[Alterations of nucleoli and nucleolar organizers in proliferative cystitis and transitional cell carcinoma of the urinary bladder].

The values of the nucleolar and granular rates as well as the number of Ag-granules were increased during the process of malignancy of transitional epithelium in proliferative chronic cystitis and transitional cell bladder carcinoma, including invasive tumours at both ultrastructural and light microscopic levels. The results may serve as additional criteria in the assessment of the risk of bladder mucosa malignancy and prognosis of the transitional cell bladder carcinoma.

[The efficacy of lanikor in treating patients with chronic cor pulmonale].

Cardiac glycoside lanicor produced in Yugoslavia ("Pliva") was administered to 43 patients with decompensated chronic cor pulmonale (DCCP). Blood concentrations of the drug given at a dose of 0.25 mg were evaluated over 24 h, in peak saturation, during therapy with maintenance doses. A daily loss of therapeutic activity of the drug was established. The total, mean and maintenance doses were determined. Tetrapolar rheography and right ventricular kinetocardiography found lanicor-related changes in the central and pulmonary hemodynamics. A positive cardiotonic response in DCCP patients appeared in 80.2% of those treated. A significant relationship existed between serum concentrations of lanicor and enhancement of myocardial contractility. The therapeutic concentration ranged from 1.2 to 1.9 ng/ml. A mean effective dose equalled 2.0 mg, whereas a recommended average maintenance dose 0.6 mg. The elimination coefficient reached 30%. Resistance to the drug occurred in 19.8% of the patients in the lack of correlation between the therapeutic dose and clinical effect.

[Effect of interferon on the course of experimental E coli-induced pyelonephritis].

The effect of the type I interferon on the development and process of experimental pyelonephritis caused by E. coli was studied on mice weighing 12 to 14 g. Interferon was administered intraperitoneally in a dose of 1000 units on days 3 and 7 of the disease. It was shown that the administration of the type I interferon to the mice with experimental pyelonephritis promoted rapid elimination of bacteria from the kidneys, prevented their penetration to the contralateral (intact) kidney, prevented marked macro- and microscopic damages in the kidneys, lowered the intensity of the inflammatory reaction, and increased the phagocytic activity of neutrophils and the number of the E-rosette-forming lymphocytes in the thymus. The data provided experimental grounding for clinical trials of interferon preparations in treatment of bacterial pyelonephritis.

[The diagnostic criteria of aspermia].

A study is presented of the clinical, endocrinological, morphological indices in 126 patients with sterility. In testicular aspermia the androgenic-estrogenic balance reduced in all kind sterility independent of the clinical signs of hypoandrogenization and degree of disorder of spermatogenesis. A correlative dependence was revealed between the level of follitropin in the blood plasma and degree of inhibition of spermatogenesis in secretory sterility.

Adult

[The morphological changes in the seminal vesicles in prostatic sclerosis].

A study is presented of morphological changes of the seminal vesicles removed during prostatectomy in 51 patients with sclerosis of the prostate and histological findings of the seminal vesicles were compared with roentgenological signs on the vesiculograms and clinical manifestations of the disease. Results indicate that the seminal vesicles undergo significant pathological changes in the development of sclerosis of the seminal vesicles requiring in some cases their surgical removal for restoration of the urodynamics in cystic-urethral segment.

[Monoclonal antibodies in the diagnosis of human tumors].

The literature reviews present the evidence that it is feasible to diagnose human tumors by applying the latest immunohistochemical techniques mainly to tissue sections. There is also evidence that monoclonal antibodies to various extracellular matrix components, cytoskeleton antigens and some specific and non-specific neoplastic antigens are employed for this purpose.

[Principles of clinical morphology in nephrology].

Causes of the numerous changes found in kidney biopsy specimens and the absence of their commonly accepted classification are revealed. Principles of clinical morphology underlying nephrology have been formulated which emphasize the need to study the qualitative and quantitative characteristics of all renal structures, maintenance of the nosological independence of primary glomerulonephritis whose diagnosis must be supplemented by the morphological characteristics of kidney biopsy specimens. Kidney affection in different diseases should be designated by the term "nephropathy" specifying the causative factor and the character of affection of renal structures.

Biopsy

Ultrastructural diagnostic markers of the urinary bladder precancer.

