Further studies on the hepatoprotective effects of Cochlospermum tinctorium rhizomes.

The hepatoprotective activity of the rhisome of Cochlospermum tinctorium was investigated using carbon tetrachloride toxicity on mouse and tert-butyl hydroperoxide in vitro induction of lipid peroxidation and hepatocyte lysis. Aqueous, hydro-ethanolic and ethanolic extracts showed significant dose-dependent hepatoprotective actions. The ethanolic extract showed a hepatoprotective activity at lower doses than silymarin. The ethanolic and hydro-ethanolic extracts exhibited remarkable effects against the induction of lipid peroxidation and hepatocyte lysis; the aqueous extract showed comparatively weaker effects. These differences were related to the chemical composition of the extracts. Among the identified constituents of the drug, phenolic and polyphenolic compounds (gallic and ellagic acids, ellagitannins, flavonoids), carotenoids, triterpenes could be related to the biological activity.

[Seroepidemiologic study of sexually transmitted diseases caused by Chlamydia trachomatis in Casablanca (Morocco)].

Screening 579 sera specimens obtained in Casablanca (Morocco) we have performed a retrospective seroepidemiological survey with regard to the prevalence of Chlamydia trachomatis involved in genital tract infections. The surveyed populations are divided into 3 groups: 177 patients affected with sexually transmitted diseases (STD), 319 reference patients, 83 maghrebian patients affected with STD but living in France. The study was performed by the indirect microimmunofluorescence technique. The prevalence of Chlamydia trachomatis ranging from 50% in the moroccan reference groupe to about 70% among patients with STD both in Morocco and Bordeaux indicates that Chlamydia trachomatis takes a significant part in STD observed in moroccan populations.

[Problems linked to trismus in a dental office].

The objective of this report was to know the most frequent etiology of trismus we meet in our dental office as well as the different treatment set up. The question is an observation of 58 cases of trismus that we received in two different dental clinics: Odontology and Stomatology Institute and Social Hygen Institute of Dakar. The results we got show that trismus is an odontologist's daily problèm. Although its treatment is relatively easy, it is good to remember that etiologies other than dental exist.

[Evaluation of the orofacial and dental status of a population of leprosy patients treated with multidrug therapy in Senegal].

The orofacial lesions of untreated leprosy have long been known. Our study concentrated on the bucco-dental and facial status of 97 leprosy patients given multidrug therapy. The number of decayed teeth and of teeth missing due to extraction proved high. Clinical manifestations of leprosy in the facial region are, however, rare and lesions of the oral mucosa, while more frequent, always exhibit scarring.

[Tumor-like focal nodular hyperplasia. Apropos of 3 cases observed at the Dakar Central Hospital from 1986 to 1989].

Authors report three observations of Focal Nodular Hyperplasia, Tumor-like on two: senegalese female patients, 16 years and 37 years old, and a young senegalese man, 24 Years old. The mean age is about 25.5 years and oestro progestative intake has bean noted once. Diagnosis of Focal Nodular Hyperplasia was difficult and has been done twice after surgical intervention for chronic cholecystitis and blood peritonitis. The only complication reported, has been a rupture of nodular, causing the death of a patient. This benign tumor that never degenerates, does not need any particular treatment unless a stopping of oral contraception.

[Surgical dentistry and geriatrics in Senegal].

INTRODUCTION: Geriatric medicine is developing more and more as a specialism. Odontology is not behindhand. That is why, even in an African setting, when life expectancy is still not as high as in the West, we are finding ourselves treating older and older patients. MATERIALS AND METHODS: During the course of the academic year 1989-1990, we treated 40 patients of both sexes, of different ethnic and social backgrounds aged from 55-89 years. Besides age, the patients had all undergone endo-buccal surgery, with the exception of parodontology surgery. According to sex: The forty patients comprised 27 men and 13 women. These figures are interesting and we will return to them during the discussion. According to age: The ages of our patients ranged from 55-89 years, with an average of 72. The distribution of age brackets shows a strong percentage (52.5%) of patients between 55 and 64 years of age. According to oral stage: In general, our patients neglect oral hygiene: 67.5% presented at least tartaric gingivitis. This negligence was evident in more men than women. According to general state: More than half the sample had at least one general pathological disorder. They ranged from cardio-vascular ailments, to differing digestive disorders, to neurological and endocrinal pathologics. METHODOLOGY: The data was collected using the following 3 methods which slowly built up: questionnaire clinical examination the operation RESULTS: The dividing-up of the different operations: we realised a total of 71 operations, this being an average 1.77 per patient. Distribution of pathological ailments according to type and nature: About 55% of our patients had a general pathology. Cardio-vascular ailments took first place with a frequency of 52.16%, followed by digestive problems with 26.08% neurological syndromes with 13.04% and ailments with endocrinal origins with a rate of 8.69%. Incidents during operation: incidents linked to the anaesthetic, incidents linked to the operation itself, post-operative incidents, healing. DISCUSSION: Age: The most representative age-band was between 55 and 64 years (with a six ration of 1:5). Multiple extractions: Apart from a defective oral state, these extractions are always motivated by the need for prosthesis. All the patients were missing at least one tooth, confirming the thesis of Dubois, according to which "the repercussion of oral ageing appears in all its simplicity in either partial or total toothlessness". CONCLUSION: Elderly Senegalese patients present serious oral problems since extraction comprises the main part of surgical odontological operations. A better way of dealing with these subjects, weakened by old age and general pathology, is desirable.

