[Sociodemographic features of suicide attempts].

Suicide accounts for about 10% of deaths among psychiatric patients, and life-threatening attempts are much more common than fatalities. Despite progress in defining risk rates and predictive factors, knowledge on which to base sound clinical and public policies regarding suicide prevention and treatment remains remarkably limited. Social risk factors and psychiatry diagnoses are the main arguments to management the suicidal behaviour in emergency room. Female gender is associate with suicide attempts while male gander is associate with completed suicide. Risk of suicide is increased in young and elderly people. Age is a confounding factor in the relationship among suicide and widow, income and professional status. Emigration and anomie are related with suicide. Social factor, as a marker of suicidal behaviour, are a promising research question.

[Entomology of onchocercosis in Soconusco, Chiapas. 6. Quantitative studies of the transmission of Onchocerca volvulus by 3 species of Simuliidae in a community with high endemicity].

In Guatemala, there is no doubt about the participation of Simulium ochraceum as vector of Onchocerciasis. However, in Mexico practically there are not studies focussed to determine the role of this species in the transmission. The objective of the present investigation was to determine which of the 3 species of Simulium founded in the Soconusco region of Chiapas, is the main, and which were secondary in the transmission of Onchocerciasis in that area. The locality of Morelos, in the Huixtla "municipio" of Chiapas, localized a 1200 meters over the sea level (mosl) were selected to carried out the present study. According to own parasitological studies, this locality is considered as highly endemic (more than 66% prevalence). From March 1979 to March, 1981, we performed captures of Simulium sp caught on human bait. Quantitative studies and of transmission potentials were also performed. The following results were obtained: a) Absolute black flies densities (nulliparous and pariparous) and infected black flies (with L1 and L2 larvae): S. ochraceum, S. metallicum and S. callidum, in that order was the distribution of densities. Infected black flies were obtained in the 3 species. However, in despite of an irregular distribution in all the year, it was possible to identify 2 peaks of maximum infection of S. ochraceum in March, 1979 and March 1981. b) Monthly bite densities and infective bites.(ABSTRACT TRUNCATED AT 250 WORDS)

[Entomology of onchocerciasis in Soconusco, Chiapas, México. VII. Quantitative variations in the transmission of Onchocerca volvulus (Nematoda: Filaroiidea) according to human activity space].

In this research we tried to demonstrate differences in transmission intensities of onchocerciasis by Simulium ochraceum in the community of Golondrinas, Municipality of Acacoyagua in the Soconusco area of the state of Chiapas. Adult females of this species were captured in the center of the villages, the coffee plantation, and the corn bean planted plots, during 7 to 10 consecutive days of the months of June, August, November and December of 1983, and January, March, and May of 1984. Captures were made from 7:00 to 11:00 h, by two persons (a human bait and a collector) in each of the mentioned areas. Variations in densities of S. ochraceum were not statistically significant among the different areas, although there was a predominance in the center of the villages during all the time periods. There was not a statistical significance in the number of oviparous females, although again the center of 50 villages showed the highest percentages (43.5%), as compared to the coffee plantation (37.8%), and the corn-bean plots (33.8%). Regarding the ratio infection: infectivity, only the infective mosquitoes with L3 showed a statistical significance in the three types of areas, being highest in the center of the villages. Transmission potentials were: center of the village 28.5 L3/human/period; coffee plantation, 73 L3/human/period; corn-bean plot, 5.8 L3/human/period. We discuss the possibility that the differences in densities among the various areas might depend on the variations of the breeding sites, mobility or permanence of the individuals during the hours of greater activity of S. ochraceum, and the wind and light local conditions.(ABSTRACT TRUNCATED AT 250 WORDS)

[Changes in arterial oxygen saturation in patients undergoing endoscopy of the upper digestive tract].

This study showed how often oxygen desaturation occurred in patients undergoing upper endoscopy procedures; extent of desaturation after sedation with diazepam, relationship between desaturation and respiratory function status and compared two types of instruments Olympus (XQ, K10). Ninety nine patients undergoing elective and emergency upper endoscopy procedures were monitorized with pulse oximeter before sedation, during and after the procedure. We did not find any change clinically significant.

[Clinical aspects of ventricular fibrillation in the development of myocardial infarction].

We report 13 patients who suffered ventricular fibrillation (VF) during the in-hospital course of an acute myocardial infarction (AMI). We compare the clinical features of this patients with other two groups; patients without VF and patients with intraventricular conduction defects. In 38% of the cases VF appeared after 5th day of the AMI. The group of patients with VF was characterized by an increased incidence of heart failure, supra-ventricular and ventricular premature beats, and also intra-ventricular conduction defects. In 2 patients VF appeared during the course of treatment with therapeutic doses of lidocaine. Sixty two percent of the patients with VF were discharged alive. We recommend that patients with AMI who display the above mentioned risk factor, should remain in the coronary care unit for a more prolonged period of time. By proceeding in this way, we had the opportunity to observe and immediately treat 5 further patients with AMI and VF. All of them recovered.

[Late arrhythmias in the operated interatrial communication. Analysis of sinus node function and the conduction pathways by His bundle electrocardiography].

Four hundred patients with atrial septal defect treated surgically were reviewed. Thirty five (8.7%) developed arrhytmias post-surgery which persisted for over a year. Sinus bradycardia was found in 10 patients, nodal rhythm in 21, and atrial fibrilation and flutter in 4 patients. Thirty five per cent of the patients with late arrhythmias developed related symptomatology. In 14 patients the function of the sinus node was studied with electrical stimulation of the atrium and with His registry. The interatrial conduction time, AV node and His Purkinje were analized employing various stimulation frequencies. All the cases studied had normal intra-atrial conduction; the response of the atrio-ventricular node to increasing frequencies was normal, an the intraventricular conduction remained constant. In 8 patients (52%), alterations of the sinus node were found; these consisted of prolonged post-stimulation pauses, Wenckebach's type sinoatrial block and suppression of sinus automatism employing vagal procedures or through electrical stimulation. A patient with severe bradycardia detected by dynamic electrocardiography had to be treated with a permanent pacemaker. We confirm that these arrhytmias are not produced by lesions of the internodal tracts, and that an alteration of the sinus node is frequent without a concomitant lesion of the intraventricular pathway. The lesion to the nutrient artery could be due to trauma and/or surgically induced. The response to anticholinergic drugs was good. Prolonged observation of these patients could increase the morbility of these arrythmias and raise doubts of the surgical indications in cases with moderate hemodynamic repercussion.
