Role of computerized morphometric analysis in diagnosis of effusion specimens.

The present study was undertaken to compare morphometric analysis of cells in 100 effusion samples, with unaided cytological diagnosis. All the cases in the form of Leishman's stained smears were subjected to computerized-interactive morphometry (CIM) by usage of Image analysis software. On routine cytological examination, 44 cases were benign, 26 were malignant, and 30 cases were placed in "atypical" category. On morphometry, values for mean nuclear area (MNA) and mean cytoplasmic area (MCA) in benign cases were found to be 58.39 +/- 12.22 microm(2) and 185.70 +/- 43.69 microm(2), while for malignant cases were 120.12 +/- 16.28 microm(2) and 274.65 +/- 61.01 microm(2), respectively (P < 0.001, P < 0.001, highly significant). Significant differences were also noted among benign, atypical, and malignant cases for MNA (P < 0.001). The differences between atypical and malignant cases for N/C (nuclear/cytoplasmic) ratio were also found to be statistically significant (P < 0.001). Finally, 46 cases were found to be benign, 39 malignant, and 15 in the category of atypical cases. CIM helps in better and quicker sorting for malignant cells. MNA and N/C ratio are the most significant parameters in differentiating various cells in effusions. MCA constitutes as another useful parameter. Salient features of CIM, including their applications in the light of current study, are discussed.

Primitive neuroectodermal tumor (PNET) as second malignancy after treatment of Hodgkin's disease.

Hodgkin's disease survivors are at an increased risk of developing second malignant neoplasms including secondary bone tumors. Common secondary bone tumors are osteogenic sarcoma and fibrosarcoma. Secondary primitive neuroectodermal tumor is extremely rare in this group. We present below, a rare case of secondary PNET in an 8-year-old child with Hodgkin's disease which developed unusually early outside the radiation portal and discuss potential factors responsible for its causation.

Morphometric evaluation of nucleolar organiser regions in reactive and neoplastic lymph node lesions.

Argyrophilic nucleolar organiser regions (AgNORs) have widely been used for making diagnostic as well as prognostic statements in neoplastic lesions. The present study was aimed at the quantitative and morphometric analysis of AgNORs in various lesions of lymph node. Thirty cases consisting 10 cases each of reactive lymphadenitis, non-Hodgkin's lymphoma (NHL) and metastatic deposits from carcinoma breast were included in the present study. Out of the ten cases of NHL, there were four cases of low grade and six cases of high grade lymphoma. The quantification of AgNOR dots and their morphometric evaluation were done using a specific programme of an automatic image analyser. The results revealed that mean AgNOR count was maximum in metastatic lymph nodes (5.37 +/- 0.35) when compared with NHL (high grade 5.12 +/- 0.27; low grade 4.61 +/- 0.13) and reactive lymphadenitis (2.45 +/- 0.49). The mean area of single AgNOR dot in metastatic lymph node, high grade lymphoma, low grade lymphoma and reactive lymphadenitis were 123.3 +/- 26.1, 95.3 +/- 12.2, 99.6 +/- 6.53 and 87.4 +/- 8.45 respectively. Similarly, the mean of total AgNOR area in metastatic lymph nodes was highest (668.4 +/- 169.9) followed by NHL (high grade 496.35 +/- 53.2; low grade 459.25 +/- 37.35) and reactive lymph node (215.0 +/- 37). However, the ratio of single AgNOR area to nuclear area was maximum in reactive lymphadenitis (0.132 +/- 0.001) followed by low grade NHL (0.083 +/- 0.0096), high grade NHL (0.073 +/- 0.0098) and metastatic lymph nodes (0.064 +/- 0.001). The ratio of total AgNOR area to nuclear area was also minimum in reactive lymph node when compared with other neoplastic lesions. Statistically, the p-value revealed a highly significant difference among all these lesions. Thus, it can be concluded from the present study that AgNOR count and other morphometric parameters are significantly altered in different lesions of lymph node which may aid in differentiation of problematic cases.

Immunohistochemical study of the expression of epidermal growth factor receptor in benign prostatic hypertrophy, prostatic intraepithelial neoplasia and prostatic carcinoma.

