Strangulated umbilical hernias in children.

Umbilical hernia is a frequent pathology of the anterior abdominal wall in children. The hernia ring closes usually before 4 years, but a strangulation can occur. It is an exceptional complication according to the literature data. Since this complication is rare, we undertook a retrospective study of these strangulations in a 5-year period from January 1997 to December 2001 at Aristide Le Dantec hospital. We collected 41 cases that underwent emergency surgery operations for strangulated umbilical hernias, which represent about 15% of umbilical hernias operated during the same period. The age range was 8 months to 10 years and the average age was 14 months. All the children were examined within 24 h after the onset of the disease and the surgery was immediately performed. In five cases the bowel was necrosed and perforated and we performed a resection followed immediately by an anastomosis. In the follow up, two children presented wound infections and a hernia recurred in one child. There was no mortality. This study conducts us to question western reports which recommend conservative therapy for umbilical hernia in children.

[Radiographic and ultrasonographic diagnosis of a left thoracic mass].

A chest roentgenogram of a 69-year-old man undergoing a check-up before prostate surgery showed a mass in the antero-inferior zone of the lung, just above the diaphragm. At ultrasonography, it was considered to be a thoracic ectopic kidney. This radiological case illustrates the place of this anomaly in the differential diagnosis of lung solitary mass, which can be explored with ultrasonography when retroperitoneal herniation is suspected.

Access and adhering to tuberculosis treatment: barriers faced by patients and communities in Burkina Faso.

SETTING: Three selected districts in Burkina Faso. OBJECTIVES: 1) To explore patients' and community members' perceptions and problems associated with accessing formal tuberculosis (TB) treatment; and 2) to identify patients' and community members' perceptions and problems associated with adhering to formal TB treatment. METHODS: Twenty-eight focus group discussions and 68 in-depth interviews with TB patients, community representatives, members of the health centre management committee, traditional healers and health professionals. RESULTS: Attending the health centre was the last resort for patients with symptoms indicative of TB. When on treatment, patients faced a number of barriers in adhering to care. These related to the centralised nature of direct observation and the problems faced whilst at the treatment unit. CONCLUSION: Patients experience three sets of inextricably linked barriers to successfully treating TB: attending the health centre initially, attending the health centre repeatedly and experiences whilst at the health centre. These barriers are further complicated by geography, poverty and gender. The challenge ahead lies in moving beyond documenting barriers from patients' perspectives to addressing them in resource-poor contexts.

[Evaluation of the management of omphalocele at Dakar].

Omphalocele is a congenital malformation of the abdominal wall of children, characterized by an ombilical defect living the abdominal organ visible through a translucent amniotic membrana. The goals of this study were to describe the epidemiological, clinical and therapeutic characteristics at the University Teaching Hospital of Dakar. We conducted a retrospective study at the UniversityTeaching Hospital of Dakar from January 1997 to December 2002. Fifty cases of Omphalocele diagnosed at the Unit of Paediatric Surgery of the Department of General Surgery at Aristide Le Dantec Hospital, were included in this study. We described the epidemiological, clinical and therapeutic characteristics of omphalocele. Omphalocele is a condition diagnosed late in boys from poor sphere. The study revealed that weight from birth, omphalocele size, local state, and existance of associated malformation correlated with death rate. The treatment option was a spontaneous epidermisation as described by Grob in the absence of omphalocele rupture wich imposed a primary closure of abdominal wall. The overall death rate was 42%. The improvement of results will need an antenative diagnosis, paediatric reanimation unit wich will notably reduce death rate.

[Results of emergengy colectomy in the management of the colon volvulus in Dakar hospital].

PURPOSE: The aim of this study was to evaluate the results of emergency colectomies for the management of colon volvulus. PATIENTS AND METHODS: This retrospective study was undertaken on 50 cases of colon volvulus operated in the Surgical Department of the Aristide Le Dantec teaching hospital of Dakar from January 1994 to December 2000. It concerned 42 men and 8 women with an mean age of 42 years. All patients presented an occlusive syndrome suggestive of colon volvulus, which required a laparotomy, through a xypho-pubic incision. The patients were divided into two groups. Group I concerned 13 patients who underwent an ideal colectomy and group II, 37 patients in whom we performed a colectomy associated with a temporary colostomy with either Bouilly-Volkman procedure (n = 23) or Hartman procedure (n = 14) followed one month later by intestinal continuity restoration. Mortality and morbidity were compared in the two groups. RESULTS: The overall mortality rate was 12% (6/50). The mortality rate was 31% (4/13) in group I and 5 % (2/37) in group II. The overall morbidity rate was 14%. In group I, the morbidity rate was 15 % (2/13) and 13,5% (5/37) in group II. CONCLUSION: In our study the mortality rate was higher after colectomy with primary anastomosis than after colectomy with temporary colostomy followed by secondary anastomosis. We recommend colectomy without anastomosis for the emergency management of the colon volvulus in Africa.

[Internal endoscopic urethrotomy for urethral stricture at the hospital of Bobo-Dioulasso: feasibility of the technique in precarious situations and short term results].

