Long term sequelae of hemispheric abscesses as a function of the treatment.

The authors compared the clinical, electroencephalographic and CT evolution of 31 cases of brain hemispheric abscesses. The vital prognosis was largely improved by the introduction of the CT scan, as compared with previous series. The functional evolution was better in the group of patients receiving high dosage (15 cases) antibiotics and/or puncture (4 cases) in the case of a large lesion, than in patients whose abscesses were removed (12 cases). The CT scan confirmed this difference, with frequent atrophy after excision.

Cervical neurological complications of rheumatoid arthritis. Surgical treatment techniques and indications.

A critical study of 11 cases with neurological complications resulting from damage to the cervical spine during rheumatoid arthritis prompts the authors to reconsider their therapeutic strategy. After recalling the various neurological complications and their mechanisms, they propose an extension of the range of indications for surgical treatment. The limits of this extension are defined according to neurological and radiological criteria and the risks inherent in each method.

Developments in the treatment and prognosis of multiple cerebral abscesses.

Multiple cerebral abscesses have been recognized with increasing frequency because of the use of computed tomography and have been the subject of various therapeutic proposals. Their prognosis has been considerably improved thanks to high dose antibiotic therapy. However, diagnostic and therapeutic problems persist.

[Computerized tomography of the distribution of N-isopropyl-p-123 iodoamphetamine in cerebrovascular pathology. Comparison with studies using the 133-xenon inhalation method].

123Iodo-Amphetamine (IAMP) brain distribution was studied with a specially dedicated multilevel tomographic system in 30 patients with vascular ischemic (18) or hemorrhagic (12) disorders. Results were compared with clinical data, CT scan and rCBF measurement obtained by the 133Xe continuous inhalation method. Abnormalities with IAMP were found in 27 cases, in good agreement with the results obtained by the Xenon inhalation technique in 21 studies from the 22 where that method was possible. For 80% cases there was also a good agreement between the results obtained by the IAMP and CT scan: one case normal, and 23 observations with abnormalities. This is true for the localization of lesions, but IAMP appears able to shown remote abnormalities, i.e. crossed cerebellar diaschisis or functional deactivation, in the absence of CT scan lesions in the corresponding areas. This is of particular interest in the protracted regressive ischemic neurologic disorders (PRIND): CT scan were normal in all five observations but IAMP showed abnormalities in four cases. IAMP is however very expensive, so it has to be used only when 133Xe inhalation is technically impossible.

[Technics and indications for surgical treatment in surgical neurologic complications of rheumatoid polyarthritis].

A critical study of 11 cases of rheumatoid arthritis with neurological complications due to involvement of the cervical region of the spinal column induces the authors to reconsider their therapeutic strategy. After reviewing the various neurological complications and mechanisms involved, they propose wider use of surgical treatment within limits defined on the basis of neurological and radiological criteria and the risks of the respective methods.
