The clinical, hydroelectrolytical and hormonal features, as well as the treatment of 3 patients with Bartter's syndrome are analyzed. The patients consulted due to severe hypokalaemia with little clinical expression (paresthesiae, carpopedal spasms, polyuria, polydipsia and/or weakness). All had normal blood pressure, hypokalemia, hyperreninaemic hyperaldosteronism and a decrease in the fractional distal chloride reabsorption; hyperuricaemia was observed in two cases and hypomagnesemia in one. Treatment with oral potassium supplements and indomethacin managed to raise serum K to around the lower limit of the normal range. However, the tubular defect and hyperreninaemic hyperaldosteronism persisted. The urinary excretion of prostaglandin E2 was normal in both the patients in whom it was measured. Given the minimal clinical expression of hypokalemia despite the difficulty in correcting it, it is probable that this syndrome often remains unrecognized.
We investigated the involvement of cutaneous structures in specific linear migration pathways of technetium-99m pertechnetate hypodermically injected at points of low electrical resistance in the metacarpus of male beagles. Skin-deep incisions were made in the front or back legs on either the same side as the 99mTc injection or on the opposite side. Incisions in the back legs did not affect the migration pattern. Incisions in the front legs before the injection of 99mTc prevented tracer migration. After the injection of 99mTc, incisions in the front contralateral leg caused sudden cessation of the migration, while incisions in the ipsilateral leg caused immediate disappearance of the pathway previously observed. Radioactivity was not detected in flaps obtained from the skin overlying the migration pathway or from the corresponding area of the contralateral leg. In conclusion, the specific linear migration pathways of 99mTc hypodermically injected at points of low electrical resistance cannot be explained by any known biological function. Although the migration of 99mTc does not seem to be strongly linked to any cutaneous structure, the skin overlying the radioactive pathway and the corresponding area of the contralateral leg must be intact if tracer migration is to take place.
Noise levels around educational centres can negatively affect the performance of both teachers and pupils. Two public schools in Valencia, Spain, were selected for study. One of these schools was exposed to excessively high road traffic noise levels while the other was located in a relatively quiet area. The socioeconomic level of those attending the schools was very similar. A set of external and internal noise measurements were carried out, along with two different attention tests among the children. Test results were consistently better (both for tests and for children from different classrooms in each school) in the quiet school. Exposure to high traffic noise levels in the noisy school over the whole school year is a plausible determinant of these results.

Nonbred and pregnant ewes were examined ultrasonographically at intervals of 4 to 6 days on Days 17 to 34 after estrus. Each ewe was diagnosed as pregnant or nonpregnant, and a score for degree of certainty in the diagnosis was recorded. The goal of the study was to define criteria that could be used for identification and accuracy of diagnosis of an early conceptus and to ascertain the confidence which the operator had in makeing the diagnosis. Pregnancy was retrospectively confirmed by ultrasonographic detection of an embryo proper and by embryonic heartbeat on Days 21 to 34, and later judged against the number of lambs born to each ewe. The percentage of ewes accurately diagnosed pregnant by ultrasonography was not significantly higher than that by guessing (50%) before Day 24, but reached 85% on Days 32 and 34. However, the ability to detect nonpregnant ewes by ultrasonography was higher (P<0.01), with a greater specificity starting on Days 21 to 23 (80%) and reaching 98% by Days 32 to 34. Before Day 24, the diagnosis of pregnancy in many cases was based primarily upon the ultrasonographic appearance of the uterine lumen and location of the uterus in relation to the bladder rather than upon detection of the conceptus. For the certainty score there was a main effect of day (P<0.01) but not for the reproductive status (pregnant vs nonpregnant). The certainty score increased in all ewes among days, and was highest on Days 32 to 34. It was concluded that real time transrectal ultrasonographic scanning of sheep between Days 24 and 34 of gestation offers a safe, accurate and practical means for diagnosing pregnancy.
