[Progressive muscular dystrophies within the scope of forensic studies].

Serum myoglobin levels were investigated in patients with different types of progressive muscular dystrophy and controls. The Mb levels were determined by Radioimmuno-assay and found to be significantly elevated in all patients. The application of a specific Mb antibody (rabbit anti-human Mb) makes it possible to recognize marked differences between the Mb bands of patients and controls. All patients with progressive muscular dystrophy had an additional fourth Mb band in contrast to controls with three Mb bands.

[Combination radiochemotherapy in the limited-disease stage of small-cell bronchial carcinoma. Results following high-dosage radiotherapy].

From 1980 through 1984, 72 patients with small cell bronchial carcinomas were treated by radiotherapy. Twenty-one patients who were in a limited disease stage (LD) received chemotherapy according to the ACO scheme and high-dose radiotherapy with 50 Gy. These patients had a complete remission rate of 67% and a partial remission rate of 33%. The median control period was 37 months. At the time of diagnosis, patients had a median survival time of ten months (six to 36 months). A recurrence in the irradiated region was seen in none of the patients, however, 14% of patients developed a recurrence beyond the irradiation field. The results are compared to literature and discussed.

[Cerebral manifestations in non-Hodgkin lymphoma and in lymphogranulomatosis on the CT image].

Primary or secondary cerebral non-Hodgkin-lymphoma or Hodgkin's disease are very rare. The incidence varies from 0.2 to 4%. There is a predilection for the basal ganglia, corpus callosum, thalami and subependymal regions. Hodgkin's disease involving the brain usually infiltrates from the meninges. Solitary tumors are rare, multifocal tumors are extremely unusual. The typical CT appearances of intracranial lymphomas are described in three cases. These consist of isodense or hyperdense intracerebral lesions with marked increase in density on contrast enhancement. Periventricular tumor expansion in NHL is characteristic.

[Radiotherapy aspects in the treatment of breast cancer].

The role of radiotherapy in breast cancer has been to supply and complete radical or simple mastectomy, conservative surgery, and to improve quality of life in cases of disseminated disease. The concept of conservative surgery and radiation is based on successful treatment while preserving the breast. In randomized trials there were no significant differences in disease-free, distant-disease free or overall survival of patients treated by radical mastectomy or segmental mastectomy followed by breast irradiation. Adjuvant radiotherapy in treatment of operable breast cancer remains controversial, although most oncologists agree that it provides local and regional control of tumor. Preirradiation multidrug chemotherapy was added in inflammatory carcinoma of the breast, in patients with high risk of dissemination, and in gross local recurrences. Radiotherapy does relieve pain and disability caused by bone metastases in 80% and effective palliation is achieved by treatment of brain metastases.

[Initial results of hyperthermia in palliative therapy of cancers in the ENT area].

Preliminary results of an interdisciplinary study, conducted to investigate the efficacy of combined local hyperthermia and radiation/chemotherapy on carcinoma of head and neck, are reported. Upto now, eleven patients with recurrent squamous cell carcinoma of head and neck were treated, all of them had loco-regional recurrence, and 2 distant metastases in addition. 7 patients were pretreated by surgery and radiotherapy, 4 by radiotherapy only, and 6 by chemotherapy. Microwave-induced low-dose local hyperthermia was combined with radiotherapy (telecobalt 1100-2400 rad) 6 cases (group A) and with chemotherapy (bleomycin and cis-platin/bleomycin) in 5 patients (group B). Intratumoural temperatures between 43 and 45 degrees C were achieved for at least 30 minutes. The loco-regional efficacy of hyperthermia was assessed. 10/11 patients with 13 tumours could be evaluated, Group A: 3 CR, 4 PR, Group B: 1 PR, 4 MR and 1 NC.

Lymphatic metastases to the esophagus.

The authors present 4 cases of esophageal involvement by lymphatic spread of tumors originating in the stomach, distal esophagus, and thyroid. Double-contrast radiographs of the esophagus showed discontinuous submucosal extension and multiple polypoid masses.

[Therapy of bronchial carcinoma from the radiotherapist's viewpoint].

The irradiation of radical resectable lung cancer is unambiguously not as effectively as surgery, and also the preoperative radiation with high tumour doses has not demonstrated any benefit in survival rates. Tumours of the superior sulcus could be an exception in both treatment modalities. As shown in randomized studies one could suggest some advantage employing postoperative irradiation of patients especially with mediastinal node metastases. The addition of chemotherapy to high dose irradiation with curative intent in inoperable non-small cell lung cancer has not been shown to increase survival. The combined chemo-radiotherapy of small cell lung cancer significantly reduced the incidence of relapse and prolonged the disease free interval but did not affect long-term survival.

