[Prognosis of remote consequences of combat mental trauma using logical statistical methods].

Prognosis of remote consequences of acute stress disorders and adaptation disorders as posttraumatic stress disorders (PTSD) is possible using relevant mathematical methods. The authors have used the following logical statistical methods: algorithms for search and statistical verification of optimal fragmentation of variables in multi-dimensional space ("Q-nearest neighbors", "statistically weighed syndromes") as well as a program of searching for multiple conjunctions of variables that allowed to single out statistically significant combinations of variables (p<0,001) and to divide the sample into two groups: with favorable and with unfavorable outcome both at the acute and remote stages of mental trauma. Predictors of unfavorable outcome found in the acute period of mental trauma were as follows: affective tension, impulsivity, high personal anxiety and depressive tendencies with reduced activity. Comparing to clinical evaluation, subjective criteria of mental state (self-rating) retained its informativeness for prognosis of protracted PTSD types. Using this set of variables, it became possible to predictt remote consequences (2-4 years) of psychogenias of combat situation with reliability of recognition of 85% for the favorable outcome and 79% for cases of protracted types.

[Modified capillary blood prothrombin-time test for the calculation of an international normalized ratio].

Prothrombin time (PT) is usually measured in citrated venous blood. This test is undesirable in pediatric practice and in patients taking anticoagulants due to frequent venepunctures. Capillary blood PT test has no adverse impact. The authors have developed a standardized modified method for PT measurement in capillary blood. The adequacy of the test is due to the fact that analyzed test packed cell volumes are kept in mind. The modified test has shown that the values of normal plasma PT may be used to calculate an international normalized ratio (INR). It makes it possible to determine capillary blood PT in the range of packed cell volumes from 0.15 to 0.7 in the examinees and to monitor anticoagulant therapy. The results of the modified test closely correlated with those when determining PT and INR in venous plasma (r = 0.99) and applying the CoaguChek test (r = 0.97).

[Combined radiotherapy and endovascular x-ray therapy for invasive cancer of the urinary bladder].

The data are presented on treatment of 131 patients with transitional cell carcinoma of the urinary bladder. Radiotherapy was received by 57, regional intraarterial chemotherapy (RIAT)--27. Radiotherapy was combined with RIAT and selective hyperglycemia (HG) in 18 cases. Local super high-frequency (SHF) hyperthermia was given additionally to another 29 patients. Radiotherapy alone was followed by primary clinical cure, a 9.6-months relapse-free period and 3-months survival (36.8%), mean survival time being 26 months. In the RIAT group, these indices were 29.6%, 10.8 mos, 51.9% and 36 mos, respectively. Combination of radiotherapy, RIAT and selective HG yielded significantly improved indices: complete response--44.4%, relapse-free period--13.6 mos, 3-year survival--66.7% and mean survival time--43 mos. After addition of SHF hyperthermia, complete response rose to 69.0%, relapse-free period--18.2 mos, 3-year survival--75.8% and mean survival time--61 mos. Joint use of radiotherapy, RIAT, HG and SHF hyperthermia caused more damage to tumor, stimulated complete response and increased 3-year survival and mean survival time.

[Clotting factor VIII in Sneddon syndrome].

Hyperactivity of coagulation factor VIII (fVIII) marks hypercoagulation. FVIII enhances activity of factor IX and their combination activates factor X, which is of primary importance in prothrombin transformation into thrombin, on the phospholipid membrane. The activity of fVIII was studied in 28 patients (26 women, 2 men, mean age 49.6 +/- 7.8 years) with Sneddon's syndrome (SS). SS manifests clinically similarly to primary antiphospholipid syndrome (PAS). The leading of them are ischemic disorders of cerebral circulation (IDCC) and advanced livedo present in all the examinees. Hyperactivity of fVIII was registered in 21 (75%) of 28 patients. Most of thrombosis-related symptoms occurred more frequently in patients with high than normal activity of fVIII: ischemic strokes (91% vs 57%, p > 0.05), repeated strokes (71% vs 0%, p = 0.0014), transient IDCC (76% vs 57%, p > 0.05), vascular dementia (43% vs 0%, p > 0.05), ischemic heart disease (43% vs 0%, p > 0.05), thickening of heart valves according to echocardiography (91% vs 57%, p > 0.05), peripheral venous thromboses (24% vs 0%, p > 0.05). In high fVIII activity cardiolipin antibodies occurred more rarely (24% vs 43%, p > 0.05) but lupus anticoagulant was seen more often (47% vs 14%, p > 0.05). High fVIII activity was in 8 of 12 aPL-negative patients. It is demonstrated that elevated fVIII activity is an essential mechanism of thrombosis development in SS. The cause of this enhanced activity is suggested to be special aPL in interaction with which fVIII becomes insensitive to inactivation with protein C. The activity of protein C was normal in all the cases.

