Intramural mesenteric venulitis. A new cause of intestinal ischaemia.

Venous damage is an uncommon cause of intestinal ischaemia. We report on a 44-year-old woman who presented signs and symptoms of acute intestinal ischaemia requiring surgical treatment. Histological examination of the resected right colon showed features of an intramural lymphocytic venulitis with no other demonstrable causes of ischaemic injury of the bowel. Extramural mesenteric veins appeared dilated and congested, without evidence of thrombotic occlusion or of inflammatory involvement. The patient, who was not taking any long-term medication and had no clinical evidence of collagen-vascular disease, promptly recovered after surgery. Follow-up for 7 months with no recurrences suggested a self-limited or indolent process. We propose the name 'intramural mesenteric venulitis' for this condition and believe that it could represent one extreme (the microscopic variant or intramural phase) of the spectrum comprising entero-colic phlebitis and mesenteric inflammatory veno-occlusive disease. The immunohistochemical evidence of a marked preponderance of T phenotype in the perivenular lymphocytes suggests lymphocyte-mediated vascular damage as the pathogenesis of the lesion.

Nodular renal blastoma in kidney with multicystic dysplasia. Report of a case.

The clinico-pathologic association of nodular renal blastema, multicystic kidney and obstructive uropathy has been recently identified. We report on a female patient diagnosed as having unilater multicystic dysplasia by prenatal ultrasonography. The patient was nephrectomized at the age of 6 1/2 months. Examination of the resected kidney revealed multiple unilocular cysts in the cortex and hypoplasia of the homolateral ureter; histological study confirmed the presence of multiple cysts limited to the renal cortex, and revealed, among them, multiple cortical metanephric blastema cells islands. Our case supports a relationship between nodular renal blastema, cortical cysts and obstructive uropathy; ureter hypoplasia could cause intraluminal back pressure, with consequent abnormal development of the ampullae, normally endowed in nephronic anlagens induction, cystic tubular ectasia and persistence of nodular renal blastema. The peripheral location of renal nodular blastema and cysts supports a late error in nephrogenesis, at the time of formation of the last generation of nephrons.

Chronic cholecystitis with features of diffuse inflammatory pseudotumour: a clinico-pathological case study and review of the literature.

A case of chronic cholecystitis with features of inflammatory pseudotumour is presented. The patient, a 69-year-old man, was admitted for epigastric pain, nausea, vomiting, and fever. Ultrasound examination of the abdomen revealed diffuse thickening of the gallbladder wall, and numerous calculi inside, without dilatation of the intra and extrahepatic biliary ducts. Cholecystectomy was performed. Histological examination confirmed the diffuse thickening of gallbladder wall, revealing a diffuse chronic inflammatory process with a huge fibroblastic and myofibroblastic proliferation associated with plasma cells, lymphocytes, macrophages, granulocytes, and rare giant multinucleated cells. Only one similar case has been quoted in the literature so far.

Congenital-infantile fibrosarcoma: study of two cases and review of the literature.

BACKGROUND: Congenital-infantile fibrosarcoma is a rare tumor of the pediatric age. It involves subjects under 5 years of age, and more than 200 cases have been reported in the literature. METHODS: The authors present the clinicopathologic findings of 2 cases and review the literature. RESULTS: Of our 2 patients, the first was a 2-years and 6-months-old female and the second a newborn male. The female presented a tumor in the retroperitoneum without recurrences or metastasis after 17 months, and the male on left foor with a recurrence after 3 months. Histologically, the tumors were mainly composed of spindle-shaped cells. Immunohistochemically, in both cases, neoplastic cells were positive for vimentin; focal positivity for muscular specific actin was present in the tumor of the female. Ultrastructurally, tumors were composed of mesenchymal cells with fibroblastic and myofibroblastic features. Flow cytometric analysis of the retroperitoneal tumor showed an aneuploid population of neoplastic cells. CONCLUSIONS: Congenital-infantile fibrosarcoma should be considered a borderline tumor; its biologic behavior is better than that of adult fibrosarcoma. Histologic diagnosis is not easy; the microscopic picture may be confused with fibromatosis or with malignant mesenchymal neoplasms. Only a follow-up of many years can confirm the benignancy or malignancy of any individual tumor, even though clinico-pathologic features may be distinctive enough to permit its recognition.

