[Staghorn calculi of the kidney and ureter as a urologic complication ina child with cerebral palsy].

UNLABELLED: We present a child with cerebral palsy, treated for renal and ureteral staghorn calculi. Calculosis was associated with neurogenic voiding dysfunction. CASE REPORT: A 10 year boy was admitted to Pediatric Surgery Clinic in Poznań because of urolithiasis. In neurologic examination four-extremities spastic paresis and convergent strabismus was found. Intellectual contact with this child was very poor. Constant urine leakage from urethra was observed. In urine analysis pH < 7, leucocytes and erythrocytes, in urine culture Proteus vulgaris > 10(5). Ultrasonography revealed three staghorn calculi in right kidney. On the plain x-ray film three calculi in right kidney and two staghorn calculi in right ureter. In urodynamic evaluation in cystometry: low bladder compliance, no detrusor overactivity, no bladder sensation, "leak point" pressure 25-27 cm H2O. In profilometry incompetent urethral closure mechanism. All stones were evacuated during one operation. Postoperative course was uneventful. Risk factors in the treatment of patients with cerebral palsy are stressed. CONCLUSIONS: 1. Dysfunction of lower urinary tract is an important clinical problem in children with cerebral palsy. 2. Voiding dysfunction could be a risk factor in the development of urinary stones. 3. In the treatment of children with cerebral palsy multispecialistic team is needed.

[The role of psychogenic factors as a cause of urinary retention in a patient with lazy bladder syndrome].

UNLABELLED: We present a child with "Lazy Bladder Syndrome". In this case psychogenic factors appeared to play an important role in the development of the voiding dysfunction in her. This factors influenced also the course of the treatment. CASE REPORT: A 14 years old girl admitted to Paediatric Surgery Clinic in Poznań with urine retention lasting for 20 hours. The child came of the pathologic family, and did not live with its parents. In the anamnesis: irregular mictions with episodes of urine retention. In clinical examination we found only bladder reaching the umbilicus, from which after catheterisation we got 1300 ml of urine. Antibacterial agent and observation was prescribed. After next 12 hours she passed no urine and intermittent catheterisation was started. Ultrasonography was normal. In urodynamic investigation we found in voiding cystometry: high bladder compliance, low bladder sensation with F.D. at capacity of 1000 ml. With normal relaxation in the EMG, detrusor contractions were observed neither in lying nor in sitting position. There was no pathology in the cystoscopy. Consulting neurologist found no pathology. EEG was normal. Conservative treatment with intermittent catheterisation, alfa-blocker and cisapride was continued. Consulting psychiatrist prescribed Mianserin. No spontaneous miction was observed after three weeks of this conservative treatment. The girl was sent to sanatorium, where she continued therapy. After 10 day resting in sanatorium she started spontaneous micturitions. She refused further cystometric examinations. In five consecutive uroflowmetries she passed 280 to 780 ml of urine in one portion with high flow rates. CONCLUSIONS: 1. Both functional and psychogenic factors are important in the development of "Lazy Bladder Syndrome". 2. In patients with voiding dysfunction the results of urodynamic evaluation are the most important. 3. Multispecialistic team is needed in the treatment of children with "Lazy Bladder Syndrome".

Determination and pharmacokinetics of a furosemide-amiloride drug combination.

The study presents an accurate and precise HPLC assay for the determination of furosemide and amiloride in human specimens. Both drugs were extracted from human plasma with ethyl acetate; furosemide was extracted at pH 1 and amiloride at pH 12. While chromatographic separation conditions, i.e., column, mobile phase and flow-rate were the same for both investigated drugs, furosemide was detected using a UV absorbance detector, whereas amiloride, because of its very low therapeutic range, was detected with a spectrofluorimetric detector. The linearity of the furosemide and amiloride assays were confirmed over the range of 30-3000 ng/ml and 0.5-30 ng/ml, respectively. These concentrations correspond well with the therapeutic ranges of both drugs. The extraction recoveries, depending on concentration, exceed 80% for furosemide and 74% for amiloride. The reported methods were applied to pharmacokinetic investigations of the two compounds taken in form of a drug combination.

[Methods of stimulation of otolithic tract: diagnostic possibilities, critique of methods].

Our modifications of the methods of otolith organs examination were reported. One of these methods is eccentric rotation without the support of the body which was held in two groups of the persons: control group of 32 healthy persons and 48 patients with unilateral vestibular loss. The second method of otolith organs investigation was evaluation of them by producing dynamic optogravitational reactions and measuring an angle of the eyeballs deviation and time of illusion. Two groups--one consisting 102 healthy persons and second 91 persons with unilateral hearing loss were examined. It was stated that the angle of eyeballs deviation was reduced and time of illusion was longer in the group patients in comparison with healthy person.

