Vaginal hemangiopericytoma: a light microscopic and ultrastructural study.

A case of a vaginal hemangiopericytoma in a 20-year-old woman was studied by light and electron microscopy. The neoplastic cells had round-to-oval nuclei with one or two nucleoli; fine chromatin; a foamy, cyanophilic cytoplasm; and an increased nuclear cytoplasmic ratio. Light microscopically, the tumor was composed of spindle-shaped or round cells proliferating around vascular spaces. Mitotic figures, necrosis, and hemorrhage were difficult to find. Electron microscopically, the neoplastic cells occurred in clusters, each of which was enclosed by a basal lamina. The cytoplasm contained abundant free ribosomes, flattened elements of rough endoplasmic reticulum, small Golgi apparatus, round or elongated mitochondria, lysosomes, glycogen particles, and sometimes pinocytotic vesicles and bundles of microfilaments with large dense spots. Desmosomes were also seen. In addition, the cytoplasmic process of the benign vascular pericytes was directly in contact with the neoplastic cells which had varying degrees of cytodifferentiation. The neoplastic cells in this lesion are consistent with an origin from pericytes, confirming the findings of this neoplasm when it arose in other sites.

The effects of several ion channel blockers and calmodulin antagonists on fertilization-induced acid release and 45Ca2+ uptake in sea urchin eggs.

The effects of calcium antagonists, diltiazem and verapamil, and calmodulin antagonists, chlorpromazine, N-(6-aminohexyl)-1-naphthalenesulfonamide hydrochloride (W-5) and N-(6-aminohexyl)-5-chloro-1-naphthalenesulfonamide hydrochloride (W-7), were tested on two responses of the sea urchin egg to insemination: (1) H+ release; (2) Ca2+ uptake. It was found that calcium antagonists inhibited both processes, while calmodulin antagonists only inhibited H+ release but not Ca2+ uptake. Verapamil and diltiazem were effective to inhibit H+ release when added to the egg suspension up to 120 sec and W-7 was effective around 150 sec after insemination. Calcium antagonists became ineffective earlier than W-7 in inhibiting H+ release. A calmodulin-dependent step may thus occur linking the Ca2+ uptake and H+ release. 4,4'-Diisothiocyanostilbene-2,2'-disulfonic acid (DIDS), an anion channel blocker, also inhibited both Ca2+ uptake and H+ release. This result suggests that an uptake of anion(s) occurs along with Ca2+ uptake.

[Basic and clinical studies on cefminox in obstetrics and gynecology].

Fundamental and clinical studies on cefminox (CMNX, MT-141), a new cephamycin antibiotic, were carried out and the results were as follows. After the intravenous one shot administration of 1 g CMNX to 17 cases, serum concentrations, tissue concentrations in pelvic organs and protein binding capacities in serum were measured. Blood concentrations ranged between 100-200 micrograms/immediately after the injection and declined gradually. Half-life (T1/2) was 75 minutes. Tissue concentrations were 30-70 micrograms/g (tissue weight) 10 minutes after injection and declined. Half-life was 90 minutes. The average serum protein binding rate was 66.3 +/- 8.4%. Correlation between tissue concentration (Y) and free serum concentration (X) was expressed as Y = 0.42 + 7.14X. Correlation coefficient (r) was 0.80 (r = 0.80). Correlation coefficient in serum concentrations between HPLC and bioassay was 0.83 and that in free concentration was 0.97. In clinical studies, CMNX was administered to 19 cases of obstetric and gynecological infections. The patients were constituted of 3 with puerperal endometritis, 3 with pyometra, 3 with pelvic peritonitis, 3 with parametritis, 3 with adnexitis, 2 with BARTHOLIN's abscess and 1 with retroperitoneal abscess and vulva abscess. Overall clinical efficacy was 89.5% (17/19). Bacteriologically 26 strains were isolated, 6 strains of E. coli, 4 strains of B. fragilis, 2 strains of P. anaerobius and P. asaccharolyticus and each one of E. faecalis, S. epidermidis, N. gonorrhoeae, S. marcescens, P. maltophilia, A. calcoaceticus, K. pneumoniae, P. mirabilis, H. influenzae, S. intermedius, E. lentum and F. varium. Twenty of these 24 strains were eliminated after the administration, K. pneumoniae was decreased, E. faecalis, S. marcescens and P. maltophilia remained unaffected. No side effects were observed but 1 case showed mild elevation of transaminases which normalized 1 week after CMNX was stopped.

[Postoperative radiotherapy for carcinoma of the uterine cervix].

During the period between 1972 and 1978, 226 patients were operated on for carcinoma of the uterine cervix at Hiroshima University, and 91 patients were treated by postoperative external irradiation. Patients with lymph node metastasis showed markedly lower five-year survival rates stage I: 44.4%; stage II: 45.0%) than those without lymph node metastasis (99.3% and 84.0%, respectively). In view of benefits of postoperative radiotherapy in the treatment of cervical cancer, only to the node positive and tumor rest patients is recommended.