Oral metastasis of renal cell carcinoma. Report of case and critical evaluation of morphologic features for differential diagnosis.

Two clear cell neoplastic lesions of the oral cavity, the first on the cheek and the second on the mucosa of the upper lip, were identified, in a 44 year old otherwise healthy man, 46 and 51 months after left nephrectomy for renal clear cell carcinoma. Basing on this case, unique for the double and limited metastasis of clear cell carcinoma of the kidney in the oral cavity, we discuss about the morphologic criteria usefull in the differential diagnosis of oral cavity clear cell tumors.

[Anatomic and surgical considerations regarding the recurrent laryngeal nerve in thyroidectomy].

The majority of surgeons view the preliminary identification of the recurrent laryngeal nerve (RLN) as an essential stage in thyroidectomy in order to preserve thyroid integrity. This conviction is based on the fact that the anatomical relations of the recurrent nerve during the cervical tract with adjacent structures are highly variable. The preparation of the RLN in a series of 42 thyroidectomies performed by the 1st Department of Pathological Surgery of the University of Florence confirmed this variability above all with regard to relations with the trachea, Berry's ligament and the branches of the lower thyroid artery. As a result, it is justified to consider the identification and preliminary preparation of the RLN on both sides as a fundamental and unavoidable stage in thyroidectomy.

Serum angiotensin converting enzyme activity in Crohn's disease and ulcerative colitis.

The relationship between serum angiotensin-converting enzyme activity (SACE) and inflammatory bowel disease was investigated in 30 patients with active Crohn's disease (CD) (CDAI > 150), 12 patients were not taking active medication, while 11 were on prednisone and 7 on mesalazine (5-ASA). Of 12 patients with active ulcerative colitis (UC), 4 patients were without treatment, 4 were on prednisone and 4 on mesalazine. Twelve patients with acute pulmonary sarcoidosis and 25 healthy subjects were studied. SACE levels in CD were not significantly different compared to healthy subjects and did not appear to be affected by the anti-inflammatory therapy. The average levels of SACE in UC were significantly higher than those of healthy subjects and CD (p < 0.005). Among the UC patients there was a trend for lower levels in the group treated with prednisone. In patients with active sarcoidosis SACE levels were extremely elevated and were significantly higher than in all other groups examined (p < 0.0001). The granulomatous inflammation in Crohn's disease differs from that of sarcoidosis, in which there is a striking elevation of angiotensin converting enzyme in serum. In other words, SACE levels seem to differentiate patients with active CD from patients with UC.

Effect of activity on performance and morphology in ischaemic rat slow muscles.

Muscle performance and structure was studied in rat soleus muscle with limited blood supply in combination with chronic muscle stimulation. Blood supply to the lower leg was restricted by ligation of the common iliac artery, electrodes were implanted in the vicinity of the sciatic nerve and ankle flexors were denervated. Three days later, soleus and gastrocnemius muscles were stimulated at 4 Hz four times a day for a period of 20 min with 2 h intervals between stimulations; this procedure was continued for 4 days. Muscle performance, histochemistry and ultrastructure were studied on the eighth day after operation in these muscles and in ischaemic unstimulated muscles with denervated ankle flexors. Both were compared with control animals. Muscles with limited blood supply developed less isometric twitch tension than control muscles (peak twitch tension in ischaemic muscle was 60.3 +/- 4.8 g g-1 muscle, mean +/- S.E.M., compared to 79.7 +/- 6.9 g g-1 in control muscle; tensions after 5 min contraction were 54.5 +/- 5.5 g g-1 in ischaemic muscle compared to 70.6 +/- 6 g g-1 in controls). Stimulated muscles with limited blood supply had higher peak (85 +/- 16.6 g g-1) and final (87 +/- 12 g g-1) tensions, and also fatigued less than muscles with limited blood supply but no stimulation. Histochemical estimation of capillary density (by staining for alkaline phosphatase) and slow (SO) and fast (FOG) fibres (by myosin ATPase staining) revealed similar capillary to fibre ratios (2.5) and a similar proportion of FOG fibres (around 18%) in all muscles. The proportion of glycogen-depleted fibres (estimated from the periodic acid Schiff reaction, PAS) in muscles removed from animals 10 min after a 5 min period of isometric twitches was significantly lower in ischaemic muscles (45.1 +/- 1.9%) than in control (80.5 +/- 1.5%) or chronically stimulated ischaemic muscles (67.3 +/- 4.0%). Electron microscopy showed disorganised myofibrils with Z-line streaming in 7.48 +/- 3.04% of fibres in muscles with limited blood supply. Swollen and degenerated mitochondria, dilated sarcoplasmic reticulum and areas of disrupted sarcolemma were also observed. Stimulated ligated muscles showed a significantly lower proportion of fibres with disorganised filaments (0.65 +/- 0.32%) and other signs of damage were much less frequent. The reduced damage and improved performance of chronically stimulated slow muscle may be the result of improved microcirculation, preventing accumulation of lactate.