[Coronary endoprosthesis. Short- and mid-term results].

Mechanical stenting of the coronary wall by an endoprosthesis is a method of treating and preventing the 2 major complications of balloon angioplasty: acute occlusion by dissection and secondary restenosis. Forty-four self expanding stents were implanted in 42 patients presenting with post-angioplasty restenosis (35 cases), stenosis of an aortocoronary bypass graft (4 cases) or symptomatic post-angioplasty coronary dissection (3 cases). The primary results featured a high early occlusion rate of the endoprosthesis (28.5%, 12 cases). Better patient selection to include patients with stable angina and coronary arteries with a diameter greater than 3 millimeters, and the administration of an association of platelet antiaggregant therapy with heparin and oral anticoagulants has led to an improvement in these results. The early occlusion rate in the first 23 patients implanted without these selection criteria was 43% (10 cases) whereas it was only 10.5% (2 cases) in the last 19 selected patients who were prescribed effective medical therapy. The medium term results of the 27 patients implanted without complications at 6 months showed restenosis in 5 cases and reocclusion (asymptomatic) in 1 case (22.2% at control angiography at the 6th month). Restenosis was only observed in cases in which the endoprosthesis did not completely cover the coronary lesion (3 out of 3 cases) or when the endoprosthesis was implanted in a saphenous vein graft (2 out of 4 cases) and the only case of reocclusion occurred on a stent implanted in a coronary artery responsible for previous myocardial infarction. In the 21 patients in whom the stenosis had been totally covered by the stent there were no cases of restenosis.(ABSTRACT TRUNCATED AT 250 WORDS)

[Asepsis and vigorous antisepsis against infectious disease transmission in the dental office].

Therapeutic acts often surgically practised in the dental office can lead to the transmission of infectious diseases such as A.I.D.S. To prevent this, the authors propose guideline base acts for antisepsis and asepsis. In conclusion, the authors exhort for an integration of these antiseptic acts in the efficient working of the dental office for an effective prevention of these infectious diseases in the area.

Antihepatotoxic actions of Cochlospermum tinctorium rhizomes.

The antihepatotoxic activity of the rhizomes of Cochlospermum tinctorium was investigated using carbon tetrachloride- and galactosamine-induced cytotoxicity in primary cultured rat hepatocytes. Because the methanol and ethanol extracts of C. tinctorium rhizomes exhibited antihepatotoxic effects, the former was fractionated in order to elucidate the active constituents. Polyphenol compounds (gallic and ellagic acids) were detected in the active fractions and may account for much of the antihepatotoxic activity. Carotenoids could also be implicated in the activity of the total extracts.

[The question of equal potency of etidocaine and bupivacaine in peridural anesthesia].

A double blind clinical trial was carried out on randomised groups of 20 patients each undergoing surgery for varicose veins to compare the actions of etidocaine 1% and bupivacaine 0.5% and 0.75% with adrenaline 1:200.000. Bupivacaine 0.75% and etidocaine 1% were found to be equipotent, with a rapid onset and long duration of anaesthesia, and a comparable degree of profound motor block. The latency period of Etidocaine was markedly increased in the L V and S I segments, probably due to its high fat solubility. 0.5% bupivacaine, compared to the other preparations, showed significantly less motor block and duration of analgesia. Unlike other investigators, "spotty-analgesia" was not found in this series.

[Comparison of halothane and enflurane on the pulmonary compliance (author's transl)].

The peroperative pulmonary compliance in spontaneous ventilation is studied in two series of ten anesthetized patients with halothane and enflurane, in closed system. The tidal volumes (inspiratory and expiratory at noflow point) are recorded by pneumotachograph (Godard 17212). The intrathoracic to mouth differential pressure is recorded by pressograph (Godard 17300). The measurements are simultaneously realized by linear recorder (Brush gould 2500) or by XY recorder (Bryans 26000 A4). At the second hour of anaesthesia with enflurane pulmonary compliance is not modified (non significant increase to 11,5 p. cent: 0,3 less than P less than 0,2) while with halothane it shows a very significant 25 p. cent decrease (0,001 less than P less than 0,01). The authors relate this difference to the specific feature of breathing in the course of such an anaesthesia with enflurane.

[Use of sodium nitroprusside in otological surgery (author's transl)].

The authors report on the experiment carried out on the use of sodium nitroprusside (NPS) in otological surgery. The acknowledged advantages of NPS over hypotensors and more particularly its spécifically vascular, périphéral, swift action, rapidly reversible, are worth mentioning. Control over the achieved hypotension however, requires careful attention to avoid overdosage which could result in severe hypotension and "cyanhydric" intoxication. The recording of capillary pulse constitutes an easy and reliable solution to the difficulties arising from permanent control of blood-pressure and of peripheral arterial perfusion. The method both produces excellent results permitting bloodless surgery, and allows a reduction of NPS doses giver, without resorting to a invasive for measuring blood pressure. The method used in surgical fields other than otology has already proved very satisfactory.
