Re-evaluation of capillaroscopy of finger nailfold in vibration-exposed workers.

Two hundred and ninety-four vibration-exposed workers were examined for vibration disease (VD) for the worker's accident compensation. Most subjects were male. Their ages ranged from 24 to 68 years. Duration of vibration exposure ranged from 3 to 30 years. After acclimatization at 25 degrees C to 27 degrees C for at least an hour, the nailfold capillaries of the ring (or middle) finger were studied with a light microscope. A cold water immersion test (5 degrees C, 10 min) was then performed. The morphological findings of the nailfold capillaries were classified based on the criteria by Fagrell & Lundberg. A hundred and eighty subjects were diagnosed as VD and 75 as non VD, even without taking the capillaroscopic findings into consideration. The classification of the capillarograms retrospectively investigated were as follows, i.e. in 98 VD subjects 40 cases were stage A, 42 stage B, and 16 cases stage C. In 26 nonVD subjects 13 cases were stage A, 10 stage B, and 3 cases stage C, respectively. When the borderline of VD and nonVD existed between stage A and stages B and C, the sensitivity was 59.2%, specificity was 50.0%, and false positive and negative were 50.0% and 40.8%. Relationship between the capillary stage and recovery (%) of the skin temperature during the cold water immersion test was also studied. The recovery in stage C was not always lower than that in stage A. Among them, arteriography was performed in 57 cases. Despite the use of vasodilator, markedly delayed perfusion of the finger tips was observed in 28 cases. Their capillary stages were A in 12 cases, B in 11 and C in 3. The peripheral microcirculatory state in VD should be carefully evaluated in combination with appropriately selected examinations, including finger nailfold capillaroscopy.

Vestibular training for benign paroxysmal positional vertigo. Its efficacy in comparison with antivertigo drugs.

A modified controlled study on treatment effects of vestibular training (VT) for benign paroxysmal positional vertigo was performed. The VT was compared with courses of patients in three different groups: patients treated by medication, by VT, and by VT with medication during 8 weeks. It was statistically confirmed by global judgments of symptoms and signs that improvement rates of positional nystagmus and vertigo were higher in the two groups treated by VT with and without medication than the improvement rate in the medication alone group. In the groups treated by VT, the effects were not influenced by time since onset of disease or by patient age. It is therefore assumed that VT can be used as a first-choice treatment in patients with benign paroxysmal positional vertigo, even in long-term cases or older patients.

Effects of inhaled ceramic fibres on macrophage function of rat lungs.

To evaluate the biological effect of ceramic fibres on the clearance function of alveolar macrophages (AMs) morphological changes and phagocytic activity of AMs were assessed. Rats were exposed to respirable ceramic fibres with a mass median aerodynamic diameter of 4.4 microns and a concentration of 20.1 mg/m3 in an exposure chamber. They were killed after one week (group A) and two weeks (group B) of exposure, and four weeks (group C) and 12 weeks (group D) after exposure for two weeks. The AMs recovered by bronchoalveolar lavage (BAL) from each test group were incubated with yeast and phagocytic activity was determined by counting the number of yeast cells in AMs. Morphological features of AMs were assessed by scanning electron microscopy and quantified according to morphological changes. Total cell counts in BAL fluid from exposed rats in group A were higher than in control rats. Phagocytic activity of exposed AMs in group B and C exceeded that of control AMs. Morphological changes of the exposed AMs in groups A, B, and C were greater than those of control AMs. These findings suggest that ceramic fibres induced the phagocytic activity and morphological changes in AMs, and that the clearance function of AMs was stimulated by the inhaled ceramic fibres.

[Experimental studies for immunoscintigraphy of adenocarcinoma of the prostate with 125I-labeled monoclonal antibody to gamma-seminoprotein].

