Primary cutaneous CD30+ large T-cell lymphoma in a patient with psoriasis treated with cyclosporine.

We report the case of a 61-year-old woman who developed an anaplastic CD30+ cutaneous T-cell lymphoma during oral cyclosporine (CsA) therapy for recalcitrant psoriasis. Two months after CsA discontinuation, clinical and histological resolution of the lymphoma was observed. However, 3 years later extracutaneous involvement of the lymphoma could be detected. The association between CsA administration and the occurrence of the lymphoma may be casual, but a relationship with immunosuppression may also be hypothesized. We have reviewed all relevant data in the literature. To our knowledge, this is the first case of primary cutaneous CD30+ anaplastic large T-cell lymphoma in a patient treated with CsA for psoriasis.

Preparation and structural study of the enantiomers of alpha,alpha'-bis(trifluoromethyl)-9,10-anthracenedimethanol and its perdeuterated isotopomer, highly effective chiral solvating agents.

Enantiopure forms of alpha,alpha'-bis(trifluoromethyl)-9,10-anthracenedimethanol and the corresponding perdeuterated isotopomers were prepared. The conformational study was carried out by (1)H NMR, and the absolute configuration was determined by the X-ray study of the crystallized diastereoisomeric carbamate derivative. This compound was tested as a chiral solvating agent (CSA). The results showed very good discrimination for several racemic mixtures that improved other classical methods. The study of diastereomeric complexes was carried out by determination of the stoichiometry of the complex and the binding constant of the equilibrium.

Granulomatous lesions of the homolateral limb after previous mastectomy.

BACKGROUND: Granulomas may be classified as palisaded, sarcoidal, tuberculoid, suppurative and foreign body. In cancer patients they are defined as tumour-related sarcoid granulomas. Otherwise they are classified as granulomatous lesions of unknown significance. It is still unknown why neoplastic diseases are sometimes associated with granulomas. OBJECTIVES: To investigate granulomas found on the same side as a previous mastectomy. METHODS: We studied four women who had previously undergone mastectomy and presented with epithelioid cell granulomas of the same side as the previous mastectomy. CONCLUSIONS: The association between breast cancer and host granulomatous reaction is uncommon. The cases discussed may correspond to cutaneous tumour-related sarcoid granuloma reactions.

Managing vulvar lichen simplex chronicus.

OBJECTIVE: To develop a flow chart indicting which tests to perform with the aim of helping physicians with the diagnosis, management and treatment of lichen simplex chronicus. STUDY DESIGN: We reviewed the data on vulvar lichen simplex chronicus patients to delineate proper management. RESULTS: Patch test should be added to the series of tests performed to ascertain the causes of vulvar lichen simplex chronicus. CONCLUSION: Among the stimuli that may trigger vulvar lichen simplex chronicus, not only infections and irritating factors but also sensitization must be kept in mind, especially in long-lasting disease.

p53, cyclin-D1, PCNA, AgNOR expression in squamous cell cancer of the lip: a multicenter study.

BACKGROUND: The development of squamous cell carcinoma of the lower lip is an interesting model of photocarcinogenesis because of the structural and topographic characteristics of the lips. The purpose of this study was to evaluate the expression of immunohistochemical markers on the lips of patients with lower lip squamous cell carcinoma (LLSCC), compared with a control population. METHODS: Of the 98 subjects involved in the study, 58 were suffering from squamous cell carcinoma of the lower lip. The remaining 40 acted as a control. The case studies were taken from six university and hospital dermatology and plastic surgery departments. Questionnaires were administered to assess the risk factors for LLSCC. The cases involving squamous cell carcinoma underwent surgical excision and punch biopsy specimens were obtained from 20 control patients. Tissues were prepared in 5-microm-thick sections to carry out the following immunohistochemical study: PCNA, p53, AgNOR, cyclin-D1, bcl-2. RESULTS: The lower lip was the predominant location of squamous cell carcinoma, with the following factors playing important roles: chronic sun exposure, history of smoking, alcohol use and familial risk of cutaneous tumors. The male/female ratio in our survey was 5:1. The p53 protein was positive in approximately 50% of SCC cases and in 20% of controls. This protein is mostly associated with chronically photoexposed skin areas. AgNOR positivity increased with the loss of cellular differentiation; a progressive increase in size and a poorly defined shape were evident in poorly differentiated carcinomas. CONCLUSIONS: The results of this multicenter study showed that there is a noticeable difference in the expression of PCNA, p53, cyclin-D1, and AgNOR in tissues from patients with LLSCC and controls.

Anaerobic sealants: still a problem today.

Acrylic anaerobic sealants are widely used in engineering and electronic industries. They may induce allergic contact dermatitis of the first three fingers and onycholysis. We report a case of allergic contact dermatitis due to anaerobic sealants and we underline some practical problems connected with the frequency of sensitization, patch-testing and material safety data sheet availability.

Vulvar hidradenoma papilliferum. A review of 10.5 years' experience.

OBJECTIVE: To analyze 10.5 years' experience with hidradenoma papilliferum in Ferrara, Italy. STUDY DESIGN: Review of 8 cases plus 25 from other area clinicians. RESULTS AND CONCLUSION: The clinical aspects of hidradenoma papilliferum are confusing. The tumor is uncommon, so physicians do not gain enough experience to recognize it. Histology is diagnostic and simple excision curative.

Serious hematoma of the vulva from a bicycle accident. A case report.

BACKGROUND: Vulvar hematomas may be caused by obstetric, athletic or sexual trauma or by car or bicycle accidents. CASE: A 29-year-old woman presented with a serious vulvar hematoma caused by a straddle bicycle accident. Monitoring of the hematologic parameters showed progressive anemia resulting from silent bleeding, which required surgical evacuation of the hematoma and complete hemostasis. CONCLUSION: The rich vulvar vascular network may be easily damaged by contusive frontal impacts, which crush the vulvar tissues against the osseous planes. Management of vulvar hematomas may range from conservative to surgical. Drainage and incision are advised in more severe cases to reduce infective complications and hospitalization.

Prevalence of superficial dermatomycoses in 73 renal transplant recipients.

BACKGROUND: Because of chronic immunosuppressive therapy, the skin of renal transplant recipients (RTR) is considered more liable to fungal infections. AIM: The aim of the study was to analyze the prevalence of superficial dermatomycoses in a chronically immunosuppressed group of RTR who live in northern Italy and to verify the eventual relationship between the onset of mycoses, the immunosuppressive regimen and the interval of time elapsed after the transplantation. METHODS: 73 RTR were submitted to a complete dermatological examination for fungal infection. Skin scrapings were taken from the upper back, from the 4th toe web of all patients and from any suspicious lesion. RESULTS: 31 patients (42.5%) were affected by dermatomycosis. Pityriasis versicolor (PV) was present in 20 RTR (27.4%), fungal infection of the 4th toe web in 10 patients (13.7%) and onychomycosis in 9 RTR (12.3%). Trichophyton mentagrophytes was the most common dermatophyte. The prevalence of dermatomycoses was higher in the group of patients treated with azathioprine-cyclosporine-steroids and in those who had received their renal transplant in the previous 1-5 years. CONCLUSIONS: PV was the most frequent dermatomycosis and showed a higher prevalence than in the normal population. The prevalence of fungal infection of the 4th toe web and onychomycosis was similar to that found in the immunocompetent population, but the length of interval after transplantation seemed to increase the probability of their occurrence and of mixed or simultaneous fungal infections in the same patient.
