Relation of severity of symptoms to transient myocardial ischemia and prognosis in unstable angina.

OBJECTIVES: This study was undertaken to compare the relative power of the severity of angina versus that of any other clinical, electrocardiographic (ECG) and angiographic findings in predicting the risk of subsequent in-hospital coronary events in patients admitted to the coronary care unit for treatment of unstable angina. BACKGROUND: The presence or absence of chest pain has traditionally been used to guide management and therapy of unstable angina. However, recent studies raised the possibility that the cumulative duration of ischemia may be an additional index of prognosis. METHODS: We studied 104 consecutive patients admitted to the coronary care unit because of unstable angina. Diaries of symptoms were accurately kept. All patients underwent Holter ambulatory ECG monitoring during the 1st 24 h and angiography within 1 week of admission. RESULTS: During the hospital stay, 41 patients (group 1) had subsequent coronary events; the remaining 63 patients (group 2) had a good clinical outcome. Recurrence of chest pain after admission was observed in 76% of patients: 36 of the 41 group 1 patients (sensitivity 88%) and 43 of the 63 group 2 patients (specificity 32%). Anginal scores (frequency and persistence of pain, duration of each single episode and pain-free interval) showed high specificity but low sensitivity for detecting evolution toward subsequent coronary events. On Holter monitoring, the duration/24 h of the total number of ischemic episodes was consistently greater in group 1 than in group 2. A cumulative duration of ischemia > or = 60 min/24 h was observed in 34 of the 41 group 1 patients (sensitivity 83%) but in only 16 of the 63 group 2 patients (specificity 75%). High risk coronary artery lesions (left main coronary artery disease or complex stenosis) were detected in 36 of the 41 group 1 patients and in 26 of the 63 group 2 patients. CONCLUSIONS: Transient myocardial ischemia detected by Holter monitoring, but not chest pain, is the best predictor of unfavorable short-term clinical outcome. The decision to perform early angiography and revascularization cannot be based on symptoms alone.

Ultrastructure of heat-deproteinated compact bone.

Heat-deproteinated bone (calcined bone) is a natural candidate for an osteoreproductive biomaterial. Ultrastructural investigation has pointed out an important aspect of the preparation technique. Treatment of bone at temperatures exceeding 500 degrees C causes complete disruption of the tissue architecture and the reorganization of the mineral phase into tightly packed, dense crystals. At 500 degrees C or less the structure and distribution of the mineral phase remain unaffected, so that cross-banded 'shadows' of collagen fibrils are still readily observable, although collagen is no longer present in the samples. By its excellent structural preservation and natural porosity, low-temperature calcined bone seems to be a promising alternative for osteoreproduction.

Effect of naltrexone treatment on insulin secretion, insulin action and postprandial thermogenesis in obesity.

For many years a series of studies has been carried out to evaluate the role of endogenous opioid peptides on glucose metabolism. In this work we studied the influence of endogenous opioid peptides on insulin response to OGTT and glucose-induced thermogenesis before and after a prolonged oral treatment with Naltrexone (50 mg/daily for 6 days), an opioid receptor antagonist, in a group of 9 obese subjects. Moreover in obese patients we evaluated the effect of this anti-opioid drug on insulin secretion and insulin sensitivity during an IVGTT using the minimal model approach. We compared the pre-treatment results with data coming from a group of 5 normal-weight subjects. We measured blood glucose, plasma insulin and C-peptide concentrations and evaluated the following parameters: first (phi 1) and second (phi 2) phase of beta-cell sensitivity, insulin sensitivity and glucose effectiveness. Obese subjects displayed an increased insulin response to oral and i.v. glucose load, due to an increased first phase of insulin secretion (phi 1), a reduced insulin sensitivity (Si) and glucose effectiveness (Sg) in respect to normal-weight subjects. They showed no difference in glucose and insulin area during oral load and in their profiles during i.v. glucose load after naltrexone treatment. Similarly no significant variation in insulin sensitivity and glucose effectiveness was observed. The glucose-induced thermogenesis, measured by indirect calorimetry, was not modified by naltrexone. Therefore our study demonstrates that endogenous opioids do not play any role in the impairment of peripheral insulin sensitivity and energy expenditure in human obesity.

Ischemia-reperfusion syndrome: an alternative experimental model.

