Efficacy of four antiviral agents in the treatment of uncomplicated herpetic keratitis.

OBJECTIVE: To evaluate the efficacy of four antiviral agents--1% idoxuridine ointment (group 1), 2% trifluorothymidine ointment (group 2), 3% acyclovir ointment (group 3) and 1% bromovinyldeoxyuridine (BVDU) ointment (group 4)--in herpes simplex keratitis. DESIGN: Randomized double-blinded clinical trial. SETTING: Tertiary care institution in New Delhi. PATIENTS: Eighty patients with uncomplicated herpes simplex keratitis of recent onset who had not previously received antiviral treatment. MAIN OUTCOME MEASURES: Cure rate, frequency and severity of side effects. RESULTS: Cure rates of 60%, 90%, 90% and 95% were obtained in groups 1, 2, 3 and 4 respectively. The average healing time was 13.4, 8.9, 8.5 and 7.5 days respectively. Side effects (follicular conjunctivitis, epithelial keratopathy and stinging) were more frequent in group 1 than in the other groups. CONCLUSIONS: BVDU has a more pronounced therapeutic effect than idoxuridine, trifluorothymidine and acyclovir in uncomplicated epithelial herpetic disease of recent onset that has not previously been treated.

The effects of corticosteroid and reduced glutathione on the endothelial viability during culture in MK medium. A light microscopy study.

The effect of addition of dexamethasone and reduced glutathione to MK medium, in a concentration of 10(-3) Molar and 0.92 g/l, respectively, was evaluated. The post preservation corneal endothelial viability was assessed using light microscopy. Our results showed that the addition of dexamethasone to MK medium increased the preservation time of dog cornea from 7 to 10 days and that reduced glutathione could further increase the storage period to 12 days. It can be concluded that the addition of reduced glutathione to MK medium is an excellent means for intermediate term corneal preservation.

Efficacy of cryotherapy in vernal catarrh.

Cryotherapy of the palpebral conjunctiva with oral aspirin has been evaluated in mixed-type active vernal keratoconjunctivitis. The symptoms and signs were recorded on a four-point scale. In 15 patients, we used cryotherapy of the palpebral conjunctiva in 30 eyes (-60 degrees C to -80 degrees C for 30 seconds, repeating the freeze-thaw cycle 2-3 times). All patients received oral aspirin 0.5 to 1.5 g in three divided doses daily over six weeks. The relief from symptoms was statistically significant (P less than .001). Objective improvement (palpebral and bulbar signs) also was statistically significant (P less than .001). Follow-up of these cases for one year showed a 3.3% recurrence rate.

Large-scale plant cell culture: methods, applications and products.

Recent significant contributions to the design of large-scale plant cell cultures for secondary metabolite production include: the development of a strategy to control the concentration of dissolved gases at constant shear; a surface immobilization technique to retain cell mass at high mixing rates; and a modified stirred-tank reactor with a mesh cage to prevent damage of hairy root cultures.

Neodymium:yttrium aluminum garnet laser anterior capsulotomy.

Anterior capsulotomy of the lens capsule is one of the important steps before extracapsular lens extraction. A neodymium:yttrium aluminum garnet laser anterior capsulotomy was done in 144 eyes with immature, mature, and hypermature senile cataracts. This noninvasive technique is a safe and useful way to treat immature cataracts. To achieve the best result, the time interval between laser capsulotomy and surgery should be within six hours. Flap types of capsulotomy give the best results.

Critical evaluation of therapeutic keratoplasty in cases of keratomycosis.

Therapeutic keratoplasty was done in 123 neglected cases of fungal ulcers that did not respond to maximal medical therapy. It is recommended that, if the surgery is contemplated, it should be done before the ulcer perforates. A 0.5mm larger donor graft than recipient bed is better than a similar-sized graft. Various complications and final results are discussed.

Prognosis of keratoplasty in viral keratitis.

We reviewed 45 eyes with active or inactive herpetic keratitis that underwent penetrating keratoplasty. In Group I, 15 eyes underwent penetrating keratoplasty without prior optical iridectomy. In Group II, 15 eyes underwent penetrating keratoplasty after prior optical iridectomy. In Group III, 15 eyes underwent penetrating keratoplasty with pupilloplasty after prior optical iridectomy. Graft clarity of 3+ or more was obtained after one year in 67%, 33%, and 60% of eyes in Groups I, II, and III, respectively. Visual outcome of 6/24 or better was found in 53% of all eyes during the one-year follow-up. The occurrence of cataract both pre- and postoperatively could have been a result of prolonged use of corticosteroids and/or uveitis. Although there were several variables apart from preoperative vascularization and cataract, prior optical iridectomy was identified as a risk factor with regard to post-operative complications, final visual acuity, and graft clarity.

The influence of cerebellum on carbon dioxide responses in the dog.

The influence of the cerebellum on the baseline ventilatory responses while breathing oxygen and O2-CO2 mixture was investigated in mongrel dogs anaesthetized with sodium pentobarbital. Ablation of the anterior lobe of the cerebellum significantly depressed both the baseline and CO2 responses. By contrast, the posterior lobe ablation did not produce any significant change. The results indicate that the anterior lobe of the cerebellum may exert a facilitatory effect on respiration. The exact nature of the underlying mechanism is not known, but the anterior lobe of the cerebellum may directly or indirectly modulate the activity of the vagal afferents.