[Radiation-induced vasculopathy of the carotid artery and vertebral artery].

The article reports on a case of radiation-induced vasculopathy of the vessels of the head and neck causing a transitory ischaemic attack 14 years after primary radiation of a squamous cell carcinoma. The correlation of the radiation-induced vasculopathy with regard to applied radiation dose and time latency is discussed on the basis of published references.

[Liver metastases: sonography or scintigraphy? A comparative prospective study].

In 278 patients with known malignancy a sonogram, scintigram and laboratory examination were done on the same day. The comparison of the examinations revealed nearly the same accuracy for both imaging modalities with 90% for sonography and 89% for scintigraphy. Sonography showed the more uniform results, because scintigraphy had a high percentage of false positive results. The value of both methods as the utility of an additional laboratory examination are discussed.

[Metastatic involvement of the breast].

The mammographic spectrum of metastases in the breast is illustrated by 43 cases with histologically confirmed findings. Haematogenous metastases appear as sharply demarcated foci, lymphogenous metastases produce thickening of the skin and reticular thickening of the subcutis and of the glandular tissue. Metastases from systemic haematological disease may appear in nodular or diffuse form.

[Irradiation of the para-aortic ducts. Presentation of an radiation technic with a largely fixed patient's position].

An irradiation planning program has been developed at the Radiologic University Hospital of Graz which allows an interactive optimization of the energy dose distribution of a Co-60 radiation. This program is running in a minicomputer of type DEC PDP-11/34 under the operation system RSX-11. With the aid of this system, a technique has been developed for irradiating the para-aortic lymph canals which complies with theoretical as well as practical optimization criteria. These optimization criteria are defined, and the usefulness of this irradiation technique is compared with a conventional method and demonstrated on the clinical example.

[Influence of the topographic situation of the uterus upon the irradiation planning in case of carcinoma of the cervix].

The authors demonstrated by means of computed tomography that some problems are offered by the standardized method of primary radiotherapy in case of carcinomas of the cervix. Especially when a low-dose curietherapy is applied without dose monitor, a radiation exposure of bladder and rectum exceeding the tolerance limit or a underdosage in the target volume are possible due to variable positions of the uterus and different thicknesses of the uterus wall, above all in the region of the cervix. The rate of complications can also be increased by a false, eccentric position of the intra-uterine applicator if the latter does not follow the cervical canal. In nearly half of 61 patients with carcinomas of the cervix of stage I or III according to FIGO, computed tomography scanning showed a false position of the intra-uterine applicator probe. Besides the correction of the probe position, the problem can be solved by a dose reduction and use of shorter applicators or by a modified relation between brachytherapy and percutaneous therapy.

[Treatment of Wilms' tumor (author's transl)].

Uniform treatment based on the therapeutic approach of the 1st and 2nd US National Wilms' Tumor Study was decided on in March 1976 by paediatricians, surgeons, urologists and radiotherapists in Austria. Wilms' tumour was diagnosed in 34 children between 1 january 1976 an 29 february 1980 (stage I: n = 11, stage II: n = 8, stage III: n = 8, stage IV: n = 7). Parents of two children refused treatments; both children have since died of metastases. Of the remaining 32 children 29 (90.6%) are alive, 10 for more than 4, 15 for more than 3 and 19 for more than 2 years after diagnosis. 21 children are without need of treatment. Three children have died, one due to postoperative complications, one due to haemorrhagic chickenpox, but free of tumour, and one after insufficient treatment. Two of the five children with a recurrence between 2 1/4 to 15 months after diagnosis had been treated inadequately in the initial phase. The tumour free survival rate in 74.2%. Two children with early occurring or recurrent lung metastases have survived for 53 1/2 and 54 months up to now.

[Intracranial germinoma; typical CT appearances and radiation therapy (author's transl)].

The typical CT appearances of intracranial germinomas are described in three cases. These consist of an isodense or hyperdense tumour in the mid-line, originating from the pineal body or infundibulum, with marked increase in density on contrast enhancement. Periventricular tumour expansion in relation to the entire supratentorial ventricular system is characteristic. The rapid tumour regression after small doses of radiation is stressed and complete regression of the tumour, as seen on CT, is a sign of its radio-curability.
