The article concerns the usage of discrete normobaric hypoxic therapy as a way to enhance non-specific resistance in epilepsy treatment. The optimising effect of hypoxia on physiological functions of the patients with epilepsy consists in an increase of haemoglobin and erythrocytes in blood, less systole frequency, a systolic and diastolic pressure reduction, prolongation of a breathing hold during a Stange's test). As a result of these changes, the frequency of epileptic attacks decrease and normalization of behavioural responses was observed.
The article presents an analysis of bioelectrical brain activity in patients with epilepsy during adaptation to intermittent normobaric hypoxia. Before adaptation and after the 1st, 4th, 20th adaptation sessions, resting EEG and EEG during hyperventilation were registered. By the end of the hypoxic trainings, a significant increase of spectral power of the EEG bands was found in resting EEG. At comparing bioelectrical activity in the resting condition and during hyperventilation, the difference between these indices decreased towards the end of hypoxic therapy course. This evidence argues for brain functions normalization during hypoxic therapy.
The paper presents the results of clinical-neurologic study of the indices of cerebral hemodynamics (ultrasonic dopplerography of major arteries of the head, transcranial dopplerography) in 114 patients aged 15-36 years with vertebral artery (VA) syndrome. In the clinical status both subjective manifestations and typical vertebral symptomatology prevailed. There were tendencies to changes of blood flow in extracranial arteries of the carotid system that differed in compression-irritative and reflex-angiospastic forms of VA syndrome. The most significant changes (decrease of blood flow velocity, increase of the indices of the kinematics of blood flow) were observed in vertebral and posterior cerebral arteries (PCA), especially in compression-irritative form of VA syndrome. Homolateral decrease of blood flow in PCA was characteristic for unilateral decrease of blood flow in VA. The results show an important role of the changes of cerebral hemodynamics in pathogenesis of the syndrome of VA.
Combined observation and treatment of 126 patients (80 men, 46 women) aged 15-67 years were carried out in the acute period of craniocerebral trauma of mild or moderate severity. Diagnostic methods including neurological examination, manual testing and X-ray investigation are discussed. The conclusion is made that craniocerebral trauma should be considered as craniocervical trauma as an indirect damage of the neck was revealed in each case in the form of myofascial syndrome pronounced in different degree; cervical syndrome of vertebral artery was also frequent. These data must be taken into consideration in pathogenetic treatment of patients with craniocerebral trauma.
The efficacy of drug (inderal, corinfar) treatment in combination with manual therapy (the course was 4-7 sessions) was examined in hypertensives with cervical osteochondrosis. The latter was evidenced by X-ray studies. Clinically, cervical osteochondrosis appeared as vertebrobasilar insufficiency. The vertebral artery bed displayed circulatory disorders documented by rheoencephalography, electroencephalography, ultrasound Doppler echography of the head and neck vessels. Treating the patients with drugs in combination with manual therapy resulted in improved circulation in the vertebral artery bed, lower clinical signs of vertebrobasilar insufficiency. There was a more persistent and more pronounced decrease in blood pressure with small-dose antihypertensive agents.

The paper is concerned with the development and specification of diagnostic approaches to patients with the title pathology. 168 patients with vascular syndromes of cervical osteochondrosis were under observation. According to the readings of ultrasound echocardiography and ultrasound double scanning, the patients were distributed into two groups: with occlusions and hemodynamically significant stenoses (from 50 to 99%) in the vertebral arteries, and without hemodynamically significant stenoses. On comparison of these two groups of patients with the disease clinical picture the first group patients demonstrated the signs of static or dynamic ataxia of varying gravity, thus allowing confirmation of the existence of the compression form of the vertebral artery syndrome--vertebrobasilar insufficiency. The authors have elaborated a complex of the treatment measures for patients with vascular syndromes of spinal osteochondrosis, including the use of medicamentous and nontraditional means (manual therapy, acupuncture), providing a possibility of carrying out successful conservative therapy. The role of vegetative formations located in the cervical part of the spine in regulation of the cerebral blood flow has been revealed.
The paper is concerned with the data on the brain stroke incidence and lethality among the population aged 25-64 years in 3 districts of Moscow during 1985. The rate of brain stroke was noted to be increased with age without any essential differences in the indicators in men and women. Comparison of the results of the investigation with the data of the Register, carried out in one of the districts of Moscow during 1972-1973 did not reveal any changes in both the disease incidence and lethality due to brain stroke. The rise of the calls on ambulances and emergency medical service for acute derangement of brain circulation is explained by the authors by a possible growth of the number of the disease cases among senile patients or by aging of the population of Moscow.
A complex examination of 60 patients with different stages of disturbances of blood circulation (by the Fontain classification) with chronic arterial insufficiency was performed. Specific features of regional hemodynamics were revealed in patient with "critical ischemia" (3rd and 4th stages of the disease). Threshold values of the "critical ischemia" were determined, most informative of them being the malleolus index and oxygen pressure on the foot. An analysis of results of the investigation in the dynamics of the treatment performed has shown possible effectiveness of restorative operations in treatment of patients with severe stages of chronic arterial insufficiency.