OBJECTIVE: To design an alternative experimental model of ischemia-reperfusion syndrome. Our model mimics the clinical pattern of the syndrome and also assesses the efficacy of therapeutical protocols. EXPERIMENTAL DESIGN: Ischemia was induced under general anaesthesia in the posterior limbs of 10 sheep by occluding the aorta and vena cava by means of two-way balloon catheters. Ischemia was stopped after 4 hours and blood and histologic parameters determined in the first three hours of revascularization. The animals were divided into three groups: a group of 3 sheep in which a sham operation was performed; a control group (5) to assess the efficacy of induced ischemia; the third group (5) to determine the effect of antioxidant and membrane protective drugs to assess the reliability of the model to study the ischemia-reperfusion syndrome. RESULTS: At the end of ischemia, skin temperature was decreased (p < 0.04) both in control and treated groups, pH decreased significantly soon after reperfusion in the control group (p < 0.04). Reperfusion in control sheep, compared with treated animals, was followed by a significant increase in CPK blood levels (p < 0.009), related to marked muscle damage, in particular after reperfusion. Tissue damage detected at TEM was less severe in treated animals. CONCLUSIONS: This model is an effective experimental strategy and a means of assessing preventive treatment.

An histologic and histomorphometric study of bone reactions to unloaded and loaded non-submerged single implants in monkeys: a pilot study.

The authors present an histologic study of bone peri-implant tissue responses to non-submerged unloaded and early-loaded titanium screw implants in monkeys. Titanium screws were placed in the maxillae of 3 Macacus rhesus monkeys. One crown for each of three implants was placed after 30 days under occlusal contact, while the other three implants were left unloaded (one pair per animal). Fifteen months later, the animals were killed, and block sections were obtained to be processed for histology. Thin ground sections were excised so that histomorphometric analyses could be performed. In all samples examined, a tight contact of new bone to the implant surfaces was observed. Moreover, around the implant necks of the early-loaded screws was observed an histologic pattern of lamellar, cortical bone, thicker than in unloaded implants. Comparison with peri-implant bone of adjacent teeth showed a similarity to the surfaces of loaded peri-implant bone.

Supracrestal circular collagen fiber network around osseointegrated nonsubmerged titanium implants.

Eight non-submerged titanium implant screws were placed in the first upper molar edentulous sites of monkeys and subsequently kept loaded with single crown prosthesis 1 month following implantation. The animals were killed after a further 14 months and specimens including implant and adjacent teeth were processed for light and electron microscopy. Histological pictures of all samples showed the neck and most of the screw body to be surrounded by new bone. The soft tissue surrounding the implant post included pocket epithelium and supra-crestal connective tissue displaying collagen fiber bundles comparable to gingival ligaments. These peri-implant collagen fiber bundles arose from the neighboring alveolar crest, root cementum of adjacent teeth or, superficially, from the epithelium and followed a circular array around the implant neck.

Implants of heterologous demineralized bone matrix for induction of posterior spinal fusion in rats.

The authors tested the osteoinductive capacity of powdered heterologous (bovine) demineralized bone matrix in rats. The first part of the study concerned a monolateral posterior spinal implant after decortication of three vertebrae, using as a control area the animal's contralateral side, in which neither bone graft nor any other material were placed. In another group of rats, a comparative evaluation was made of powdered heterologous demineralized bone matrix and fresh autologous bone. In the same animal, autologous bone was implanted to realize a thoracic posterior fusion and demineralized bone matrix, to induce a posterior fusion in the lumbar area. All data obtained suggested a good osteoinductive activity of heterologous powdered demineralized bone matrix. The two posterior spinal fusions done in the same animal with heterologous demineralized bone matrix or autologous bone, respectively, had similar callus development and required the same time for formation.

Particular structure of the anterior third of the human true vocal cord.

The histological aspects of the true vocal cord mucosa change in the anterior third compared with the posterior two thirds. The anterior third is characterized by an epithelium where the ridges, marked in the posterior two thirds, are very slight or even absent. The underlying basement membrane, which is thin in the posterior two thirds, here appears particularly thick. At the ultrastructural level in this area, beneath a normally thickened basal lamina, a thick layer of finely granulated electron-dense material, interspersed with thin and randomly scattered collagen fibrils and proteoglycan filaments, is detectable. Beneath this thickened basement membrane, a layer of small undulated collagen fibril bundles with very numerous interspersed oxytalan fibres is found. The collagen fibrils, small in diameter (30-40 nm), seem to continue with the collagen fibrils of the basement membrane. In this layer numerous blood vessels with a very thick, delaminated basement membrane are also observed. The underlying area is characterized by the vocal cord ligament, composed by large compact collagen fibril bundles with interspersed elastic fibres. The particular features of the thick basement membrane, the thick-walled and delaminated vessels and the modular distribution of the elastic system together may well form the basic structure enabling the functional integration of the vocal ligament into the overlying mucosa and the underlying vocal muscle.

