Workplace interventions for workers with musculoskeletal disabilities: a descriptive review of content.

BACKGROUND: Despite the convergence of scientific data to the effect that interventions in the workplace promote a healthy return to work, the interventions carried out in the real work environment appear to be very heterogeneous and ill-defined. OBJECTIVE: The goal of this review is to identify the different objectives pursued through the workplace interventions carried out in the context of a rehabilitation program, and to describe the activities involved. METHODS: A descriptive review of the literature, including various research designs, was carried out. RESULTS: This review reveals great heterogeneity in the content of interventions offered in the workplace to workers with musculoskeletal disabilities. The objectives of workplace interventions may range from gathering information in order to reproduce work demands in a clinical setting, to gradually exposing workers to the demands of the real work environment, or permanently reducing the demands of the work situation. A descriptive analysis of the literature also brings to light the diversity of actions carried out, human resources used, and workplace environments involved, while highlighting the few documented process outcome evaluations that have been done of workplace interventions. CONCLUSION: It is recommended that in future research in this area, efforts be made to better describe the components of the interventions, to develop process outcomes representing the multidimensional results obtained in the workplace, and to differentiate between temporary and permanent modifications made to the work situation.

Alkaloids and amides from Triclisia sacleuxii.

A new alkaloid, named gasabiimine (1), together with four known bisbenzyltetrahydroisoquinoline alkaloids (phaeanthine, 1,2-dehydroapateline, N-methylapateline, O-methylcocsoline) has been isolated from the roots of Triclisia sacleuxii. The structure of the new alkaloid was elucidated by spectroscopic data analysis.

[Clinical study of cysts jawbones].

The objective of this study was to appreciate the clinical and paraclinical aspects of cysts jawbones. It was an exploratory study about 61 cases of jawbones cysts (24 men and 37 women). It took place in the Odontostomatology Department of the General Hospital of Grand Yoff at Dakar, during a period of 5 years (February 1996 to December 2000). The mean age of patients was 29.67 years with extremes of 14 and 55 years. The mean reason of consultating was an important perish-jawbone tumefaction which can be associated with dental displacements, mobilities or pains. The radiographic assessment was essential for the diagnosis. The radiological image was characterized by an osteolysis with regular contours. We have noted the invasive appearance of the osseous lysis which is developed in several teeth concerning 50 cases (81.97%). The anatomo-pathological exam which is essential to confirm the diagnosis has been possible only for 9 patients by lack of means. Thus, in our study the diagnosis of jawbones cyst was retained while being based primarily on clinical experience and the radiological assesment. The types of diagnosed cysts are: 53 perish-apical cysts (86.89%), 5 dentigerous cysts (08.20%), 2 globulo-jawbone cycts (03.28%), and only one case of paramount cyst. The jawbones cysts are often of aggressive development with various etiologies. They may be voluminous. Their diagnosis must be exact in order to allow an adapted surgical technics and to prevent the reccurence.

[Surgical and orthodontic treatment of inclusions and dental retentions: report on the upper incisives and canines block].

We have consecrated this retrospective report to the clinical study and treatment of 18 cases of inclusions and dental retention observed in the services of orthodontics and oral surgery of the department of odontology of the medical faculty of the Dakar University. From that study it arises that 50% of the inclusions and retentions are casually discovered and that canines and central incisives take up to the first place. With regard to the surgical treatment associated to orthodontics, it is only seldom worked up because of its duration and the late appearance of the results. However it agrees to retain that the treatment of these affections can be conceived only in the point of view of a multidisciplinary approach.

Immunoaffinity chromatography for the sample pretreatment of Taxus plant and cell extracts prior to analysis of taxanes by high-performance liquid chromatography.

The application of immunoaffinity chromatography for the purification of Taxus plant and cell extracts prior to the HPLC analysis is described. Polyclonal antibodies raised against 10-deacetylbaccatin III (10-DAB III), paclitaxel's main precursor in plant, were characterised by enzymed-linked immunosorbent assay. Immunoglobulins from selected antisera were immobilised on CNBr-activated Sepharose 4B. The immunoaffinity column was used for the purification of plant and plant cell culture extracts prior to their analysis by HPLC. Immunoaffinity chromatography enabled the selective concentration of taxoids and enhanced sample clean-up.

