[Teleradiography and tomography in the search for pulmonary metastases in 132 cases of soft tissue and bone neoplasms].

In recent years whole lung tomography (WLT) has been considered mandatory in the staging of some neoplastic diseases, particularly of soft tissues and bone tumors. Since WLT is an exacting roentgen examination, its usefulness was evaluated by analysing 132 consecutive patients submitted to orthogonal chest roentgenograms and WLT, from January 1979 to October 1981 at the National Cancer Institute of Milan. A comparison between WLT and traditional chest X-ray examination was performed in order to evaluate how much tomography is significantly useful in improving diagnosis of lung metastases. From this analysis, the authors conclude that WLT is generally unnecessary when chest roentgenograms are negative, whereas it is useful when single or multiple lung metastases are already evident by traditional X-ray examination. In these cases WLT allows a more accurate identification of the metastases from the point of view of their number and site, which is important not only for the treatment choice but also for the check of its effectiveness.

[Radiological and telethermographic diagnosis of breast lymphomas (28 cases)].

Twenty eight patients with histologically proven primary or secondary malignant lymphoma of the breast were evaluated by mammography and telethermography between 1968 and 1980 at Istituto Nazionale Tumori of Milan. This histological type is rather unusual and poor is the radiological literature on the matter. The roentgenographic findings showed especially isolate nodes with clear-cut or irregular outline, multiple displasic lesions or diffuse inflammatory-like disease. Because of such a variety of radiological matters, diagnosis is often uncertain. No significant difference between primary or metastatic disease was seen in radiological patterns of non-Hodgkin lymphomas. In the three cases of Hodgkin's lymphoma radiological findings suggested inflammatory disease. Telethermography was usefully associated showing, in the majority of cases, high hyperthermia also in "benign" mammographic patterns.

[Radiography signs of non-Hodgkin lymphomas with pulmonary localization].

The authors have examined a series of 992 patients with non-Hodgkin lymphomas treated at the National Cancer Institute of Milan from 1966 to 1977. The pulmonary parenchymal lesions were studied. The lung involvement by lymphomas is unusual and show a difficult X-ray diagnosis. A classification deriving from the experience of the various roentgenologic findings is proposed. Furthermore the most important differential diagnostic difficulties, some of which have an impossible solution even with the most recent X-ray techniques, are analized.

Prognostic significance of the growth rate of breast cancer: preliminary evaluation on the follow-up of 196 breast cancers.

The doubling time (DT) of 196 consecutive breast cancers was studied by means of a double mammographic examination (average time between the 2 mammographies, 30 days) and calculated with the formula of exponential growth. On the basis of DT values the case series was divided into 3 groups of growth: fast (DT from 1 to 30 days), 31 cases (15.8%), intermediate (DT from 31 to 90 days), 84 cases (42.9%), slow (DT more than 90 days), 81 cases (41.3%). No relationship was found between growth rate and size of tumor, or menopausal status of the patient. After mastectomy fast and slow cases were equally distributed in the N- and N+ groups, whereas for the intermediate cases the N- : N+ ratio was 1 : 2. One hundred and thirty-four cases were followed for a period of 12 to 52 months. Evaluation was done on the basis of the subdivision into N- 2nd N+, and the latter group into N+ (1-3) and N+ (greater than 3). For N- tumors the course of the disease was apparently not affected by the growth rate. However, the case of fast growing tumors showed a higher proportion of recurrences with respect to N+ slow growing tumors. This difference was even more noticeable the higher the number of involved lymph nodes, but not statistically significant. The course of slow growing tumors was identical in the N- and N+ groups, but all the N+ tumors were subjected to adjuvant chemotherapy.

[The value of mammography in 1203 breast cancers (author's transl)].

The value of clinical mammography is reviewed on 1203 consecutive cases of histologically proven breast malignancies examined from 1971 to 1978 at the Istituto Nazionale Tumori, Milan, and at the Radiological Department, University of Milan. The diagnostic effectiveness of mammography in this case-list corresponds to approximately 80%: the influences of the histotype and of the tumor's size on these results are reviewed and discussed. The main radiographic signs of malignancy are presented and their importance in reaching a correct diagnosis is outlined. The advantages and drawbacks of mammography are discussed and, amongst these latter, emphasis is given to structurally dense breasts as an intrinsic factor limiting the effectiveness of mammography.

[Radiological diagnosis in 229 cases of breast fibroadenoma].

From 1976 to 1977, 705 patients with a fibroadenoma of the breast, histologically proven by biopsy, were observed at the Istituto Nazionale Tumori of Milan. In 229 cases, the clinical examination was followed by mammography or xeromammography; in 65/229 patients telethermography was also carried out. The aspects of fibroadenoma at x-ray examinations are described together with the difficulties to reach an exact diagnosis: the cases in which a false positive diagnosis of cancer was done are emphasized.

[X-ray diagnosis of neoplastic breast calcifications after radio- and chemo-therapy (author's transl)].

The morphological aspects of calcifications in inoperable breast cancers were investigated through repeated mammographic examinations before and after radiotherapy and/or chemotherapy. Aim of the study was to find out the modifications induced by therapy. Different radiological patterns observed during treatment and follow up are described in details. Some findings concerning calcifications, such as modifications of number, distribution, size and shape or new evidence of micro- and macro-deposits are analyzed and discussed. A correlation between radiological findings and clinical response to radio- and chemo-therapy is attempted, but wider experience and information are needed.

[The solitary pulmonary nodule. A radiologic and clinical approach (author's transl)].

The cases of pulmonary coin lesions operated at the National Cancer Institute of Milan during the last ten years are presented. Both the disadvantages of early thoracotomy as well as watchful waiting are discussed. Therefore different parameters with the aim to select the patients with the highest probability of cancer are analysed. Finally we tried to identify a diagnostic-therapeutic approach to reduce the complications and risks of thoracotomy in non-oncologic patients.

Pulmonary and mediastinal xerotomography: a critical appraisal.

Xerotomography was performed on 148 patients with proven or suspected tumors of the lung or mediastinum. Retrospectively they were compared to film tomography in 108 cases. In 64 patients, the results of histological examination were also available. Xerotomography proved diagnostically more effective than film tomography in 52 cases (48%), less effective in 6 cases (6%) and about equivalent in the remaining 50 cases (46%). Xerotomography appeared to be more effective in visualization of small peripheral nodes of moderate opacity. Xerotomography has to be a selected second-line technique for special cases, in particular for the study of coin lesions, for the search for cavitations and calcifications and for the study of pulmonary vessels.

[X-ray diagnosis of malignant breast calcifications. (author's transl)].

The authors have taken into consideration the different features of malignant breast tumor calcification. Frequent, rare and extremely rare radiological pictures will be shown and a differential diagnosis with benign micro-calcifications will be considered. A correct mammographic and xerographic technical performance is absolutely necessary to enhance the images of the smallest micro-calcifications. At last we assess the importance of a radiographic control of the surgical blocks, after conservative surgery of the breast, to assure a complete removal of microcalcifications.

Bronchial involvement by Hodgkin disease.

Two cases of Hodgkin disease, lymphocyte depletion type, with bronchial involvement are described. One patient revealed involvement of the right upper lobar bronchus, the other patient of the left main bronchus. The correct diagnosis was made by fiberbronchoscope examination with cytologic brushing and biopsy. That bronchial involvement by Hodgkin disease is very rare is confirmed by the literature, where only 20 cases have been so far reported.