[Mass screening for breast cancer--the results for 15 years].

Mass screening for breast cancer has been carried out in 13 cities in Osaka Prefecture for the past 15 years. The screening method was inspection and palpation by the physician, and examination by mammography was performed for the women with abnormal findings as the second screening. Among the total of 73,488 examinees (actual number was 44,835 examinees) 3,022 (4.1%) were examined by mammography and 106 (0.14%) patients with breast cancer were detected. Seventy-two of them were found at the first screening, 15 at the subsequent screening, and 19 were interval cases. Sixty-two cases of breast cancer (63.9%) were without lymph node metastasis, and cancer in Stage I and TIS reached 37.8% in this series. These results show that our screening method is valid for detecting the earlier stages of breast cancer.

Histologic grade and karyotype of immature teratoma of the ovary.

Seven cases of ovarian "pure" immature teratoma were encountered in patients 10 to 38 years of age, six cases being in Stage Ia and one case in Stage IIc. The primary tumors and recurrent growth observed in one case were histologically graded from 0 to 3 according to the criteria of Norris et al. Karyotypes of the tumors and the patients were determined using culture and banding techniques. The only nonsurviving case was in Stage IIc. Four primary tumors belonging to grades 0, 1, and 2 showed a normal 46,XX female karyotype and the patients are alive and healthy. Three grade 3 tumors showed various types of karyotype abnormalities (48,XX,+14,+21; 47,XX,+20; 47,XXX). One patient died, one is alive after experiencing a recurrent tumor, and one has only been followed for 22 months. All seven patients had a normal 46,XX female chromosome constitution. Evidence to date indicates that karyotype of ovarian immature teratoma is either normal female 46,XX or a slight deviation from normal. It is postulated that in ovarian immature teratoma normal 46,XX karyotype is an indicator of favorable prognosis, whereas deviations in karyotype suggest a possibility of poor prognosis.

Histogenesis, cytodifferentiation, and its subcellular steroidogenic sites in the virilizing ovarian Leydig cell tumor: light microscopic dry-mounting radioautography for [3H]cholesterol and electron microscopic cytochemistry for 3 beta-hydroxysteroid dehydrogenase activity.

A case of a virilizing ovarian Leydig cell tumor in a 38-year-old woman with a marked elevated plasma testosterone level was investigated using light microscopic dry-mounting radioautography and electron microscopic cytochemistry. Following a total abdominal hysterectomy and bilateral salpingo-oophorectomy, the plasma testosterone level decreased abruptly. Light microscopic dry-mounting radioautography for [3H]cholesterol showed silver grains localized mainly over the cytoplasm of the neoplastic Leydig cells which were of three different cell types: fibroblast-like cells, Leydig cells (steroid-secreting cells), and transitional cells (partially or incompletely differentiated Leydig cells). Reaction products for 3 beta-hydroxysteroid dehydrogenase activity were localized on the tubular or lamellar cristae and inner membranes of the mitochondria, and on the membranes of smooth endoplasmic reticulum in the transitional cell as well as in the Leydig cell. From these observations, it is suggested that the Leydig cell tumor is derived from the fibroblast-like cell and from the transitional cell, morphologically falling between the fibroblast-like cell and the Leydig cell, has fine structural evidence of steroidogenic activity, and has an ability to secrete testosterone.

Chlamydial cervicitis and cervical intraepithelial neoplasia: an immunohistochemical analysis.

Studies using serological and culture techniques indicate that chlamydial infection is frequently associated with cervical intraepithelial neoplasia (CIN). This relationship was investigated by examining a series of biopsies containing normal epithelium, non-neoplastic condylomatous epithelium, and neoplastic epithelium (CIN). In each case the degree of inflammation and the presence of reparative atypia were recorded from an examination of the hematoxylin and eosin stained sections and serial sections were stained for chlamydial antigens using a polyclonal antichlamydial antibody and the immunoperoxidase technique. Overall, staining for chlamydia was positive in 0, 2, and 16% of biopsies with mild, moderate, and severe inflammation, respectively. In cases of severe inflammation positive staining was present in 20, 25, and 8% of biopsies containing non-neoplastic, condylomatous, and neoplastic epithelium, respectively. In all positive cases the staining was most prevalent in areas of the most intense inflammation. A spectrum of squamous epithelial changes was found in the infected biopsies and their distinction from CIN is discussed. This study suggests that tissue staining for chlamydia is related more to the extent of the coexisting inflammation than the presence or absence of CIN. Chlamydial infection, however, was frequently associated with inflammation-related squamous atypia in the transformation zone, which may be confused histologically with CIN.

Penitricin, a new class of antibiotic produced by Penicillium aculeatum. I. Taxonomy of the producer strain and fermentation.

