[Abortion following recent myocardial infarct].

There is reported about a 42 years old woman suffering from a myocardial infarction in the 9th weeks of pregnancy. According to her wish the pregnancy was interrupted in the 22nd gestational week by hysterotomy and sterilization of tube. Probable causes, frequency and prognosis are discussed. References are made to the therapeutical action.

The effect of caries-preventive measures in mothers on dental caries and the oral presence of the bacteria Streptococcus mutans and lactobacilli in their children.

Seventy-seven first-time mothers were selected on the basis of high salivary counts of Strep. mutans [greater than 10(6) c.f.u. (colony forming units) per ml saliva]; 40 mothers were in the control group and 37 in the test group. Their infants were 3-8 months of age at the start of the study. A prophylactic programme for the test mothers, aiming at a reduction of Strep. mutans, was repeated at intervals of 2-4 months as and when necessary until their children were 3 years old. The test mothers as a group showed approx. 10-fold fewer Strep. mutans during the test period. At the age of 3 years, 70 per cent of the children in the control group carried Strep. mutans, compared with 41 per cent in the test group (p less than 0.01). Fifty-two per cent of the children who carried Strep. mutans had caries at this age, compared to 3 per cent of the children without this organism. The time when Strep. mutans was first detected in the children seemed to influence subsequent development of caries because 77 per cent of the children who carried Strep. mutans at the age of 15 months had caries at the age of 3 years. Approximately 40 per cent of the children in both the control and the test group had detectable lactobacilli in their saliva at 3 years. [Ambulatory cryosurgical restoration of benign cervical lesions].

The area of ectopic epithelium and transformations zone of the uterine cervix was treated cryosurgically by liquid nitrogen in 252 patients. The influence of deep freeze lasted four minutes. Neither analgesia nor anaesthesia was necessary. After a single cryosurgery cure rate was 92 per cent, after a second application 98 per cent. Cryotherapy can be performed ambulatorily and is nowadays to be regarded as method of choice to restore benign cervical lesions.

Preventive measures in mothers influence the establishment of the bacterium Streptococcus mutans in their infants.

First-time mothers who had a high salivary number of Strep. mutans [greater than or equal to 10(6) colony-forming-units (c.f.u.) per ml] were selected. Every second mother was given a special preventive programme to reduce her salivary level below 3 x 10(5) c.f.u. per ml. Where a reduction of Strep. mutans was achieved in the mother, the establishment of Strep. mutans in her infant was prevented or delayed. Thus, 28 mothers were successfully treated until their infants were 23 months old and only 3 of their infants (11 per cent) were infected with Strep. mutans, compared with 17 out of 38 infants in the control group (45 per cent). In both groups, the percentage of infected infants increased with increasing age, although at all ages fewer infants were infected with Strep. mutans in the test group than in the control group. Sixteen infants of successfully treated mothers had reached the age of 36 months. Three were infected (19 per cent) compared with 17 out of 27 in the control group (63 per cent). These findings show that the spread of Strep. mutans can be delayed or prevented by measures directed against the main source of infection, an approach which is successful in the prevention of other infectious diseases.

[Results of cryosurgical treatment of conization wounds in comparison with electrocoagulation and Sturmdorff suture].

There is reported about results of curing and complications of cryosurgery on conisation-wounds in 102 patients using spray-freezing as well as contact method.--The results were compared with those of wound-care by electrocoagulation and Sturmdorff-suture technics. The evaluation was performed according to absolutely clinical criterions. Physical parameters like freezing depth and diffusion of temperature have not been measured.--Cryotherapy showed less complication and produces good cosmetic results. Cryotherapy with a sure aim means an enrichment of our therapeutic possibilities in treating conisation-wounds.

Effect of caries preventive measures on Streptococcus mutans and lactobacilli in selected mothers.

Mothers of first-born babies were selected on the basis of high salivary counts of Streptococcus mutans. All experimental subjects were given a basic caries preventive program in order to decrease the level of S. mutans in saliva. The program included dietary counseling, which stressed the role of sucrose in the accumulation of S. mutans. The treatment was carried out in a public dental office. The effect of the treatment was assessed by microbiological examination of salivary levels of S. mutans and lactobacilli. A statistically significant reduction in both S. mutans and lactobacilli was found. The basic preventive program was effective in reducing the number of S. mutans below a selected threshold value of 300 000 CFU per ml saliva in 60% of the mothers. In the remaining subjects chlorhexidine treatment was required to suppress the salivary levels of S. mutans below this value.

Streptococcus mutans in plaque and saliva and the development of caries.

Plaque samples from 10 different tooth surfaces of 10 schoolchildren with varied caries experience (DFS 10-33) were collected five times during 2.5 years. The samples were examined with an immunofluorescent technique for identification and enumeration of Streptococcus mutans serotypes c/e/f and d/g. At each sampling occasion the children were scored for caries. A stimulated saliva sample was also collected and the number of S. mutans per ml saliva was determined. The saliva level of S. mutans was shown to reflect the prevalence and proportion of this microorganism on the selected surfaces. Five surfaces carried S. mutans at each sampling. Four of these surfaces showed progressive cries S. mutans infection was also found to precede the development of incipient caries on four surfaces. Eighty percent of the surfaces that stayed sound were only transiently carriers of S. mutans in mainly very low numbers. Serotype c/e/f dominated in prevalence and proportion on the surfaces with a history of caries during the study.

Adherence and Streptococcus mutans infections: in vitro study with saliva from noninfected and infected preschool children.

An in vitro adherence experiment was designed to mimic the transmission of Streptococcus mutans from mother to child to test the hypothesis that differences in initial adherence reflect differences in susceptibility to infection. The data show that the pretreatment of S. mutans cells with the saliva of the mother in a mother-child pair and the pretreatment of spheroidal hydroxyapatite with that of the child may result in combinations which counteract or foster the initial adherence to a varying extent. The findings indicate that such combinations may determine the risk of S. mutants infection.

[Comparison of the efficiency of the Soviet culture medium no. 3 and the media used for cultivation of mycobacteria in routine in the GDR (author's transl)].

In cooperation of ten laboratories the Soviet culture medium Mo. 3 (FINN) was compared with the media used in the GDR for primary cultivation of mycobacteria in routine (German Pharmacopoeia (diagnostic laboratory methods) GDR). The efficiency of the media tested in the study was nearly the same, not only concerning the number of positive cultures but also in time of growth and the number of colonies.

[Anaesthetic block close to spinal cord of gynaecological high-risk patients (author's transl)].

Conduction anaesthesia was applied close to the spinal cord of 91 gynaecological patients whose risk in the context of surgery had been found to be increased on account of advanced age or for severe secondary internal diseases. The rate of complications of these patients with regional anaesthesia was compared to that of another group of 91 patients on whom operations were performed under general anaesthesia. The group with anaesthetic block close to the spinal cord exhibited less intra-operative variation in arterial pressure and less postoperative disorders of the respiratory organs. The metabolism of diabetic patients was more easily controlled and managed under regional anaesthesia. Conduction anaesthesia close to the spinal cord is recommended for patients in geriatric condition or with concomitant internal disorders.