We have studied the effects of the unilateral electrolytic lesion of the medial forebrain bundle at the level of the lateral hypothalamus on the density and functionality of histamine H1 receptors in rat brain cortex. The treatment resulted, after two and four weeks, in an increase in the maximal phosphoinositide breakdown induced by histamine, which can be accounted for by the appearance of a higher potency component for the response. On the other hand, the density of cortical histamine H1 receptors, determined by the specific binding of [3H]mepyramine to membranes, remained unchanged two weeks after the lesion but after four weeks a small but significant increase was also found. These results suggest that the denervation hypersensitivity developed may initially be the result of a more efficient coupling of the H1 receptors to the effector system prior to the subsequent increase in receptor numbers.
A V/Q mismatch (segmental contour pattern) is described in a patient suffering from dyspnea. Chest roentgenography revealed a mild interstitial pattern and enlarged pulmonary arteries. A lung biopsy demonstrated pulmonary venoocclusive disease.
Radioimmunoscintigraphy (RIS) of colorectal carcinoma with a 111In-labelled anti-TAG-72 monoclonal antibody (CYT-103) has been performed in 24 patients with five primary lesions and 10 suspicious of recurrence (in one of these patients two RIS were made). Histopathological confirmation of the disease and surgical liver examination were carried out in all primary tumours and in 12 possible recurrences. In the remaining eight patients a final diagnosis was established according to the clinical course and other diagnostic procedures. Planar and tomographic scans were obtained at 48 and 72 h postinjection in all patients. All primary tumours were detected by RIS. In the group with recurrences confirmed pathologically the results were nine true positive, two true negative and one false positive in a patient affected only with liver disease. The nine true positive studies corresponded to four positive by computed tomography (CT), four negative by CT and one nonconclusive by CT. Surgical liver examination results were 15 true negative and two false negative. No correlation was found between serum levels of carcinoembryonic antigen or TAG-72 and the detection of the lesions. In 10 patients human anti-mouse antibody (HAMA) levels were studied. In conclusion, RIS with an anti-TAG-72 monoclonal antibody is a useful technique for the study and localization of colorectal tumours, mainly in cases of recurrence, being also indicated in patients with normal TAG-72 serum levels.

The therapeutic efficacy of a preparation containing 2.5% bifonazole was investigated by comparing three different treatment modalities--A, B, and C. Group A used bifonazole only on Days 1, 2 and 3, and the Group C on Days 1, 3 and 5. Of the patients in Group A 56% had a negative mycological examination at the end of the study. The results obtained in Groups B and C were not significantly different: 92% of the patients had a negative mycological examination at the end of the study. Electron microscope (EM) studies showed morphological alterations such as loss of cytoplasmic organization with shrinkage and folding of the cell membranes after 1 week of treatment only in Groups B and C. We conclude that 2.5% bifonazole is a highly effective treatment for Pityrosporum ovale infection when applied using a 3-day schedule.
A transversal epidemiological study was carried out to discover the most important cardiovascular risk factors in the child population of Asturias, Spain. The study took place over a period of 4 years (1987-91) and consisted of a representative sample of boys and girls (6-13 years of age) from different socioeconomic and cultural backgrounds. Biochemical factors (serum total cholesterol, HDL-cholesterol, serum triglycerides and glucose) and anthropometric measures to evaluate obesity were analysed. Hypercholesterolemia appeared to be the most prevalent risk factor with a prevalence of 38.27% and a mean value of 178.31 mg/dl, followed by obesity (15.58%), hypertriglyceridemia (14.05%) and hyperglycemia (0.77%). Risk factors were more frequent in girls than boys, and in younger children in relation to older ones. Hypercholesterolemia and obesity were more prevalent in children from an urban environment while hypertriglyceridemia was the main risk factor in those from a rural one. Considering the high prevalence of the evaluated risk factors, the prevention and control of such is of extreme importance in leading children to becoming healthy adults.