In patients with atherosclerotic encephalopathy (AE) the major indices of blood rheology, hemostasis and its cells' functional state were under investigation. Different patterns of blood rheology changes were detected with increased counts of pathological red blood cells and hypercoagulability. These disorders correlated with the rate of major neurological signs and syndromes, and with the disease stage as well. A multilevel positive effect of hemosorption was shown in different pathogenic links of AE related to disorders in lipid metabolism, blood rheology, shape and functional activity of erythrocytes, shifts in the homeostatic systems. This suggested the use of hemosorption as a tool in prevention of ischemic stroke.
The inclusion of hemosorption through activated coals into the comprehensive treatment of 580 patients sick with atherosclerosis has made it possible to achieve a positive clinical effect among 81 per cent of the patients sick with the ischemic heart disease, 79 per cent of atherosclerotic discirculatory encephalopathy and 76 per cent of obliterating atherosclerosis of lower limbs. The positive dynamics is especially visible among patients who are administered the programmatic hemosorption, making it possible over a long period of time (3-5 years) to sustain remission in the clinical state and the normal level of the lipids in the blood. The direct effect is expressed in the improved microcirculation, disappearance of the sludge-syndrome and the reduction of ischemic disorders. The remote effect of hemosorption is connected with the repeated removals of cholesterin of the atherogenic lipoproteids, which leads, we believe, to the interruption or even regression of the atherosclerotic process.
In a series of 84 patients with various stages of atherosclerotic circulatory encephalopathy (ADE) the authors studied the possibility of detecting by ultrasound double scanning (UDS) of occlusive lesions of the brachiocephalic arteries and compared the results with the findings provided by cerebral angiography (AG). In a study of 35 healthy subjects normal parameters of the spectrograms of the extracranial arteries were established. In 84 patients with ADE, ultrasound double scanning revealed stenoses and occlusions of the brachiocephalic arteries in 47 cases. They were confirmed by AG and were classified into five categories. Comparison of the data demonstrated that the diagnostic reliability of UDS versus AG was 95.9%. UDS makes it possible to diagnose accurately occlusive lesions of the brachiocephalic arteries in patients with ADE and to determine indications for AG and thus may be used for screening purposes.
Hemosorption was included in combined treatment of 110 patients with atherosclerotic dyscirculatory encephalopathy combined with coronary heart disease. The effect was assessed on the basis of clinical improvement, psychological tests, blood lipid assays, compressed spectral EEG analysis and ultrasonic flowmetry of the carotid arteries. The effectiveness of hemosorption was demonstrated at all stages of the disease, with the regression of general cerebral and focal neurologic symptoms preceding clinical remission of coronary heart disease. Hemosorption was shown to have good effects on blood lipids, cerebral circulation and EEG spectrograms.
A total of 140 patients with atherosclerotic encephalopathy were subjected to a multiple modality treatment. The efficiency of the treatment with (n-90) and without (n-50) hemosorption was compared. The results of the treatment were assessed by neurologic and neuropsychological examinations, spectral analysis of the EEG and the study of blood lipids. Hemosorption was found to be highly effective in the treatment of patients with atherosclerotic encephalopathy, which was expressed in the regression of clinical manifestations, a positive time-course of the electrical activity of the brain and lipid metabolism indicators.
The characteristics of lipid metabolism investigated in 114 patients with dyscirculatory encephalopathy of the atherosclerotic genesis were collated with the clinical manifestations of the disease and the results of patients' psychological testing. Ten neurological syndromes and symptoms occurring most frequently in dyscirculatory encephalopathy were specified. The data of the psychological testing also changed with the severity of the disease. The most commonly identified abnormalities included dyslipoproteinemia, types II b and IV of hyperlipoproteinemias according to Frederickson as well as hypertriglyceridemia and an increase in the atherogenic coefficient.
The peculiarities of consciousness disturbances in 289 patients with grave brain stroke were studied. Eight informative clinical syndromes, including the awakeness level, brain stem reflex functions as well as vegetative-visceral functions were described. Two levels and eleven corresponding to them phases of damaged consciousness were singled out. The scale of quantitative estimation of each phase of the consciousness damage was established for practical use by the computer method of plural step by step regression; dynamics of phases was shown also.
The elasticity of the craniospinal system was examined in 26 patients with grave forms of cerebral stroke. A certain role of the relationships between the intracranial pressure and contents in the stroke pathogenesis is demonstrated. The initial and "optimal" CSF pressure, the elasticity gradient, and the index of the craniospinal system reserve utilization were found to be the most informative indications of the elastic properties of the craniospinal system. Criteria for individual evaluation of the degree of the intracranial hypertension compensation are determined. A possibility of compensating the intracranial volume increase by a diminution of the cerebral blood flow volume is demonstrated.
