Cytokeratin expression in preneoplastic lesions and early squamous cell carcinoma of the bronchi.

Cytokeratins are expressed in varying combination in the course of differentiation of epithelial cells and tumor cells. This is the first report of systematic immunohistochemical (ABC-method) investigations concerning the expression of different cytokeratin types (KL 1, CK 4, 10 and 17) within the transformation of the bronchial mucosal epithelium with epithelial hyperplasia to potential preneoplasia and early squamous cell carcinoma of the lung. In bioptical obtained specimens from 35 patients, 63 areas with diagnosis of regular bronchial mucosa (n = 10), basal cell hyperplasia (n = 9), goblet cell hyperplasia (n = 10), squamous cell metaplasia (n = 11), dysplasia I-III (n = 13), carcinoma in situ in the border zone of squamous cell carcinoma (n = 10) and in 5 surgically obtained specimens with findings of an early squamous cell carcinoma of the bronchus (n = 5), the expression of different cytokeratin types was investigated. The specimens were fixed in formalin and embedded in paraffin for lightmicroscopical and immunohistochemical investigations (ABC-method). KL 1 was detected in all regular and pathological mucosal epithelia, CK 4 in basal cells, squamous cell metaplasia, potentially preneoplastic changes and early squamous cell carcinoma of the bronchus. CK 10 was expressed only in early squamous cell carcinoma of the bronchus, and CK 17 in varying quantity in hyperplastic, preneoplastic and cancerous lesions of the bronchial mucosa. Our results reveal an increasing expression of squamous epithelial type-cytokeratins (CK 4, CK 17) in hyperplastic and metaplastic lesions of the bronchial mucosa. In dysplastic lesions and carcinomata in situ, a quantitative, as well as qualitative decrease of the CK 4- and CK 17-expression with a heterogeneous expression pattern for CK 17 was found. The heterogeneous neo-expression of CK 10 in early squamous cell carcinoma of the bronchus has to be emphasized.

Basement membrane in middle ear cholesteatoma. Immunohistochemical and ultrastructural observations.

We investigated the distribution of basement membrane zone (BMZ) components collagen type IV, collagen type VII, and fibronectin in human middle ear cholesteatoma, auditory meatal skin, and middle ear mucosa using both immunohistochemical and ultrastructural methods. Collagen type IV immunoreactivity of skin and middle ear mucosa is continuous in the BMZ, whereas cholesteatoma frequently showed absent immunoreactivity or focal discontinuities. Collagen type VII immunoreactivity is detected similarly within the BMZ of cholesteatoma and skin. Fibronectin immunoreactivity is observed within the dermoepithelial junction of skin and middle ear mucosa. In cholesteatoma, however, fibronectin immunoreactivity is markedly increased within the extrinsic BMZ and the subepithelial connective tissue. The ultrastructural arrangement of the BMZ of cholesteatoma is like that of skin; however, it exhibits distinct alterations of the lamina fibroreticularis and lamina densa. Our results outline cholesteatoma as a disease with disturbed cell matrix interactions analogous to those of wound reepithelialization.

Expression of a cell-cycle-associated nuclear antigen (MIB 1) in cholesteatoma and auditory meatal skin.

Middle ear cholesteatoma is often invasive with consequent bone destruction. Inflammatory stimulation of the underlying connective tissue, as well as an autocrine mechanism, may be responsible for the dysregulation and abnormal proliferative features of the keratinocytes in cholesteatoma. Comparative investigations were performed to assess the epithelial cell kinetics of cholesteatoma and normal auditory meatal skin. Monoclonal antibody MIB 1 immunostaining (which recognizes a nuclear antigen expressed by dividing cells) was applied using the alkaline phosphatase antialkaline phosphatase immunolabeling method. Specimens of normal auditory meatal skin (n = 7) revealed an average MIB 1 score (quotient of the MIB 1-positive cells and the total number of cells) of 7.6 +/- 2.2%. Cholesteatoma samples (n = 13) showed an average MIB 1 score of 17.4 +/- 8.9% and a heterogeneity of proliferating epithelial areas. Epithelial cones growing toward the underlying stroma exhibited high mitotic activity. Statistically, the results of this study confirm a highly significant increase in the proliferation rate of cholesteatoma keratinocytes, which had an MIB 1 score that was 2.3 times higher than the score for keratinocytes of normal external auditory meatal skin.

