[Distribution of collagen in human livers].

The distribution of collagen (determined as hydroxyproline) is compared with the histological index of fibrosis in 32 cadaver livers. The results indicate a partly heterogeneous distribution of collagen between three topographic different regions of the liver. The possible reasons for the different distribution of collagen were discussed and compared with data of the literature. Our results suggest that one biopsy only is not representative for the whole organ in every case.

Pharmacokinetics of iron salts and ferric hydroxide-carbohydrate complexes.

Pharmacokinetic measurements of ferrous sulphate and ferric hydroxide-polymaltose complex on anaemic and non-anaemic rats have shown different postabsorption serum iron levels, invasion and elimination constants and distribution volumes for the two preparations. But nevertheless the absorption and utilization ratios of iron from both preparations are equal. It is therefore not justified to calculate utilization ratios for chemically different iron preparations on the basis of postabsorption serum iron measurements. Similar pharmacokinetic differences of these two iron preparations could also be found in clinical trials, which means that different serum iron increase results in an equal utilization ratio for both preparations. It is clear that ferrous sulphate and ferric hydroxide-polymaltose complex must have a different absorption mechanism. The postulated absorption mechanism is in agreement with all the facts known today and does explain the pharmacokinetic difference between the investigated preparations.

[The fluorescein dilaurate serum test following metoclopramide and secretin stimulation for evaluating pancreatic function. Contribution to the diagnosis of chronic pancreatitis].

A new method of exocrine pancreatic function testing is described. After oral administration of a standard meal containing fluorescein dilaurate (FDL) metoclopramide (10 mg) and secretin (1 U/kg) are given as intravenous bolus injection. The best discrimination between normal and abnormal pancreatic function was found to be the individual peak serum fluorescein concentration within 180 minutes. 184 patients were studied and divided into 4 groups according to the final diagnosis: group 1: 44 healthy controls; group 2: 87 patients with chronic pancreatitis; group 3: 21 patients with different pancreatic diseases; group 4: 32 patients with different gastrointestinal disorders. Sensitivity of the FDL serum-test in detecting chronic pancreatitis was 87%, specificity was 97%. Considering patients with different gastrointestinal disorders specificity dropped to 77%. FDL serum-test revealed superior as compared to the FDL urine test. Correlation between FDL serum-test and secretin ceruletide test was significant (r = 0.72, p less than 0.001).

A binary vector for transferring genomic libraries to plants.

The transformation of mutant plants with a complete recombinant library derived from wild-type DNA followed by assay of transformed plants for complementation of the mutant phenotype is a promising method for the isolation of plant genes. The small genome of Arabidopsis thaliana is a good candidate for attempting this so-called shotgun transformation. We present the properties of an A. thaliana genomic library cloned in a binary vector, pC22. This vector, designed to introduce genomic libraries into plants, contains the oriV of the Ri plasmid pRiHR1 by which it replicates perfectly stably in Agrobacterium. Upon transfer of the library from E. coli to A. tumefaciens large differences in transfer efficiencies of individual recombinant clones were observed. There is a direct relation between transfer efficiency and stability of the recombinant clones both in E. coli and A. tumefaciens. The stability is independent of the insert size, but seems to be related to the nature of the insert DNA. The feasibility of shotgun transformation and problems of statistical sampling are discussed.

Renal effects of aluminium in uraemic rats and in rats with intact kidney function.

The effect on renal function following administration of aluminium (i.p., five times weekly (0.05 or 0.5 mg kg-1 body weight) for 12 weeks) to partially nephrectomized (5/6 NX) or intact female rats was examined. The observed loss of concentrating ability, characterized by increased urine volume and an increased sodium excretion, as well as increased renal excretion of p-aminohippurate (demonstrable after low-dose treatment with nephrotoxins) can be interpreted as an initiation of kidney function injury. No distinct differences in response were observed between partially nephrectomized and intact animals.

Correlation of clinical signs with CT findings in patients with cerebellar disease.

The severity of cerebellar signs and the degree of cerebellar atrophy depicted by computed tomography (CT) were independently graded in 108 patients with cerebellar disorders. The overall agreement between these independently scaled measures was only 28%. In patients with involvement of the cerebellar hemispheres and anterior lobe, clinical signs tended to be more pronounced than the cerebellar atrophy revealed by CT. The opposite was true for patients with lesions of the caudal vermis. Patients with Friedreich's ataxia had no or only minor CT abnormalities. Close correlation between the degree of infra- and supratentorial atrophy was found only in chronic alcoholics. The poor correlation between changes in cerebellar structure detected by CT and clinical disability suggests the need for caution in CT interpretation.

Effect of bumetanide on potassium transport and ionic composition of the arterial wall.

Bumetanide (B) (1 mM) decreased K content of dog carotid arteries (Ka) by 11-15%, K turnover (lambda 2) by 31%, and steady-state K fluxes (JK) by approximately 48%. The drug also reduced intracellular (ic) Cl, H2O, and occasionally ic Na. The half-maximal inhibitory concentration of B on JK was close to 40 microM. However, 10 microM still fully reduced ic K, Cl, and H2O but not ic Na. Replacement of external Cl by sulfate or nitrate mimicked the B effects on JK and reduced its capacity of inhibition by approximately 40 and approximately 80%, respectively. Replacement of Na by choline decreased Ka and JK by 90 and 96%, respectively, and rendered B totally ineffective. B also decreased Cl uptake and content of cultured vascular smooth muscle and ic Cl and H2O of whole arteries. Ouabain (Ou) (1 mM) decreased Ka 90%, accelerated lambda 2 four- to fivefold, and reduced JK 45%. Addition of B or removal of external Na or Cl in the presence of Ou returned lambda 2 to normal levels and reduced the residual JK by approximately 90% but failed to further decrease the Ka. According to these data, B mainly inhibits a coupled KCl passive self exchange plus a smaller active net KCl influx. Ou abolished the active component without affecting the passive self exchange. The presence of Na is necessary for the operation of the KCl-coupled self exchange, and Na gradients may provide the energy for the uphill KCl movements.

[The value of the free hydroxyproline and the N-acetyl-beta- glucosaminidase for the observation of the course of malignant tumors].

In 172 patients with various malign tumours the free hydroxyprolin (HP) and N-acetyl-beta-glucosaminidase (beta-NAG) in the serum were determined. The established values were analysed according to the kind of tumour, spread of tumour, behaviour of growth and histological classification and statistically evaluated. In mamma carcinoma progressively metastasing into bones HP is significantly increased in its mean value. 71.4 per cent of the patients have pathological values. The parameter for bone-marrow diagnostics failed in 28.6 per cent. Increases may only occur until 9 weeks before traditional diagnostics of metastases. beta-NAG values lie within the normal range for clinically not metastasing mamma carcinoma. The enzyme level will increase significantly after each tumour progression into an organ. The diagnostic sensitivity for the general diagnostics of metastases amounts to 97.7 per cent. Increases without any evidence of metastases are either due to other diseases with connective tissue changes or to occult micrometastases. An exact control is required for these patients. Likewise, HP and beta-NAG have a prognostic importance for malign testicle tumours from nonseminomes character and malign lymphomas, because with tumour progression they will turn into pathological ones. Cytostatics have no direct impact on the serum level of HP and beta-NAG.