Internal optical urethrotomy is one of the harmless and less expensive methods in the treatment of urethra stricture. The aim of this study was to analyze the feasibility, the safety and the short-term results of this method in our urologic department. Seventy patients (age range 17-90 years) treated from 01/01/94 to 31/12/00 were included in this retrospective and descriptive study. Two principal aetiologies were pointed up: infectious strictures (69%) and traumatic strictures (13%). The procedure has been successful in 74% of all cases with 16% of complications. The mortality rate of the procedure was zero. The results were good in 67% of cases at one month and in 44% during a average period of 4.5 months. In conclusion, internal optical urethrotomy in our medical context is feasible with few complications. These ones can be reduced by avoiding unfavourable indications. Since urogenital infection is the predominant aetiology, the best management of urethral stricture is the prevention of these infections.

[Radiography and ultrasonography in the management of bladder tumors: 71 cases at the National Hospital Center of Yalgado Ouedraogo (Burkina Faso)].

The bladder's cancer is frequent in West Africa. Urinary schistosomiasis endemicity helps to explain this high incidence. It is a pathology of late diagnosis, little or badly explored by imaging. Through a retrospective survey of 71 patients' files aged in average of 51.7 years, all having a bladder's tumour which is clinically shown by an haematuria, the major symptom, often by a pelvic volume, and who have all gone through an abdominal echography and/or intravenous urography (IVU) and/or retrograde urethrocystography (UCR), we have tried to point out the role of imaging in the caring of this pathology in our working context. Imaging, with a 98.5% sensibility for sonography and 100% for IVU, took part in all the cases to the diagnosis, to the search of urinary signs of reflux, associated signs authorizing a diagnostic orientation, but was excluded from the evolutive follow-up due to the poverty of our populations. So, despite some limits specific to the survey, particularly the absence of historadiological comparison for all the files, echography and IVU with cystography have always led to malignancy diagnosis. Therefore they should be requested for any patient consulting for haematuria.

[Etiologies of ocular and eyelid trauma in Burkina Faso].

PURPOSE: To identify the epidemiological features of ocular traumas in Burkina Faso. METHODS: A retrospective study was conducted in the eye department of the National Hospital of Ouagadougou, from January 1995 to 31 December 1997. All cases of eye trauma during this period were included. RESULTS: The study showed that ocular traumas represented 9.80% of the reasons for consultation in the unit. 73% of the patients were male. The median age was 21. School children and students were the most frequently involved 25.8%) and 58.9% of the patients came from Ouagadougou while 41.1% came from the rural areas. Public thoroughfare accidents (were the main causes of the trauma (25%). Among the traumatic agents, 26% involved vegetal materials. CONCLUSION: Ocular traumas are a major cause of blindness in Western Africa and concern mostly young people. Prevention strategies based on education and information about road safety could reduce the incidence of ocular traumatisms.

[Warm saline enema for reduction of intestinal invagination under the ultrasonographic guidance: preliminary results apropos of 2 cases].

The authors describe the use of isotonic normal saline enema under US guidance in the treatment of acute uncomplicated intestinal intussusception in infants. The technique was successfully used in two patients aged respectively 18 and 16 months, with 11 and 9-month follow-up. The various steps of this technique and the results are analyzed and discussed. It is a valuable alternative to surgical treatment or hydrostatic reduction with opaque x-ray contrast under fluoroscopic guidance.

[Contribution of medical imaging to visceral manifestations of sickle cell anemia in the child].

UNLABELLED: Sickle cell anemia is one of the qualitative hemoglobinopathies which are the most widespread in the world. In a retrospective study on 11 drepanocytary patients (10 SS and 1 SC) observed at the BORDEAUX children's hospital. 3 Processus were identified: Hemolysis: 3 hepatomegaly, 3 splenomegaly, 1 hematochromatosis and 4 pigmentary lithiasis; Thrombosis: 1 infarctus case of spleen, kidney, lung, 1 papillary necrosis and 1 brain vascular ischemic appearance; INFECTION: 1 spleen abscess and 1 pneumopathy. In 15% of instances, visceral appearances are allowed to discoverer sickle cell anemia. Hemolysis type appearances were currently observed (63.6%) like pigmentary lithiasis and hepatomegaly. The authors analyze the major radiological appearances of this hemoglobinopathy and recall the interest in knowing this disease even in low endemicity countries.

[Survey of nosocomial infection prevalence in the surgery department of the Central National Hospital of Ouagadougou].

The objective of this study is to collect preliminary epidemiologic data about nosocomial infections (NI) in the National Hospital of Ouagadougou. A prevalence survey of NI was realized in surgical wards (116 patients). All hospitalized patients were included, excepted those stayed less than 48 hours. Using the definitions of the "Conseil Supérieur d'Hygiène Publique de France", the nosocomial infections studied were surgical wards, pneumonia and urinary tract infection. The result showed that 19 patients (16.4%) had 26 nosocomial infections (prévalence: 22.4%). Six patients had at least 2 nosocomial infections. The infection rate increases with operation, debilated preoperative physical statute, and invasive care. The average increasing of duration of hospital stay for infected patients was 10 days. This data confirm the importance of nosocomial infections and emphasize the need of surveillance and control of nosocomial infections. But deeper epidemiological studies must be undertaken before.