Cost-benefit and cost-effectiveness analysis of a disability prevention model for back pain management: a six year follow up study.

AIMS: To test the long term cost-benefit and cost-effectiveness of the Sherbrooke model of management of subacute occupational back pain, combining an occupational and a clinical rehabilitation intervention. METHODS: A randomised trial design with four arms was used: standard care, occupational arm, clinical arm, and Sherbrooke model arm (combined occupational and clinical interventions). From the Quebec WCB perspective, a cost-benefit (amount of consequence of disease costs saved) and cost-effectiveness analysis (amount of dollars spent for each saved day on full benefits) were calculated for each experimental arm of the study, compared to standard care. RESULTS: At the mean follow up of 6.4 years, all experimental study arms showed a trend towards cost benefit and cost effectiveness. These results were owing to a small number of very costly cases. The largest number of days saved from benefits was in the Sherbrooke model arm. CONCLUSIONS: A fully integrated disability prevention model for occupational back pain appeared to be cost beneficial for the workers' compensation board and to save more days on benefits than usual care or partial interventions. A limited number of cases were responsible for most of the long term disability costs, in accordance with occupational back pain epidemiology. However, further studies with larger samples will be necessary to confirm these results.

[Limit of root canal obturation by gutta percha compaction technique in single rooted teeth. Clinical data apropos of 168 canals].

After the introduction of the lateral compactage of gutta percha in endodontic clinic of OdontoStomatology Institute of Dakar in 1995; the immediate evaluation of the quality and the limit of the canal obturation due to a prospective study on 157 monoradicular teeth so be 168 canals (11 supplementary canals) has shown that 75% of canals are of type I of Vertucci and that the apical limit of security is reached within 80.25% of cases. The manual step back canal preparation (79.17%) and the technique of canal obturation by lateral compactage of gutta percha [(64.88%); Apical limit of security: 59.52%] permit to obtain a densities (95.83%) and homogeneous (79.57%) canal obturation. The indication of gutta percha compactage on monoradicular teeth (hermetic and tridimensional root canal filling) must avail to apical curved roots of lateral incisive, the use of adapted instruments in case of canines (31 mm) and apical stop cone on necrotic pulp teeth.

[The place of radiography in endodontic treatment carried out in general practice in Dakar].

An investigation conducted over dental surgeons of 52 dental offices in Dakar and it's near suburb (40 from private sector and 12 from public and quasi-public) has shown that the use of retro-alveolar radiography during endodontic treatment was not systematic, due to the price of retro-alveolar film, waste of time and relative facility of the endodontic treatment of monoradicular teeth. The preparatory negative is favored (32.70%) particularly by private dental surgeons, who use two retro-alveolar films for endodontic treatment: pre and post operative radiography (30%). The immediate post operator control is executed only in case of post-operator pains by public and quasi-public dental surgeons (16.67%) with one retro-alveolar film on average for the endodontic treatment; at term any radiographic control is done. Three retro-alveolar films per endodontic treatment were exceptionally used (9.61%) and offset incidences (15%) are not much used as well as support-films (angulators). The retro-alveolar radiography is still a reliable guide for a clinical success in endodontic treatment with a minimum of three retro-alveolar films and offset incidences.

[Evaluation of the height of the apical delta of maxillary anterior teeth. Surgical significance].

It's a study dealing with the apical delta position in the management of peri-apical lesions. The study was carried out from the observation of root canal systems of 150 teeth mainly central, lateral incisors and canines. The study has been bounded by the conservative dentistry and oral surgery departments of the UCAD Dental Institute. It lasted from February to November 2001. We noted that the maximal height of the apical deltas comparison to the apices is 1.6 mm. So, the height of 3 mm section proposed by MEYER during the apicectomies is enough to eliminate the major part of the accessory canals.

[Clinical study of alveolites and their treatment in multiple practice].