[Preliminary results of selective combined therapy for patients with urinary bladder cancer].

The paper discusses the methods of complex treatment for invasive bladder cancer which includes radiation therapy, regional intraarterial chemotherapy with cisplatin drugs, selective hyperglycemia and local SHF-hyperthermia. Complete local response was reported in 18 (56%) out of 32 patients with stage II-IV tumors, partial--8 (25%), no response--6 (19%). The method appeared effective and safe; it offers good perspectives in bladder cancer treatment.

[Use of domestic thromboplastins with attested international indices of sensitivity in the treatment of thrombophilia].

Clinical evaluation of thromboplastins manufactured by RENAM (Moscow) with international index of sensitivity (IIS) 1.1-1.5 and comparison of the results with the data obtained in the same patients with foreign thromboplastins (Dade, USA; Behring, Germany; Diamed, Switzerland) and Mediolab thromboplastins (Russia) manufactured from different raw materials (rabbit brain and human placenta) showed that by sensitivity to indirect anticoagulants, Russian thromboplastins made by RENAM and Mediolab firms are not inferior to the best foreign analogs.

Hyperfine interactions in the iron cores from various pharmaceutically important iron-dextran complexes and human ferritin: a comparative study by Mössbauer spectroscopy.

Mössbauer spectroscopy has been used to study the hyperfine interactions in the iron cores of pharmaceutically important industrial and elaborated iron-dextran complexes (ferritin models) and human ferritin. Mössbauer spectra of frozen solutions and lyophilized samples of iron-dextran complexes at 87 K demonstrated magnetic, superparamagnetic and paramagnetic states of iron in various complexes. Mössbauer spectra of human ferritin in frozen solution and lyophilized form showed paramagnetic state of iron at 87 K. Small variations of Mössbauer hyperfine parameters were observed for different samples at 87 and 295 K, respectively, supposing the homogenous iron cores. The values of quadrupole splitting for iron-dextran complexes and ferritin in frozen solutions at 87 K varied from 0.639 to 0.744 mm/s while those of lyophilized samples at 87 K varied from 0.714 to 0.788 mm/s. The values of quadrupole splitting for iron-dextran complexes and ferritin in lyophilized form at 295 K varied from 0.687 to 0.741 mm/s. The values of hyperfine magnetic fields on the 57Fe nuclei in several iron-dextran complexes at 87 K varied from approximately 231 to approximately 485 kOe. These small variations of the hyperfine parameters were related to several types of the hydrous iron oxide microstructural modifications in the core and variations of the iron core size. The influence of lyophilization on the iron core structure was also assumed. In addition, Mössbauer spectra were evaluated in supposition of heterogeneous iron core in all samples.

[Dependence of the formation of the addictive personality on the predisposing factors].

During observation of 100 patients with consequences of different types of drug-addictions (heroin, opium, poly-drug, pervitine-ephedrine) a complex of social and biological factors was analyzed: heredity, premorbid personality, somatic diseases, nurture, age of the onset of drug-addiction, duration of drug abuse, a social behavior. Influence of these factors was studied according to the following parameters: alteration of personality; degree of the moral-ethic decline and of the intellectual-mnestic disorders; somatic complications; the presence of the suicidal trends; frequency of self-damages; a professional level; family interrelations; frequency of the delinquencies (including conviction). On the basis of the correlation analysis it was established that the most significant unfavorable prognostic factors, which had determined a rate of the formation of the addictive personality were the following (in order of the decrease of significance): perinatal pathology; family history of alcoholism, drug-addiction and other mental diseases; personality deviations in premorbid period; an early age of the onset of drug addiction; a type of the drug; education; alcoholic abuse before the addiction; duration of the addiction.

[Electric stimulation of the brain in patients with hepatocerebral Wilson-Konovalov's degeneration (neuromorphological and neurophysiological analysis)].