The dependency of collagen fibrillogenesis in vitro on fibroblast culture conditions. Fibroblasts in mono- and multi-layers.

The extracellular matrix produced by monolayer and tridimensional cultures of fibroblasts was investigated using histochemical and ultrastructural methods. In monolayer cultures, collagen and proteoglycans produced by fibroblasts could not be organized into morphologically recognizable structures. Tridimensional fibroblast cultures produced a well organized matrix with periodic, parallel ordered collagen fibrils of 50 nm diameter, criss-crossed by alcianophylic segments 6-10 nm thick in diameter and 100-300 nm in length, parallel to each other, perpendicular to the collagen fibrils and spaced 67 nm from each other. Some alcianophylic segments lay perpendicular to the above described ones, with maximum lengths of 65-70 nm. Alcianophylic segments are the ultrastructural evidence of structural proteoglycans. These observations suggest that the culture conditions influence the collagen and proteoglycans secretion, so that the final organization of the matrix results quite different.

Ultrastructure of the extracellular matrix of bovine dura mater, optic nerve sheath and sclera.

The sclera, the outermost sheath of the optic nerve and the dura mater have been investigated histologically and ultrastructurally. Although these tissues appear very similar under the light microscope, being dense connective tissues mainly composed of collagen bundles and a limited amount of cells and elastic fibres, they exhibit subtle differences on electron microscopy. In the dura and sclera collagen appears in the form of large, nonuniform fibrils, similar to those commonly found in tendons, while in the optic nerve sheath the fibrils appear smaller and uniform, similar to those commonly observed in reticular tissues, vessel walls and skin. Freeze-fracture also reveals these fibrils to have different subfibrillar architectures, straight or helical, which correspond to 2 distinct forms of collagen fibril previously described (Raspanti et al. 1989). The other extracellular matrix components also vary with the particular collagen fibril structure. Despite their common embryological derivation, the dura mater, optic nerve sheath and sclera exhibit diversification of their extracellular matrix consistent with the mechanical loads to which these tissues are subjected. Our observations indicate that the outermost sheath of the optic nerve resembles the epineurium of peripheral nerves rather than the dura to which it is commonly likened.

The human anterior cruciate ligament: histological and ultrastructural observations.

In transverse and longitudinal paraffin-embedded sections, the human anterior cruciate ligament (ACL) is made up of wavy bundles of collagen fibres arrayed in various directions, the majority around the axis of the ligament with a few running parallel to it. The fascicles making up the larger bundles are also characterised by this undulating appearance. In thin sections 2 types of collagen fibrils are observed: small (with a single diameter peak at 45 nm) and large (3 peaks at 35, 50 and 75 nm respectively), organised into distinct areas made up of either large or small bundles of fibrils. The numerous fibroblasts that are present appear elongated in the direction of the bundles with branches and short cytoplasmic processes. The elastic system is made up of both elastic and oxytalan fibres. The varied orientation of the bundles in the ACL, the complex ultrastructural organisation and the abundant elastic system make it very different from other ligaments and tendons, providing a structure able to withstand the multiaxial stresses and varying tensile strains imposed upon it.

Anterior Cruciate Ligament

Calcium phosphate materials containing alumina: Raman spectroscopical, histological, and ultrastructural study.

Variable alumina quantities were added to two types of calcium phosphate materials-hydroxyapatite ceramics with Ca/P = 1.67 and calcium metaphosphate glass with Ca/P = 1--in order to increase their mechanical properties. Raman spectroscopy shows that alumina interacts with the phosphate group of these materials, while thermomechanical analysis shows that their elastic modulus has a value similar to that of bone. Histological sections demonstrate that the surface in close contact with ceramic materials shows a good integration between bone and biomaterial. All ceramic specimens are penetrated by well-stained, presumably glycoprotein, matrix, that consistently forms a thin network in close contact with the implant. After 6 weeks bone growing on both ceramic and glass shows signs of maturation with a lamellar structure and an apparently normal mineralization. In the case of the glass, inside this newly formed bone, were often observed two layers, the internal one showing a well defined lamellar structure.

A reduced sampling schedule for estimating the parameters of the glucose minimal model from a labeled IVGTT.

The minimal model of tracer glucose disappearance allows the measurement in man of important metabolic parameters from a labeled IVGTT (intravenous glucose tolerance test). Usually 30 blood samples are drawn to measure the insulin and the labelled glucose concentration time courses, which provide the data base for the model identification. In this paper, we address the problem of reducing the number of blood samples withdrawn without, however, deteriorating parameter estimation. By using a stepwise approach, largely based on optimal experiment design theory, we have derived a reliable reduced sampling schedule, which comprises only 14 total samples. The practical application of this reduced schedule has been validated by applying it in a new set of human experiments, completely independent of the ones used in the design phase.

