[Stereotaxic radiosurgery:methods and indications in the treatment of cerebral arteriovenous malformations].

After Leksell and Steiner's pioneering experience with the use of the gamma units, another technically different system has been developed, using a linear accelerator. It's a very precise stereotactic radiosurgical approach which allows to deliver the necessary dose of radiation to the target volume while sparing the surrounding structures from potentially dangerous levels of radiation. This hyperselective irradiation is a new and complementary method of treatment of arteriovenous malformations located in central or functionally critical areas of the brain. The aim is to obtain a progressive and total obliteration of the lesion. The place of radiosurgery is obviously reserved for the patients who do not seem able to profit from the benefits of open surgery and/or embolization techniques.

[Clinically localized cancers of the breast. Therapeutic consequences of the evaluation of extension].

The therapeutic modifications induced by pretreatment evaluation were studied in a consecutive series of 852 asymptomatic women with newly diagnosed primary breast cancer attending our center between 1980 and 1984. Staging tests included chest X-rays in 851 patients, bone X-rays in 831, alkaline phosphatase in 826, hepatic enzymes in 818, liver echography in 750 and bone scintigraphy in 504. The intended local treatment was changed for a systemic one in 8 patients due to suspicious abnormalities. The follow-up confirmed evidence of metastases in 6 out of 8 patients (bone: 4; liver: 1; lung: 1). Mastectomy, initially avoided in these 6 patients, was subsequently performed in 2 of them owing to slow progression of distant metastases. On the basis of the current study, pretreatment staging in asymptomatic primary breast cancer cannot be recommended due to the low prevalence of detectable metastases.

Characterization of insulin-like growth factor 1 receptors (IGF1-R) in human breast cancer cell lines.

The binding characteristics of IGF1 on membranes prepared from 5 human breast cancer cell lines were investigated in detail. The presence of one class of high affinity IGF1 binding sites was demonstrated (BT-20: n = 230 fmol/mg protein, Ka = 0.7 nM-1; MCF-7: n = 124 fmol/mg protein, Ka = 1 nM-1; T-47D: n = 61 fmol/mg protein, Ka = 1.1 nM-1; HBL-100: n = 18 fmol/mg protein, Ka = 3.2 nM-1; MDA-MB-231: n = 7 fmol/mg protein, Ka = 2.8 nM-1). Chemical cross-linking of 125I IGF1 to breast cancer cell membranes then sodium dodecyl sulfate-polyacrylamide gel electrophoresis revealed one major band of relative molecular weight 130000. The specificity of these receptors was studied: native or recombinant IGF1 had the same potency to inhibit 125I IGF1 binding; IGF2 was able to compete for binding, whereas insulin competed with a potency lower than 1/100 that of IGF1. These characteristics of IGF1 binding sites in breast cancer cell membranes correspond to the previously described binding unit of type I IGF receptors (IGF1-R). Finally we determined that for a routinely used standard assay, it was necessary to incubate for 5 h at 4 degrees C a high amount of membrane protein (400 micrograms) and 200,000 cpm of tracer. Considering the known effect of IGF1 on breast cancer cell multiplication, it is tempting to suggest that this factor might play a major role in the growth of breast cancer: the measurement of IGF1-R, using this standardized method, will give an assessment of these tumors IGF1 sensitivity; it can be performed on the membrane fraction obtained when preparing cytosol for steroid receptor assay.

[Intracerebroventricular morphine therapy. Apropos of 79 patients].

The authors report their experience in the treatment of 79 patients suffering from intractable cancer pain with intra-cerebro-ventricular morphine. They obtain a long-lasting analgesia (34.2 hours at the beginning of treatment and 27.9 hours at the end; mean follow-up of 65 days) with a very low daily morphine consumption. The results obtained, good and very good in 95% of the cases, could be explained by patients' selection criteria and quality of the follow-up (prospective sheet given to the patients, regular phone calls and consultations in the pain clinic). The authors discuss the indications of this technique in relation with other neurosurgical procedures, taking into account the neurophysiological mechanism of action of morphine.

Insulin-like growth factor 1 receptors in human breast cancer and their relation to estradiol and progesterone receptors.

