[Findings on retinal surface by scanning electron microscopy. III. Epivascular glial clusters (author's transl)].

The findings by scanning and transmission electron microscopy of multiple glial excrescences from the retina into the cortical vitreous body are described. These formations are located in lines above superficial retinal vessels. The findings are compared with those from light microscopy of similar formations. The pathogenesis and the possible significance as retinovitreal attachments for rhegmatogenous defects of the retina and for posterior vitreous detachments are discussed.

Aged

[Necrotizing hypersensitivity-angiitis Zeek with ocular involvement (author's transl)].

A clinical and pathological report is given of a case of necrotizing angiitis Zeek. This rare condition is related to polyarteriitis nodosa, but has an acute course and involves the small vessels. The histology of ocular complications has not been observed up to now. Clinically it appeared as a severe panuveitis and hemorrhagic retinitis. Histologically the vessels of the retina, the choroid, the sclera and the optic nerve show inflammatory signs similar to those of the viscera and the brain. The literature concerning the ocular involvement due to necrotizing angiitis is reviewed and compared with the present case.

Acute Disease

[Trabeculectomy: surgical technique, results, indications (author's transl)].

In this teaching-course special emphasis is given on surgical techniques and postoperative care of trabeculectomy. The follow-up of 114 eyes of 103 patients is reported. High tension in the early postoperative phase allowed no prediction of the later results. After 3 months 11%, and after one year 39% of the operated eyes with simple glaucoma had a tension higher than 22 mm Hg, even if drugs were given. Different sites of the excision in regard to the trabecular meshwork and Schlemm's canal gave no statistically different results. The filtering blebs have the tendency to develop in two directions, either scarring or becoming cystic. The cystic filtering blebs give better functional results. The development of the filtering bleb was inversely proportional to the height of the tension but went parallel to the facility of outflow. The visual acuity diminished in 35%, mostly due to cataract-formation and improved in 3%. Different complications happened in 6%. The authors consider the trabeculectomy to be the operation of choice for almost all forms of glaucoma.

Follow-Up Studies

[Findings on retinal surface by scanning electron microscopy. I. Retinal holes (author's transl)].

The findings by scanning electron microscopy on three retinal holes are described. An outgrow of a glial membrane with microvilli from the holes to the retinal surface can be seen. This epiretinal cell membrane grows on a felt of collagen fibres of the vitreoretinal borderlayer. Rudiments of visual receptors project from the deep walls into the holes. The layer of the pigment epithelial cells beneath the holes is remarkable by the abundance of microvilli.

[Adenoma of the ciliary epithelium of unusual structure and localisation (authors transl)].

The authors describe an adenoma of the ciliary epithelium of low degree of malignacy. The tumor is clinically and histologically remarkable because of its uncommom site behind the iris and the abundance of mucoid stroma. The clinical data, the course as well as the histological findings are described and compared with the rare similar cases in the literature. Electron microscopic data of this kind of tumor are reported for the first time. The tumor are characterised by basal membranes, microvilli, desmosomes, occasional melanin granules, a well differentiated RER and specially by the massive content of intracytoplasmatic filaments.

Adenocarcinoma

[Ultrastructural studies on the pterygium. I. Epithelium and glands of the conjunctival part (author's transl)].

13 pterygia from patients aged between 39 and 81 years are studied by electron microscopy. The pterygium is covered by a multilayered, highly differentiated cylindrocellular epithelium. Arrangement and height of the microvilli of the superficial epithelial cells differ from those of the conjunctival epithelium. Morphological signs of secretion and resorption are found. As a characteristic the basal epithelial cells of the pterygium are rich in metaplasmatic filaments. Tubular glands are localised underneath the covering epithelium; they may reach the deep substantia propria. Their walls consist of a single layer of cylindrical cells. A varying number of goblet-cells are embedded between them. The secretion by the epithelial formations of the pterygium is of the merocrine, holocrine and microapocrine type.

[Ultrastructural studies on the pterygium. II. Connective tissue, vessels and nerves of the conjunctival part (author's transl)].

The stromal part of the pterygium is remarkably rich in plasma cells. The connective intercellular substance shows two different pathological modifications: A clotting together of collagen fibres to broad, osmiophilic, elastoid bands and a widening of the spaces containing the granular nonfibrillar ground-substance . The vascular system consists mostly of fenestrated sinusoid capillaries. Their endothelial cells frequently show degenerative changes and the basement membrane of the vessels can be as thick as up to 50--100 fold the normal size. For the first time nervous formations could be found in the pterygium. Axons of the autonomic system join the basal epithelial cells, where they form terminal endings. Encapsulated myelinated and nonmyelinated corpusles probably stem from the sensory nervous system.

[Selective atrophy of the radial peripapillary capillaries and visual field defects in glaucoma (author's transl)].

The existence and distribution of atrophic radial peripapillary capillaries in 33 autopsy eyes of patients with chronic glaucoma or different optic nerve atrophies are compared with their visual field defects and stage of atrophy or excavation of the disc. No correlation of defective RPC and visual field defects was found. The selective atrophy of RPC is therefore supposed to be secondary to different atrophies of the disc.

The clinical features and pathological characteristics of a case of apocrine mixed tumour, a rare sweat gland neoplasm located in the lid, are described.

Adenoma, Sweat Gland

[Are the symptoms "white with pressure" and "white without pressure" related to peripheral retinal sclerosis?].

Collagenic formations occur increasingly with age in the retinal periphery of adults. They grow from the vitreous cortex and from the vessels. In extensive growth they form intraretinal networks. Different inflammatory, degenerative and malformative diseases of the retina lead secondarily, and this already in young persons, to an extensive development of such structures. The name 'peripheral retinal sclerosis' is proposed for such alterations in the peripheral retina. Since the peripheral retinal sclerosis occurs at the same areas where the symptom 'white with pressure' is found, one believes that these formations are its organic substrate. The opacification is thought to be a consequence of the stretching of these collagenic formations during the indentation. Deposition of lipids in these formations can show the symptom 'white without pressure'.

[Balloon cell nevi of the conjunctiva (author's transl)].

The clinical and histological features of three cases of conjunctival balloon cell nevi are described. This peculiar form of nevus is very rare in the conjunctiva. The findings are compared with the descriptions in the literature of dermal balloon cell nevi. They demonstrate, that the conjunctival and dermal tumours are of idential histological structure. The proliferations of the conjunctival epithelium often found in conjunctival nevi do not modify the balloon cell nevi. These can not be diagnosed clinically. The problems of the pathogenesis of the balloon cell nevi are discussed.
