1. Acute cytogenetic effects expressed as sister-chromatid exchanges (SCE) in peripheral blood lymphocytes were investigated after in vitro exposure to low vibration regimes (5-30 Hz. at 0.1 cm amplitude) for 20 min intervals and compared to non-vibrated control cultures. 2. There was no significant (P < 0.05) increase in the range of SCE incidence at any vibration frequency compared to the mean value of control; 6.88 +/- 0.11 to 7.97 +/- 0.22 versus 6.83 +/- 0.86, respectively. 3. When cultures were incubated for 1 h in the presence of 0.5 microgram ml-1 mitomycin C (MMC; the positive control) significant elevations in the SCE frequency were observed (19.81 +/- 0.67), indicating the sensitivity of the system to in vitro SCE induction. 4. Other cell parameters, such as mitotic index and cell proliferation index were statistically similar in the experimental cultures and the negative control. 5. The observed differences in the investigated end-points are considered within those typically found in vitro. This suggests no relation between low frequency vibrations and cytogenetic effects.
BACKGROUND: Very large cervical polyps are rarely reported. CASE: A giant cervical polyp, 17 x 10 x 5 cm, protruded through the vaginal introitus in a 27-year-old, sexually inactive woman. CONCLUSION: Although carcinomatous change occurs in 1.7% of cervical polyps, malignant degeneration did not occur in the six reported cases. Thus, biopsy of these tumors before excision may not be necessary.
To evaluate the possible cytogenetic consequences of exposure of human populations to petrochemicals, the frequency of chromosomal aberrations was studied in blood lymphocytes obtained from 233 persons working at different locations in a large oil refinery (184 exposed and 49 worksite controls) and from 47 individuals who served as community controls. Chromosomal aberrations occurred with frequencies (mean +/- SEM) between 0.023 +/- 0.013 and 0.037 +/- 0.011 breaks per cell (1.56 +/- 0.87% to 2.66 +/- 0.54% aberrant cells) in the exposed groups. These values, with the exception of those observed in two workshops (crude oil unloading as well as petroleum tanker washing and lubrication), were significant compared to the presumably spontaneous (control) levels (0.015 +/- 0.004 to 0.021 +/- 0.007 breaks per cell and 1.17 +/- 0.29% to 1.46 +/- 0.40% aberrant cells). Cigarette-smoking was not associated with increased chromosomal aberrations in the exposed and in the control groups.
OBJECTIVE: To evaluate pregnancy outcome following uterine unification procedures in patients with uterine anomalies. METHODS: A retrospective survey included all abdominal metroplasty procedures performed on 43 patients at the American University of Beirut Medical Center between January 1, 1974 and December 31, 1991. Age at metroplasty, type of anomaly and surgical procedures, as well as preoperative and postoperative reproductive performance were all recorded. RESULTS: Forty (93%) out of 43 patients who underwent metroplasty had postoperative live births compared with five (12%) prior to surgery. The fetal wastage rate dropped from 93% pre- to 16% postoperatively. All seven patients with a history of primary infertility conceived and had live births. CONCLUSION: Our data suggest a remarkable improvement following abdominal metroplasty in patients with both typical and uncharacteristic preoperative reproductive performance.
The use of alkylating agents has been reported to be a cause of secondary leukemia particularly in patients surviving for 2 years or more after treatment of ovarian carcinoma. The risk of developing leukemia is possibly related to the duration of treatment and the total dose of alkylating agents administered. We report a patient with epithelial ovarian carcinoma who received chlorambucil for 130 consecutive months with no clinical or laboratory evidence of leukemia.
The genotoxic effects of inorganic fluorides were investigated by treating cultured rat bone marrow cells with varying concentrations (0.1-100 microM) of potassium fluoride (KF) and sodium fluoride (NaF) for different durations (12, 24 and 36 h) and measuring the incidence of cells with aberrations and number of breaks per cell. Both forms of fluoride were found to be weak mutagens relative to the positive control N-methyl-N-nitro-N-nitrosoguanidine (MNNG). A specificity of fluoride ion in inducing chromosome aberrations (CA) was indicated by the observation that both NaF and KF behaved almost equivalently in this study and at significantly higher variations from the results with potassium chloride (KCl) and sodium chloride (NaCl).
