Managing vulvar lichen simplex chronicus.

Abstract

OBJECTIVE: To develop a flow chart indicting which tests to perform with the aim of helping physicians with the diagnosis, management and treatment of lichen simplex chronicus. STUDY DESIGN: We reviewed the data on vulvar lichen simplex chronicus patients to delineate proper management. RESULTS: Patch test should be added to the series of tests performed to ascertain the causes of vulvar lichen simplex chronicus. CONCLUSION: Among the stimuli that may trigger vulvar lichen simplex chronicus, not only infections and irritating factors but also sensitization must be kept in mind, especially in long-lasting disease.

In suspected acute pulmonary embolism, the measurement of the d-dimer is now an established screening method. Further diagnostic measures include the electrocardiogram, echocardiography and conventional chest radiography, as well as the analysis of blood gases. The specific demonstration of a pulmonary embolism is achieved with a helical CT of the thorax, pulmonary angiography and ventilation/perfusion scanning. Apart from continuous (infusion) of unfractionated heparin, the low molecular weight heparins also are of potential value in the treatment of pulmonary embolism. A larger percentage of patients benefit from the use of fibrinolytic agents than was previously thought, so that the indication for fibrinolytic therapy should not be made dependent on the presence of hemodynamic instability.

Diagnostic criteria for Parkinson's disease.

The diagnosis of Parkinson's disease is not easy. Developments in basic research have indicated pathophysiological links among parkinsonian syndromes that are still classified as independent entities. On the other hand, genetic studies are dividing forms that fit into the clinical diagnosis of Parkinson's disease. The diagnostic criteria used in current practice are by no means satisfactory, but cannot yet be replaced by new comprehensive criteria based on laboratory evidence.

Application of decision trees to the analysis of soil radon data for earthquake prediction.

Different regression methods have been used to predict radon concentration in soil gas on the basis of environmental data, i.e. barometric pressure, soil temperature, air temperature and rainfall. Analyses of the radon data from three stations in the Krsko basin, Slovenia, have shown that model trees outperform other regression methods. A model has been built which predicts radon concentration with a correlation of 0.8, provided it is influenced only by the environmental parameters. In periods with seismic activity this correlation is much lower. This decrease in predictive accuracy appears 1-7 days before earthquakes with local magnitude 0.8-3.3.
