Epidemic of fatal encephalopathy in preschool children in Burkina Faso and consumption of unripe ackee (Blighia sapida) fruit.

BACKGROUND: On March 21, 1998, the Regional Health Authority of Bobo-Dioulasso, Burkina Faso, asked the Centre Muraz to investigate an unexplained outbreak of epidemic fatal encephalopathy (EFE). We aimed to identify the cause of this epidemic. METHODS: We identified cases retrospectively through review of health-service records and interviews of family members, village chiefs, and local healers. Active surveillance was started in administrative divisions within the study area in April, 1998, to identify further EFE cases. We did a case-control study of households to investigate the risk from various environmental and health factors. Blood and urine samples were collected if possible and urine dicarboxylic acid concentrations measured by gas chromatography. FINDINGS: 29 cases of EFE were identified from January to May, 1998. Estimated age-specific attack rates (2-6 years) ranged from 31 to 847 per 100,000 population (p<0.001). The most common symptoms were hypotonia, vomiting, convulsions, and coma. All children died in 2-48 h. The only factor associated with EFE was the presence of ackee trees (Blighia sapida) within 100 m of households (odds ratio 5.1 [95% CI 1.8-14.7] p=0.001). Poisoning with unripe ackee fruits was suggested by urine concentrations of dicarboxylic acids four to 200 times higher in cases (n=2) than in controls (n=3). CONCLUSION: Consumption of unripe ackee fruit probably caused this epidemic and may lead to a substantial number of unexplained deaths in preschool children in west Africa every year. Educational campaigns have the potential to prevent these deaths.

B-SPID: an object-relational database architecture to store, retrieve, and manipulate neuroimaging data.

We propose a hardware and software architecture to respond to crucial problems in the neuroimaging field: storage, retrieval, and processing of large datasets. The B-SPID project, here discussed, concerns the processing of neuroimages and attached components stored in an object-relational multimedia database management system (DBMS). Advanced bioinformation concepts are exploited in this project such as large scale data storage, high level graphical user interfaces and 3D graphical processing and display of data. Our database implementation is based on standard programming components, runs on several UNIX platforms and is written to be evolutive. Queries on this database are designed to obtain and display from neuroimaging data several types of results (pictures, text, or 3D graphical shapes) on heterogeneous systems.

Epidemiology of the incidence of oro-facial noma: a study of cases in Dakar, Senegal, 1981-1993.

Oro-facial noma is an oral gangrene occurring in early childhood in extremely poor areas. As many as 70-90% of those with noma die, and to date, there is no satisfactory treatment to fight this disease. Within the context of the World Health Organization international program against noma, a 13-year retrospective study based on clinical records was carried out in Dakar, Senegal in an attempt to understand the epidemiology of noma. Between 1981 and 1993, 199 cases of noma were identified, among them; 36.7% were acute cases and 63.3% showed sequelae. Chronic sequelae of noma were seen in patients 2-41 years of age, but the acute phase of noma was found only in young children (77.7% in those 1-4 years of age, maximum age = 9 years, mean age +/- SD age = 3.4 +/- 1.9 years). A total of 73.1% of the cases with acute disease were reported in the Dakar, Diourbel and Kaolack regions during the dry season (57.0% of the cases). The lesions of progressive noma were localized mainly on the upper lip (42.4%) and the cheek (31.1%). A total of 96.9% of the patients with acute diseases were had poor general health with serious associated diseases; only 20.0% had a good vital prognosis. The development of epidemiologic surveillance programs for noma should be a public health priority in Senegal.

A review of database management systems suitable for neuroimaging.

This study comprises a technical assessment of Database Management Systems (DBMS), which may be of use in the analysis of data obtained from human brain mapping procedures. Due to the large expansion of the neuroimaging field, the use of specialized database software to store and process neuroimages and their attached components is inevitable. The advent of multiple software products, a wealth of technical terms and a wide variety of other applications make the choice of a suitable program sometimes difficult. Through the inclusion of some basic and pertinent criteria (e.g., performance, ease of opening, standardization and portability), we present a descriptive comparison of 12 DBMSs currently available in the commercial and public domain. We have compared and tested three main architecture models which are currently available and assessed their potential applications for imaging purposes: relational, object-oriented, and hybrid. The findings of our study demonstrated that the Illustra software was the best suited for a neuroimaging environment because of its intrinsic ability to handle complex and large objects, such as 3D volumes or geometric structures.

Anatomical congruence of metabolic and electromagnetic activation signals during a self-paced motor task: a combined PET-MEG study.