We established LNCaP tumor in male nude mice after inoculation of LNCaP cells. The tumor produces the organ-specific glycoproteins in the human prostate, gamma-seminoprotein (gamma-Sm), which has been verified by immunohistochemistry and enzyme immuno assay. The LNCaP tumor is composed of 2 distinct portions, namely GEL/FLUID portion and SOLID portion. The GEL/FLUID portion contains much more gamma-Sm than the SOLID portion does. The monoclonal antibody against gamma-Sm (murine IgG1, K) was radio-labeled with iodine-125 by lactoperoxidase method followed by passage over column chromatography. Finally the conjugates with specific activity of 49.8-56.6 KBq/micrograms were obtained. Biodistribution of 125I-labeled monoclonal antibody to gamma-Sm (125I-gamma MAb) was evaluated by whole-body autoradiography (ARG) and by determination of label uptake (DLU) by the LNCaP tumor using the well type gamma counter. The ARG were obtained 1-9 days after intravenous administrations of 32-38 micrograms of 125I-gamma MAb to 12 nude mice bearing LNCaP tumor. The DLU were evaluated 3, 4 and 7 days after intravenous administrations of 18-25 micrograms of 125I-gamma MAb to 9 nude mice bearing LNCaP tumor. The DLU by organs (GEL/FLUID portion, SOLID portion, liver, kidney, spleen, lung, etc.) were expressed as F-values (%) which were calculated by the following formula: F = radioactivity per gram of tissue (dpm/g) divided by injected dose per gram of animal (dpm/g) x 100 and tissue-to-blood ratios (T/B). The specific uptake by the LNCaP tumors, especially in the GEL/FLUID portion, was observed on ARG, with optimal images of the tumor evaluated 4-7 days after administrations. The F-values and T/B of the LNCaP tumors were always higher than those of other organs. Especially T/B of GEL/FLUID portion were of 6.7 and 6.3, evaluated 4 and 7 days after administration of 125I-gamma MAb respectively. These results suggest that the radio-labeled gamma MAb is applicable for immunoscintigraphy of adenocarcinoma of the prostate.

[Clinical feature and quality of life of urinary diversion patients with bladder cancer].

A clinical survey was performed on 95 cases who were treated by total cystectomy with urinary diversion at our hospital between August 1971 and March 1992. Urinary diversions were classified by ureterocutaneostomy (Group I) in 27 patients, ileal or colon conduit (Group II) in 45 and continent urinary reservoir (Group III) in 23. The patients ranged in age from 29 to 76 with an average age of 60.6 year and the male to female ratio was 68 to 27. Histopathological typing of bladder cancers showed 82 cases of transitional cell carcinoma, 8 cases of squamous cell carcinoma, 4 cases of adenocarcinoma, and 1 mixed carcinoma with transitional cell carcinoma, squamous cell carcinoma and adenocarcinoma. At present, patients alive of Group I, II and III were 14, 27 and 16, respectively, totally 57 (60.0%) patients were alive. Five-year actuarial survival rates of Group I, II and III were 45.2%, 54.1%, and 56.8%, respectively. A questionnaire survey was carried out to assess the quality of life (QOL) of 57 alive patients. The analysis of replied questionnaire showed that even all patients of each groups adapted to the basic life activities after urinary diversion, though a step forward quality of life with hobby, travel and job were decreased postoperatively in 3 groups. We obtained results that the patients of Group III were more satisfactory with daily (bath) and social life (job) than the patients of Group I, II and preoperatively. We have a new comprehension of importance of enough informed consent and mental support to the urinary diversion patients preoperatively, and of a new operative procedure not to impair the QOL.

Determination of blood and urine manganese (Mn) concentrations and the application of static sensography as the indices of Mn-exposure among Mn-refinery workers.

Early detection of neurophysiological abnormalities is believed to be most effective for the early diagnosis of chronic manganism. The static sensography was applied during the periodical health examination and the significance of total distance of the postural sway (i. e. postural sway index) as a neurophysiological index was studied in relation to the blood and urine manganese concentrations. Sixty-six workers in a manganese (Mn)-refining factory, aged 29-59 (mean 47) years, were examined from 1984 to 1989. Mn-exposed workers had been engaged in alternating shifts and the mean duration of Mn exposure was 22.6 years before the health examination in 1984. Air-borne dust and Mn levels in the working environment were 0.07-2.74 mg/m3 and 0.02-0.46 mg/m3. The mean values of the parameters in Mn-exposed workers fluctuated as follows: blood Mn concentrations 19.1-26.9 micrograms/l [control; Mean 17.8 (Standard deviation 5.2) micrograms/l], urine Mn concentration 2.60-4.22 micrograms/g Creatinine [control; Geometric mean 1.16 (Geometric standard deviation 1.93) micrograms/g Creat.] and postural sway index 51.4-94.6 cm/30 sec [control; Geometric mean 59.7 (Geometric standard deviation 1.4) cm/30 sec.]. Although there were no significant correlations between the postural sway index and blood and urine manganese concentrations, the usefulness of this kind of simple neurophysiological test should be further investigated in combination with other established examinations.

A case of cavernous hemangioma in the internal auditory canal.

A case of cavernous hemangioma in the internal auditory canal was reported. The chronology of this case was similar as one of Ménière's disease. The neurotological findings in this case were no specific findings to differentiate cavernous hemangioma from acoustic neurinoma. The high-resolution computed tomography (HRCT) revealed calcium stippling in the lesions, whereas magnetic resonance imaging (MRI) demonstrated a low signal intensity on both T1- and T2-weighted images. Comparison of HRCT and MRI findings is most useful for differential diagnosis of intracanalicular tumors.

