Endometrial carcinoma: 128 cases at stages I and II.
The authors have re-examined 146 patients with stage I and II endometrial carcinoma according to pathological findings. An understaging of FIGO classification of 9.8% and 14.2% in stages I and II, respectively, was found. Also evaluated was the influence of some risk factors (histologic grade, myometrial infiltration) on survival rates which was highly significant for both stages I and II. Surgery was the main form of treatment with an operability rate higher than 90%. The bilateral salpingo-oophorectomy appeared to improve the survival at stage I, 85.8% versus 63.6%, while postoperative external radiotherapy (ERT) seemed to be ineffective.
Squamous cervical cancer at stage IB and II: a ten years experience, analysis of 231 cases.
The authors have reviewed their experience on treatment of carcinoma of the uterine cervix at stage I and II, with a special regard for prognostic factors. The best survival results at 7 years for patients at stage I B and II A were obtained with integrated treatments, while for stage II B patient no treatment proved to be really effective. Lymphnodal infiltration, increasing with the stage, was the most important prognostic factor, but stage must always be taken into account, especially in N- patients. Some considerations are then made on the importance of prognostic factors such as lymphnode status and tumour volume, in order to improve the survival results.
Early hemostatic disorders in patients with gynecologic tumors.
One hundred and seventeen female subjects were studied: 23 patients with cervical carcinoma; 25 with endometrial carcinoma; 29 with benign uterine diseases; 14 with ovarian carcinoma; 26 patients with benign ovarian tumors. These patients, together with 25 healthy female control subjects, underwent several coagulation tests including Beta-Thromboglobulin (Beta-TG) and Platelet Factor 4 (PF4) plasma levels. Among all Beta-TG and PF4 exhibited the most interesting results. They were increased in four groups of patients: those with malignant 92.2% (13/14) and benign 50% (13/26) ovarian tumors and those with endometrial 64% (16/25) and cervical carcinoma 69.5% (16/23). Our study showed a high incidence of abnormalities of Beta-TG and PF4, early signs of hemostatic disorders, in gynecological malignancies especially in ovarian carcinoma. These data suggest a possible value of these tests as tumor-markers and in order to detect the patients who develop thrombo-embolic accidents.
Evaluation of circulating immune complexes (C.I.C.) in endometrial cancer.
We evaluated the presence of CIC in 32 patients with histological proven cancer of endometrium: 23 stage IA or IB (i.e. localized disease), 9 in stage II or III (i.e. advanced disease). The determination of CIC was done at the time of the diagnosis before the patients underwent any surgical or medical therapy. The presence of CIC was demonstrated in 17% (4/23) of the patients with localized disease and in 56% (9/9) of the patients with more advanced disease. Our findings indicate that the presence of CIC is higher in patients with advanced endometrial carcinoma (stage II, III), than in those suffering of a more limited disease (stage IA, IB) and this seems could be consistent with a possible role in modulating the immune response against tumour itself.
Pelvic exenteration in gynaecologic oncology: analysis of 44 cases.
The Authors review 44 cases of pelvic exenteration performed at the Department of Obstetrics and Gynaecology of the Università Cattolica del S. Cuore - Rome, from 1967 to 1981, in patients affected by advanced or relapsed gynaecological malignancies. Operative mortality, medical and surgical complications and survival are analysed. Some characteristics of the disease are outlined as the most important criteria for patient selection, and the possible curative role of this operation is stressed.
Evaluation of the feto-placental function by means of intraamniotic administration of dehydroepiandrosterone-sulphate.
It is well known that in pregnancy dehydroepiandrosterone-sulphate (DHEAS) (90% synthetized by the fetal adrenals, 10% by the maternal ones) is converted into estrogens mainly in the placenta. In order to evaluate the steroid metabolizing capacity of the placental enzymatic complex under load conditions, a test was devised based upon a rapid introduction of 200 mg of DHEAS into the amniotic compartment during the third trimester of gestation. The purpose of the present investigation was to establish the clinical utility of such a test in comparison with the simple urinary estrogen determination test.
Water and ion metabolism in placenta. II. Water compartments and electrolytes in slices of rabbit placenta, at different periods of gestation, incubated at 0-1 degree C.
The movement and the distribution of water, sodium, potassium, chloride, magnesium and calcium have been studied in slices prepared from rabbit placentae at different periods of gestation, incubated in condition of depressed metabolism (0-1 degree C). In these conditions the tissue takes up water from the external medium up to a maximum of about 2.0 kg/kg d. wt., which represents 30 per cent of the initial H2O content of the fresh tissue. The extracellular compartment swells progressively and proportionally to the age of the placenta. The sodium and chloride content of the tissue increases while that of potassium decreases and their intracellular concentration reaches, after 120 min, that of the external medium. Magnesium does not show appreciable changes and calcium, too, despite its extreme variability, does not seem to undergo significant variations during the cold incubation. The results obtained show that placenta, like most of other mammalian cell systems, possesses specific metabolism-dependent mechanisms responsible for the maintenance of water distribution and ion gradients among the different tissue compartments. The characteristics and the regulation of these mechanisms are discussed in detail.

