Familial Wolff-Parkinson-White syndrome.

Familial occurrence of Wolff-Parkinson-White (WPW) syndrome is uncommon and to date only seven instances have been documented in the literature. We describe another unusual family in which the proband, his father and two of his five brothers (including one who died suddenly) showed WPW conduction. A younger sister showed evidence of Lown-Ganong-Levin (LGL) pre-excitation. This pedigree is consistent with autosomal dominant inheritance and supports the importance of testing asymptomatic family members of patients with WPW because it is a potentially life-threatening conduction disorder.

Anomalous chordae tendinae. A source of echocardiographic confusion.

A unique instance of an anomalous chorda tendinae visualized in the left ventricular outflow tract during M-Mode and two-dimensional echocardiography, and subsequently confirmed during surgery for concomitant severe rheumatic valvular disease, is described. The M-Mode appearance can be confused with more serious forms of heart disease manifesting extraneous echoes in the outflow tract, and an approach to these echoes is described, based on a review of the literature.

The immediate hypotensive effect of oral nifedipine.

The immediate hypotensive effect of oral nifedipine was assessed in 15 patients with moderate to severe hypertension. Supine and erect blood pressure and heart rates were measured before and 30 minutes after 10 mg nifedipine. The results showed that there was a significant (p less than 0.001) reduction in both systolic and diastolic pressures. There was a slight increase in heart rate and mild postural hypotension but neither appeared to be significant. It is concluded that nifedipine given by the oral route is an effective agent for the rapid lowering of elevated blood pressure and deserves further evaluation as an agent for the management of hypertensive emergencies.

Cardiac amyloidosis--diagnosis by 2-dimensional echocardiography.

In the past, the precise diagnosis of amyloid cardiomyopathy without resort to invasive techniques has been difficult. Recent experience with 2-dimensional echocardiography in cardiac amyloidosis has suggested that there are characteristic, and perhaps unique, features that are regularly imaged. We describe a case where usage of this non-invasive technique allowed accurate definition of cardiac involvement by amyloid.

The current status of coronary care units in the treatment of acute myocardial infarction.

Intensive monitoring for patients suffering from acute myocardial infarction first began in USA in 1962. In the past 2 decades, major progress has taken place in the development of coronary care units. The first coronary care unit in Singapore was introduced in 1967. Review of the results obtained from this unit shows that the hospital mortality from acute myocardial infarction can be lowered by intensive cardiac monitoring because of better treatment of cardiac arrhythmias and cardiac failure. However, the outcome of patients with cardiogenic shock is still dismally poor and the mortality in this group of patients is about 85 to 100%. Although the hospital mortality of acute myocardial infarction has been decreased with the introduction of coronary care units, the mortality of this condition in the community is still distressingly high. This is so because the majority of deaths in acute myocardial infarction occur early and outside the hospital environment. Currently the salvage of ischaemic myocardium and the continuing search for an effective treatment for severe pump failure remains the greatest challenge in the present and future management of acute myocardial infarction.

Diffuse conduction abnormalities in an adolescent with familial sinus node disease.

We report a case of familial sinus node disease with associated conduction abnormalities in the atrioventricular node and distal conduction system Spontaneous atrial pacemaker activity was absent though the atrium could be depolarized. The pacemaker activity of the heart resided in the atrioventricular junction. The AV node showed impaired automaticity and abnormal conduction properties which partially improved after vagal blockage.

Coronary artery-left ventricular fistula.

Coronary artery-left ventricular fistulae are exceedingly rare and to date only 19 cases have been reported in the literature. We describe another four cases which we encountered recently. The first patient was a 10-year-old asymptomatic Indian male who was referred because of a to-and-fro murmur. Haemodynamic and angiographic studies revealed an aneurysmally enlarged right coronary draining into the left ventricle. The fistula was ligated and the patient has remained well over a 21/2-year follow-up period. The second patient was a 13-year-old asymptomatic Chinese female referred also because of a to-and-fro murmur. Haemodynamic and angiographic studies showed a grossly enlarged right coronary artery draining into the left ventricle. This patient refused operation and has remained well over a 2-year follow-up period. The third patient was a 43-year-old Chinese male who presented with a 3-year history of angina pectoris and was admitted to hospital because of crescendo angina. The resting electrocardiogram showed marked ischaemic changes and the treadmill exercise stress test was also positive. Selective coronary angiography showed markedly dilated right and left coronary arteries. Multiple fistulae from the three major coronary arteries were seen to communicate with the left ventricle. It was elected to treat this patient medically with propranolol. He has remained well over a 6-month follow-up period. The fourth patient was a 5-year-old asymptomatic Chinese girl. Cardiac catheterization and selective coronary angiography revealed marked dilatation of the branches of the left coronary artery which communicated via a maize of fine vessels into the left ventricle. This patient has remained well over a 5-year follow-up period. The clinical presentation, investigations and treatment of coronary artery-left ventricular fistula are discussed and the literature regarding this subject is reviewed.

Digitalis-induced cardiac arrhythmias.

Six cases of digitalis intoxication presenting with cardiac arrhythmias are described. Multiple cardiac arrhythmias consisting of multifocal, or unifocal, multiform ventricular ectopic beats, bidirectional tachycardia, complete heart block, accelerated junctional rhythm with exit block, nonparoxysmal junctional tachycardia, atrial fibrillation, atrial tachycardia with block, and multifocal tachycardia were observed. The presence of such multiple arrhythmias occurring simultaneously should always suggest the high possibility of digitalis intoxication.