[The pathology of the pulmonary arteries in lung tumors].

A study was undertaken to analyse the type and extent of pathological changes in the pulmonary arteries in non-small cell malignant tumours of the lung. Large-section histological preparations were made from 33 squamous cell carcinomas and 30 adenocarcinomas (T1 and T2 tumours) and classified according to tumour margin area (zone 1), intermediate area (zone 2) and tumour centre (zone 3). Transmural tumour growth with intraluminal cell formations in the pulmonary artery branches were found in the centre of all adenocarcinomas and 86% of squamous cell carcinomas, involving subsegment, prelobular and lobular arteries. Obstructive and obliterative changes in the pulmonary arteries as the result of tumour compression and secondary fibrosing changes predominantly occurred in the centre of all tumours. They were less common and less marked in zones 1 and 2. -Pulmonary artery branches in lung tumours of stages T1 and T2 showed marked infiltrating, obliterative and secondary inflammatory changes as far as complete vascular occlusions. These observations indicate that cytotoxic drugs, introduced via the systemic circulation, cannot reach and therefore not exert their effects in extensive areas of tumour.

[Differential diagnosis of synovitis. Correlation of arthroscopic-biopsy to clinical findings].

Histological examination of 1660 biopsies (2-5 mm) of the synovial membrane of 317 patients were obtained by arthroscopic dissection; in 70% of all cases non-specific changes were revealed and in 30% specific changes. The findings included different stages of proliferation of the superficial layer, uncharacteristic inflammatory reactions of the stratum synoviale and fibrosis of the stroma, vascular proliferation and pseudopolypoid thickening of the synovial membrane. The histomorphological findings can only be interpreted reliably and a pathogenetic classification obtained, e.g., detritus synovialitis, rheumatoid arthritis or post-traumatic changes, if the arthroscopic findings conclude an exact documentation of the biopsy site and the clinical findings.

Lendrum (-MSB) staining for fibrin identification in sealed skin grafts.

The significance and effect of fibrin sealant systems for woundhealing are still unknown, because of the use of insufficient, conventional staining methods for the demonstration of the fibrin components used by sealant systems. From 21 patients with extensive burns of 2nd and 3rd degree biopsies of the skin were obtained during consecutive operations to cover the defect of the skin with split-thickness skin grafting. In the present paper morphological results concerning the demonstration of fibrin components and morphological differences in woundhealing of sealed and unsealed skin grafts are presented using Lendrum (-MSB) staining. With this staining method it is possible to identify exogenous fibrin components of the sealant system and to differentiate between fresh and older fibrin components, due to colour changes depending on time.

AgNOR counts in preneoplastic lesions of the bronchus.

Silver staining of argyrophil nucleolar organizer regions (AgNORs) has become a valuable method to assess cell kinetics in paraffin sections and separate benign from malignant lesions. As a putative technique for the distinction between regenerative and preneoplastic alterations of the bronchial mucosa we employed the Ag-staining method, evaluating the number of dots/nucleus and shape of the dots. Dots/nucleus were counted by by standardized methods, and mean and standard deviation were calculated. Comparative investigations were performed in bioptically obtained specimens of normal bronchial epithelium (1.94 +/- 0.1 dots/nucleus, n = 12), basal cell hyperplasia (2.22 +/- 0.13 dots/nucleus, n = 15), goblet cell hyperplasia (2.38 +/- 0.16 dots/nucleus, n = 17), squamous cell metaplasia (2.58 +/- 0.17 dots/nucleus, n = 25), squamous cell metaplasia with mild and moderate dysplasia (2.50 +/- 0.5 dots/nucleus, n = 15), squamous cell metaplasia with severe dysplasia and carcinoma in situ (3.10 +/- 0.47 dots/nucleus, n = 17), micropapillomatosis (2.20 +/- 0.21 dots/nucleus, n = 13) and early squamous cell carcinoma of the bronchus (4.40 +/- 0.74, n = 13). In preneoplastic lesions a variability in form and size of the dots was found. Though showing an increase in the number of dots/nucleus coincidentally with the degree of proliferative activity, the AgNOR method cannot be considered as a statistically significant tool in the evaluation of regenerative and preneoplastic lesions in bioptically obtained specimens.