Insulin-like growth factor 1 (IGF1) binding sites were characterized in breast cancer. We demonstrate the presence of one high affinity binding site. Chemical cross-linking of 125I-IGF1 to breast cancer membranes in reducing condition and sodium dodecyl sulfate-polyacrylamide gel electrophoresis revealed one band with an apparent molecular weight of 130,000. The specificity of the binding was studied. IGF2 was a good competitor whereas insulin competed with a potency lower than 1/100 that of IGF1. This IGF1 binding corresponded to the previously described type 1 IGF receptor (IGF1-R). IGF1-R was determined in 76 human breast cancer biopsies. Ninety-three % of the tumors were positive. The specific binding range was 0-16.4%; the geometric IGF1-R mean level was 3.9%. There was a relation (chi 2 test) between IGF1-R and progesterone receptor positivity rates (P = 0.002). The IGF1-R concentrations were correlated (Spearman test) with those of estradiol receptor (P = 0.0018) and progesterone receptor (P = 0.0011). A positive linear correlation existed between IGF1-R and estradiol receptor (P = 0.006) and between IGF1-R and progesterone receptor (P = 0.003). Our demonstration of the presence of IGF1-R in human breast cancer biopsies suggests that IGF1, acting either via the endocrine, paracrine, or autocrine pathways, could stimulate tumor growth.

Presence and characterization of insulin-like growth factor 1 receptors in human benign breast disease.

Insulin-like growth factor 1 binding sites were characterized in human benign breast disease. We demonstrated the presence of one high affinity binding site. Chemical cross-linking of [125I]IGF1 to benign breast disease membranes in reducing condition and sodium dodecyl sulfate-polyacrylamide gel electrophoresis (SDS-PAGE) revealed one band with an apparent Mr of 130,000. The specificity of the binding was studied: IGF 2 was a good competitor whereas insulin competed for binding with a potency lower than 1/100 that of IGF1. This IGF1 binding corresponded to the previously described type I IGF receptor (IGF1-R). IGF1-R was assayed in 35 cases of benign breast disease and two samples of normal breast tissue. Forty-three per cent of the lesions were IGF1-R positive. The mean geometric level of specific binding was 1.98% in the whole population, it was significantly lower in adenofibromas (1.55%) than in epithelial hyperplasia (2.5%); it was 2% in dystrophic disease. IGF1-R was undetectable in normal tissue. Considering our previous results showing that almost all the breast cancers contained IGF1-R, these data suggest that the increase in IGF1-R could be a marker of malignant tumor development.

Prognostic significance of prolactin receptors in human breast cancer.

Overall survival and relapse free survival (RFS) were studied in 547 patients according to the presence of prolactin receptors (PRL-R), either free or total (after 3 M MgCl2 desaturation). All these patients were surgically treated for locoregional disease in the same institution between 1978 and 1984. In actuarial survival studies, RFS was higher in total PRL-R positive patients in the whole population (P less than 0.02). When the population was divided into two groups, according to either the presence or the absence of node metastasis or the presence or absence of estradiol receptor, the higher RFS was restricted to node positive (P less than 0.001) and to estradiol receptor positive patients (P less than 0.01). The Cox analysis on RFS showed that free PRL-R alone was a significant prognostic factor in estradiol receptor positive patients; total PRL-R alone was never significant; when considered together with steroid receptors, free as well as total PRL-Rs were significant prognostic factors in some subgroups of patients.

Variations in lipoproteins during aminoglutethimide therapy.

Plasma lipids, apolipoproteins, and gamma-glutamyl-transpeptidase (GGT) were measured in 28 patients receiving aminoglutethimide (500 mg) and hydrocortisone (30 or 40 mg) for advanced breast cancer. A rise in cholesterol (CHOL), LDL-CHOL, apoprotein B, apoprotein CIII, and GGT was observed after 45 days. When the patients were divided in two groups according to lipid basal plasma levels, those with high CHOL and triglyceride did not experience any modification of lipid parameters (only GGT were elevated). Conversely normolipidaemic patients experienced an increase in CHOL, triglycerides, LDL-CHOL, apoproteins B and CIII, and GGT.

[Low-dose aminoglutethimide (500 mg/day) and hydrocortisone (30 mg/day) in advanced cancer of breast].

One hundred-twenty-seven advanced breast cancer patients who had been -for most of them- heavily pretreated with chemo- and/or hormone therapy received 500 mg aminoglutethimide and 30 mg hydrocortisone/day. The response rate was 20% (58% when stabilizations were included); the best responses were observed in skin and node metastases. The response rate in bone metastasis was 16%. The general condition of the patient (according to the Karnofsky index) was improved in 50% of the responders (including stabilizations) and in 23% of the non-responders. The survival of patients who were only stabilized was close to the survival of responders. The tolerability of this treatment was very good.

[Carcinomas of the upper aerodigestive tract before age 40 (excluding children). Apropos of 100 cases].

