Caffeine-containing beverages, total fluid consumption, and premenstrual syndrome.

The main objective of this study was to evaluate whether daily consumption of caffeine-containing beverages is related to the prevalence and severity of premenstrual syndrome apart from any effects of daily total fluid consumption. A secondary objective was to determine whether daily total fluid consumption itself is related to premenstrual syndrome. The study is based on 841 responses to a questionnaire probing menstrual and premenstrual health, and daily fluid consumption, which was mailed to female university students in Oregon. Analysis of the data revealed that consumption of caffeine-containing beverages was strongly related to the prevalence of premenstrual syndrome. Among women with more severe symptoms, the relation between consumption of caffeine-containing beverages and premenstrual syndrome was dose-dependent, with prevalence odds ratios equal to 1.3 for consumers of one cup of a caffeine-containing beverage per day and increasing steadily to 7.0 for consumers of eight to 10 cups per day. The effects were apparent among both caffeine-containing tea/coffee consumers and caffeine-containing soda consumers. The observed effects were only slightly reduced when daily total fluid consumption was controlled. Daily total fluid consumption also was related to the prevalence of premenstrual symptoms although the effects were large only for consumers of 13-19 cups of fluid per day (the largest amount studied).

[Dilated cardiomyopathies in children].

Dilated cardiomyopathies occur mostly in infants and affect their vital and functional prognosis. They may be primary or secondary to various pathologies such as congenital cardiac malformation, toxic myocardial drugs or mucoviscidosis. Medical treatment may only be transiently effective and brutal or progressive aggravation may lead to cardiac transplantation being considered in those cardiomyopathies which are primary (without metabolic causes) or chronic secondary. Echocardiographic survey is the most efficient way to follow the evolution and to detect complications (cardiac thrombosis).

Employment status and the frequency and causes of burn injuries in New England.

We analyzed a subset of data from the New England Regional Burn Program (NERBP) to assess the effects of employment status on the rates and causes of burn injuries in New England. The subset of the NERBP data analyzed pertained to residents of the six New England states who were hospitalized for the treatment of a burn injury sustained between July 1, 1978 and June 30, 1979 and who were aged 20 years or older at the time of the injury. Analysis of the data identified that men, particularly black men and young men, experienced higher burn rates than did their female, white, and older counterparts among both employed and unemployed persons. High burn rates were observed in Maine; low rates were observed in Rhode Island. Scalds were the most common type of burn among work-related burns to women and to men aged 20 to 54 years, and flame or flash burns were the most common otherwise. The majority of work-related burn injuries were caused by activities related to food preparation or consumption, motor vehicle repair and maintenance, and use of flammable liquids. Persons employed as operatives and laborers, or persons employed in the service occupations, appeared to have the highest risk of sustaining a burn injury while at work.

Tea and premenstrual syndrome in the People's Republic of China.

We evaluated the hypothesis that tea consumption causes premenstrual syndrome by studying 188 nursing students and tea factory workers in the People's Republic of China via questionnaires distributed in classes (nursing students) or during routine physical examinations (tea factory workers). Analysis of the data revealed that tea consumption is strongly related to the prevalence of premenstrual syndrome and that the effects are dose-dependent.

[Left pulmonary artery sling associated with right pulmonary hypoplasia and ventricular septal defect].

The pulmonary artery sling is a congenital malformation where the left pulmonary artery forms a vascular sling that bends around the right border of the lower trachea. Its clinical manifestations are signs of tracheal compression in the first months of life. The diagnosis of this rare anomaly is suspected on radiological and echocardiographic grounds and confirmed by oesophagography, tracheoscopy and angiocardiography. Diagnostic problems may be encountered when the condition is associated with tracheobronchial (50% of the cases) or cardiovascular malformations. In the case reported here respiratory symptoms and heart failure were present in a 1-month hypotrophic infant who also had ventricular septal defect and dextrocardia due to right lung hypoplasia. The pulmonary artery sling was diagnosed by angiography. The severity of the clinical signs precluded all attempts at surgical repair of the cardiovascular anomalies. The child died at the age of 2 1/2 months.

[Treatment of essential hypercholesterolemia by normolipidemic drugs in children].

