Stimulation of reparative osteogenesis after xenotransplantation of human prenatal mesenchymal stem cells and chondroblasts.

Regeneration of the bone tissue after unilateral xenogeneic transplantation of a suspension of human mesenchymal cells and chondroblasts was studied in rats with experimental damage to both femurs. The state of animals was satisfactory and non-depressed in the early and late postoperation period. No local pathological reactions and complications were seen. Administration of mesenchymal stem cells and chondroblasts into the area of bone defect accelerated regeneration on days 10 and 30 (compared to the control group). Implantation of mesenchymal stem cells more significantly stimulated reparative osteogenesis compared to treatment with chondroblasts. This was mainly associated with increased ratio of mature lamellar bone tissue. Unilateral transplantation of the cell suspension stimulated regeneration in the contralateral limb due to accelerated maturation of the bone tissue. The bone tissue formed after transplantation of mesenchymal stem cells and chondroblasts was integrated into bone organ and underwent complete remodeling. Xenotransplantation of prenatal mesenchymal stem cells and chondroblasts without immunosuppression was not followed by the development of early and delayed complications or local reactions of graft rejection.

Reparative osteogenesis during transplantation of mesenchymal stem cells.

Reparative osteogenesis was studied after xenotransplantation of suspension cell graft from human mesenchymal stem cells. A model of experimental damage to rat femoral diaphysis was developed. The state of animals was satisfactory and non-depressed in the early and late postoperation period. We revealed no local pathological reactions and complications. Administration of mesenchymal stem cells into the area of bone defect accelerated and improved regeneration. Unilateral transplantation of the cell graft stimulated regeneration in the contralateral limb due to acceleration of bone tissue maturation. On day 90 after treatment the bone regenerate was completely developed in the area of defect in animals of various groups. The newly formed bone tissue was well integrated into the bone organ.

[Diagnostic potential of computer tomography in odontogenic cysts study in cases of their contiguous invasion of sinuses].

Questions of diagnostics and treatment of odontogenic cysts with contiguous invasion of sinuses are discussed. It was shown that the use of computer tomography increased considerably its diagnostic value in determination of size and localization of the cyst, its relationship with sinus membranes. It helps to plan the operation correctly and prevents complications. The results of clinical-roentgenological examination and treatment of 10 patients with odontogenic cysts with contiguous invasion of sinuses are offered.

[Lactation mastitis: problems and perspectives].

Results of treatment of 664 patients with lactation mastitis were analyzed. It was demonstrated that Staph. aureus was the main infectious agent of the disease. Antibacterial therapy must be started with cephalosporins and aminoglycosides and corrected after tests for antibiotics sensitivity. In purulent mastitis breast feeding must be stopped, and drugs suppressing lactation should be added. In severe forms of intramammary purulent mastitis semi-oval incision by Rovinsky is recommended. In infiltrative-purulent mastitis it is better to resect the affected part of the mammary gland and apply delayed sutures in 5-7 days. Surgery must be performed by an experienced surgeon and assistant under general anesthesia.

[Diagnosis and treatment of advanced stomach cancer].

800 patients with stomach cancer were operated in Saratov Military Medical Institute from 1990 to 2000. Results of radical surgery in advanced stomach cancer are presented. The most frequent postoperative complications and causes of lethal outcomes are described. Treatment and diagnostic algorithm in stomach cancer is proposed to optimize work of general surgery units.

[Results and perspectives of development in dental implantation in Russia].

The basic trends of scientific research carried out at the implantation department of CRID (Central Research Institute of Dentistry), referred to the fundamental problems in medicine and biology including a variety of not only experiments and clinical study but also a complex of technical aspects. Our long-term research and valuable clinical material collected resulted in confirming the necessity and importance of complex approaches developed by the Institute in dental implantation.

[Use of implants in congenital anodontia].

Use of screw implants for repair of the maxilla and mandible defects with congenital anodontia is discussed. Surgical methods are described. Clinical examples are offered. Immediate and remote results are illustrated.

[Preparation of bone tissue of the jaw to implantation of abutment elements of dentures].

A complex multi-staged method for stimulation of bone tissue regeneration in the jaws after removal of teeth and preparation of bone tissue for subsequent implantation, stimulation of bone tissue regeneration after implantation, and adaptation of bone tissue of the jaws to masticatory pressure is developed. The method is based on utilization of magnetic laser exposure with a universal Optodan laser device. In case of pronounced inflammation and pyodestructive processes after implantation, these processes are liquidated due to detoxifying effect of the blood, which is stimulated by irradiation of the carotid sinuses on both sides. Granulation tissue round the heads of implants is removed by laser coagulation with surgical lasers Lancet or Stokos.

[Maintenance of anesthesia and intraoperative safety in dental implantology].

Most patients who need dental implantation suffer from somatic diseases. Therefore, an anesthesiologist's participation is highly desirable, since he supports the stability of vital functions. The authors used mixed anesthesia involving traditional field-block and general anesthesia. By choosing doses of anesthetics, the authors achieved ataraxia which preserved the minimum contact with a patient and a complete analgesia, which gave rise to psychoemotional stability.

[Use of X-ray techniques in dental implantology].

This paper deals with some aspects of using radiographic methods in dental implantology. Analysis of the published data and their own findings have brought the authors to the conclusion that the most widely used method involved is a panoramic technique; however, CT scan is required in some cases demanding accurate measurements; which, at the stage of preoperative planning, enables us to specify surgical indications and choose the optimal form of implants.