A series of 100 young adults with carcinoma of upper respiratory and digestive tracts; and examined successively, is reviewed. As in older adults, the oropharynx was the most frequent localization with, in contrast, fewer endolaryngeal tumors and more rhinopharyngeal cancers. Progression appeared to be more rapid in young adults and alcohol and smoking a more important factor than in the older age group. Overall prognosis in the older age group. Overall prognosis seemed somewhat better for these younger patients. Epidemiologic surveys would appear to be essential.

[Carcinomas of the upper aerodigestive tract in the female. Apropos of 132 cases].

A series of 132 female patients with carcinoma of upper respiratory and digestive tracts, and examined successively, is reviewed. The mean age of the group was sensibly higher than that of the group of male patients used as reference. Progressive stages of the disease were similar in the female and male groups. The must frequent localization was buccopharyngeal, naso-sinusal and nasopharyngeal lesions being observed more frequently than in men. Alcohol and smoking were perhaps less important factors in women. Overall prognosis was better in the female group. Detailed epidemiologic surveys involving. This population would appear to be essential.

Correlation between prolactin receptors (PRL R), estradiol (ER) and progesterone receptors (PgR) in human breast cancer.

Free (n = 432) and total (n = 387) prolactin receptors (PRL R) (after 3 M MgC12 desaturation) as well as estradiol (ER) and progesterone receptors (PgR) were measured in 547 breast cancer patients surgically treated in the Oscar Lambret Centre. Free PRL-R were found in 43% of the cases, total PRL R in 72%, ER in 81% and PgR in 55%. A statistically significant correlation was found by the Spearman test between ER on the one hand free PRL R (P less than 0.02) and total PRL R (P less than 0.001) on the other and between PgR on the one hand, free PRL-R (P less than 0.05) and total PRL R (P less than 0.01) on the other. A linear correlation test could be done on subgroups of values, excluding zero values, of each of the receptor type; a statistically significant correlation could be found between ER and total PRL R (P less than 0.001) and between PgR and total PRL R (P less than 0.05). These results confirm, on a large series, the relation between PRL R, ER and PgR in breast cancer.

[Cancer of the prostate: therapeutic strategy in 1985].

After extensive staging, localized prostate carcinomas are treated by radical prostatectomy, external beam radiation therapy or interstitial radioactive implantation therapy combined or not with external radiation. For these patients, it is not demonstrated that adjuvant hormonotherapy increases survival duration. For metastatic carcinomas, orchiectomy or low dose of DES are possible. When carcinoma patients escape to endocrine treatment, there are given chemotherapy. Hormonal associations with central and peripheral action mechanisms could modify, in early future, present treatment options for advanced carcinomas.

Platinum concentration in human tumors of head and neck, uterine cervix, and breast following treatment with cisplatin.

Intratumoral platinum concentrations were measured in three tumor sites (head and neck, uterine cervix, and breast) 48 h after cisplatin administration according to the same protocol. The platinum levels were in the same order of magnitude in all tumors, but the concentration in breast tumors was found to be higher than that in tumors of the head and neck and of the uterine cervix.

Estradiol and progesterone receptors in breast cancer: prognostic value after relapse.

The prognostic significance of estradiol (ER) and progesterone receptors (PgR) for survival from relapse has been studied in two groups of breast cancer patients: group 1, 35 patients in whom receptor levels were measured at the time of mastectomy; group II, 49 patients in whom receptor levels were measured at the time of recurrence. ER+ (greater than 10 fmoles/mg) patients had a better survival from relapse than ER- patients. High levels of PgR (greater than 50 fmoles/mg) had a prognostic significance only in group II patients.

[Aminoglutethimide-induced dyslipemia. Clinical study].

Plasma levels of cholesterol, HDL cholesterol (HDL chol), LDL cholesterol (LDL chol), triglycerides, Al apoprotein and B apoprotein were studied in 73 patients receiving 500 mg aminoglutethimide (AG)/day and 40 mg hydrocortisone/day for advanced breast cancer. These dosages were done before treatment and then repeated during AG therapy. When all patients were considered together, a significant increase of cholesterol, HDL chol, LDL chol and Al apoprotein was noted. If one considers two groups of patients: group A (where cholesterol and triglycerides plasma levels were normal before treatment) and group B (where cholesterol and/or triglycerides plasma levels were high before treatment), it appears that variations differ in both groups. In group A patients we found an increase in cholesterol, chol LDL, B apoprotein and to a lesser extent Al apoprotein plasma levels. In group B patients there was an increase in cholesterol, HDL chol and Al apoprotein plasma levels.
