Advance directives: the New Zealand context.

Advance directives convey consumers' wishes about accepting or refusing future treatment if they become incompetent. They are designed to communicate a competent consumer's perspective regarding the preferred treatment, should the consumer later become incompetent. There are associated ethical issues for health practitioners and this article considers the features that are relevant to nurses. In New Zealand, consumers have a legal right to use an advance directive that is not limited to life-prolonging care and includes general health procedures. Concerns may arise regarding a consumer's competence and the document's validity. Nurses need to understand their legal and professional obligations to comply with an advance directive. What role does a nurse play and what questions arise for a nurse when advance directives are discussed with consumers? This article considers the cultural dimensions, legal boundaries, consumers' and providers' perspectives, and the medical and nursing positions in New Zealand.

Interprofessional practice: learning how to collaborate.

Interprofessional practice is a way of practicing that is based on collaboration. We cannot assume that health professionals have either the skills or attributes required for interprofessional practice. They may need to learn how to collaborate. Developing interprofessional practice requires a commitment to engage in shared learning and dialogue. Dialogue has the potential to encourage collegial learning, change thinking, support new working relationships, and improve client care.

Setting the standard? New Zealand's approach to ensuring health and disability services of an appropriate standard.

This article critiques how health and disability providers' competence is monitored in New Zealand. The New Zealand Health and Disability Commissioner has the responsibility to uphold health and disability consumers' rights, including the right to an appropriate standard of care. Analysis of opinions examines how services of an appropriate standard are measured. Changes have been made to the Commissioner's investigative process since the current Commissioner took office in 2000, and comparison between opinions from 1999-2000 and 2004 assesses the impact of changes for providers and consumers. Currently, service providers interpret standards; a new approach is required to ensure consistent standard application so that consumers' rights are promoted according to the legislation.

Interdisciplinary team leadership: a revisionist approach for an old problem?

Understanding of interdisciplinary teamwork is evolving. During health care restructuring, leaders across organizations have challenging responsibilities when work groups must integrate changing organizational values with new modes of service delivery. In this environment, a well-functioning interdisciplinary team in which clinicians work as member-leaders has the potential to further organizational change and foster improvements in patient outcomes. In this paper it is argued that the term interdisciplinary team leadership should be embraced cautiously as it may be a revisionist approach to an old problem, namely a means to modify existing theories of leadership that have been vague and continue to be poorly understood despite considerable effort to explicate knowledge over several decades. Preliminary research suggests that interdisciplinary team leadership is a model of shared leadership that requires more development if it is to become the cornerstone of interdisciplinary team practice in a radically reforming health sector. Stewardship is proposed as a potential philosophy for interdisciplinary team leadership, and a new, shared leadership role of practice leader is suggested.

Grappling with the literature in a grounded theory study.

Student researchers often struggle to understand how to use literature in a grounded theory study where timing and knowing what to read are critical. Despite substantive theoretical documentation on this topic the reality of working through abstract ideas is more challenging. There is a fine line between not doing a literature review in the area of study and being informed so that a study is focused. In this paper a practical example will be presented illustrating how the student can integrate literature yet stay away from preconceived notions. The topic is interprofessional practice.
