Prostaglandins in semen and their relationship to male fertility: a study of 145 men.

Prostaglandin (PG) F2 alpha and PGE were measured in 163 semen samples from 145 men attending our male infertility clinic. In addition, each semen sample was analyzed for 13 different fertility parameters. Blood plasma levels of testosterone, follicle-stimulating hormone, and luteinizing hormone were also determined in many of the patients. The data obtained were then analyzed using multiple regression analyses on each PG for all of the parameters studied. The results indicate seminal PGF2 alpha and PGE concentrations were 2.78 +/- 0.24 micrograms/ml and 46.0 +/- 4.5 micrograms/ml, respectively. The seminal parameters that were significant predictors of seminal PGF2 alpha included: Ca++ concentration (P less than 0.001), Zn++ concentration (P less than 0.01), and percentage of tapered sperm (P less than 0.05). The seminal parameters that were significant predictors of seminal PGE included: Ca++ concentration (P less than 0.01) and sperm motility (P less than 0.05). Plasma testosterone was also a significant predictor of seminal PGE (P less than 0.05). These results suggest that seminal PGs are important to the human male fertility potential in that their levels are significantly interdependent with specific parameters of male fertility.

The effects of sulfasalazine on human male fertility potential and seminal prostaglandins.

Fertility parameters of 10 men with chronic inflammatory bowel disease under treatment with sulfasalazine for at least 5 years were compared to those of 19 control subjects. Seminal parameters examined included ejaculate volume, sperm number and concentration, sperm motility index, sperm viability, pH, zinc concentration, prostaglandins E and F2-alpha, prolactin and 7 classes of sperm morphology. In addition, plasma concentrations of follicle-stimulating hormone, luteinizing hormone, testosterone and prolactin were noted. The data indicate that sulfasalazine therapy reduces semen quality and that this effect can be reversed upon removal from therapy. This reversal is independent of seminal prostaglandin concentrations.

Direct fibrinolytic therapy for renal vein thrombosis: radiographic followup.

Renal vein thrombosis is a rare entity whose true incidence is unknown. The disease occurs most frequently in patients with the nephrotic syndrome but it also can occur in the presence of other hypercoagulable states. Previous modes of therapy have been limited to systemic anticoagulation or surgery. We report a case of successful dissolution of acute renal vein thrombosis using direct fibrinolytic therapy, which was documented radiographically during treatment and at 3-month followup.

Simplified new approach to endoscopic photography.

Information obtained by endoscopic examination can be shared with others by viewing sequentially through the same endoscope, "teaching attachments" allowing viewing ports through the same instrument, drawings based on the observers' recollection of the findings, and by endoscopic photography. Of these methods, endoscopic photography has the important advantages of speed and faithfulness of reproduction. Herein, we describe a simple technique for endoscopic color still-photography with illustrations of normal and pathologic findings.

Effect of low-dose androgen and zinc sulfate on sperm motility and seminal zinc levels in infertile men.

Beneficial effects of zinc therapy in some infertile patients with low seminal zinc concentrations and decreased sperm motility have been reported recently. The effects of zinc therapy, low-dose (5-10 mg/day) androgen therapy, and combination therapy of both zinc and low-dose androgen, on sperm motility and seminal zinc concentration were studied in patients referred to the andrology clinic. Low-dose androgen therapy increased seminal zinc concentrations and sperm motility, only when pretreatment seminal zinc concentrations were low. Furthermore, patients with low seminal zinc concentrations and poor sperm motility showed greater improvement of sperm motility and seminal zinc levels in response to zinc sulfate with fluoxymesterone than did fluoxymesterone alone or zinc sulfate alone, possible due to a synergistic effect of zinc and androgen.

Significance of testicular size measurement in andrology. I. A new orchiometer and its clinical application.

When performing a physical examination in the andrology clinic, accurate measurement of testicular size is important. Two methods for measuring testicular size have been developed using calipers for measurement and comparison with plastic testicular models. We have developed a new orchiometer, which consists of a graded series of punched-out elliptical rings with the volume of the ellipsoids indicated on each ring. Testicular sizes measured by our orchiometer had a better correlation with real testicular size determined by water displacement of testes after castration when compared with those data obtained by using calipers. By the use of this orchiometer, the normal range of the adult testicular size was greater than 14 ml in Japan and greater than 17 ml in the United States. This orchiometer is valuable for the rapid and accurate assessment of sexual maturation in children and men.

Spontaneous intrapartum rupture of renal hamartoma.

Spontaneous rupture of a renal hamartoma during pregnancy is a rare condition. Increased blood volume, increased renal blood flow, and vascular changes secondary to a change in the hormonal milieu may predispose to rupture in pregnancy. The diagnosis is made difficult by a desire to proceed cautiously and avoid the inherent risks of interventional radiography, aggressive surgical therapy, or conservative management as well. However, a conservative approach may be successful in selected cases.

Urinary incontinence.

Accurate diagnosis and successful management depend on knowledge of the anatomy and regulatory physiology of micturition. Incontinence is classified according to its mechanisms--passive incontinence, active incontinence with or without complete emptying, and overflow incontinence. Approaches to management include pharmacotherapy, surgical procedures and important steps in supportive care. This is a problem of any age. Educating the patient is essential.

Porcine sensitized lymph node cells (immunotherapy) and attenuated irradiation for infiltrative transitional cells carcinoma of bladder.

Thirty-four patients wih infiltrative bladder carcinoma, Stage B2C or higher were treated with immunotherapy and irradiation. Seventeen patients are alive, and 17 have succumbed to their disease. Eight patients underwent cystectomy after immunotherapy and irradiation; 6 of 8 are alive and well at the present time. The technique of immunotherapy is outlined. New methodology for sequential CT scans and scheduled bladder biopsies is mentioned. The 17 patients have survived twelve to sixty-nine months after immunotherapy and irradiation. Downstaging is demonstrated based on sequential CT scans of the bony pelvis and histologic biopsy. The biopsies reveal eosinophilia and multinucleated giant cells, a specific response to immunotherapy. A prospective randomized study will be initiated.

Surgery and immunotherapy in renal cell carcinoma involving inferior vena cava.

We herein report 4 cases of renal cell carcinoma with vena caval extension. Appropriate diagnostic studies and a planned surgical approach is the most effective treatment for intravascular spread. Accurate preoperative staging is important for a successful outcome. Although the technical challenges involved in performing a nephrectomy are substantial, these cases illustrate that standard nephrectomy with evacuation of tumor thrombus, ligation of the left renal vein, and even partial resection of the inferior vena cava can be performed with an acceptably low morbidity.