[The usage of nasal drops Tyzine in otorhinolaryngology].

In this research the authors presented their experiences in the treatment of non-complicated and complicated acute mucous membranous rhinitis with application of shrinking nasal drops Tyzine as a locally affective remedy assisting the therapy. The aim of this research was the estimation of the efficiency of Tyzine drops with mucous membranous rhinitis, their effect on the efficiency of the ciliary transport and stating the probable side effects occurring after their application. 80 persons were observed--60 adults who were treated with 0.1% solution oftetrazoline hydrochloride and 20 children who were given 0.05% solution. To state the condition of the ciliary transport within mucous membrane, a saccharine test was conducted with all examined persons on the 1st and 5th-7th days of treatment and on the 21st day after completing the therapy. The results of the observation seem to be positive as for as the efficiency of nasal drops Tyzine under the stipulation that the drops are reasonably applied and according to the doctor's recommendation.

Simultaneous determination of spironolactone and its metabolites in human plasma.

This study describes a specific, precise, sensitive and accurate method for determination of unchanged spironolactone and its major active metabolites in human plasma. After one-step liquid-liquid extraction, analysis of the parent drug and its metabolites was performed in one chromatographic run, using a high performance liquid chromatography (HPLC) method with a programmed switchover of the UV wavelength. Spironolactone and 7 alpha-thiomethyl-spironolactone were detected at 245 nm, while canrenone and internal standard were detected at 280 nm. The column used was an S5 ODS2 (500 mm x 4.6 mm i.d.). The mobile phase was a mixture of acetonitrile-aqueous orthophosphoric acid (pH 3.4). Chromatographic separations were performed at 5 degrees C. The standard curves were linear over the range 10-400 ng ml-1 for spironolactone and 10-600 ng ml-1 for 7 alpha-thiomethyl-spironolactone and canrenone. The precision and accuracy of the method were confirmed by relative standard deviations below 10% for different concentrations, except for the concentration equal to the quantitation limit, where these parameters ranged from 12-15%. The recovery was above 80% for all investigated compounds and for the internal standard. The assay proved to be suitable for pharmacokinetic studies of spironolactone.

Influence of Ukrain on patients with surgically treated breast cancer (introductory remarks).

Studies were undertaken to evaluate the influence of Ukrain on the postoperative period in patients with breast cancer. The group under Ukrain medication was comprised of 10 patients in stage T1-3NO-1MO. The control group was comprised of 22 patients in the same clinical and laboratory staging as those treated with Ukrain in the first year of postoperative follow-up metastatic lymph nodes were observed in one patient from the control group. It is planned to enlarge the groups and to continue the observation up to the third year to obtain reliable and more statistically significant data.

Influence of Ukrain on patients with surgically treated breast cancer. Part I. Clinical and laboratory parameters.

Studies were undertaken to evaluate the influence of Ukrain on clinical and laboratory parameters in ten patients with breast cancer treated with the drug in the preoperative phase. The control group was composed of eight patients of similar age and advancement of the disease who did not receive Ukrain before mastectomy. Data from the present studies indicate that the drug is distinctly helpful from the surgical viewpoint by increasing tumour hardness and enlargement of metastatic lymph nodes. Furthermore, treatment with it is fully safe, with no side effects or allergic reactions.

Influence of Ukrain on patients with surgically treated breast cancer. Part II. Hormonal profile.

The present study was undertaken to evaluate the influence of Ukrain on the hormonal status of ten patients with breast cancer, treated in the preoperative phase by Ukrain. A control group consisted of eight patients of similar age and advancement of the disease, who did not receive the drug before mastectomy. The data were compared to those obtained in ten healthy women of similar age. The results indicate that Ukrain only slightly influences the hormonal dynamics and its destructive mode of action on neoplastic tissue does not significantly affect hormonal activities.

Influence of Ukrain on patients with surgically treated breast cancer. Part III. The immune system.

The present study was undertaken to evaluate the influence of Ukrain on immune parameters in ten patients with breast cancer, treated with the drug in the preoperative phase. The control group consisted of eight patients of similar age and advancement of the disease, who did not receive Ukrain before mastectomy. The data were compared with the parameters obtained in 15 healthy blood donors. It was found that treatment with Ukrain distinctly regulates cellular and, slightly, humoral immunity, non-specific immunity and also complement activity.

[Elastin-derived peptides and anti-elastic antibodies in children with recurrent or obstructive bronchitis].