Four cases of pregnancy with low estrogen production due to placental enzymatic deficiency.
4 cases of normal pregnancies associated with low estriol excretion and resulting in healthy infants are described, in which by in vitro studies on the placentas a steroid 3-sulfatase deficiency was found in 3 cases and a lack of activity of the aromatizing system was found in the 4th case. A reduced activity of the aromatizing system was also shown in one of the cases affected by steroid 3-sulfatase defect. The placental 3 beta-hydroxysteroid dehydrogenase, delta 4,5-isomerase system was measured in 3 cases and results were normal. The repetition of the specific lack of steroid 3-sulfatase was observed in 2 successive pregnancies of the same patient (case T.C.). It is interesting to note that, at variance with other reports, this patient delivered 2 fetuses, both female.
The behaviour of microsomal mono-electron carriers in adult and fetal liver and in placenta throughout pregnancy.
The activities and contents of microsomal mono-electron carriers (NADPH-cytochrome c reductase, NADH-cytochrome b5 reductase, cytochrome P-450 and cytochrome b5) in the liver of 160 pregnant rabbits and their response to phenobarbital (PB) administration were found to be different to that of non pregnant animals and to vary depending on the period of gestation. In fetal liver and in placenta the activities and contents of mixed function oxidase system enzymes are very low and increase from the mid-gestation up to the term of pregnancy. PB has either effect or decreases the enzyme levels. The variations of enzyme content and activity in maternal liver and the different response to PB are related to the hormonal modifications of pregnancy. The behaviour of mono-electron carriers in fetal liver can be related to the process of maturation of this organ. The effect of PB can be due to the interference of the drug with steroid metabolism. The behaviour of mixed function oxidase system enzymes in placenta can be related to the processes of maturation and ageing of organ.

Water and ion metabolism in placenta. I. water and ion content of rabbit placenta in different periods of gestation.
Total tissue water of rabbit plancenta decreases by 18% between the 9th day and the end of gestation. In the same period intracellular water decreases by 40% and the extracellular compartment swells. Total tissue sodium and chloride content increase by 34% and 27% respectively from the 16th to the 33rd day while potassium decreases by 30%. The intracellular concentration of these ions does not vary greatly during gestation. Moreover the total tissue magnesium content decreases by about 23% at the end of gestation while its intracellular concentration does not show marked changes. Calcium content of rabbit placenta substantially increases during gestation and intracellular calcium shows a fivefold increase from the 16th to the 33rd day of gestation. The results are discussed in terms of relation between water and ion content and placental tissue growth and aging.

In vitro biosynthesis of rabbit blastokinin.
The authors have incubated, in vitro, estrous rabbit endometrium with 3H-leucine and pseudopregnant rabbit endometrium with 14C-leucine. By the double-labeling technique and the use of the 14C:3H ratio, it was shown that blastokinin (BKN) is neosynthesized in vitro and that this neosynthesis probably also affects other protein components. By ion exchange chromatography of the supernatant of the pseudopregnant endometrium and previously purified BKN, it was possible to confirm the in vitro biosynthesis and to exclude interfering phenomena. Time dynamics showed that biosynthesis was more marked between the 18th and 24th hours of incubation.
The behaviour of microsomal mono-electron carriers in human placenta during late pregnancy.
The activities and the content of enzymes involved in mixed function oxidase systems (cytochromes P-450 and b5, NADPH-cytochrome c reductase and NADH cytochrome b5 reductase) in microsomes from 114 human placentae were found to increase from the 34th up to the 38th-39th week of gestation and later to decrease until the 43rd week. Such a process can be related to the following: (i) the phospholipid content of microsomal membranes; (ii) the protein synthesis possibly induced by steroids such as androstenedione and testosterone.
"Timing" of ovulation by laparoscopy and evaluation of some physico-chemical parameters.
The Authors describe the laparoscopic aspects of spontaneous human ovulation and correlate it with the evaluation of basal body temperature, the total urinary oestrogens, pregnanediol and urinary LH and some hematic values of 17-beta-estradiol, 17-alpha-Hydroxyprogesterone and rogesterone. They find that the rise in temperature and the oestrogenuria peak precede ovulation by some hours in the great ovulation by sompe hours in the great majority of cases. They analyze the validity of the other parameters studied, and show a relatively constant behavior, with regard to ovulation, in the mean evaluation of several cases. In the individual analysis, on the contrary, this relation is difficult to find and less significant. The Authors stress the importance of comparing the physico-chemical evaluations with the biological phenomenon of ovulation directly ascertained by means of laparoscopy.
