Vincristine (NSC-67574) in the treatment of small-cell anaplastic carcinoma of the lung.

In a phase II trial the effect of vincristine as a single agent was evaluated in patients with small-cell anaplastic carcinoma of the lung. The dosage of vincristine was 1.5 mg/m2/week given iv for 4 weeks, followed by 1.5 mg/m2 given every other week with dose modifications according to neurologic tolerance. Nineteen of 27 patients included in the study were evaluable. Objective response was observed in eight patients (42%) including three who were not previously treated and five who were previously treated. The median duration of response was 60 days (range, 21-182 days) with the response always occurring within 4 weeks. This study demonstrates that vincristine is an active agent for small-cell anaplastic carcinoma without cross resistance to CCNU, cyclophosphamide, or methotrexate.

Intracardiac rhabdomyosarcoma: transesophageal echocardiographic findings and diagnosis.

Transesophageal echocardiography (TEE) was performed on a 41-year-old woman who presented with a cerebrovascular accident. TEE confirmed the presence of a morphologically bizarre biatrial tumor with precarious, vigorous motion throughout the cardiac cycle. Surgical intervention was decided on, and the patient underwent cardiac surgery for tumor excision 16 hours after TEE. Intraoperative frozen section diagnosis was spindle cell sarcoma, and subsequent immunohistochemical analysis showed the tumor to be a rhabdomyosarcoma. The data are presented here, and the role of TEE to establish a preoperative diagnosis of intracardiac tumor is discussed.

Acetabular changes in an experimental model of developmental dysplasia of the hip (DDH).

We wished to correlate the morphological acetabular changes in developmental dysplasia of the hip (DDH) with changes at the tissue level. A secondary aim was to develop a way of measuring the dysplastic acetabulum. We studied the changes in rabbit acetabula after maintaining knee extension, measuring the major diameter from the acetabular notch to the farthest opposite point on the acetabular margin. The minor diameter was at right angles to this. The dysplastic acetabula showed elongation along the major diameter compared to control hips. Microscopic sections were made along major and minor diameters. The posterosuperior lip of the dysplastic acetabulum showed an early eversion of the acetabular cartilage. Growth of the articular cartilage in this new direction accounted for the acetabular elongation. As well as providing insight into the manner of acetabular shape change, the findings suggest the nature of remodeling required before normal acetabular growth can resume during treatment.

Partial albinism, immunodeficiency, and progressive white matter disease: a new primary immunodeficiency.

We studied 12 children who presented with a recently recognized syndrome. The salient features of this new syndrome were recurrent fever; hepatosplenomegaly; pancytopenia; blond, golden to silvery gray hair; hypopigmented skin, progressive white matter demyelination; and early death. Seven patients died, four with severe central nervous system (CNS) involvement, and three with bone marrow failure and sepsis. Cutaneous anergy to recall antigens was present in all patients. Other immunological abnormalities were poor antibody responses; deficient T-cell responses to phytohemagglutinin (PHA), concanavalin A (Con-A), and allogeneic lymphocytes; hyperresponsiveness of B lymphocytes to pokeweed mitogen; and variable phagocytic defects. Histopathologic examination of the hair and skin biopsies showed characteristic distribution of melanin with melanocytes present in normal numbers but with fewer short dendritic processes. Langerhans' cells were present in normal numbers in some patients and sparse in others. This syndrome seems to cluster into two tribes from two different geographical areas in the Arabian Peninsula. In the eight families studied, 12 other siblings and close relatives were found to be affected. The mode of inheritance in this syndrome is that of an autosomal recessive pattern. We propose the term "PAID syndrome" to identify patients with the above features.

Spindle cell carcinoma of the larynx.

Histology, histogenesis, biology and treatment of the rare spindle cell carcinoma of the larynx are presented. The histogenesis of spindle cell tumours has been much debated, but electron microscopic investigations have shown that most of the laryngeal lesions are squamous epithelial carcinomas that have undergone transformation to a spindle cell pattern. As confusion with supporting tissue neoplasms may occur in the light microscope, the use of the electron microscope in differential diagnostics is recommended.

Collagen fibres in otosclerosis and in osteogenesis imperfecta tarda. A light and electron microscopic study.

Skin biopsies taken from patients with osteogenesis imperfecta tarda and otosclerosis were compared blind by light- and electron microscopy with similar biopsies taken from normal volunteers in an effort to find common pathological features in the collagen fibres from the two patient groups, thus corroborating the theory that osteogenesis imperfecta and otosclerosis might be due to a common genetic anomaly in the form of a mesenchymal hypoplasia. We were not able to find differences in structure or quantity of the collagen-, elastic- or reticular fibres between normals and the two patient groups. We cannot, on the basis of our previous work and on the present morphological investigation, give support to the above-mentioned theory.