The authors list the principal normolipidemic drugs. Only cholestyramine (Questran) and some fibrates (fenofibrate: Lipanthyl for instance) are registered for pediatrics. Cholestyramine acts by sequestering biliary acids, and thereby inhibiting intestinal cholesterol resorption. It is the primary drug used for children, when diet has been unable to restore normal cholesterol values.

Simulations of enhanced malaria transmission and host bias induced by modified vector blood location behaviour.

Monte Carlo simulations were developed to assess the potential impact of parasite pathology on vector salivary function as well as of host haemostasis on transmission. Assuming that a proportion of desisting vectors switch host following failure to locate blood, we demonstrate three possible consequences: (1) infected vectors contact more hosts than non-infected host, (2) non-infected vectors are biased to infected hosts, independently of attraction, and (3) an exponential relationship exists between parasite load and transmission. We discuss possible epidemiological implications.

[Atheromatous lesions of the proximal aorta. Severe complications of homozygous type IIa hypercholesterolemia in children].

Coronary lesions with atheromatous deposits occurring in later childhood characterize homozygous type IIa hypercholesterolaemia and condition the somber prognosis of a disease which affects one subject in a million. However, aortic lesions are constantly found, as shown by routine ultrasonographic and angiographic studies in these children. The walls of the proximal aorta are cardboard-like and thick, the origin of the aorta is narrow and the semilunar aortic valves are thickened. The valvular or supravalvular aortic gradient may be considerable; it is often progressive, but is sometimes stabilized or made regressive by medical treatments combined with plasmapheresis or porto-caval shunt. Aortoplasty or aortic valve replacement being difficult to perform in these patients, more aggressive therapeutic procedures, such as liver or heart transplantation, have been suggested. The last generation cholesterol-lowering drugs seem to offer some hope of success.

[Restrictive ostium secundum: a new fetal malformation syndrome].

Echocardiographic examination of a 23 weeks' hydropic fetus disclosed abnormal kinetics of the valve of the foramen ovale. This valve was constantly bulging out, dome-like, into the left atrium, and on TM-mode tracings in transatrial projection the alpha and beta peaks which occur respectively during the opening and closure of the atrioventricular valves throughout fetal life were missing. This anomaly suggested that the ostium secundum was restrictive; the foramen ovale itself was not restrictive. Such abnormal kinetics have not been encountered among 16 other cases of hydrops fetalis of cardiac or other origin, or in a control series of 81 normal fetuses, which clearly shows that the restriction was primitive. At birth, the child presented with aneurysm of the foramen ovale, probably due to the restrictive ostium secundum.

Video display terminal use and reported health symptoms among Massachusetts clerical workers.

Few well-controlled studies have assessed the extent and magnitude of adverse health conditions reported by video display terminal (VDT) operators. To evaluate the association between typical, daily VDT use and the prevalence of adverse conditions related to vision, musculoskeletal conditions, headaches, and colds or sore throats, we conducted a cross-sectional, epidemiologic study of 1,545 Massachusetts clerical workers. The study results indicated an increased prevalence of adverse conditions pertaining to vision, musculoskeletal discomfort, and headaches among clerical workers who used VDTs. The magnitude of the increased prevalence was dose dependent. The effects related to musculoskeletal discomfort and headaches were higher among VDT workers in computer and data processing services, public utilities, and the Commonwealth of Massachusetts than among workers in banking, communications, and hospitals. These latter effects, however, may have been due to random variation.

Epidemiology of work-related burn injuries in Massachusetts requiring hospitalization.

To describe the epidemiology of hospitalized, work-related burns among Massachusetts residents aged 16 years or older, 825 Massachusetts residents who were burned between 1 July 1978 and 30 June 1979, and who were treated as hospital inpatients in any of 240 New England hospitals, were studied. Two hundred forty (29%) of the 825 identified burns were known to be work-related. Our findings suggest that the workplace is a major contributor to hospitalized burn injury among employed males, accounting for 60% of their burns. Young workers and black workers experienced the highest rates of burning, with rates two and four times higher than their older and white counterparts, respectively. Scalds were the most common type of work-related burn injury for individuals in each age category, accounting for 45% of the burns overall.