Functional characterization of middle ear mucosa residues in cholesteatoma samples.

Cholesteatoma epithelium is characterized by a keratinocyte dysregulation with an aggressive growth that leads to the destruction of normal middle ear mucosa. The abnormal behavior of cholesteatoma epithelium seems to be induced by the presence of a heavy immune cell infiltrate releasing different cytokines and growth factors in high amounts. Middle ear mucosa rests are often observed within the cholesteatoma stroma or adjacent to the advancing front of cholesteatoma epithelium. This study investigated the presence of interleukin-1 (IL-1), transforming growth factor-alpha (TGF-alpha), epidermal growth factor (EGF), and epidermal growth factor-receptor (EGF-R) in the mucosa rests as well as the expression of an activation marker, 4F2. The findings were correlated with the features of a surrounding stroma with an enhanced immune cell infiltrate. Cholesteatoma epithelium showed a high staining intensity of IL-1, TGF-alpha, and EGF-R. In contrast to this, middle ear mucosa did not show any positive reactions for the mentioned factors. Epidermal growth factor immunoreactivity was found in neither cholesteatoma epithelium nor in middle ear mucosa residues. The authors found a high concentration of lymphocytes and macrophages in the surrounding stroma. Most of these cells expressed TGF-alpha, IL-1, and 4F2, suggesting an activated form. Results indicate that keratinocytes present in the middle ear mucosa do not appear to react to the stimuli released by the inflamed stroma, reflecting important differences in the cell biological features of the keratinocytes that form parts of both types of epithelium.

[Anti-human collagenase type IV expression in preneoplastic lesions and early squamous cell lung carcinoma].

Proteolytic enzymes like collagenase type IV are relevant to the destruction of matrix-components within the scope of invasive tumor growth. Preneoplastic lesions of the bronchial mucosa and early squamous cell carcinomas were investigated immunohistochemically using anti-human-collagenase type IV monoclonal antibodies due to the question of enzymatic proteolysis of basement membrane structures. With increasing malignant epithelial transformation an increase of collagenase type IV expression was observed in single atypical cells of severe dysplasia, carcinoma in situ and early cancer of the bronchus. Collagenase type IV expression can be evaluated as enhanced proteolytic potency for degradation of matrix structures of the basement membrane as one factor for early infiltration in preneoplastic lesions up to early squamous cell carcinomas of the lung.

Arthroscopy and chondrocalcinosis.

Different pathogenetic factors have been associated with the development of chondrocalcinosis. Under normal conditions, previous degenerative meniscus changes with proliferation of "pseudochondrocytes" and chondrocalcinosis have been found coincidentally. Small fragments can be excised during arthroscopy for histological assessment of the developmental stages and localization of chondrocalcinosis. We investigated 3,350 specimens histologically and correlated the results with clinical data and data on previous trauma to establish the role of surgery or trauma in the induction or promotion of chondrocalcinosis. The findings allow us to draw the conclusion that contrary to previous study findings, chondrocalcinosis is a highly significant factor in posttraumatic and/or degenerative meniscal changes.

Localization of elastase and tumor necrosis factor alpha mRNA by non-radioactive in situ hybridization in cultures of alveolar macrophages.