A morphological analysis of 346 biopsy specimens from 140 patients with chronic cystitis permits one to evaluate the proliferative forms of the disease, the cases with atypical hyperplasia of the urothelium or metaplasia of the squamous cells with acanthosis and hyperkeratosis of the group at risk, belonging to bladder precancer. The early ultrastructural signs of urothelial atypia are determined, among them the thickening of the glycocalyx layer (up to 30-60 nm) irregularly covering the entire surface of atypical cells, the disappearance of vesicular apparatus and AUM, the widening of the intercellular spaces. It was found that the squamous epithelium, formed by atypical cytodifferentiation of urothelium, is inadequate for performing the barrier function inherent in normal urothelium.

[Morphologic changes of the kidneys in patients with vasorenal hypertension].

A study is presented of the state of blood vessels and parenchyma of the kidneys in patients with vasorenal hypertension. Morphological changes of the renal parenchyma at different stages of the disease are described. The authors compared clinico-morphological findings and give practical recommendations for the improvement of diagnosis of the disease and timing of surgical treatment of vasorenal hypertension.

Biopsy

[Characteristics of morphogenesis of cryptorchism in childhood].

The main morphological features of undescended testes in children of 2-14 years old were incomplete cytodifferentiation of Sertoli cells (absence of transition of Sa = Sb cells into Sc type), reduced number of spermatogonia and dystrophic changes in germ cells. Intense progressive sclerosis developed in the interstitium and walls of seminiferous tubules. The dark Sertoli cells were described as a distinct cell type for the first time. The results of the morphological study indicated that cryptorchism should be surgically corrected during the first two years of life.

[Effects of chemotherapy on regional metastases of non-seminomatous tumors of the testis].

A comparative evaluation of histologic patterns of non-seminomatous testicular tumors was carried out in 2 groups of patients: those receiving platinum derivative-based preoperative chemotherapy (31) and chemotherapy-free cases (36). Preoperative cytostatic treatment potentiated cell differentiation in retroperitoneal metastases in some patients, the latter effect being considered prognostically favorable.

[Precancerous importance of chronic cystitis and papilloma of the bladder].

Analyzes the findings of histologic studies of biopsies of a pathologically changed and visually intact bladder mucosa from 334 patients with chronic cystitis with nonproliferative, proliferative, and metaplastic changes (n = 164), papillomas (n = 62), and carcinomas (n = 108). Demonstrates the relationships between these conditions and their evolution over a follow-up of 36-72 months. Outlines the most typical patterns of their development and progress eventuating in noninvasive or various forms of invasive carcinoma.

Biopsy

[Fibronectin, its role in the processes of tissue differentiation, regeneration and neoplastic transformation].

The data from literature on certain biological and functional peculiarities of fibronectin which is a high molecular connective tissue glycoprotein of the intercellular matrix are reviewed. Various features of fibronectin, such as its promoting action in cell adhesion, participation in cytodifferentiation, embryogenesis and regeneration are discussed. Several points of view about fibronectin participation in neoplastic transformation are mentioned. Diagnostic and prognostic value of the quantitative and qualitative estimation of plasma fibronectin in the human tumour investigation are emphasized.

[Morphology and histogenesis of yolk sac tumors in the testis].

The differentiation of yolk sac tumor cells was similar to the processes of development of embryonic and extraembryonic endoderm during the first 7 wk after cleavage. Two ways of yolk sac tumor histogenesis are discussed: differentiation of embryonal carcinoma cells, and redifferentiation of the polypotent undifferentiated anaplastic seminoma cells. Histogenesis features described may be responsible for aggressive nature and high malignancy of yolk sac tumors.

Cell Differentiation

[Histogenesis of human embryonal cancer of the testis].

Complex morphological study of 45 germ cell tumors of the testis revealed two types of histological structure of embryonal carcinoma. Tumour histostructure was related to the ultrastructural features of cells (I-IV types) forming them. The data obtained indicate that the process of differentiation of these tumours resembles initial stages of embryogenesis (morula, blastula, egg cylinder). Embryonal carcinoma cells appeared to be tumor stem cells in the development of teratoma, chorionepithelioma, yolk sac tumour.

Embryonal Carcinoma Stem Cells

[Electron microscopy in the diagnosis and prognosis of transitional-cell bladder tumors].

The electron-microscopic analysis of 65 different transitional-cell tumours of the urinary bladder revealed quantitative correlations between the tumour cells belonging to different groups, types and forms of transitional-cell papilloma, transitional-cell carcinoma and undifferentiated carcinoma of the bladder. These correlations are of importance for histogenetic and differential diagnosis of these tumours. The correlation between the changes in cell composition of transitional-cell papilloma or transitional-cell carcinoma and the subsequent clinical course of the disease (prognosis) is determined.
