The trial entered 41 patients with chronic renal failure. Group I (15 patients) received conventional therapy involving infusion detoxication, antibiotics, immunostimulators, anabolic agents. 26 patients of group II in addition to standard therapeutic measures underwent plasmapheresis. The effect was assessed by immunological and renal function parameters. Group I patients have benefited from conventional treatment, but signs of immunodeficiency remained: B-lymphocyte, IgG and IgA levels were lowered and elevated, respectively. Patients of group II improved clinically, got rid of uremia and immunodeficiency.
61 patients suffering from chronic pyelonephritis in the stage of active inflammation associated with either urolithiasis (67.4%) or prostatic adenoma (32.6%) were divided into three groups. Group 1 received conventional antibacterial treatment, group 2 conventional treatment + local laser radiation (LLR), group 3 received combined therapy including intravascular laser irradiation of blood (IVLIB). The treatments efficacy was controlled by urine seeding for microflora and its sensitivity to antibiotics, by immunological supervision. The patients were found to have immunological abnormalities in the form of cellular and humoral immunity suppression, neutrophil phagocytic hypoactivity, elevated levels of medium-molecular peptides nonresponsive to standard antibacterial therapy. Adjuvant use of LLR improved humoral immunity and reduced total urine bacterial count. LLR replacement for IVLIB resulted in a complete regression of immunological shifts and more pronounced reduction of urinary bacterial contamination.
After-treatment immune effects of therapy were assessed in 57 pregnant females with pyelonephritis. Eleven patients received standard antibacterial treatment (ABT), 25 patients ABT plus plasmapheresis, 21 patients underwent UV irradiation of blood and ABT. ABT alone resulted in improved general condition of the patients, in reduced renal inflammation, but it failed to remove completely signs of immunodeficiency, B-lymphocyte and Ig levels remained low. The combined treatment with UV blood irradiation and plasmapheresis promoted rapid disappearance of clinical and laboratory evidence indicating pyelonephritis. UV irradiation could not correct deficiency of cellular immunity, while adjuvant plasmapheresis led to a complete cure of immune deficiency symptoms.
Treatment outcomes are available for 502 patients with benign prostatic hyperplasia exposed to transurethral microwaves (TUMW) on Prostatron unit. Of the patients whose age ranged from 55 to 84, 328 had stage I disease, the rest stage II. 32 patients had previously undergone cystostomy. Acute urine retention was registered in 67 patients on day 1-4 after microwave therapy because of edema of the tumor. To remove the urine, cystostoma and catheter were placed in 18 and 49 patients, respectively. In half a year a positive effect occurred in 80% of cases. As microwave therapy relieves symptoms, increases the quality of life, improves urine flow and reduces the tumor size it can be recommended for the treatment of benign hyperplasia of the prostate.
4 rare cases are reported of adult Wilms' tumor in females aged 35-65. It had been developing subclinically for a long time and was detected at palpation or at abdominal ultrasonography. In the 35-year-old woman the weight of the removed tumor reached 3.5 kg. Wilms' tumor in the adults seems to run asymptomatically and, consequently, is likely to be diagnosed late.
The paper analyzes treatment outcomes in 72 patients with chronic pyelonephritis in the phase of active inflammation. The patients have received combined therapy including laser blood radiation to correct hemostatic defects. The latter manifested as DIC syndrome resistant to antibacterial treatment. Attempts to apply transcutaneous laser radiation (TLR) and intravascular laser radiation (ILR) showed that DIC syndrome may be cured only in the combined use of the above modalities.
Ultrasonic transrectal triplex investigation of the prostate with assessment of its echo structure and vascularization, hemodynamics in the vessels has been performed in 68 patients with benign prostatic hyperplasia (BPH). The examination was conducted before transurethral microwave therapy (TMT) and 2 weeks, 1, 3 and 6 months after it. 2 weeks after TMT ultrasonic picture of the prostate exhibited foci of BPH destruction, inflammatory edema of the peripheral tissues. Later, fibrotization of the prostate occurred. Reduced circulation in the prostate developed by the end of the 6th month.
Urinary levels of L-, Y-glutamyl transferase, alkaline phosphatase, L-leucine arylaminidase, lactate dehydrogenase, N-acetyl-beta-D-glucose aminidase, pseudocholine esterase, neutral L-glucosidase were examined in 76 urolithiasis patients. The activity of the above enzymes was found enhanced. This may be due to dysfunction of the tubular system. The content of L-leucine arylaminidase, N-acetyl-beta-D-glucose aminidase, L-glucosidase provide the most complete diagnostic information compared to the other enzyme tests.
