[Compressed spectral EEG analysis of patients with acute cerebral circulatory disorders].

The use of a new examination method -- compressed spectral analysis of EEGs by way of computerized Fourier's transformation -- is described. The method was used for examining 43 patients with cerebral stroke. Real-time comparisons of EEGs and data of their compressed spectral analysis were made in healthy subjects and patients with cerebral strokes of various character. Data on long-time on-line registration of the EEG spectrograms in patients with the cerebral strokes are presented, and the time course of the changes of those spectrograms in patients with the ischemic and hemorrhagic stroke is followed up. Prognostic criteria of the EEG spectrograms in cerebral strokes of various gravity are determined.

[Compressed spectral EEG analysis in patients with consciousness disorders complicating stroke].

In 26 patients with stroke accompanied by consciousness disorders, 48 monitoring recordings of compressed spectral array of EEG were performed by means of computerised real-time fast Fourier transformation of initial EEG with digital assay of total spectral power and percentage of frequency components in the brain hemispheres. Bilateral increase in the total spectral power and in slow-wave spectral components correlated with diffuse alterations in the initial EEG. 3 types of spectrograms were distinguished in accordance with the degree of consciousness disorders. Possibilities and prospects for application of the new method in patients with stroke are discussed.

[Transoral ultrasonic flow metering of the vertebral arteries].

Using the method of ultrasonic flowmetering based on Doppler's effect with transoral positioning of the sensor occluding lesions of the vertebral arteries were diagnosed in 32 patients, in 22 of whom also the carotid arteries were involved. The method has proved to be effective in diagosing the subclavicular steal syndrome due to occlusions of the subclavicular arteries in 13 other patients. The agreement between the data of the transoral measurements of the blood flow in the vertebral arteries on the one hand, and the angiographic findings on the other was 81.1%. Disagreements were observed mostly in cases of stenosis of a vertebral artery combined with occlusions of the opposite vertebral artery.

[Allergic encephalomyelopolyradiculoneuritis from a wasp sting (clinico-pathologic report)].

The authors discuss clinico-pathomorphological observations of allergic encephalomyelopolyradiculoneuritis, due to wasp stings. A lethal outcome followed the 14th day of the disease. The clinical picture and morphological symptoms were characterized by allergic reactions of immediate and retarded types leading to severe lesions of the vital organs and the nervous system. Special attention is drawn to a selective lesion of the pallidonigral system conditioning the clinical picture of acutely developing parkinsonism. The polyradiculoneuritis and myelitical syndrome were also expressed.

[Rheologic properties of the blood in stroke patients].

The rheological blood properties were studied by means of a graphical registration of platelet and erythrocyte agregation, as well as the rotational viscosimetry in 163 patients in the acute period of hemorrhagical, ischemic and complicated ischemic strokes. Changes in the rheological blood properties detected a dependence on the character of the stroke, age of the patients and duration of the disease. The types of rheological blood properties, related to the character of the stroke are described. The role of rheological blood properties examinations in the study of the pathogenesis in brain strokes and differentiated therapy is stressed.

[Rheologic properties of the blood and lipid metabolism in patients with hemorrhagic infarct of the brain].

The clinical development, changes of the rheological blood properties and lipid metabolism were analysed in 70 patients with acute and subacute hemorrhagical infarctions into the hemispheres. Two clinical types in the development of hemorrhagical brain infarctions are described. The rheological blood properties and lipid metabolism had certain differences in comparison to patients with hemispheric hemorrhagical and ischemic strokes. They were also changed depending upon the age and duration of the illness. The pathogenesis of the changes detected is substantiated and their significance for microhemocirculation in patients with hemorrhagical brain infarctions is discussed.

[Ultrasonic flow metering in the diagnosis of occlusive lesions of the extracranial portions of the carotid arteries].

The paper is concerned with the results of a new method of investigation--ultrasound fluometry, based on the Doppler effect. This method was applied in patients with atherosclerotic lesions of the carotid arteries. The authors studied 57 patients in the acute stage of the ischemic stroke and 32 patients with chronic insufficiency of the brain blood supply. The report describes the principle of the ultrasound debitmeter action. Some features of a total and subtotal occlusion and stenosis of the extracranial part of the carotid artery and the features of collateral circulation were detected. The authors point of comparable data of ultrasound fluometry and contrast angiography, and the valuability of ultrasound fluometry for the determination of indications to angiographic studies. The advantage of the new methods is the possibility of its use in outpatient practice and in hospitals for evaluating the effectiveness of the used treatment as well as for observing cerebral hemodynamics during the postoperative period.

[Emergency medical services for acute cerebral circulatory disorders].

An analysis of annual reports of the station of ambulance and emergency medical aid during the past 6 years demonstrated a significant increase in the number of patients with acute cerebral circulation disorders due to reorganization of ambulance medical aid services. The authors discuss some scientific-methodological organizational problems of neurological emergency medical services, the tasks of specialized neurological teams questions of the diagnosis of brain stroke and the necessary level of eupplementary studies and treatment measures in the prehospital stage. A certain importance is allocated to a continuity of patient treatment in the hospital and perfection of the system of hospital care to patients with acute disorders of cerebral circulation.

[Semiologic, biochemical and electrophysiologic analysis of the stages of apopleptic coma].

The paper contains some results of a comprehensive clinical, biochemical and electrophysiological study of 120 patients with different stages of an apoplectic coma. The authors studied the gas content, acid-base balance of the cranial blood inflow and outflow in the CSF and changes of EEG and EMG. It was shown that there were different levels of CNS lesions (cortico-subcortical, meso-diencephalic, meso-pontile and bulbar level) which corresponded to certain correlations between clinical, biochemical, EEG and EMG findings. The authors describe a classificational scheme of 4 stages of an apopletic coma. Special significance in the pathogenesis of a coma along with structural lesions in the different areas of the CNS is attached to metabolic disturbances and extracerebral factors (respiratory and circulatory disorders).