We have investigated the degree of spatial correlation between the cerebral blood flow variations measured by positron emission tomography (PET) and the electromagnetic sources as measured by magnetoencephalography (MEG) in five subjects while performing a self-paced right index finger tapping task. Data were processed independently for each technique using both single-case and intersubject analysis. PET and MEG were coregistered with anatomical magnetic resonance images for each subject. Both extension and flexion motor-related fields were extracted from the MEG signal. Using the single dipole model we identified the motor evoked field 1 (MEF1) in all subjects and the motor field (MF) in three subjects. Individual and intersubject averaged PET data showed consistent contralateral primary sensorimotor (PSM) hand area and bilateral supplementary motor area activation. MEG individual and intersubject averaged results demonstrated that both MEF1 and MF dipoles were localized within the PSM PET activated area. Individual PSM mass center to dipole distance was 12 and 15.3 mm on average for the MEF1 and the MF component, respectively. For the same components, the intersubject averaged analysis shows distances between the PET Z-score maximum and the dipole locations of 6.3 and 15.0 mm, respectively. These results show that PET and MEG MEF1 activation signals spatially coincide within instrumental, registration, and modeling errors.

The venom of Conus pennaceus inhibits the binding of [3H]neuropeptide Y by direct interaction with the radioligand.

The venom from the marine snail Conus pennaceus inhibits the binding of [3H]neuropeptide Y to calf brain membranes (Czerwiec et al., 1996a) and, in the present study, also to rat forebrain membranes. These membranes contain about 80% Y1- and 20% Y2-receptors. The inhibition by the venom was concentration-dependent with an IC50 value of 3.4 micrograms ml-1. However, the venom also inhibited the binding of [3H]neuropeptide Y to the glass fibre filters and to the previously discovered ANPY toxin from the venom of Conus anemone (Czerwiec et al., 1996b). This inhibition was related to the ability of one or more of the venom components to bind directly to the radioligand instead of the initially assumed interaction with the neuropeptide Y receptors present in membrane preparations. The complex with Conus pennaceus venom was not retained by the glass fibre filter during the separation of the bound from the unbound [3H]neuropeptide Y. Gel filtration chromatography and denaturing sodium dodecyl sulfate-polyacrylamide gel electrophoresis revealed that the active [3H]neuropeptide Y-binding component is likely a approximately 30 kDa polypeptide. Binding of [3H]neuropeptide Y to the venom component(s) was not displaced by 20 microM of the (1-24) N-terminal and the (25-36) C-terminal neuropeptide Y fragments. It is therefore likely that the recognition of the venom component(s) requires both the C- and the N-terminal segments of the neuropeptide Y molecule.

[Malignant kidney tumors in adults in Senegal: diagnostic and therapeutic problems].

The aim of this study was to analyze problems posed by adult renal malignant tumors both in the diagnostical and therapeutical viewpoints. We have performed a retrospective study including 48 cases of confirmed renal cancer. Have been included adult patients who presented a cancer of the kidney operated or not. Studied parameters were age, sex, clinical signs, complementary explorations i.e. biology, renal ultrasonography, IVU, Computerized tomography, the operative indication and the prognosis. We have used Robson's staging to classify our patients. The average age was 51 years with extremes of 18 and 83 years. However 60% of them were less than 50 years. Adenocarcinoma was the main histological form encountered. It has been found in 93% of cases. The flank mass has been the mode of discovery in 70.8% of cases. The classic symptomatic triad associating hematuria, pain and the renal mass has been found in 20.8% of cases. Ultrasonography and Intravenous urography (IVU) have allowed to pose the diagnosis in 93.75% of cases. Patients were often referred very late with an average delay of consultation of 14 month. So, 25% of them presented with metastasis at the moment of the diagnosis. Radical nephrectomy was performed in 60% of cases. 40% of patients have not been operated due to the advanced evolution of the tumor. The global mortality to 1 year was estimated at 38%. We conclude that renal malignant tumors in the adulthood, in our practice, presents some particularities. Indeed, patients are referred late when clinical symptoms are sufficiently evocative to suspect the diagnosis. At these advanced stages, there is no curative therapeutic. So, a precocious diagnosis is necessary in order to propose to patients radical nephrectomy which remains the only efficient therapeutical procedure of the localized renal cancer in the adulthood.

[Maternal morbidity associated with delivery apropos of 124 cases].