Results of penetrating keratoplasty in vascularized corneas.

A retrospective longitudinal study of 105 penetrating keratoplasties for vascularized corneal opacities was done. The results of cases with an etiology of healed trachoma or healed corneal ulcers were compared. The vascularization was graded from Grade I to V depending on the quadrant and type of vascularization. Graft clarity of 2+ or more in cases of trachomatous opacity was achieved in 93% of cases and that after postinfectious opacity in 71.8%. Prognosis was poor in cases where deep vascularization was present in more than two quadrants or more than 180 degrees.

Sickle haemoglobin, G-6PD deficiency and malaria in western Orissa.

Sixty cases of malaria were screened for sickle haemoglobin and G-6PD deficiency. Plasmodium vivax was detected in 40 (66%) and Plasmodium falciparum in 21 (35%) cases, with six of the latter having cerebral manifestation. Sickle Hb was found in 7 (11.5%) patients and G-6PD deficiency in 3 (5%) cases. Both patients with SS disease had vivax malaria, while of 5 with sickle cell trait 3 had only vivax, one only falciparum and one mixed infection. Amongst G-6PD deficient patients one had vivax and two falciparum malaria. One of the latter had both SC trait and G-6PD deficiency. Thus, adult persons with SS disease or SC trait were not found to be resistant to either vivax or falciparum malaria. A high frequency (5%) of G-6PD deficiency amongst malaria patients warrants a caution against indiscriminate use of 8-aminoquinoline drugs.

Management of human keratomycosis with miconazole.

A prospective clinical study was undertaken to evaluate the efficacy of 1% miconazole in 85 cases of smear-positive fungal corneal ulcers. Miconazole therapy healed 44 ulcers, thereby achieving a success rate of 64.7%. The most common organism encountered was Aspergillus followed by Fusarium, Philophora and Candida. The average healing time was found to be 22 days. The drug was well tolerated by the eye and no symptoms of ocular toxicity were observed. The findings of this study suggest that miconazole is an effective agent for the management of human keratomycosis.

Ocular manifestations of rheumatoid arthritis.

One hundred and seven patients of definite rheumatoid arthritis were screened for the presence of ocular manifestations of which 21 (19.63%) were observed to have ocular changes. Of these keratoconjunctivitis sicca in 19 (17.7%) patients and episcleritis in one (0.93%) were attributable to rheumatoid arthritis. Bilateral retinal haemorrhage in one patient was due to grade 3 hypertensive retinopathy. Absence of posterior subcapsular cataract in 106 (99.06%) patients who did not receive corticosteroids supports the hypothesis that steroids are the aetiologic agents and not rheumatoid arthritis.

Keratoplasty in children.

One hundred sixty-two eyes of children less than 14 years of age with corneal disease underwent keratoplasty over a period of eight years. The cases were divided into two groups and different subgroups, and the results were compared. The results of the recent group of cases after introduction of microsurgery were definitely better. However, being a referral center, the cases came to us at a late stage with complications and required larger grafts. Interestingly, graft transparency has a direct correlation with the size of the graft and type of corneal disease. Lastly, there was a marked reduction in frequency of therapeutic keratoplasty later, a result of the availability of various drugs.

Microbial contamination of donor eyes.

A total number of 1,516 donor eyes received from various sources during the years 1973-1985 were subjected to the isolation of bacterial contamination. The bacterial cultures taken from the pretreatment eyeball showed culture growth in 366 (24.1%) eyes. Of the 366 positive cultures, 331 (21.8%) were bacterial and 35 (2.3%) were fungal. Amongst the bacterial the major contamination was by staphylococcus aureus and albus and pseudomonas aeruginosa. Gentamicin was found to be the most sensitive antibiotic against a wider group of organisms, the next being chloramphenicol. Thus, treatment of a cadaver eye with a solution of normal saline containing 0.1-0.5 mg/ml of gentamicin is recommended before and after the donor eye is enucleated.

Corneoscleral ulceration in congenital erythropoietic porphyria (a case report).

A case of congenital erythropoietic porphyria, with corneoscleral ulceration, a rare association in this disorder, was reported. Anterior segment ischemia is believed to play an important role in the etiopathogenesis of corneoscleral ulceration. Remarkable improvement occurred with use of topical heparin drops (500 units/ml).

Genetic appraisal of congenital cataract.

A total of 45 congenital cataract cases were studied for chromosomal aberrations and dermatoglyphic patterns. Whereas parents showed no marked differences, patients showed marked differences in different dermatoglyphic traits as compared to age-sex matched controls. Out of all the patients Rubella, hereditary and undetected actiology groups showed variations differently, SCE were seen in all cataracts irrespective of their aetiology as compared to controls.

Cystic lesions of conjunctiva.

Cystic lesions constituted 37% of all the conjunctival specimens received over a period of 6 1/2 years. Epithelial inclusion cysts were the most common, followed by dermoid and parasitic cysts. A clinicopathologic correlation, along with an attempt at classification of cystic lesions of conjunctiva, is made in this article.