Synergistic effects of mineral fibres and cigarette smoke on the production of tumour necrosis factor by alveolar macrophages of rats.

The objective of this study was to evaluate the combined effects of mineral fibres and cigarette smoke on the production of tumour necrosis factor (TNF) by alveolar macrophages. Rats were exposed to cigarette smoke in vivo, and production of TNF by alveolar macrophages was measured in the presence of mineral fibres in vitro. For smoke exposure, rats were divided into two groups. Five were exposed to a daily concentration of 10 mg/m3 of cigarette smoke for an eight hour period, and five rats (controls) were not exposed to smoke. Bronchoalveolar lavage was performed after exposure to smoke and the recovered alveolar macrophages were incubated with either chrysotile or ceramic fibres on a microplate for 24 hours. Activity of TNF in the supernatant was determined by the L-929 fibroblast cell bioassay. When alveolar macrophages were not stimulated by mineral fibres, production of TNF by rats exposed to smoke and unexposed rats was essentially the same. When alveolar macrophages were stimulated in vitro by chrysotile or ceramic fibres, production of TNF by alveolar macrophages from rats exposed to smoke was higher than that by alveolar macrophages from unexposed rats. The findings suggest that cigarette smoke and mineral fibres have a synergistic effect on TNF production by alveolar macrophages.

Reduction by niceritrol treatment of serum lipoprotein(a) in normolipidemic patients with coronary artery disease.

The effect of niceritrol at an ordinary dose (1500 mg/day) on the serum lipoprotein(a) (Lp(a)) concentration was investigated in 25 normolipidemic patients with coronary artery disease. The serum Lp(a) level was reduced by approximately 21% after 3 months of treatment (before treatment, 30.3 +/- 4.1 mg/100 ml; during treatment, 22.6 +/- 2.4 mg/100 ml; p < 0.01). By one month after the drug was discontinued, the Lp(a) returned to a pretreatment level (30.8 +/- 2.8 mg/100 ml). The levels of LDL-cholesterol and apoB were decreased significantly by the drug therapy; LDL-cholesterol increased closely to a pretreatment level after withdrawal of the drug. The percentage reduction of Lp(a) was significantly correlated with that of fibrinogen (r = 0.763, p < 0.01). Plasma concentration of fibrinogen was decreased in the patients whose reduction rate of Lp(a) was 30% or more. These results indicate that niceritrol has Lp(a)-lowering effect; sufficient reduction of Lp(a) may improve dyscoagulopathy of patients with coronary artery disease.

Prolonged dysequilibrium in three cases with vestibular neuronitis: efficacy of vestibular rehabilitation.

The clinical course of acute unilateral vestibular disturbance in 3 cases with vestibular neuronitis and a case with labyrinthine destruction was surveyed for a long-term period. Vestibular training with a modified Cawthorne-Cooksey's exercise was done in 2 of 3 cases with vestibular neuronitis and a case of labyrinthine destruction. All had no severe troubles in activities of their daily lives over one year after the onset of the disease, but the central compensation was not completed, even in a young patient, in a case of labyrinthine destruction and in a well trained case. Spontaneous nystagmus to the affected side and increase of body sway with eyes closed were revealed by examinations in all cases. Physical exercise was found to be effective for relief from vestibular ataxia, but it was difficult to keep the effect even after training.

[An experience with a continent appendix stoma].

The submucosally embedded in situ appendix guarantees an ideal continence mechanism in patients with ileocecal urinary reservoirs. To date this modification of the Mainz pouch technique was performed successfully in 10 patients between September 1990 and October 1992. Pouch capacity ranged from 350 ml to 900 ml (mean: 565 ml), frequency of intermittent self-catheterization ranged from 3 to 7 times per day (mean: 4.4-5.4 times) and complete urinary continence without difficulty of self-catheterization was obtained in eight patients. Two patients with stomal stenosis needed to indwell a catheter. The follow up periods ranged from 3 to 24 months (mean: 14.7 months). The main advantages of appendix stoma technique are shorter operation time, a short ileal segment, no risk of nipple gliding or prolapse, no need for staples, thereby decreasing significantly the risk of stone formation, and facilitating easy catheterization. This method seemed to be valuable for continent urinary reservoir assuring patients of high quality of life.

[Immunohistochemical study of the prostatic tissues with monoclonal antibody against gamma-seminoprotein. Analyses of benign prostatic hyperplasia, metastatic foci from adenocarcinoma of the prostate and malignant neoplasms other than adenocarcinoma of the prostate].

