The place of attachment in the life events model of stress and illness.

Recent evidence indicates that perceived adequacy of support has substantial predictive power for the subsequent onset of minor psychiatric symptoms. Such findings suggest that social support is not purely a property of the environment, but is also a function of personality. The problem, however, with a re-emphasis on personality variables is that any one of scores of dimensions can be chosen for study. This paper argues that precision may be gained by conceptualizing personality variables in terms of basic behavioral systems that have been extensively investigated in ethological and developmental studies. One such basic system is attachment. The present paper discusses the relevance of attachment theory for understanding susceptibility to stress and individual differences in the use of social support during times of stress.

Help for families of suicide: survivors support program.

The Survivors Support Program attempts preventive intervention with a high risk population. As with most preventive efforts in psychiatry, it is difficult to predict who will be most at risk, what services will best meet their needs, and what positive effects are due to the services. To answer these questions through properly controlled and designed experimental programs conforming to scientific methodology would require the investment of much time, personnel and money. To postpone attempts at intervention until answers are provided by such experimental programs would be to ignore the evidence of common sense and clinical experience. The Survivors Support Program demonstrates that a volunteer self-referral service, organized through professional liaison with a community organization and committed to providing service and gathering information, can suggest interim answers to the questions while providing support and counselling to a needy population.

An agency model for developing and coordinating psychiatric aftercare.

The development of coordinated systems for delivery of aftercare services to psychiatric patients has lagged far behind the theoretical emphasis on community maintenance. From a collaboration of 29 treatment and rehabilitation facilities, an independent agency was established to improve aftercare services in Metropolitan Toronto. Known as Community Resources Consultants, the agency was designed to facilitate and rationalize the use of existing services, to identify gaps in service, to initiate or cooperate in the development of new services, and to involve hospital and community service personnel in raising the level of expertise in the provision of aftercare. Formal and informal assessments indicate that CRC has had a positive effect on the provision of aftercare services and on professionals' level of awareness of aftercare priorities.

A self-help program for widows as an independent community service.

Community Contacts for the Widowed, a self-help group in Toronto, offers individual and group counseling and volunteer work to widows. Beginning as a demonstration project funded by the provincial health ministry, the program became an independent charitable organization in 1976. Like many new self-help groups, the program employs peers who receive training in helping their clients by establishing a one-to-one supportive relationship. These peers, called widow contacts, also arrange group sessions and conduct community education on behalf of the program. Although reliance on a self-referral process limits the number of lower-class, immigrant, and other widows unwilling to reach out for help, since 1976 the program has helped more than 1500 women through their bereavement.

HLA-DRw7 and steroid-responsive nephrotic syndrome of childhood.

We searched for possible immunogenetic markers in steroid-responsive nephrotic syndrome of childhood (SRNS). The incidence of HLA-DRw7 was significantly greater in SRNS patients than in controls (71% in patients, 29.8% in controls, P < 0.005). The HLA-A and -B locus antigens occurred in normal frequencies. The relative risk factor for HLA-DRw7 in SRNS was 5.9. This report failed to show a relationship between HLA-DRw7 and atopy, use of alkylating agents or occurrence of relapse after remission induced by these agents. We believe that SRNS may be an immunologic disorder whose pathogenesis is related to an MHC-linked Ir gene.

The Janus principle.

The leaders who guide health organizations into formal and informal alliances must serve their own organizations while meeting the demands of a larger environment. Those who recognize the need for change in leadership style are most likely to help their organizations to maintain their strengths within new structures. The authors examine several theories of leadership style, describe the developmental stages involved in forming an inter-institutional organization, and discuss the new leadership tasks and skills that collaboration mandates.

Management uses of the Delphi.

In addition to its simplicity, the Delphi approach to planning and problem solving invites candor and uninhibited response from participants. It may also serve as a bridge to team building within the health care institution.
