[Patient's reaction to surgical trauma in surgical treatment of duodenal ulcer].

Evaluation of metabolism and state of central hemodynamics in 134 patients with duodenal ulcer before the operation and at the early postoperative period was carried out. It was established that pronouncement of postaggressive response of an organism to trauma in surgical treatment of a duodenal ulcer didn't depend on a type of the operation due to use of the modern methods of anesthesia. Therefore, moderate pronouncement of such a response after the organ-preserving operations can't be a criterium for the wide use of surgical treatment of an uncomplicated duodenal ulcer.

[The prospects for using extracorporeal detoxication methods in disaster medicine].

A lot of specific injuries caused by catastrophes provoke endogenic intoxication. In this situation the application of extracorporal detoxication methods will be conditioned by pathogenetical mechanisms of their action. There are indications for ED management in mechanical injuries, prolonged squeezing effects, thermal injuries, acute poisoning, irradiation injuries, acute infectious diseases, acute reactivity state. The experience of 163 operations on 111 wounded in catastrophes proves the worth-while character of work in this sphere. 10 variants of extracorporal operations including combined methods were applied.

[Sudden death after an operation for gall stones].

During 10 years 3000 patients were operated upon for chronic calculous cholecystitis. Twenty of these patients died during 30 days after operation, in 16 of them death resulted from incompetence of the vital organs. Endotoxicosis and polyorganic insufficiency after technically correct operations for chronic calculous cholecystitis are thought by the authors to be a variant of anaerobic infection with possible development of sepsis.

Cause of Death↗

[Preventive use of immunostimulants in patients with infectious endocarditis].

Leakadine++, levamisole++, proper-myl were used in the postoperative period in 36 patients operated upon for infectious endocarditis with regard for the character of the immunopathology found. The directed immunostimulation results in resolution of immunodeficiency and normalization of the immune status of the patients.

[An evaluation of conductometric methods for determining circulating blood volume].

An analysis of the theory and practical check-up of the methods of determination of the circulating blood volume by the conductometric methods in 86 surgical patients with different phases of compensation of disturbance of water-electrolyte metabolism have shown them to be incompetent. The authors propose a new way to search for and develop a noninvasive impedance measurement method of determination of the extracellular fluid on the basis of Thomasset principle corresponding to possibilities and specific features of national rheographic apparatus.

Blood Volume↗

[Hemodynamic effect of pericardiectomy].

The method of integral rheography was used for the examination of 126 patients with different forms of pericarditis. The authors have established different nature of the blood circulation insufficiency (heart compression and myocardiodystrophy), which should be taken into consideration in choosing the methods of treatment.

Chronic Disease↗

[Clinical importance of changes in thrombocytic hemostasis in lung cancer].

Clinico-laboratory examinations of 86 patients have shown that lung cancer develops against the background of activation of the thrombocytic link of hemostasis which manifested itself in thrombocytosis, greater degree of destruction of the lysosomal membranes, lower energy resources of the cells, their tendency to slow and little reversible aggregation. The authors consider that the indicators of the ADP-induced aggregation of thrombocytes may be used for prognosis of postoperative pyo-inflammatory and thromboembolic complications. HBO is good for correction of alterations in the thrombocytic link of hemostasis in lung cancer and its suppurative complications.

[Hemodynamic indices in refractory anemia subsequent to lung resection].

Results of the investigation of hemodynamics in 208 patients three days after resection of the lungs are presented. Reduction of the stroke volume and cardiac output was established as well as increased respiration energy, expansion of fluid volume which increased with reduction of oxygen capacity of blood despite the unaltered blood volume and were independent of the volume of surgery. To prevent these alterations of special significance are the restoration of oxygen blood capacity with the help of hemotransfusions.

[Prevention of wound infection after pulmonary resection of acute suppurative disease].

The appearance of suppuration of the operation wound is real after resection of lungs for a restricted and spread gangrene despite prophylactic use of antibiotics and washing of the wound with antiseptics. Early diagnosis of local suppuration of the wound, effective drainage of the focus and increase of non-specific resistance give a way out of the situation. Choice of diagnostic criteria of suppuration and a method of correction of healing the thoracotomic wound are presented.

Combined Modality Therapy↗

[Hemodynamic and homeostatic effects of the isoperistaltic preparation of the large intestine in colorectal surgery].

Whole-gut irrigation was used in 471 observations in 411 patients. High efficiency of the method of preparation of the intestine for colorectal procedure or endoscopy was shown. Whole-gut irrigation does not exert a substantial influence on vital functions and homeostasis of the patients. Main contraindications for using the method of a rapid purgation are the following: colonic obstruction, severe arterial hypertension, considerable cardio-pulmonary incompetence and renal insufficiency.

[Intravenous monoanesthesia and antianesthetics in emergency surgery].

Profiles of using the intravenous mononarcosis (sodium hydroxybutyrate, viadryl , ketamin , sombrevin, seduxen) in urgent surgery and traumatology are analyzed. Choice of certain narcotics is motivated for patients with blood loss and shock, intoxication, insufficiency of kidneys, adrenals and liver, cardio-vascular and respiratory disorders. The problem of antinarcotics is considered with reference to the efficiency of specific (bemegride, gutimine , amtizol , cytochrome "C") and nonspecific ( osmodiuretics , infusion media containing thawing water) antinarcotics . A preliminary assessment of the efficiency of different drugs of antinarcotic action is given.

[Expansion of the lung after partial resection for gangrene].

On the basis of the experience with the treatment of 69 patients with lung gangrene the authors recommend to use a combination of active aspiration from the pleural cavity, occlusion of the segment bronchus and pneumoperitoneum for the efficient expansion of the lung after its resection in the early postoperative period.

[Major trends in the treatment of patients with septic shock].

The authors formulate main tasks to be solved which will result in successful treatment of septic shock in the surgical clinic. Choice of the methods affecting the septic focus is determined including measures from the rational systemic antibacterial therapy to interventions on the focus with its ablation or draining. The infusive and medicamentous programs are formulated, debatable questions in the treatment of such patients are discussed on the basis of the personal experience: the application of vasoactive drugs, corticosteroids, heparin.