The immunohistochemical specificity of the monoclonal antibody (43-21-1-1) against gamma-seminoprotein (gamma-Sm) in the prostatic tissues was evaluated by avidin-biotin peroxidase complex method. The normal tissues of various organs, other than male genitourinary organs already examined in the previous study, the brain, skin, spinal cord, tongue, esophagus, stomach, small intestine, rectum, trachea, lung, pleura, diaphragm, liver, spleen, pancreas, mesentery, kidney, lymph node, bone, bone marrow and striated muscle were examined for control study. The prostatic tissues obtained by surgery or biopsy, benign prostatic hyperplasia (BPH; 72) urethral polyp with prostatic-type epithelium (1), bone (3) or testicular (2) metastases from adenocarcinoma of the prostate and malignant neoplasms other than adenocarcinoma of the prostate, including primary small-cell carcinoma (1), secondary embryonal carcinoma (1) and secondary mucinous adenocarcinoma (1) of the prostate were then examined. Since all but 2 (97%) specimens of BPH were obtained by transurethral resection (TUR), which might cause non-specific staining due to electro-mechanical effects, the tissue of BPH obtained by suprapubic prostatectomy were examined simultaneously, serving as a control for immunostaining. The normal tissues of various organs were never stained positively for gamma-Sm. Positive reactions of gamma Sm with this monoclonal antibody were recognized in the cytoplasms of urethral polyp with prostatic-type epithelium, bone or testicular metastases from adenocarcinoma of the prostate. The malignant neoplasms other than adenocarcinoma of the prostate examined in this study, were never stained positively. There was obviously evidence of non-specific staining in the TUR tissues of BPH.(ABSTRACT TRUNCATED AT 250 WORDS)

[Immunohistochemical study of monoclonal antibody against gamma-seminoprotein in the prostatic tissues. A significant correlation between the degree of staining and the histological grade of adenocarcinoma of the prostate].

The tissues consisted of adenocarcinoma of the prostate were examined by immunohistochemical study with monoclonal antibody (43-21-1-1) against gamma-seminoprotein (gamma Sm)2). The degree of staining regarding any correlation with the histological grade was evaluated. The prostatic tissues were obtained by transurethral resection or by fine needle biopsy from untreated 38 patients. The avidin-biotin peroxidase complex technique was used to stain on 3 microns-sections of 10% formalin fixed, paraffin embedded tissue. In addition, immunohistochemical staining with the commercialized antibodies to prostate-specific antigen (PSA; polyclonal, DAKO) and to prostatic acid phosphatase (PAP; polyclonal, DAKO) was done simultaneously for a comparative study. The degree of immunoperoxidase stain was classified into two categories, namely location and pattern, and was graded from 0 to 3, respectively. The product of the degree of location and the degree of pattern was noted as the total score. The mean of score was calculated in each histological grade. Then the means of total scores were compared and evaluated as having any statistical difference by Student's t test among 3 histological grades as well as among 3 primary antibodies used in this study. When the monoclonal antibody to gamma-Sm was used for immunoperoxidase staining, the means of total scores and the rates of negative reactions (% Negative) in 3 histological grades were 6.8 +/- 1.8 (M +/- SD) and 0% in well (N = 9), 4.4 +/- 2.4 and 14% in moderately (N = 22), and 1.8 +/- 2.3 and 54% in poorly differentiated lesions (N = 11), respectively. [Clinical analysis in 2266 cases of transurethral resection of the prostate].

During the past 20 years from August 1971 to July 1991, 3215 cases of transurethral resection were performed at Kitasato University Hospital, and consisted of 2008 benign prostatic hyperplasias, 692 bladder tumors, 258 prostate cancers, 167 bladder neck contractures, 38 urethral strictures, 20 chronic prostatities and 32 various urologic diseases. On 2266 transurethral resections of the prostate including 2008 benign prostatic hyperplasias, and 258 prostate cancers analysis was conducted. Patient age ranged from 44 to 97 (mean, 70.1). Resected tissue varied from 1 to 177 (mean, 26.9) grams. The time required for resection was from 9 to 245 (mean, 73.0) minutes. The volume of irrigation fluid was from 4 to 92 (mean, 25.0) liters. The duration of catheter placement after operation was 3 to 44 (mean, 4.1) days and hospitalization, 10 to 81 (mean, 12.1) days. The number of complications and blood transfusions were 308 (13.6%) and 381 (16.8%) respectively. Resected amount of tissue, volume of irrigation fluid, duration of catheter placement and frequency of complication in the benign prostatic hyperplasia group exceeded those in prostate cancer group. The length of hospitalization and blood transfusion rates were higher in the prostate cancer group. The rate or frequency of each item increased with the operating time and amount of resected tissue. Today, 60 minutes of operating time as a limiting factor of transurethral resection is not considered a important factor. Based on the results of the 2266 TURP cases, an attempt will be made to establish safer methods of treatment.