Hospitalized burn injuries in Massachusetts: an assessment of incidence and product involvement.

We assessed the frequency of hospitalized burn injuries in Massachusetts, and product involvement in causing burns, by reviewing the hospital inpatient records and emergency room logbooks for 240 of New England's 256 acute-care hospitals. Children less than two years of age, males, and Blacks experienced higher burns rates than did older individuals, females, or Whites. Products frequently associated with burn injuries included those involved in food preparation and consumption, flammable liquids, and clothing.

Consumer products and hospitalized burn injuries among elderly Massachusetts residents.

This study is based on data for Massachusetts residents who were burned between July 1, 1978 and June 30, 1979 and who were treated as hospital inpatients in any of the 240 hospitals participating in the New England Regional Burn Program. Demographic information and information regarding the nature of the burn injury and consumer products involved were obtained by review of the medical record for each case. One hundred seven (9%) of the 1237 burn victims identified were aged 65 years or older, yielding a burn incidence rate equal to 15.5 burns per 100,000 person-years compared with a rate of 23.8 burns per 100,000 person-years for younger Massachusetts residents. The burn mortality rate for the elderly who survived long enough to be hospitalized was 2.6 deaths per 100,000 person-years compared with a rate of 0.4 deaths per 100,000 person-years for younger individuals. For survivors, the average length of hospital stay for elderly victims was 28.6 days compared with 12.0 days for younger individuals. Clothing ignition, stoves and ovens, smoking materials, and bathroom showers and tubs contributed significantly to the causes of burning among the elderly. In addition, evidence was found suggesting that elderly patients receive medical care relatively later after injury, thus considerably complicating their illness.

Caffeine-containing beverages and premenstrual syndrome in young women.

I evaluated the hypothesis that consumption of caffeine-containing beverages causes premenstrual syndrome by studying 295 college sophomores. Data about premenstrual syndrome and beverage consumption were collected by a self-administered questionnaire. Analysis of the data revealed that consumption of caffeine-containing beverages is strongly related to the presence and severity of premenstrual syndrome and that the effects exist for most of the premenstrual symptoms studied.

[Bourneville's tuberous sclerosis of neonatal disclosure. Apropos of 2 cases].

A cardiac murmur was found in a newborn, after 12 hours of life. It was related to an intracardiac tumor, and we made the diagnosis of tuberous sclerosis. The same diagnosis was made in another patient, with a tumor diagnosed by obstetrical echography. In these two cases, seizures occurred rapidly during the evolution. In one case, the heredity was dominant, in the other one, it was a "de novo" mutation. It is unusual to diagnose tuberous sclerosis during the neonatal period, so we report these two cases.

[Extrinsic allergic alveolitis in children. Apropos of 4 cases].

Four cases of extrinsic allergic alveolitis are reported. About them, we discuss: The difficulties of diagnosis that we can only affirm on the association of several clinical, biological, and radiological arguments. The physiopathology: association of immune complex hypersensitivity type III and immediate hypersensitivity type I. It's important to always search for a probable starting infectious factor (mycoplasma). The therapy: antigenic exclusion is the only way to avoid a lung fibrosis.

A measure of the extent of burn injury based on fluid resuscitation.

This study was undertaken in an attempt to develop an objective method of measuring the severity of burn injury not subject to the personal bias of the observer. The classic method of assessing burn size--the estimation of the percentage body surface area burned--provides a systematic error peculiar to each observer so that it is not possible to compare results from one burn treatment centre to another. To assess the accuracy of a new objective method for measuring the extent of burn injury--the estimation of fluid retention over the first 48 h after burning--and to compare the new method with the classic method, we measured net fluid retention and the percentage body surface area burned for 52 patients who were treated at the Massachusetts General Hospital or Shriners Burns Institute, Boston Unit, between November 1968 and July 1980, and compared these measurements with clinical scores indicating the extent of burn injury. Analyses of the data showed that net fluid retention is an accurate measure of the extent of burn injury, and approximately equivalent to the percentage body surface area burned when the estimates of percentage burn are made by a small, consistent clinical team in a single burn care system.
