[Myocardial infarct immediately after a normal exercise test].

Two cases of myocardial infarction immediately following a normal stress testing, are described. The incidence and possible pathophysiological mechanisms are discussed. In one of the patients it was difficult to establish the pathophysiological mechanism which was the cause of the ischemic event. In the other, the coronary arteriography revealed only minimal obstructive disease. Therefore, coronary vasospasm with thrombus formation as a cause of the infarction ia an interesting speculative possibility in view of the angiographic findings. Acute myocardial infarction after a normal electrocardiographic response to maximal exercise testing is extremely rare, and the precise pathophysiologic mechanism that leads to his complication is not clear.

[Wolff-Parkinson-White syndrome with orthodromic supraventricular tachycardia associated with a "hyper-conductor" atrio-ventricular node. A therapeutic challenge].

One case of Wolff-Parkinson-White Syndrome with paroxysmal supraventricular tachycardia related to orthodromic atrioventricular reentry using an accessory pathway for retrograde conduction an a rapidly conducting AV node for anterograde conduction is present. The pharmacological therapy with Digoxin, Propranolol, Quinidine, Disopyramide and Propafenone was not effective. An electrophysiologic study showed a reciprocating tachycardia induced by spontaneous ventricular beats. Both the effective refractory period of the AV node and the anterograde effective refractory period of the accessory pathway were minor or equal to 220 msec which made the control of the arrhythmia difficult. Amiodarone was able to suppress the premature ventricular beats, depress conduction and prolong refractoriness in both, the AV node and accessory pathway to prevent recurrences of atrioventricular reentry. In this patient a false positive test with ajmaline was documented. The electrophysiologic study showed the association of Wolff-Parkinson-White Syndrome with an enhanced atrioventricular nodal-conduction and allowed the selection of an appropriate antiarrhythmic agent.

[Amiodarone in the control of supraventricular tachyarrhythmias].

The clinical efficacy of amiodarone was observed in a group of 20 patients (9 women and 11 men) with a total 21 supraventricular tachyarrhythmias (16 paroxysms or premature supraventricular beats (PSB) and 5 with established arrhythmias), to whom amiodarone was given during one month: 800 mg/day the first week and 400 mg/day for 3 weeks. An electrocardiogram and a 24 hour Holter were taken before and after treatment. The results observed for paroxysmal arrhytmias and PSB were excellent in 94% and satisfactory in one patient. As for established arrhythmias the results were excellent or satisfactory in 80%. We conclude that ammiodarone is an effective drug with a wide security range in the treatment of supraventricular arrhythmias.

[Electro-vectorcardiogram in myocardial infarct with involvement of the right ventricle in humans].

The electrocardiograms of 7 patients with anatomical diagnosis of right ventricular infarction, were studied. Electrocardiographic and vectorcardiographic records of 11 patients surviving the acute stage of myocardial infarction, were also analized. In 10 cases, both ventricles were involved. All patients had echocardiograms, ventriculograms and cineangiograms of coronary arteries, which showed the damaged vessels and the severity of their alterations. A severe obstrucción of the right coronary artery was found in all the cases, except one, in each group. The signs of necrosis were recognized in DIII, aVF, right unipolar thoracic leads and abdominal leads MD, ME, less frequently MI. They consisted in QS, Qr or W complexes in the absence of intraventricular blocks, and in slurred QR complexes in the cases with right proximal or distal blocks. Q wave duration fluctuated between 35 and 60 msec. The orientation of the initial vectors resulting from the ventricular activation sequence (20 - 40 msec) as well as ARST were different if obtained from usual precordial loads or the right thoracic ones. The latter was concordant with the orientation observed in the standard leads. VCGF suggested the posterior-inferior necrosis by its clockwise rotation and delayed downward crossing (32.5 - 50 msec) or by the situation of the entire ventricular curve above and to the left of E point. The duration of QF loop as increased in 9 (17.5 - 35 msec) Analysis of VCGH made possible the diagnosis of anterior septal necrotic zone because of the changes in QH loop. VCGS generally started upward and showed a delayed crossing of the anterior-posterior axis downward.

[Exercise tests in pneumopathy patients].

In this prospective work eleven pneumopathes patients were studied, ten of them with HAP, which were subjected to effort prove in endless band, in order to know the electrocardiographic alterations presented during the exercise. The VO2 máx was determined simultaneously in four patients. In the four, the value of this parameter was found under the normal value, which showed us a poor increase of the cardiac expense for the demands in the exercise. If the effort proves had not been practiced, this fact could not have been known. Related to the electrocardiographic alterations, cellular diastolic depolarization signs (cellular damage), product of the cellular damage, were found. The mechanics of this cellular damage, as well as the one of the poor increase of cardiac expense, are mentionned in the discussion.
