[The effect of extracorporeal shockwave lithotripsy on the indices of urinary enzymes in nephrolithiasis patients].

Immediate and long-term effects of extracorporeal shock-wave lithotripsy (ESWL) on urine enzymes were studied in 200 patients with nephrolithiasis (NL). The patients had abnormally high urine excretion of AP, GGT, APA, LDH and NAG, whereas aminotransferases excretion was low. Pathobiochemical and diagnostic implications of these findings are discussed. Immediately after its conduction ESWL entails a mild injury to tubular portion of the nephron. The causes of long-term stay of enzymuria after ESWL in NL patients are discussed.

[Role of hepatic monooxygenase activity study in optimization of immunosuppressive therapy and optimal use of ketoconazole in patients with transplanted kidney].

The aim of the study is to evaluate the activity of hepatic monooxidase enzymic system (MOS) in patients with chronic renal failure before and after transplantation of the kidney, to elucidate the relationship between MOS activity and clinical features of the disease, to validate effective dose of MOS inhibitor ketokonasole. MOS activity was measured by antipirine half-life. By initial MOS activity, three groups of oxidizers were recognized: fast, moderate, slow. Native population of the North and some ethnic groups prevailed among fast "oxidizers". MOS activity measured 2 months after transplantation of the kidney was characterized by a spontaneous fall of MOS activity, recovery of renal elimination. Fast "oxidizers" demonstrated greater frequency of acute rejection, less frequent episodes of cyclosporine nephrotoxicity, higher cumulative dose of steroid hormones and weak response to azatioprin. Effective dose of ketokonasole was within 100-200 mg/day under the same pharmacodynamic effect. It was found reasonable to measure MOS activity for decision about administration of optimal immunosuppressive therapy and ketokonasole.

[Morphological alternations and hemostatic disturbances as manifestation of mesenchymal dysplasia in nephroptosis patients].

The examination of 36 nephroptosis patients (33 females and 3 males, mean age 23 years) included assessment of hemostasis before and after nephropexia (15 patients), histological investigation of renal parenchyma biopsies (15 patients) and those of the skin from the lateral abdominal surface and musculus lumborum fascia (5 patients). Changes characteristic for an early stage of microcystosis were found in all the renal tissue biopsies. In the skin and muscular connective tissue there were dystrophic changes. Collagen fibers were undergoing elastoid degeneration. As to hemostatic disorders, there were diverse platelet dysfunctions and/or thrombocytopenias, 9 patients had low plasma level of Willebrand's factor, 28 showed impaired terminal clotting. Combination of the above defects was registered in 12 of 36 examinees. After nephropexia, platelet count and fibrinogen level noticeably increased, hemostatic disorders aggravated. Thus, nephroptosis is manifestation of connective tissue pathology associated with marked hemostatic changes.

[Changes in the enzyme level of the sperm of men with infertility].

Spermatic enzymes (fructose, alkaline phosphatase, gammaglutamyl transferase, alpha-glucosidase, dipeptydil aminopeptidase IV) were measured in 200 patients with secretory-toxic infertility. It is shown that in some variants of infertility changes in the levels of spermatic enzymes are compensatory. These changes may also reflect qualitative characteristics of spermatozoa.

[Effects of plasmapheresis and lasers on the state of urinary microflora in chronic pyelonephritis].

Bacteria isolated from the urine of 142 patients with chronic pyelonephritis (CPN) were examined for pathogenic properties of the strains (bacteriuria, hemolytic, proteolytic properties, urease, adhesive activity, antibiotic resistance, the ability to inactivate bactericidal activity of the serum) to control the effect of the treatment: antibiotics combined with plasmapheresis or antibiotics combined with laser radiation (intravascular, transcutaneous, or both). Combined application of intravascular and transcutaneous laser irradiation in multimodality treatment reduces the number of highly pathogenic strains as well as antibactericidal activity of the urine strains. It also promotes normalization of bacteriuria level. Plasmapheresis is inferior to laser radiation but ranks the second in efficacy of action on urinary microflora. Thus, use of efferent methods, especially transcutaneous plus intravascular laser radiation, plasmapheresis, in combined treatment of pyelonephritis decreases pathogenicity of urine strains and normalizes bacteriuria.