[Duodenal diaphragm in children (two cases)].

Duodenal diaphragm is a rare cause of intestinal obstruction. Two pediatric cases are reported in which vomiting of bile-stained material and distension of the epigastrium were the main clinical signs. Roentgenograms and sonograms were highly suggestive. Duodenal bypass was easy to perform and effective. Nevertheless, it must be kept in mind that associated anomalies are frequent and may be severe.

[Breast cancer: diagnostic and therapeutic problems at the Ougadougou University Hospital Center].

A retrospective study on a 4 years period (1991-1994) enabled the authors within the digestive and general surgery of the Ouagadougou Teaching Hospital Center in Burkina Faso to list 39 breast cancers. During the same period, 3788 patients were hospitalized within the service out of which, 120 for mammary tumoral pathology. Patients, most of the time come late and sometimes with very severe and progressive lesions. Mammography and extemporaneous biopsy were not possible. Fine-needle aspiration for cytologic diagnosis practiced in 17% cases revealed to be insufficient. PATEY's mastectomy was done on 59% of patients. As far as chemotherapy is concerned, it is too expensive and inaccessible, only 9 patients have benefitted from this practice. Te histological test on surgical parts retrieved ductal infiltrating carcinoma 61.5%, medullary infiltrating carcinoma 15.3%, comedo-carcinoma infiltrating 5.1%, lobular infiltrating carcinoma 5.1%, papillary carcinoma 5.1%, carcinoma with fibrous stroma 2.5%, undifferentiated carcinoma 2.5%, scirrhous carcinoma 2.5%.

[Salivary gland tumors. Apropos of 48 surgical cases].

The interest of tumors of salivary gland has increased according to the nosology, diagnosis and therapeutic aspects. This study object to appreciate the frequency of this pathology at the ENT Department of the University Hospital Center of Ouagadougou and to report our experience in their diagnostic and therapeutic management. We have collected 48 cases, representing 2% of the department surgery activity and 7% of the tumors operated between 1985 and 1996. This report has concerned 32 parotid tumors (66.7%), 10 submaxillary tumors (20.8%), 6 minor gland tumors (12.5%). This series was composed of 28 women and 20 men with an average age of 41 years. Diagnosis management was based on clinical, radiologic and echographic examination. The correlation between preoperative and histological diagnosis was correct in 88.7% cases. Benign tumors represented 83.3% of the cases with essentially pleomorphic adenomas, and the malignant tumors 16.7% cases. Post operative evolution was simple in 85.4% cases. After 3 years, local recurrence occurred in 4 cases. The prognosis of malignant tumors has been unsatisfactory: the survival rate at 3 years has been 20%.

[Management of the diabetic foot, apropos of 42 cases at the Ougadougou University Hospital Center].

Feet lesions are the most important cause of diabetic's consultation in general surgery. A retrospective study was conducted in Ouagadougou National Teaching Hospital, in order to analyse epidemiology, clinic, and evolutive aspects of this affection. In the same period, 222 cases of diabetes mellitus were admitted in general surgery and internal medicine. We noticed 42 cases of feet lesions. The mean age of patients was 53 years. Anterior knowledge of diabetes has not been a factor of early consultation. Consultation mean delay was a month. In 28 cases, feet lesions were 4 or 5 Wagner grade. The role of ischemia and neuropathy have not been evaluated, but sepsis was highly determinant factor. Insulin has been used in 83.3% of cases. Amputation concerned 19 patients. Mean length of hospital stay was 16.6 days and mortality rate was 38.1%. Prevention of complications can be much more effective if it is applied as a part of global approach to diabetes mellitus management.

[Short- and long-term results of parotidectomy, apropos of 24 cases at the Ougadougou University Hospital Center].

We reported the short and medium results of the 24 parodectomies performed at the ORL Department of the NHC of Ouagadougou between 1985 and 1993 in order to assess the frequency of the complications and to search for their causes. It is a retrospective study on 15 women and 9 men aging between 14 to 56 years. The research for the VII has been always done at the trunk. In the absence of an extemporan histological test, 3 exofacial parotidectomies have been performed for slight injuries of the superficial ear-lobe, 2 total parotidectomies in the other cases. All our sick persons have a 3-years recession. We registered: 1 case of post surgery haemorrhagia (4%) 2 cases of additional infection (8%) 3 cases of facial paralysis that are recessive or not (12.5%) 6 cases with Frei syndrome (25%). Multiple factors can explain the profile that follows surgery of our sick-persons. We insist on the fact that there is minimum specialist that are used to this kind of surgery and to the smallest of the technical board. Complications and after effects after-parotidectomy were in our view almost frequent in our conditions of work. It is true that it is a delicate surgery, but the after effects can be better improved by additional specialist in ORL and with an adequate equipment.
