Study of prior biopsies of endometrial cancer patients and controls.

Prior endometrial biopsies were studied for evaluation of the relationship between hyperplastic endometrial changes and the development of endometrial cancer. Our findings were: A trend for the histologic diagnosis to be more severe in the reexamination. Endometrial hyperplasia preceeded cancer in 20 out of 21 cases. The severe hyperplastic lesions were found 1 to 6 years prior to the diagnosis of endometrial cancer.

[An epidemiological study of endometrial cancer. Controlled trial of 501 cases (author's transl)].

In a retrospective case control study, 501 cases of endometrial cancer were observed from 1955-1975 and an equal number of matched control cases were compared regarding the following parameters: obesity, hypertension, diabetes, age a menopause, parity, age at first and last delivery, age at menarche, menstrual abnormalities, history of prior endometrial biopsies, concomitant ovarian tumours, family history of cancer and history of exposure to radiation. The statistical analysis revealed significant differences regarding the incidence of obesity, hypertension, late menopause, low parity, prior endometrial biopsies and prior exposure to ionizing radiation. However, no significant difference was found between endometrial cancer patients and control patients as to the incidence of diabetes, age at menarche, age at first delivery and family history of cancer. The results of the investigation are discussed and compared with similar reports from the literature.

[Papillomatous changes in the cervix uteri].

A case of condyloma acuminatum of the uterine cervix and a case of papilloma on the cervix with concomitant adenocarcinoma of the cervix are described. The diagnostic biopsy revealed only the papillomatous lesion, whereas the adenocarcinoma was diagnosed in the surgical specimen after hysterectomy. Referring to the studies of Meisels et al. on condylomatous lesions of the cervix, the possible frequent incidence of this cervical lesion and its role as one possible first step in the pathogenesis of cervical cancer are discussed. The in toto excision is recommended because of diagnostic and therapeutic reasons. Clinical, colposcopic and cytologic check-up has to be done carefully for years after primary treatment.

[Examination of two sampling methods (pistolet and jet washing) for endometrial tissue (author's transl)].

The Pistolet-Aspiration and Gravlee Jet-Wash Techniques were used to examine 112 patients at random and the efficacy of each technique compared. The Pistolet-Aspiration technique is more often applicable and significantly provides sufficient material for cytologic and histologic evaluation. The histologic diagnosis was compared with the histologic diagnosis of the uteri after hysterectomy in all cases. In consideration of our results, the easier practicability and because of its economy we prefer the Pistolet-Aspiration technique versus Gravlee Jet-Washing. A catalogue of indications for Pistolet-Aspiration is also given in the discussion.

[Vaginalcytology and late pregnancy (determination of term, fetal monitoring, correlation with hormon patterns) (author's transl)].

The "before term" pattern allows to exclude over-maturity with probability of 95--100%. The "at term" pattern lets expect spontaneous labour in next five days just so with great probability. We attribute no importance to cytology as a method of control for high risk pregnancies. At last we didn't find any correlation with statistical methods between the alterations of the cell patterns some days before delivery and values of hormons.

[The problem of Caesarean section (author's transl)].

The Caesarean sections at the Universitäts-Frauenklinik Innsbruck from January 1st, 1969 to December 31st, 1974 have been surveyed. Related to the total number of 12 579 deliveries 627 Caesarean sections have been performed. Two 3-year-intervals have been compared: 1969-1971 (group I) without and 1972-1974 (group II) with modern practices of prenatal supervision. There was a high significant increase of the section rate in group II (5,8% versus 4,1% which has been related to the increasing number of mainly fetal indications. The maternal morbidity didn't rise with the increasing section rate. A significant decrease in total infant mortality rate and mortality rate at vaginal deliveries has been found with the increasing incidence of Caesarean sections; whereas the number of depressed infants (Apgar score less than 7) was increased in group II. The reasons therefore have been discussed. The rise of the number of Caesarean sections in group II performed because of breech presentation (14,2% versus 6,2%) was correspondent with a statistically significant decrease in the mortality rate from 11,7 to 4,1%. A Caesarean section rate between 7% and 10% seems to be the limit at least in our hospital--the total infant mortality rate cannot be reduced by further increasing the section frequency: the infant mortality rate possibly could be lowered by a better and more wide spread prenatal care especially by taking more care for prematurity and by intensifying the perinatal period.

[Urinary oestrogens and vaginal cytology in late pregnancy (author's transl)].

72 vaginal wall smears were taken from 51 women in late pregnancy. The following indices were evaluated: Eosinophilia, pyknosis, Crowded cell and Folded cell index. These indices were compared with urinary oestrogen levels. Statistical evaluation showed no relation between these indices and the total oestrogens in the urine. The Aetiology of the oestrogenic appearance of the smear some days before delivery in spite of constant oestrogen values is discussed.

[Problem of circulation disorder in the inferior vena cava in newborn infants].

The authors present a case of congenital obliteration of the inferior vena cava in a newborn baby. The obliteration was distal to the confluence with the hepatic veins. In addition, there was obliteration of the renal and iliac veins as well. Pathogenesis, clinical manifestations and prognosis were discussed in more detail. The development of the venous system and diagnostic and therapeutic possibilities were shortly referred to.

Cytology at the end of pregnancy: significance of determinations of the eosinophilic and karyopyknotic indices.

The clinical usefulness of the cytologic findings "pre-term," "at term" and "post-term" are discussed briefly. For this investigation, the eosinophilic and pyknotic indices were obtained by counting one hundred cells in each of four microscopic fields. A separation function was calculated using the eosinophilic and pyknotic indices. The results of our study permitted a highly significant separation of the collectives "pre-term" and "at term."