The alvéolite is an post-extractionnel accident, very frequent, unforeseeable, characterized by a deterioration of the physiological process of formation of the clot and occurring as well at the healthy subject at the degenerate. The objective of this work is to make an exploratory study on the cases of alvéolites which have occurred in three structures of odontological care in Dakar between October 2000 and June 2001. We studied the clinical and therapeutic aspects alveolites among 55 patients having undergone 168 extractions. On the whole 68 cases of alveolites were indexed including 18 suppurées forms and 50 dry forms. The alveolite was occured among 44 women and 11 men and the section of age which most touched ranges between 20 and 29 ans (51 %). The evoked factors etiologic are multiples(taken oral contraceptives, use of vasoconstricteurs. technical of extraction etc?). The treatment is local by type of antiseptic paste and general with base of antibiotic, antiseptic and anti-inflammatory. We don't have real coding of the treatment. The prevention remains the best attitude and consists with: - To carry out the acte operational with a rigorous asepsis - To avoid the inopportune instrumental traumatisms - To Irrigate the bone in permnance during the utilisation of rotary instruments.

[Oral hygiene habits and dental caries among students. Investigation of 150 students in university housing in Dakar, Senegal].

The objective of this study is to describe the oral hygiene habits and the status of the teeth decay of 150 senegalese students living in a university campus. The results showed that the toothbrush as mean of oral hygiene supplanted the traditional means such as "cure dents" or water rinse with finger. Results showed also a high rate decay (60%) but the CAO index remained low (2.04%). We founded an improvement of dental decay status with an increasing daily number of tooth brushing: we noted a decreasing dental plaque deposition rate, a decreasing decay rate and a CAO index remaining low until adult age.

[Pain in pediatric dentistry: etiology and management].

From January 1998 to January 1999, 458 files of 2 to 15 years old patients have been collected to indexthe different causes of pain in pediatric dentistry and to record the treatments applied to face that. It arises from this study that BAUME category II takes up from as far to the first place (77.7 %) followed by the category III (2.3) with a notable difference which nevertheless, can be explained by the transient nature of this category in the child. However, according to our study, pain abolition doesn't exclusively depend on drugs, some surgical procedures are indispensable. At the end of this study, it is to retain that, in spite of the importance typically dental pain, the problem of pain in pediatric dentistry must be approached in a entire sight.

[New attitudes to the treatment of recurrent idiopathic pneumothorax].

We examine the classical indications for thoracoscopy in case of pneumothorax and recall the therapeutic modalities of thoracoscopic treatment. We then discuss the long-term incidence of this approach, examining outcome in terms of post-operative pain, respiratory function, cost and complications. A comparison of these parameters after thoracoscopy with those observed after classical surgery would favor thoracoscopic management.

[Analgesic effect of Pilostigma reticulatum (nguiguis)].

Mankind has always given himself means to fight pain by using at first, means offered to him by his environment particularly the plants. African pharmacopoeia is rich of thousand of plants. It changes in term of its ecosystem and its vegetation. Decocted leaves of pilostigma reticulatum (nguiguis in ouolof) are used in western Africa, because of its analgesic properties in case of "borom bop" literally meaning headache associated to odontalgias and mumps. In our study we used lyophilisate obtained from dry leaves of the plant which has been used for experimentation while the decocted dry leaves have been used for clinical application. So, it appeared that leaves of pilostigma reticulatum are almost atoxic when administrated by oral tract (DL50 = 17 g/kg) according to GLEASON classification which recognizes as atoxic every substance having a DL50 higher to 15 g of lyophilisate by kilogram of corporal weight. Elsewhere the study of the peripheric analgesic activity (according to the acetic acid test) has shown a very significant peripheric analgesia since the dose of 750 mg/kg which climbs with it. The clinical survey carried out at the dental community center of Pikine Icotaf based on the usual method of the utilization of the decocted (as mouth rinse) has shown that, this plant procures pain sedation in 97% of the patients having undergone dental avulsion and in 78% of the cases of desmondontal syndromes. This inaugural report must be continued in order to certify the pharmacologic or toxic effects of that plant and define clinical doses from experimental doses we have cleared.