The present study provides an analysis of immunohistochemical expression and localization of epidermal growth factor receptor (EGFR) in formalin fixed paraffin embedded specimens of prostate. Thirty-five cases each of benign prostatic hypertrophy (BPH) and prostatic carcinoma and 30 cases of prostatic intraepithelial neoplasia (PIN) were taken up for study. Streptavidin biotin peroxidase method was employed for immunohistochemical staining. EGFR positivity was observed in all the cases (100%) of BPH and PIN and in only 10 cases (28.5%) of prostatic carcinoma. In both BPH and PIN the basal cells revealed significantly higher intensity and percentage cell positivity than the luminal cells. Intensity and percentage of positively stained basal cells in BPH was higher than PIN basal cells but the difference was not statistically significant. The intensity and percentage cell positivity of BPH basal cells and PIN basal and luminal cells were significantly greater than the epithelial cells of prostatic carcinoma. Presently, the significance of variable expression of EGFR in various types of prostatic lesions is unknown.

Fibromatosis of the breast--a case report.

A case of fibromatosis of the breast in a 30 years old female is described. Only a few cases have been previously reported. It is anticipated that fibromatosis of breast behaves in a similar fashion to fibromatosis occurring in other sites. It is a locally aggressive lesion which exhibits a high incidence of local recurrence following incomplete excision.

The authors have described the different perspectives like epidemiology, biology and outcome, barriers to optimal treatment of childhood cancers in India along with a brief review of literature in the present article.

Cystic lymphangioma of the spleen--a case report.

Splenic cystic lymphangioma is a very rare condition. It occurs commonly in children in whom it is often an incidental imaging finding. In the absence of histologic confirmation, it usually mimics hydatid disease and other cystic vascular proliferations of the spleen. We present a case of cystic lymphangioma of the spleen in 15 years old female.

Extraskeletal chondroma of the hand--a case report.

Extraskeletal chondroma is a rare entity. A fairly benign condition, it is usually seen in adults. It presents as an enlarging mass, most commonly in the hand. Local excision is the treatment of choice. We present a case of extraskeletal chondroma of hand in a 12 year male child. Its variable histological appearance not infrequently leads to a mistaken diagnosis of chondrosarcoma.

Malignant Brenner tumour--a case report.

Malignant Brenner tumour is a rare pathological entity. Apart from identification of typical benign, metaplastic and/or proliferating components, stromal invasion must be observed for diagnosis of Brenner tumour. A case of malignant Brenner tumour is described along with a brief review of strict criteria of diagnosis and its biological behaviour.

Mesoblastic nephroma of infancy.

This report presents features of mesoblastic nephroma, a rare benign tumor of kidney observed in perinatal period of life and highlights the role of imaging investigations in the proper management of this tumor. Awareness of this tumor may facilitate prevention and management of severe obstetric and neonatal complications and inadvertent and vigorous therapy compatible with that of Wilms' tumor can be avoided.

A 16-year study of HIV seroprevalence and HIV-related diseases in a teaching tertiary care hospital in India.

A total of 183,912 persons were screened from September 1986 to May 2002 in and around Haryana, out of whom 1178 were reactive (0.64%) for anti-HIV antibodies. The overall incidence of HIV-1 in the seropositive patients was 98.5%, HIV-2 alone was 0.8% and 0.7% had a mixed infection with both HIV-1 and 2. High seropositivity (22.8%) was observed among the relatives of these HIV-positive individuals. The heterosexual route (78%) remained the predominant mode of transmission. Among the jail inmates only one individual out of 1306 (1986 to 1993) was found to be positive. Tuberculosis (46.7%) was the most common opportunistic infection in these seropositive patients. Significant titres of Venereal Disease Research Laboratory tests were observed in 8.8% seropositive patients, 7.9% were positive for hepatitis B surface antigen and only two patients were suffering from chancroid. Forty-six of these HIV-seropositive patients had already died. In order to cope with this epidemic, new models of care and cost-effective preventive measures are needed.

Papillary carcinoma thyroid: FNAC diagnosis.

Thyroid carcinoma in children is currently assuming greater importance due to increase incidence in the recent times. In carcinoma thyroid, the age at the time of diagnosis is an important prognostic factor and children in particular are considered to have an excellent prognosis. We came across 3 cases of papillary carcinoma thyroid involving both the lobes and having lymph node metastasis in children, the youngest being 4 years of age at the time of diagnosis. All these cases were diagnosed on aspiration cytology.

Pancreatoblastoma (infantile pancreatic carcinoma)--a case report.

Pancreatoblastoma or infantile pancreatic carcinoma is a rare pancreatic tumor with distinct acinar and squamoid cell differentiation that generally affects infants and young children. Ultrasound and CT scan may be useful but preoperative diagnosis is often quite difficult. The outcome is generally favourable. A such case of 10 years old boy with an abdominal mass is being presented.