[Pre-operative, operative and postoperative complications in 2266 cases of transurethral resection of the prostate].

Pre-operative and operative complications in 2266 patients having undergone transurethral resection of prostate (TURP) for the past 20 years at Kitasato University Hospital were analyzed. They consisted of 2008 benign prostatic hyperplasia and 258 prostate cancer patients. Seven hundred and fifty four patients showed some of physical disorders prior to TUR:hypertension in 147 cases, diabetes mallitus in 87, ischemic heart disease in 46, chronic obstructive lung disease in 41 and others. Operative and postoperative complications of TURP were seen in 308 cases (13.6%). Perforation of the prostatic capsule was seen in 100 cases (4.4%) and bladder perforation into intraperitoneal cavity in 6 cases (0.3%). Transurethral fulgulation for postoperative hemorrhage was conducted on 79 cases (3.5%). Hyponatremia lower than 130 mEq/L was noted in 14 cases (0.6%). Severe urinary tract infection leading to bacteremia was observed in 9 cases (0.4%). Postoperative epididymitis was evident in 20 cases (0.8%). There was postoperative urinary incontinence in 19 cases, 3 of which was treated with Teflon-paste injection successfully. One patient had to undergo AMS-800 artificial sphincter implantation. The number of postoperative urethral stricture patients requiring urethral dilatation or internal urethrotomy was 12 (0.5%) and postoperative bladder neck contracture was seen in 20 cases (0.9%). One patient (0.04%) who developed DIC after profuse postoperative hemorrhage died on the 37th postoperative day. The efficiency of TURP depends not so much on the skill of cutting as on the speed and accuracy of orientation and haemostasis. The quick recognition of anatomical landmarks will assure effective and safe resection.

[Biodistribution of 125I labeled monoclonal antibody against gamma seminoprotein in the nude mice bearing human benign prostatic hyperplasia xenografts].

The biodistribution of 125I labeled monoclonal antibody against gamma seminoprotein (gamma-Sm) in the nude mice bearing human benign prostatic hyperplasia xenograft was evaluated by whole body autoradiography and by counting of radioactivity in organs. The monoclonal antibody (murine Ig G1, K) to gamma-Sm which was established in this institute and its F(ab')2 fragment were radioiodinated using Iodogen method. The autoradiograms demonstrated specific uptake of 125I-intact Ig G as well as 125I-F(ab')2 within the prostatic adenoma xenografts 4 days after intravenous administrations. Radioactivity in the xenograft was relatively higher than those in the liver, kidney or lung. These results suggest that radio-labeled monoclonal antibody against gamma-Sm might be applicable for radioimmunodetection of prostatic tumors which produce gamma-Sm.

Biological monitoring of workers exposed to acetone in acetate fibre plants.

Concentrations of acetone in urine, alveolar air, and blood were measured by gas chromatography with flame ionisation detection for 110 subjects occupationally exposed to acetone (mean 372 ppm) in three factories. Significant relations were found between the time weighted average environmental concentration and the concentration in the biological samples. The strongest correlation was between the concentration of acetone in urine and the degree of exposure (r = 0.71, 95% CI 0.64-0.77). This suggests that urinary acetone concentration is the best biological index of occupational exposure to acetone.

[Anti-bacterial zeolite balloon catheter and its potential for urinary tract infection control].

We present here a production of anti-bacterial zeolite balloon catheter and investigated its potential for controlling urinary tract infection. This anti-bacterial balloon catheter showed a bactericidal effect against Pseudomonas aeruginosa, Staphylococcus aureus and Escherichia coli in vitro studies. The antibacterial effects were correlated with the concentration of anti-bacterial zeolite and size of catheter. We tried this catheter for 11 various urological patients who needed a long-term indwelling of a balloon catheter for lower urinary tract obstruction and neurogenic bladder. All patients were already indwelled silicon balloon catheter for 3 to 6 months and suffered with complicated urinary tract infection. Nine patients who had this anti-bacterial zeolite balloon catheter indwelled for 3 to 7 months and exchanged every 2 to 4 weeks, and no patient was taking antibiotics during this trial. Two patients (22.2%) showed good results by the urinary tract infection (UTI) criteria and 5 patients (55.4%) showed good effects by doctor's judgment. This anti-bacterial zeolite balloon catheter might be useful for patients who need long-term balloon catheter indwelling.