Variation in thin filament size in the skeletal muscle of the frog.

The sartorius muscle of Rana esculenta was fixed and dehydrated by different methods and cross-sections of sarcomeres of different lengths were examined by electron microscopy. The area within the outlines of the thin filaments in negatives was measured using a high-resolution television system and a graphics tablet or by using a microdensitometer and computer processing the images with a density threshold above the background level. In all specimens the area of the thin filaments in the I-bands was found to be larger than in the S-zones of the A-bands where the thick and thin filaments interpenetrate. The difference was statistically significant and independent of the length of the sarcomere. The results are in agreement with previous observations in glycerol-extracted fibres. It is suggested that the change in size of the thin filaments might be accounted for by some interaction with the thick filaments.

Lymphocyte proliferation in neonatally thymectomized rats.

Autoradiography has been used to evaluate lymphocyte proliferation in the neonatally thymectomized rat in comparison with the normal animal. The data obtained show that the proliferative activity of lymphocytes is greatly increased in the thymus-dependent areas 4-6 weeks after thymectomy, whereas it is normal or slightly increased 3 months later. It seems plausible to assume that a thymus factor or chalones in situ produced by specific cells normally regulate the proliferation of thymus-derived cells. The increase of the proliferative activity accounts for the repopulation of the thymus-dependent areas, which are completely replenished in the older animals. Recirculation of the thymus cells is not confined to the thymus-dependent areas.

Effects of dimethyltriazenes on in vitro Lewis lung carcinoma tumor lines with different metastatic capacity.

The effects of a selective antimetastatic agent: the aryldimethyltriazene derivative 1-p-(3,3-dimethyl-1-triazeno)benzoic acid potassium salt (DM-COOK) have been examined on two in vitro tumor cell lines derived from lung metastases of Lewis lung carcinoma. These stabilized in vitro tumor cell lines named C108 and BC215 have been reported to differ in their metastatic potential evaluated as lung colony forming ability and as the number of spontaneous metastases produced after intramuscular implant of tumor cells. The cytotoxic effect of DM-COOK in vitro was also compared with the one demonstrated by the structure-related compound 4-(3,3-dimethyl-1-triazeno)imidazole-5- carboxamide (DTIC) on the same variant lines. Survival curves show a different chemosensitivity of the two in vitro lines to the DM-COOK treatment, whereas no differences were detected between C108 and BC215 after exposure to DTIC. Moreover, DM-COOK and DTIC exhibit different trends of cell killing, implying different mechanisms of action for the two drugs. Results are discussed in view of the selective in vitro action of the aryldimethyltriazene derivative DM-COOK on cells which express a high metastatic potential.

Expression of tumor-associated antigens and kinetic profile of two in vitro human melanoma cell lines.

The growth characteristics and the antigenic profile of two human melanoma established cell lines in vitro are reported. The growth curves of the two cell lines show some peculiar features which are also detected in their DNA distribution content. In the late stages of growth, in fact, the cell DNA content resumes the initial distribution, suggesting a partial recovery of the cell proliferating potential. When the two cell lines were tested by indirect immunofluorescence with monoclonal antibodies to human melanoma associated antigens, a rather constant expression of these epithopes was observed.