The stability of relationships between harem stallions and mares (consort relations) was investigated and the durations of inter partum intervals were determined in order to establish if there was any correlation between the stability of consort relation and reproductive success of mares in Misaki feral horses. Thirty-four mares were observed continuously for more than 5 years. The lifetime stability was 80-100% (mean 92.4%) for 16 mares, 60-79% (mean 70.4%) for 10 mares and 0-59% (mean 27.9%) for 8 mares. The continuous length (years) of specific consort relations was 2-10 years and was found to correlate significantly with lifetime stability. There was a significant positive correlation of lifetime stability with lifetime reproductive success for 34 mares observed, and the correlation was higher when the data of 8 wandering mares (<60% in lifetime stability) were omitted. The mean +/- s.d. delivery interval of 25 stable mares was 364.5 +/- 11.0 days, whereas that of 8 unstable mares and stable mares who changed stallions was 387.0 +/- 40.2 days. There was a significant difference between delivery intervals of stable and unstable mares. Significant correlations between the stability of consort relations and both the foaling rates and delivery intervals suggest that mares may obtain major reproductive advantages if they maintain long term and stable consort relations with a particular stallion throughout their reproductive life span.
AIM: To describe the thin CT scans findings in AIDS patients with intrathoracic Kaposi's sarcoma (KS). MATERIAL AND METHODS: Fifty-three CT scans of patients with KS were retrospectively reviewed. The diagnosis of intrathoracic KS was established histologically (n = 17) or on the association of skin KS and the visualization of characteristic endobronchial lesions (n = 36). CT scans were performed with thin slices (2 mm) obtained at 10-mm intervals, and a 512 x 512 reconstruction matrix. No patients had Pneumocystis carinii pneumonia within the 3 months preceding the CT scan examination. RESULTS: Numerous nodules (n = 42), tumoral masses (n = 28), bronchovascular pathways thickening (n = 35), and pleural effusions (n = 28) were the most frequent patterns. Septal lines (n = 15), ground-glass opacities (n = 3), and mediastinal adenopathies (n = 8) were not frequent. CONCLUSION: Numerous nodules, tumoral masses, bronchovascular pathways thickening, and bilateral pleural effusions were the main signs of intrathoracic KS; their association (66%) is very characteristic. An opportunistic infection or mycobacteriosis must be sought if the thin CT scans reveal ground-glass opacities and/or mediastinal adenopathies.
The present report describes a 27-year-old woman diagnosed with Ann Arbor stage IE primary malignant lymphoma of the cervix. Radiation therapy was instituted as conventionally advocated at this early stage of the disease. Despite adequate local tumor response, central nervous system metastasis occurred a few months later. She succumbed to her condition 7 months after initial diagnosis. As occult distant foci may not be amenable to detection by available imaging techniques at the time of initial work-up, combination chemotherapy alone or in conjunction with radiation therapy may be the most appropriate first line of therapy in patients with stage IE primary malignant lymphoma of the cervix.
A case of fibroepithelial polyp arising from the labium majus is described. The tumor measured 12 cm in its largest diameter and was connected to the left labium majus by a 12-cm pedicle. The microscopic examination showed hypocellular connective tissue and focal myxoid areas. One year following surgical excision, the patient did not manifest any signs of recurrence. This case is very unusual in that almost all reported vulvar fibroepithelial polyps are small and sessile.
Solitary nerve sheath tumors such as benign schwannomas arising in the pelvic retroperitoneum are infrequently reported. We report a case of a benign retroperitoneal pelvic schwannoma that presented with pelvic pain and an adnexal mass. Complete surgical excision was achieved only after transection of the S1 nerve root on the left side. The adjacent vascular and urinary channels sustained no injuries and the patient had minimal neurologic deficit.
