[Cataract surgery results with intraocular lens implantation in children and youth in the years 1990-1995 (personal material)].

PURPOSE: Analysis of cataract surgery with intraocular lens implantation in children and youth performed between 1990 and 1995 in Bialystok University Eye Clinic. MATERIALS AND METHODS: Twenty patients between 5 and 19 years of age were operated. The subjects comprised 15 cases of traumatic and 5 of congenital cataract. Intraocular lens implantation was performed as a primary procedure in 18 patients and as a secondary procedure in 2 cases. RESULTS: Visual acuity of 5/50 to 5/12 was achieved in 14 patients and 5/10 to 5/5 in 6 patients. Posterior capsule opacification requiring YAG-laser capsulotomy was found in 12 patients (60%). CONCLUSIONS: Intraocular lens implantation may be a safe and effective technique for optical correction in children with traumatic and congenital cataracts.

[Neodymium-YAG laser iridotomy in subacute and acute angle closure glaucoma].

PURPOSE: To evaluate efficiency of Nd: YAG-laser iridotomy in the treatment of angle closure glaucoma. MATERIAL AND METHODS: 33 eyes of 22 patients were examined before and after the treatment. Intraocular pressure (IOP) was measured 24, 48 and 72 hours after iridotomy and then in (after) 1 and 6 months. Visual acuity, static perimetry and optic disc evaluation was performed before the treatment and after 1 and 6 months. RESULTS: In 90% of the eyes, 24 hours after the laser, IOP was below 22 mmHg and it was normalized during the whole follow-up. There no differences between the results of visual acuity, perimetry and optic disc evaluation before the treatment and during follow-up. CONCLUSIONS: Nd:YAG-laser iridotomy is an efficient method of lowering of IOP in angle-closure glaucoma.

Cathepsin D and an alkaline protease activities in subretinal fluid.

It was found that subretinal fluid contains at least two different proteolytic enzymes which digest various protein substrates. The main of them was identified as cathepsin D. This protease is very active towards the denatured haemoglobin. The highest activity is apparent at pH 3-4 and it is completely inhibited by the action of pepstatin. The cathepsin activity increases with the duration of retinal detachment. The other proteolytic enzyme acts at slightly alkaline pH. Its activity is lower and does not change with the duration of the disease.

Body Fluids↗

[Behavior of selected cytokines in subretinal fluid].

The aim of the investigations was to search the subretinal fluid for the inflammatory interleukins which influence T and B lymphocytes. The levels of IL-1, IL-2, IL-4 and IL-6 were determined. Only IL-6 was found in a significantly high level, especially in cases with retinal detachment lasting 5-8 weeks. The level of other interleukins examined was not significant.

[Proteolytic and collagenolytic activities of the bovine vitreous body].

The vitreous contains collagen and soluble non-collagen proteins. The degeneration of collagen takes place in a different manner than in the other proteins. It has been detected that the cattle vitreous does not show any collagenolytic activity against the basic types of collagen (I, II, IX, and XI). The vitreous digests haemoglobin only in the acidic section of pH (optimum 3.5-4.5). No proteolytic activity towards Hb in physiological pH has been found.

[Vitreous body proteins. IV. Proteolytic activity of the vitreous body in pathological conditions of the eye].

Two groups of proteins are present in the vitreous: collagen and non-collagen, deriving probably from the blood serum. The authors examined the proteolytic activity against haemoglobin in the course of some ocular pathological conditions. It was demonstrated that the vitreous digests haemoglobin in the acid division of pH (optimum--3.0-4.5) with various activity dependent on the pathological condition of the eye.

Cataract↗

[Parasitic uveitis in children].

Seven children with an atypical parasitic uveitis were treated in the Department of Ophthalmology in Bialystok in the period 1985-1989. Parasitic background had been discovered in all the children: toxocara in 3, lambliasis in 2 and nematodiasis also in 2. Specific treatment achieved an improvement of the visual acuity and the inflammatory process abated.

Antinematodal Agents↗

Proteolytic activity of vitreous-humour.