The aim of the study was to examine the effects of bronchitis on elastin-derived peptides (EP) and antielastin antibodies (AEAb) in serum. Using an ELISA technique, these parameters were assessed in children aged 6 months to 15 years: 36 had recurrent bronchitis (RB), 34 had obstructive bronchitis (OB) and 37 were healthy children (C). A comparison of the levels of the investigated compounds in the acute phase (I) and stage without signs of diseases (II) was studied. The EP data show non-significant changes in the RB and OB groups in comparison with group C. Children with OB had significantly lower levels of AEAb in IgA, IgG and IgM class antibodies, while those with RB in the IgG class in comparison with group C in both examinations. In examination 1, AEAb in IgA and IgM classes were significantly lower in children with OB than with RB.

[Plasma elastase alpha 1-proteinase inhibitor complex in children with bronchitis].

The objective of this study was to assess the diagnostic usefulness of plasma levels of polymorphonuclear neutrophil elastase-alpha 1-proteinase inhibitor complex (E-alpha 1PI) in children with bronchitis. One hundred and seven children aged 6 months to 15 years were studied: 36 with recurrent bronchitis (RB), 34 suffering from obstructive bronchitis (OB) and 37 disease free-control group (C). Systemic inflammatory response by ESR, total leukocyte (L), polymorphonuclear count (PMN), alpha 1-proteinase inhibitor (alpha 1PI) and C-reactive protein (CRP) in the blood was monitored simultaneously. A comparison of the levels of the investigated indicators in the acute phase (I) and in the stage without signs of diseases (II) was carried out. Upon examination 1, about 90% of the patients in both groups had mean levels of E-alpha 1PI significantly elevated (p < 0.001) over the control group. There was no significant correlation between the E-alpha 1PI concentration and other analyzed indicators of inflammation. These results show that E-alpha 1PI may serve as a sensitive indicator for granulocyte activation during the acute course of the disease, even in neutropenia.

[Level of GM-CSF and IL-4 in serum of children with bronchial asthma].

Serum IgE, GM-CSF and IL-4 concentrations were measured in 35 children (3 to 18 years of age) with bronchial asthma in I and II clinical stage. Control group consisted of 10 healthy children (6 to 9 years of age) without any symptoms of inflammatory process. All measurements were performed by ELISA tests. The concentration of serum IL-4 was statistically significantly higher in children with bronchial asthma (0.06 ng/ml) than in the control group (0.01 ng/ml). Serum concentration of GM-CSF was lower in children with bronchial asthma than in control group (4.4 and 8.44 pg/ml respectively).

[The significance of rotation tests in diagnosis of vestibule and otolith].

The authors describe the most useful tests of rotatory stimulation of vestibular and otolith organs. Estimation of threshold of during the rotative excitatory test, acceleration-deceleration test and reactions post stimulation of otolith organs are represents. Examinations of otolith organs are exactly discussed. The physiological and pathological results of somatic "deviation" test are showed.

Captopril: determination in blood and pharmacokinetics after single oral dose.

The study describes a specific, precise, sensitive and accurate method for determination of unchanged captopril, an angiotensin-converting enzyme inhibitor, in human plasma. Captopril was stabilized by forming an adduct with p-bromophenacyl bromide and this adduct was measured by high-performance liquid chromatography with UV detection. The standard curve was linear over a range of 30-800 ng ml-1. The average yield of derivatization of the unchanged captopril was 73.6% and the recovery of captopril-adduct reached 93.1%. The limit of detection was 15 ng ml-1, while the quantitative limit was 30 ng ml-1. Inter- and intra-assay RSD was below 9%, but inter- and intra-assay accuracy was below 8%. On the basis of elaborated method, a single-dose pharmacokinetics in 12 men, in two doses (25 and 50 mg of captopril) has been investigated. The comparison of the pharmacokinetic parameters obtained from both doses of the drug have been made.

Evaluation of chromatographic methods for the determination of nifedipine in human serum.

Gas-liquid chromatography and high-performance liquid chromatography were compared for the identification and determination of nifedipine in biological samples and the elaboration of the optimum liquid-liquid extraction procedure. The determination limits were 2 and 10 ng/ml, respectively, and the detection limits were 1 and 5 ng/ml, respectively. The calibration graphs were linear in the ranges 2-300 and 10-500 ng/ml, respectively. Recoveries based on three different concentrations were 88.7-95.8% and 93.7-104.2%, respectively. Both methods are sensitive, specific and reproducible enough for pharmacokinetic studies and therapeutic drug monitoring.