The diagnosis of leptomeningeal relapse in a patient with epithelial ovarian cancer was confirmed by the presence of malignant ovarian cells in the cerebrospinal fluid. There was no clinical evidence of tumor spread elsewhere. Therapy, including intrathecal methotrexate and whole-brain irradiation led to transient clinical improvement. International literature review revealed only 13 other cases of leptomeningeal carcinomatosis in epithelial ovarian cancer; all died within 15 months following the diagnosis of leptomeningeal spread.
The effects of sodium and potassium fluoride (NaF and KF) at concentrations ranging from 10(-7) to 10(-2) M for 12, 24, or 36 h on cultured rat bone marrow cells have been studied with respect to cytotoxicity and induction of sister-chromatid exchanges (SCE). At the three exposure times, cell survival progressively decreased with increasing concentrations. Treatment with 10(-2) M fluoride resulted in a statistically significant death (62-65%) of cells. Similarly, no dividing cells were encountered at concentrations of 10(-3) M and 10(-2) M, and significant reductions in mitotic index (MI) were calculated at 10(-4) M. In contrast, cell kinetics, expressed as cell proliferation index (CPI), revealed no significant inhibitory effect of fluoride on cell proliferation. Furthermore, the mean SCE score reached a maximum (7.64 SCE/cell) in the 24-h-treated cultures. This value was not significantly different from that observed in sodium chloride (NaCl) at 10(-2) M (5.42 SCE/cell) and distilled water (4.86 SCE/cell) controls. In comparison, mitomycin-C (MMC, positive control) at 5 x 10(-8) M caused an average of 22.13 SCE/cell. These results indicated an inhibition of cell division and death of cells with high doses of fluoride with no effect on SCE frequencies.

In order to assess the potential genotoxic effects of occupational exposure to petrochemicals, the incidence of sister-chromatid exchanges (SCE) in cultured lymphocytes was studied. Blood samples were taken from 233 individuals (184 exposed and 49 worksite controls) in an oil refinery and from 47 community control persons. The data showed a non-significant elevation of SCE frequency in occupationally exposed workers when compared to non-exposed individuals. The mean SCE frequency per cell ranged from 7.55 +/- 0.55 in blood of lube oil blending and canning (LOBC) workers to 9.13 +/- 0.71 in catalytic cracking and water treatment (CCWT) workers. The control values were 6.2 +/- 0.67 and 7.21 +/- 0.45 in the community and worksite individuals, respectively. Furthermore, the SCE frequencies were influenced neither by age nor by smoking.
Since chest X-ray and CT scan features of Kaposi's sarcoma (KS) are nonspecific, we wanted to test the hypothesis that the histological components of this tumour and/or the associated haemorrhagic component, may result in a characteristic signal pattern on magnetic resonance imaging (MRI). Thoracic MRI was performed in a prospective manner in ten patients with acquired immune deficiency syndrome (AIDS) and pulmonary KS. MRI examinations (1.5 Tesla) included Spin-echo T1 (SE-T1), before and after gadolinium injection, as well as T2-weighted sequences (SE-T2). For each sequence the signal intensity of lesions was measured and compared with each other as well as with the signal intensity of muscle. Results were compared to the hemosiderin content of macrophages in the bronchoalveolar lavage (BAL) in all patients and with histological findings in three. The results were compared to values obtained in a control group of seven patients with pneumocystis carinii pneumonia. SE-T1 showed focally increased signal intensity in the pulmonary parenchyma (n = 5). Signal enhancement in parenchymal lesions (n = 10) and along peribronchovascular trees (n = 5) was observed after gadolinium injection. The second echo of SE-T2 showed a markedly reduced signal intensity in pathologic areas (n = 10). This last finding was not observed in the control group. In conclusion, we have identified a pattern of MRI signal abnormalities suggestive of Kaposi's sarcoma. The MRI signal intensity of KS lesions may be related to the angiomatous and fibrous components of the tumour.(ABSTRACT TRUNCATED AT 250 WORDS)
Edema is a frequently encountered medical sign in pregnancy. Edema confined strictly to the labia, however, is not as common. This report describes severe labial edema in a patient with a twin gestation while on intravenous magnesium sulfate tocolysis. The tocolysis could have initiated the process of labial swelling in the presence of other, predisposing factors.