36 stereotactic operations were carried out in 27 patients. Either the total lack of hyperkinesis or its significant decrease was found in 17 cases; 5 patients died. There was no correlation between manifestations of hepatocerebral degeneration and relatively normal quantitative characteristics of both cortical and subcortical biopotentials. These potentials are produced on the background of the microstructural changes in the neurons of the same regions. It is suggested that qualitative importance of such biopotentials lies in their exceeding pathologic impulsation, arising hyperkinesis. That was confirmed by the fact, that hyperkinesis eliminates after surgical destruction of the ventrolateral nucleus of the thalamus and the subthalamic structures.

[Study of thromboplastin sensitivity].

Comparative analysis of foreign and Russian thromboplastins demonstrated differences in the notions "activity" and "sensitivity" and the significance of low value of International Index of Sensitivity (IIS). Sensitivity of thromboplastin manufactured by RENAM Firm, Russia (IIS = 1.5) to deficiency of prothrombin factors II, VII, X has been demonstrated, which means that this thromboplastin can be used for diagnosis of deficiency of each of these factors. The authors consider that testing of thromboplastin sensitivity to factor VII is the main criterion of its fitness for monitoring anticoagulant therapy.

[Effect of composition of reagents for activated partial thromboplastin time on their sensitivity during analysis of blood coagulation factors].

Brain cephaline-based reagents for evaluating activated partial thromboplastin time (APTT) and soybean phosphatides with ellagic acid complex activator with intermediate metal ions have been studied. The sensitivity of these reagents to internal clotting factors (VIII and IX) and heparin is determined by phospholipid nature and type of metal. The results help obtain highly active and sensitive APTT reagents.

["Addictive" personality].

The paper presents an observation of 100 patients (84 men, 16 women) aged 13-50 years (mean 25 +/- 6.7 years) with different forms of drug addiction, namely: heroin, opiate, pervitin-ephedrone addiction as well as polynarcomanias. The duration of drug addiction ranged from 1 to 24 years (mean 4.35 +/- 2.25 years). No matter what the type of a drug was abused, all patients had levelled off individual personal traits and some formed peculiar narcomanic defect. It was characterized by elevated excitability, gradually progressing affective disorders in form of dysphoric or apathoabulic depressions, affective lability, prevailing hysteric-excitable forms of responses, psychosocial dysfunction as gradually decreasing interests, different anomalies of the emotional sphere, impuls control disorders, including sexual ones. A moral-ethic decrease was pronounced as well as transient intellectual-mnestic disturbances. By and large personality disorders were qualified as an "addictive personality".

[Test for detection of activated partial thromboplastin time using ellagic acid].

A simple and sensitive method for estimation of activated partial thromboplastin time (APTT) is developed, making use a complex reagent containing the activator (plant phospholipids) and contact factor (ellagic acid). The test requires additionally only 0.025 M CaCl2. The test is more sensitive to the presence of heparin in the blood and to insufficiency of blood clotting factors VIII and IX than the reagents containing insoluble substances (kaolin and animal phosphatides). Addition of soluble ellagic acid into reagent for APTT estimation allows studies on optic coagulometers.

[The psychological characteristics of patients with drug addictions].

The paper presents results of the examination of 70 patients with different forms of drug addiction (opium, heroin, pervitin addictions, including patients with polynarcomania). Psychologic tests were used to evaluate personal traits of the patients (MMPI variation), peculiarities of the emotional sphere (test of Lusher) and the intellectual level (Progressive Matrices of Raven). It was revealed that chronic drug dependence resulted in considerable disorders of personality--psychopathization and schizoidization. Intellectual abilities of the patients were on the low normal level.

[Modern principles of standardization in detection of factor VIII activity in human plasma].

The authors propose a modified single-stage method for measuring the activity of factor VIII in human plasma, consisting in the utilization of modern philosophy of standardization of coagulological studies, accepted at the National Reference Institute of Great Britain. The authors draw attention to plotting the line of tested plasma titration, which is to be parallel to the calibration curve.

[How to achieve analytical reliability of hemoglobinometry].

Three kits of reagents manufactured by RENAM Research and Production Unit provide the analytical reliability of hemoglobinometry. The kits contain: 1) dry powder transforming agent; 2) four calibration standard solutions of hemoglobin cyanate requiring no dilution; 3) three highly purified stable solutions of hemoglobin (simulating human blood) with attested concentrations of hemoglobin from 50 to 200 g/liter.