A novel antibiotic, penitricin (Ro 09-0804) was discovered in the culture filtrate of a fungal strain NR 5165. Taxonomic studies of the producing organism resulted in its assignment to Penicillium aculeatum. Further examination on penitricin production by other strains of this species and related taxa revealed that penitricin was produced by several other strains of P. aculeatum, but not by any available strains of the closely similar species, P. verruculosum. It was also found that copper ion was essential for production of penitricin.

[Effect of pre-operative biopsy for breast cancer on survival rate].

To study the effect of pre-operative biopsy for breast cancer on the 10-year-survival rate, 1123 cases with single cancer radically operated at the Cancer Institute Hospital between Jan. 1960 and Dec. 1969 were analyzed. The survival rate in 292 biopsied and non-biopsied patients with cancers less than 2 cm in diameter and no lymph node metastasis was not significantly different. There was no difference between incisional- and excisional-biopsied patients. However, the survival rates tended to decrease as the interval between biopsy and radical surgery increased. We conclude that pre-operative biopsy does not worsen the prognosis in patients with breast cancer. Early detection and treatment are of great importance.

[Kinetics of protein synthesis in the human granulosa lutein cell during pregnancy as revealed by electron microscopic autoradiography of 3H-leucine].

Kinetics of protein synthesis in the human granulosa lutein cell during pregnancy were investigated using the electron microscopic autoradiography of 3H-leucine. The corpus luteum of early pregnancy was chased at 10 and 30 min, 1, 3, 6, 12, 24 and 48 h after pulse labelling (10 min) with 100 microCi of 3H-leucine. Although the whole silver grains were very few at 10 min postpulse, silver grains were localized mainly over the rough endoplasmic reticulum (r-ER, 34.3%) and there were no silver grains over the smooth endoplasmic reticulum (s-ER). At 30 min postpulse, the ratio of the grain number to r-ER is decreased (18.7%) and silver grains begin to appear over the s-ER (5.4%). The total number of silver grains over the cell increases with the lapse of time, the ratio of the grain number over the r-ER slightly increases and then decreases afterwards, while that over the s-ER continues to increase gradually. From these facts, it is felt that some protein synthesized on the r-ER might be transported to the s-ER in the human granulosa lutein cell during pregnancy showing steroidogenic activity.

Inhibition of DNA polymerases of sea urchin by palmitoyl coenzyme A.

Palmitoyl CoA noncompetitively inhibited the activities of DNA polymerase alpha and gamma, prepared from sea urchin germ cells, with Ki values of 28 microM and 116 microM, respectively. Myristoyl CoA also inhibited DNA polymerase alpha and gamma, while coenzyme A, short chain fatty acyl CoA's, Na-myristate and Na-palmitate failed to inhibit the enzymes. It was concluded that both the long hydrocarbon chain and CoA moiety of long chain fatty acyl CoA's are necessary for inhibition of DNA polymerase activity. DNA polymerase beta was not inhibited by long chain fatty acyl CoA's.

Inhibitory effect of long chain fatty acyl CoAs on RNA polymerase from Escherichia coli.

RNA polymerase from Escherichia coli was inhibited by long chain fatty acyl CoAs, such as myristoyl CoA (Ki = 17.2 microM), palmitoyl CoA (Ki = 8.9 microM), oleoyl CoA (Ki = 5.5 microM), and stearoyl CoA (Ki = 0.94 microM). The inhibition by these CoA thioesters was non-competitive against nucleoside triphosphates. Short chain fatty acyl CoAs, such as acetyl CoA, propionyl CoA, acetoacetyl CoA, butyryl CoA, and decanoyl CoA, failed to inhibit RNA polymerase. CoA, Na-myristate, Na-palmitate, Na-oleate, Na-stearate, palmitoyl carnitine, and carnitine did not inhibit the enzyme. The inhibition of RNA polymerase by long chain fatty acyl CoAs was competitive against template DNA.

Degeneration of archenteron in sea urchin embryos caused by alpha,alpha'-dipyridyl.

Degeneration of the archenteron in middle gastrulae occurred in the presence of alpha,alpha'-dipyridyl or Zn2+, inhibitors of prolyl hydroxylase. In the presence of these substances the archenteron degenerated and was eventually destroyed. Adding Fe2+ to the embryo culture containing alpha,alpha'-dipyridyl protected the archenteron from further degeneration, but the collapsed archenteron was not restored to the upright position. At the late gastrula stage, alpha,alpha'-dipyridyl did not cause the degeneration of the archenteron. Treatment of the embryos by alpha,alpha'-dipyridyl, starting at the swimming blastula state, resulted in the production of many mesenchyme-like cells but archenteron was not produced in the embryos. Addition of Fe2+ to alpha, alpha'-dipyridyl culture, just before the beginning of gastrulation of normal embryos, resulted in the formation of normal archenteron. alpha,alpha'-Dipyridyl inhibited hydroxylation of proline residues of collagen in sea urchin embryos and Fe2+ prevented the inhibition by alpha,alpha'-dipyridyl. Respiration was not inhibited by alpha,alpha'-dipyridyl.