Digoxigenin is a new tool for labeling probes which can be detected with the help of specific antibodies in the cell by indirect or direct immunostaining. In contrast to the biotin-reaction, the advantage of digoxigenin is that it does not appear in animal or human cells in nature. In comparison to radioactive labeling methods it is favorable in terms of short exposure time and precise localization of signals in the cell. In this paper we describe the localization of elastase and tumor necrosis factor alpha (TNF alpha) mRNA by non-radioactive in situ hybridization of rat alveolar macrophages in cell culture after stimulation with welder steam dusts. Using digoxigenin labeled probes the determination of specific mRNA's expression and their precise localization in the cytoplasm of the cell could be achieved within one day.

[Small cell bronchial cancer--pathologic anatomy].

Examinations of 635 biopsies and 163 surgical specimens resulted in a morphological description of characteristics of small cell cancer of the lung (cell diameter 9 microns, medial diameter of nuclei 7 microns). Histological diagnosis reflects only a rough phenotypic parameter of this tumor entity. At thorough examination of small cell cancer squamous, adenoid and/or giant cell differentiation can be found within the same tumor tissue in about 20% of specimens. Histological examination of spontaneous and therapeutical induced tumor regression in resected specimens yielded findings such as necrosis, scar development and endangitic blood vessel transformations. Results of electron microscopy, immunohistochemistry and in situ hybridisation also underline a wide range of heterogeneity in the special group of small cell cancer of the lung. The documentation of a broad spectrum of morphological findings in specimens from resected tumors demonstrates that a 2-mm biopsy represents only a rough parameter of a biologically complex tumor.

Extracellular matrix in preneoplastic lesions and early cancer of the lung.

Characterization of preneoplastic lesions mainly concentrated on cellular, nuclear and epithelial atypias. The extracellular matrix was almost neglected, although malignant tumour cells interact with extracellular matrix molecules during tumour invasion. Until now systematic investigations of extracellular matrix components in preneoplastic lesions and early stages of lung cancer are lacking. 150 preneoplastic lesions, 10 specimens of early cancer and 30 specimens of normal bronchial mucosa were examined by means of immunofluorescence microscopy. The distribution of collagen type I and III and the non-collagenous glycoproteins laminin and fibronectin have been investigated by an indirect immunohistochemical method. With an increasing degree of preneoplasia an increased matrix disarrangement of the basement membrane zone could be observed. In severe dysplasia and carcinoma in situ neosynthesis of collagen type III and especially laminin in close association to neoangiogenesis could be demonstrated besides a disintegration of extracellular matrix components. In early lung cancer numerous laminin positive basement membrane like structures are situated around tumour cells. An enhanced deposition of collagen type I and III fibres could be demonstrated around tumour cell islets. The results indicate a partial loss of function in preneoplastic basal cells in cases of dysplasia and carcinoma in situ. In early stages of invasive squamous cell carcinoma of the bronchus, extracellular matrix components obviously could be produced by tumour cells.

[Dust deposits in lung tumors--cause and significance].

In the diagnosis of a so-called scar carcinoma, one of the accepted morphological characteristics is the deposition of dust in tumours of the lung. In 50 bronchial carcinomas in former miners and patients not occupationally exposed to dust, the cause and significance of such lesions were investigated in autopsy and surgical material. Impregnation with dust must be considered to be a secondary phenomenon of the infiltrative tumour growth. Such depositions depend upon previously present dust depositions in the pulmonary parenchyma and on the localisation of the tumour, and reveal a clear correlation with the histological type and degree of differentiation of the tumour.

[Mucoviscidosis: late diagnosis despite typical clinical features].

An 18-year-old girl was hospitalized because of a treatment-refractory broncho-pneumonia requiring artificial ventilation. She had been suffering from recurrent respiratory infection since the age of three years. Chest X-ray films demonstrated bronchiectasis: Pseudomonas, Klebsiella and Candida albicans were isolated from the respiratory tract. The correct diagnosis of the underlying disease had not previously been made. A sweat test, when aged 15 years, was borderline abnormal. Despite the application of all available therapeutic measures she died in cardiorespiratory failure from an overwhelming pulmonary infection. The typical clinical features and the findings at autopsy established the diagnosis of mucoviscidosis.
