Contributions to the biology of the hypoxic liver. Note II. Histologic, electron microscopic and biochemical aspects in the course of open heart surgery under extracorporeal circulation.

A study was carried out on the evolution of histological and ultrastructural lesions of liver fragments harvested at different time intervals in the course of extracorporeal circulation in 62 patients operated for acquired and congenital heart disease, as well as that of serologic tests, pre-, intra- and postoperatively up to seven days. Morphologically, it is only the ultrastructural examination that detects the accentuation of preexisting hypoxic lesions within the framework of a state of "controlled shock", noting especially accentuated dilation of the endoplasmic reticulum, lysosomal activation, mitochondrial lesions and a tendency to ribosomal and glycogenic depletion. The lesions did not exceed the limits of reversibility, excepting the cases with advanced heart failure and cardiac cirrhosis. Lending support to these data is the decrease of proteinemia and the dynamics of LDH, SDH, G1DH, gamma GT and transaminases increase after 24 h, then fall to normal values within seven days.

Myocardial ultrastructural lesions in septic valvulopathies.

The electron microscopic aspect of the myocard fragments was examined during surgical extirpation and prosthesis of some septic valvulopathies. Various and disseminated lesions were observed. We remarked the range of mitochondrial lesions, glycogenic deletions, lysosomal activations, the intensity of interstitial and intrasarcoplasmatic oedema that disintegrate sarcolemal structures and intercalary disks. The difficulties concerning the surgical valve prosthesis on the severe toxicoseptic and hypoxic myocardial alterations were mentioned.

Contributions to the biology of the hypoxic liver. Note I. Histological and electronmicroscopical aspects of the liver in chronic congestive heart failure.

A study was carried out on the histological and ultrastructural aspects of liver fragments harvested from 62 patients with acquired or congenital heart disease before open heart surgery under extracorporeal circulation. Against a background of passive congestion in the pericentrolobular and mediolobular areas, various mitochondrial lesions and dilation of the endoplasmic reticulum with a reduced number of ribosomes, the presence of microbodies, biliary pigments, lipid vacuoles, lysosomol hyperplasia and activation, glycogen depletion could be seen, as well as extensive collagenization of Disse's spaces, fibroblast hyperplasia and Kupffer cell activation. These lesions are more reduced in the periportal zones. In the advanced stages of heart failure, there appeared a cirrhogenic organization due to extensive pericentrolobular and periportal fibrosis.

Effect of age on the heart electrical phenomena, left ventricular wall thickness and blood pressure in men.

The effect of age on the maximal spatial QRS vector (MSQRSV) in the Frank lead system was correlated with th posterior left ventricular wall thickness (PLVWT), the interventricular septum thickness (IVST) determined by echocardiography, and with blood pressure in 300 normal men, distributed by age groups (20-29, 30-39, 40-60). With advancing age, a significant decrease of MSQRSV accompanied by a significant increase of PLVWT was observed while the increase of IVST and of blood pressures with age were found non significant. It was concluded that: 1. blood pressure is not the sole determinant of hypertrophy; 2. myocardial hypertrophy is not the sole determinant of cardiac voltage increase; 3. there is some degree of myocardial sclerosis in healthy people aged over 40-60 years; 4. in left ventricular hypertrophy the left ventricular muscular mass cannot be accurately predicted from the heart electrical phenomena; 5. the results obtained were in agreement with those of authors describing different aspects of normal Frank ECG and vectorcardiogram (VCG) in subjects under and over 40-50 years of age; 6. the variability of the degree of age influence on normal ECG from one country to another could explain the different diagnostic criteria for the same cardiac disease in computerized programs.

[General surgical operations in patients with prior cardiac operations. Anesthetic and surgical risk].

The authors make an analysis of a group of 39 patients with cardiac interventions in their antecedents that later underwent general surgery. Of these 25 underwent planified surgery while 14 were operated as emergency cases. The surgery performed was of variable extension. Nine fatalities were recorded, representing 23,09%. Evaluation of the anesthetic and surgical risk factors revealed their dependence both on the myocardial hemodynamic efficiency (as determined by the previous heart surgery), and on the second intervention, emergencies presenting with a potentially increased risk factor. The authors stress the necessity to adapt the attitude of the anesthetist, as well as the intensive care, to particularities of the patient, and those determined by the second intervention.