Blood Glucose

Ultrastructure of the bovine nuchal ligament.

Nuchal ligament is composed almost exclusively of elastic fibres and collagen fibrils, interwoven very closely and lying parallel to the main ligament axis. Elastic fibres are very large, straight and roughly cylindrical; the collagenous matrix consists of septa of diminishing size forming a 3-dimensional matrix that envelops fibre bundles as well as individual elastic fibres. In all areas examined, collagen fibrils are of very uniform size and, on replicas, they reveal a spiral subfibrillar arrangement with an inclination angle of 17 degrees. Collagen fibrils appear to adhere to the elastic fibres very closely, conforming to their irregular shape. Sometimes they impinge directly upon the elastic fibres, while in other cases a space is visible between collagen fibrils and elastic fibres that contains a rich fabric of intermediate filaments. The collagen-elastin complex of the ligamentum nuchae may be considered a fibre-reinforced composite material comprising tough fibres immersed in an amorphous elastic matrix. Its mechanical behaviour is the result of the combined properties of its components and their interactions.

Tricalcium phosphate endosseous implants in dentistry: ultrastructural findings.

beta-Tricalcium phosphate magnesium substituted (beta-TCMP), consisting of one part small unsintered and one part large sintered granules was placed in upper and lower jaw surgical cavities of monkeys. At light and transmission electron microscope the biopsies taken at 2, 4, 8 and 24 weeks showed that both sintered and unsintered granules were well tolerated by the host tissue and result biodegradable over time. beta-TCMP may stimulate the direction of bone growth enhancing osteoblasts activity and new bone deposition in direct contact and in the micropores of the biomaterial. beta-TCMP containing Mg ions reabsorbs slowly when in sintered granules form and rapidly as unsintered granules. When it is placed in bone cavities, if bone reabsorption is biologically necessary, the biomaterial can be reabsorbed with bone, while if osteogenesis is required beta-TCMP provides a more durable matrix to support new bone growth.

Histologic and ultrastructural findings of tissue ingrowth. The Leeds-Keio prosthetic anterior cruciate ligament.

A light and electron microscopy investigation was performed on a Leeds-Keio ligament removed because of rupture 18 months after implantation to repair an anterior cruciate ligament. The investigation showed fibrous connective tissue on the plane of the main stress force. There was elastin and adequate vascularization interspersed with Type I collagen fibrils in the area most distant from the ligament. The tissue near the Dacron fibers was highly cellular with a matrix of infrequent, thin collagen fibrils and abundant fine granular material. The growth of the host tissue occurred in and around a Leeds-Keio ligament in response to tensile stresses.

Distribution of elastic system fibres in the rat tail tendon and its associated sheaths.

The distribution of elastic, elaunin and oxytalan fibres in tendons and tendon sheaths was studied by means of light and electron microscopy. The different fibres were characterised by comparing their typical ultrastructural pictures with a combination of selective staining techniques for light microscopy with the appropriate enzymatic digestions. Since it was observed that the rat tail tendon and its associated sheaths are not homogeneous structures from the point of view of the distribution of the elastic system fibres, a systematic description of the differential distribution of the oxytalan, elaunin and elastic fibres in the tendon and in the endo-, peri-, epi- and paratendineum is presented. Mature elastic fibres are present in the tendon and in the para-, epi- and endotendineum, whereas these fibres could not be detected in the peritendineum. Very many elaunin fibres are characteristically recognised in the endotendineum; these fibres are present in the peritendineum and in the tendon fascicles too, while the epi- and paratendineum are devoid of elaunin fibres. The peri- and endotendineum contain numerous oxytalan fibres, whereas these fibres are absent from the tendon fascicles and the epi- and paratendineum. The functional implications of the foregoing findings are discussed.

Subfibrillar architecture and functional properties of collagen: a comparative study in rat tendons.

Collagen fibrils from different rat tendons have been investigated by freeze-fracture and transmission electron microscopy. In all cases, marked differences in both fibril morphology and subfibrillar organisation have been consistently found between the tendon core (composed of large and heterogeneous fibrils comprising tightly-packed, straight, parallel molecules) and sheath (showing small, uniform collagen fibrils with a helical arrangement of the molecules). The bio-mechanical requirements to which these tissues are subjected suggest, as do previous observations on other tissues, that a causal correlation exists between substructure and collagen fibril function.
