Ileo-sigmoid knotting: a clinical study of 11 cases.

Ileo-sigmoid knotting is an uncommon lesion that has a high mortality (27.3% in this study). This is due to the rapidly deteriorating systemic disturbances that can follow gangrene involving both the ileal and sigmoid loops (63.6% of cases had gangrene of both loops in this series). Isolated gangrene of the sigmoid colon is uncommon. The recommended treatment consists of resection and anastomosis of the small bowel, and a Mikulicz type exteriorisation of the gangrenous sigmoid. However, Hartmann's procedure is recommended when gangrene has extended down to the upper rectum.

Cecal volvulus in children.

Volvulus of the cecum is very uncommon in children. Of 189 children operated upon from 1970-1977 for acute intestinal obstruction, in only six children, was cecal volvulus the cause. The etiology and treatment and factors affecting mortality are discussed.

Infarction of the greater omentum.

Infarction of the greater omentum is an uncommon entity in children. The etiology of this disease is unknown. It is difficult to differentiate this condition from acute appendicitis in children. Two cases from the pediatric age group are reported.

Sigmoidal volvulus in childhood: report of two cases.

Volvulus of the sigmoid colon is a very uncommon cause of acute obstruction in children. Although common in adults in India, it was found to account for only 0.8 per cent of all acute obstructions in infants and children in this institution. It causes a proximal torsional obstruction of the colon with an acute onset of symptoms. The onset of volvulus is characterized by colicky pain over the left lower quadrant, vomiting, tenderness, and rigidity in te left lower quadrant. A scout film of the abdomen may be inconclusive, but a barium-enema examination is diagnostic. The number of cases reported is too small to allow conclusions about the best treatment for children who have sigmoidal volvulus.

Primary antiphospholipid syndrome in stroke in the young.

Antiphospholipid syndrome is an uncommon cause of stroke. A 12 year old girl with this syndrome is reported who presented with thrombotic stroke and high titres of anticardiolipin (aCL) and lupus anticoagulant (LAC). The patient improved subsequently and was put on aspirin. The present report highlights the importance of screening for aCL and LAC in cases of stroke in young patients.

Leucocytoclastic vasculitis as a presentation of adenocarcinoma rectum.

Vasculitis has been linked to several processes, like infections, drugs and allergic, rheumatologic and neoplastic diseases. Neoplasm-associated vasculitis described in the medical literature has mostly been reported in association with haemotological neoplasms. Adenocarcinoma of rectum presenting as leucocytoclastic vasculitis is rare. We present a case of a 43-year-old male with paraneoplastic leucocytoclastic vasculitis preceding the manifestation of adenocarcinoma rectum. The vasculitis subsided on resection of the rectal malignancy and the patient did not require steroid therapy thereafter.

Fine needle aspiration cytologic diagnosis of the solitary cold thyroid nodule. Comparison with ultrasonography, radionuclide perfusion study and xeroradiography.

Thirty-six cases of solitary and scintigraphically "cold" thyroid nodules were studied by fine needle aspiration (FNA) cytology, ultrasonography, radionuclide perfusion study (RPS) and xeroradiography with the aim of differentiating the neoplastic from the nonneoplastic nodules. Histologic study of the excised specimens provided the definitive diagnosis in all cases. Of the techniques used in this study, FNA cytology and RPS had the highest sensitivities and specificities. Ultrasonography and xeroradiography were of limited use due to their low sensitivity rates.