It has been found that the bovine vitreous--humour did not digest in vitro collagen at physiological and acidic pH. A proteolytic activity against haemoglobin in acid pH was found both in bovine and human vitreous-humours. The activity of human vitreous-humour increased significantly in endophtalmitis and glaucoma. In all pathological conditions studied the pH optima were at more acidic values than in control.

[Function of the visual system after orbital surgery. Late observations].

Forty six persons with an orbital tumor were operated in the period of 1980-1988. A control examination performed in 39 persons did not show in 38 any characteristics of recurrence of the tumor; the motility of the eye ball was normal. In 29 persons the visual acuity of the eye at the side of the operated tumor was over 5/6.

Bcl-2 expression in primary uveal melanoma.

AIMS: Uveal malignant melanoma is the most common intraocular tumor. The aim of this study was the analysis of bcl-2 oncoprotein expression in this tumor type. The melanomas were evaluated according to tumor location and patient age and sex. The relationship between bcl-2 expression and histological type, clinicopathologic stage and the presence of a set of predetermined morphological parameters was analyzed. METHODS: The study involved 39 patients with ocular melanomas treated with surgery alone between 1983 and 1997. Formalin-fixed, paraffin-embedded tissues were treated with anti-bcl-2 antibody (Dako No M0887). Immunolocalization of the bcl-2 oncoprotein was performed using the labeled streptavidin biotin (LSAB) method. bcl-2 expression in neoplastic cells was evaluated in a semiquantitative manner: lack of reactivity was defined as bcl-2 negative, reactivity present in less than 30% of cells as low bcl-2, and reactivity in more than 30% of cells as high bcl-2. The percentage of cells with a positive reaction was assessed independently by two pathologists, and the results were subjected to statistical analysis using Fischer's exact test. RESULTS AND CONCLUSION: No statistically significant correlation was found between the expression of bcl-2 oncoprotein and the clinicopathologic features analyzed. However, the high percentage of tumors with positive expression of this oncoprotein suggests that it plays a significant role in the biology of uveal melanoma.

Ultrastructural changes in the trabecular meshwork of congenital glaucoma.

Trabeculectomy specimens from 10 patients with congenital glaucoma were studied by electron microscopy. In all cases proliferation of fibrous connective tissue was found from the inner wall of Schlemm's canal. Accumulation of collagen fibres, sometimes of varied thickness and irregular striation, and agglomerations of microfibrillar material with the structure of collagen microfibrils were observed. This material formed basement membrane-like structures and 'shaggy' or fingerprint-like patterns. The ultrastructural examination of the trabecular meshwork revealed no significant differences between the morphological picture of primary congenital glaucoma and that found in Axenfeld and Rieger anomalies. Ultrastructural pictures of glaucoma cases examined in the present study converged also with those of juvenile glaucoma cases reported in literature, which may indicate the existence of a pathogenic factor common to these forms of glaucoma.

[Photocoagulation in diabetes mellitus--our results].

Fifty six diabetic patients aged 25-74 were under ophthalmological control in the period 1984-87. The mean time of duration of diabetes amounted 14 years. In 29 persons diabetes was insulinodependent and in the remaining insulino-independent. In 11 eyes the fundus exhibited diabetic changes with characteristics of angiopathy, in 43 eyes--of exudative retinopathy and in 58 eyes of proliferative retinopathy. Photocoagulation with a xenon-arc lamp of the diabetic changes was performed in all the patients. In dependence of the extent of the changes one used either a focused or a panretinal photocoagulation. In the majority of cases one could stop the progress of the diabetic changes on the fundus and preserve a fair visual acuity.

[Evaluation of the surgical treatment of retinal detachment in children. Personal data].

Twenty nine children aged 8-18 years with retinal detachment were treated in the period 1980-1987. The cause of detachment was in 27 cases injury and in 2 cases rupture of a congenital retinal cyst. In the majority of cases the detachment was connected with a dialysis at the ora serrata and comprised two quadrants of the fundus' periphery. The operation in 25 children consisted on implantation of a silicone sponge. A hard silicone implant situated intrasclerally with a band round the globe was used in 3 persons. A cerclage of the eye ball with a silicone sponge was performed only in one child. In 25 children the retina reattached after operation.
