[The lipoproteic state in patients with coronary heart disease (author's transl)].

The lipoproteic state of 28 patients with coronary heart disease (CDH) and 44 normal subjects (total N. 72) has been analyzed. In normal subjects the results showed an increase of triglyceride (TG), VLDL with age. For subjects between 20 and 40 years of age, higher values of HDL and lower RF were observed in women than in men. In CHD patients, between 40 and 60 years, higher values of total cholesterol (TC) were observed in women than in men (p less than 0,05). The comparison between male CHD patients and normal male subjects indicated a reduction of HDL and an increase of the risk factor (RF) (p less than 0,05) in coronary patients. However, the female patients showed lower levels of HDL in combination with higher values of TC, LDL, RF, and TG when compared with the control group.

[The antithrombophilic complex of the arterial wall in the therapy of obliterating arteriopathies in lower limbs (author's transl)].

For 30 patients affected by obliterating arteriopathy in lower limbs, the use was made of an antithrombophilic complex of the arterial wall (aorta) consisting of both anti-coagulating and fybrinolytic principles. The results obtained, as regards both subjective and objective symptomatology, clearly showed a considerable increase in the hematic flow in the limbs affected by arteriopathy. Also the flow of the collateral circulation was improved. At the same time, a meaningful reduction in the abnormal cholesterol values was found. The preparation showed no antigenic activity. As or the oral administration, this preparation proved to be an effective one. Tolerance was completely satisfactory.

[An isometric exercise for the diagnosis of chronic obliterating arteriopathies of the lower extremities].

A non invasive method for the diagnosis of occlusive diseases of the peripheral arteries and the follow-up of therapy is proposed. The method consists in the performance of an isometric effort of the inferior limbs and the Doppler detection of the dorsalis pedis systolic pressure. 41 patients with different degrees of peripheral artery involvement have been studied and the results compared with those obtained in a control group of 12 normal subjects, equally matched for ages and sex. In the normal subjects the maximal dorsiflection of the feet for 30 seconds caused a mean increase of the heart rate of 20% and no changes of the systolic blood pressure. In the patients with peripheral artery disease the isometric effort caused, contemporary to the increase of the heart rate, a decrease of the dorsalis pedis systolic pressure, that varied from 15 to 100% according to the degree of the arterial involvement. A close correlation was found between the degree of the percent reduction of peripheral systolic pressure and the entity of the arterial involvement, as evidenced by other invasive and non-invasive methods. The method proposed appears useful for a proper diagnosis of peripheral vascular diseases in those patients who cannot undergo invasive investigations or the treadmill effort test.

Plasma levels of cyclophosphamide in patients under polychemotherapeutic regimens.

Plasma levels of non metabolized CPA were measured in two groups of lung cancer patients receiving polychemotherapy. There was extreme variability in the data obtained during the first hour after CPA administration. After this period of time a wide range of T 1/2 was found particularly in one of the two groups of patients. "Border-line" patients (with T 1/2 at the upper or lower limits of the range found) are discussed as regards the possible influence of ancillary treatment.

Bone metastases from breast cancer treated with calcitonin. Case report.

A case of bone metastases from breast cancer is reported. After 6 months of therapy with calcitonin, the skeletal radiological examination showed an evident change in the roentgenographic pattern of a large metastasis of the left femur. A possible relationship between the calcitonin treatment and the radiological change is discussed.

Respiratory response to carbon dioxide after propranolol in normal subjects.

The ventilatory response to carbon dioxide, using the rebreathing technique, was investigated in 5 healthy nonsmoker volunteers, without obstructive bronchopathy. The administration of propranolol (20 mg) in a single oral dose did not produce significant modifications of the slopes of the response curves, but caused a significant increase of the intercept of the curves (p less than 0.05). Since no changes of the spirographic values were noted, the results obtained were attributed to a decrease of ventilation. It is concluded that propranolol, even at the dose of 20 mg, is able to induce a depression of the respiratory center, concomitant with significant reduction of heart rate and arterial blood pressure.

[Effort test on a treadmill in peripheral arteriopathic obstructions (author's transl)].

20 males, 48 to 66 of age, whose 10 normal and 10 with chronic arteriopatic obstruction at inferior limbs, were submitted to effort on a treadmill for 5 minutes or until pain. The clinostatic systolic aterial pressure was obtained in the four limbs before and at fixed intervals of time from the exercise. Arterial pressure increases after effort in the normal limbs; the length and the amount of the systolic pressure decrease after exercise are in correlation with the degree of the arterial involvement. The exercise on treadmill seems useful when the clinical and other diagnostic data are inadequate for a reliable diagnosis and to evaluate the progression of the arteriopathy through a non-invasive, and easy to repeat, method. The possible mechanisms of the pressor decrease in the arteriopatic limbs are discussed.

[Reflex pressor response in young subjects with labile hypertension (author's transl)].

A study on the pressor response to various tests of sympathetic stimulation has been performed in 36 young subjects, 9 with labile hypertension and 27 normotensive. All have been studied with "tilt test", "cold pressor test" and "mental arithmetic stress". In the labile hypertensive subjects the "tilt test" induced increases of the systolic and diastolic pressure significantly higher than in the normotensives. The "cold pressor test" produced an increase of the arterial pressure and of the heart rate significantly lower in the labile hypertensive than in the normotensive. The pressure responses and the changes of the heart rate produced by the "mental arithmetic stress" did not show significant differences in the two groups of patients studied. The results are discussed.

Primary carcinoma of the colon and hyperlipemia: a paraneoplastic syndrome.

The human counterpart of the association of hyperlipidemia with cancer is rare, as compared with the relative frequency of the syndrome in experimental animals. A case is presented of adenocarcinoma of the colon with a non casual relationship between the presence and the progression of the tumor and hyperlipemia. Surgical resection and antiblastic chemotherapy moreover seemed to influence the metabolic anomaly.
