Morphological features of red blood cells in subjects with sickle cell trait: changes during exercise.

Occasional complications and even death in subjects with sickle cell trait have been attributed to severe physical exertion. However, the extent to which sickling actually occurs during exercise has not been reported. This study examined the red blood cell morphological features immediately following near maximal upright graded bicycle exercise in five asymptomatic black subjects with hemoglobin AS. Exercise produced minimal sickling in vivo, which was not proportional to the intensity of exercise. The amount of sickling in vivo was small in comparison to that observed in the presence of severe hypoxia in vitro, never exceeding 0.75%. in seven normal subjects with hemoglobin AA, exercise did not cause changes in red blood cell morphological features. We conclude that exercise may initiate sickling in subjects with sickle cell trait.

[Scintigraphy with thallium chloride in a case of clear cell sarcoma of the tendons and aponeurosis].

Clear cell sarcoma of the soft parts (CCSSP) is a rare and highly malignant tumor. This is a case report of a 31 years old woman who presented with a tumor in the internal face of left thigh. The tumor biopsy was suggestive of a possible malignant tumor of the peripheral nerve sheath (malignant schwannoma). Biochemical analyses, computed tomography and magnetic resonance were performed and reported a sarcoma of soft parts (CCSSP) without abnormal inguinal lymph nodes. With this diagnostic suspicion, the patient was sent to the Nuclear Medicine Service of our center where a 201Thallium scintigraphy study was performed. In this study, it showed the primary tumor together with a hot spot in the homolateral inguinal region, suggestive of the presence of a metastastic lymph node. This finding would change the surgical attitude in the patient, including inguinal lymphadenectomy. The post-surgical histological study confirmed a clear cell sarcoma of soft parts (CCSSP) with a metastatic inguinal lymph node.

[Pesticide application practices in agricultural workers].

OBJECTIVE: We describe the sociodemographic characteristics and determinants of pesticide exposure in agricultural workers applying pesticides. METHODS: The workers selected were included in a case-control study carried out in the Autonomous Community of Valencia in Spain. Contact was made by telephone and individuals who had been involved in agricultural work during the relevant period of exposure were interviewed face-to-face to gain information on the following determinants of pesticide exposure: crops and periods worked, mixing of products, treatment equipment, participation in the washing of equipment, use of personal protection during the treatments and knowledge of the risks of pesticide exposure. RESULTS: Eighty-nine workers, aged between 16 and 46 years old, were included. Most of the interviewees had primary education or less. Pesticide treatments were mostly applied on high crops (82%), with manual equipment (61%) and throughout the year (45%). Workers frequently performed tasks involving additional exposure to pesticides (mixing chemicals, 66%, or washing equipment, 60%). Sixty-five percent of the workers used no personal protection or used it defectively. No differences were found in personal protection use according to age, family income or education. Most of the workers (90%) reported knowledge of the health risks of pesticide exposure and 21% of them rated the risk as null. CONCLUSIONS: Workers involved in pesticide application use personal protection measures very defectively. There is a clear need to develop specific prevention programs for these workers. The determinants of pesticide exposure in agricultural workers described in this study should be properly assessed in epidemiological studies of the health effects of pesticides on agricultural workers.

[Interleukin levels in umbilical cord blood: relationship with a family history of allergic disease].

BACKGROUND: The development of allergic pathology takes place as result of an alteration of the immunity and alteration of the corporal mechanism of protection, giving rise to an erroneous answer or exaggerated forehead to innocuous antigens, that it generates clinical symptoms with cutaneous, digestive or respiratory manifestations. The frequency and distribution of this process have undergone an increase from the 1970, which causes that a greater interest in the knowledge of the mechanisms exists that produce this clinic. The answer by immunoglobulin E is regulated by the answer of lymphocytes T-helper-1 represented by interleukin 2 and gamma-interferon that inhibits their production, and the answer of lymphocytes T-helper-2 formed by interleukin 4, interleukin 10 and interleukin 13 that stimulate the production of immunoglobulin E. METHODS AND RESULTS: A study of cases and controls with a sample of 70 appears new born considering antecedent relatives of first degree of allergic disease (47 no, 23 yes). Values in umbilical cord blood were moderate of interleukins 4, 10, 13 and gamma-interferon (kit CLB, and method ELISA). Values for interleukin 4 and interleukin 13 have not been obtained. One has been greater values of interleukin 10 in children of mother or brother affection (mother affects IL 10 = 48.7 pg/ml, in front of mother does not affect IL 10 = 31.62 pg/ml, p = 0.081, no signification), (brother affection IL 10 = 72.8 pg/ml, in front of brother no affection IL 10 = 32.31 pg/ml, p = 0.0062, is significant). Difference for gamma-interferon was not obtained. CONCLUSIONS: As it has already been shown in other studies, interleukin 10 increases in cord blood in children whose mother is the one who presents the disease. It emphasizes the increase of interleukin 10 in blood of umbilical cord of children with brother affection of allergic disease. Still it is left much to do and by means of later pursuit it is hoped to obtain interest results.

[Auricular Q wave in right precordial derivations. New sign of left auricular growth].

The authors present four cases of rheumatic heart disease with severe dilatation of the left atrium which reached the right profile in the radiologic study. An initial negative deflexion of the P-wave (qP) in the right precordial leads was recorder in these cases. The presence of qP was interpreted in view of the atrial activation and the solid angle of the right precordial leads as a variation of potential of the right atrial ceiling and/or of the high interatrial septum, a consequence of the dislocation of the right atrium produced by severe dilatation of the left atrium, demonstrated radiologically in the cases studied. Thee presence of qP is another undoubtably valuable sign for inferring a severe displacement of the left atrium toward the right profile.