In a prospective survey, the authors related 124 cases of maternal traumatic lesions during a child birth. They were apper lesions in 13% of uterine rupture, and lower lesions in 87% cervical vaginal and vulvoperineal ruptures). Adolescent primiparas were the most concerned with an incidence of 9.12% followed by the greater number of multiparas 8.24%. Age, parity, badly or unassisted deliveries were the main factors of risk. For the uterine rupture surgical intervention was preservative in 62% of cases. Suture has been essential in lower lesions. One maternal decease is registered, the foetal forecast is bad (14 foetal decease out of 16) in the uterine rupture. Morbidity was important: 64% in cicatricial perineum, 62% in cicatricial uterine, 37% in hysterectomy. In results, that all the cases of maternal traumatic lesions observed were avoidable. The authors recommended a better pregnancy and delivery care.

[Post-partum malignant amebic colitis. Apropos of a case].

A 26-year old female developed acute fulminant amoebic colitis during the post-partum period, successfully managed by subtotal colonic resection without anastomosis. Fulminant transmural amoebic colitis is a rare life-threatening complication of invasive bowel amebiasis. Pregnancy, delivery, diabetes mellitus and immunodeficiency are the main risk factors. At pathologic examination, bowel wall necrosis can be seen with amoebae present in the lumen of capillary vessels. The diagnosis of amoebic colonic perforation is difficult, especially in a non-endemic area. Conservative surgical management is required in non-perforated forms. If perforated, the bowel must be resected, limited to macroscopic lesions.

[Benign maxillary tumors. Epulis in Senegal].

The epulis is a hyperplasic tumour restricted to the gum. It is the most widely spread tumour among the benin gum tumours. It has various etiologies. The authors emphasize the epidemiological and clinical aspects of 96 cases. Four types of epulis are observed, the most common type being the acquired epulis. On an etiological level, the factor related to oral hygiene is the most criticized. Their frequency in the oral pathology should lead the dental surgeon to master his treatment.

Immunological detection and quantitation of 10-deacetylbaccatin III in Taxus sp. plant and tissue cultures.

A high-sensitive ELISA method was developed for the detection and semi-quantitative determination of 10-deacetylbaccatin III and its structurally related compounds in crude extract of Taxus sp. plants and tissue cultures. The antibodies were raised in rabbits using 7- or 10-succinyl-10-deacetylbaccatin III-BSA conjugate as immunogen. The working range of the assay was from 0.003 to 1.000 ng (0.09 to 31.33 nM) of 10-deacetylbaccatin III per assay. The cross-reacting material in crude plant extract was examined by chromatographic (silica gel CC, HPLC) and immunoassay methods. Study on the evaluation of cross-reacting material in crude Taxus plant extracts showed that at least 80% of the immunosignal correspond to 10-deacetylbaccatin III in the extract. The ELISA method was applied to investigate the 10-deacetylbaccatin III equivalent content in crude extracts of 19 plants species including Taxaceae, Taxodiaceae and Pinaceae species. The 10-deacetylbaccatin III-like structure was only detected in Taxus and Torreya sp. The results indicate that this immunoassay is a useful tool for the rapid screening of species, varieties or individual plants out of a wide population. The distribution of 10-deacetylbaccatin III equivalent content in 9-month old Taxus plantlets cultivated in vitro as well as in callus culture was investigated.

[Evaluation of a direct serologic card agglutination test for the diagnosis of camel trypanosomiasis caused by Trypanosoma evansi].

The results of a novel direct serological card agglutination test for the diagnosis of camel trypanosomosis due to Trypanosoma evansi (CATT/T. evansi) were compared with those obtained by direct detection of parasites in a study using 1,093 sera from camels raised in northern Mali. A good correlation was revealed between the percentage of positive results obtained by CATT and the presence of trypanosomes (89%), as well as a good coincidence between the percentage of positive results obtained by CATT and low haematocrit values (packed cell volume). CATT revealed a global serological prevalence of 30.6%, whereas trypanosomes were found in only 5.85% of the corresponding animals. CATT/T. evansi is a quick and easy-to-read test, which merits further evaluation in camel-rearing countries.

[Antibiotic sensitivity of germs isolated from cellulitis of dental origin in Senegal (results of a survey of 49 cases)].

Irregular results sometimes found during antibiotic treatment of perimaxillar cellulitis, conducted the authors to study bacteriology of these infections from dental origin. This study confirmed the aero and anerobic polymicrobism of these cellulitis. Antibiograms realised in 49 cases showed occurrence of streptococcal and anaerobic germs resistance to antibiotics usually prescribed. A nearly constant success of other antibiotics allowed the authors to define a first intention antibiotherapy for the treatment of these infections.
