The European Community of twelve and the drug demand. Excerpt of a comparative study of legislations and judicial practice.

The imminent birth of a United Europe has called forth a survey of national 'drug' legislations in the twelve member countries of the European Community. Chapters of possible interest and concern to the professions addressed by Drug and Alcohol Dependence are reproduced, the permission for which is gratefully acknowledged. The divergence of concepts and measures embodied in national legislations is staggering--not the least in the light of results of scientific research. Effects of an early weaning on phosphate transport maturation in the rat kidney: influence of the phosphate content of the diet.

The renal phosphate (Pi) transport system matures during the 3rd postnatal wk in the rat by an increase in the carrier affinity for sodium-cotransported phosphate. This study examines the ability of pups to adapt their renal Pi transport to the dietary phosphorus content during this period of carrier affinity maturation, corresponding to the weaning period in the rat. Clearance experiments and brush border membrane studies were performed on 21-d-old rats weaned early on d 16 onto a low phosphate diet (LPD, 0.19 g/100 g), a normal phosphate diet (control, 0.78 g/100 g), or a high phosphate diet (HPD, 1.5 g/100 g). In LPD rats, the Pi fractional excretion (0.3 +/- 0.1%) was lower than in controls (18 +/- 3%, p < 0.001). It remained very low (0.21 +/- 0.05% in LPD rats versus 40.5 +/- 6.3% in controls, p < 0.001) after Pi perfusion (1.5 mumol.min-1.100 g-1) and the reabsorbed Pi per min, corrected for the glomerular filtration rate, was higher than in the two other groups. The calcium fractional excretion (12.6 +/- 1.02%) in the LPD rats was much higher than in the controls (0.42 +/- 0.2%, p < 0.001). In contrast, HPD rats had an elevated Pi fractional excretion (41 +/- 4%, p < 0.001), whereas reabsorbed phosphate per min corrected for the glomerular filtration rate was not increased by a Pi load.(ABSTRACT TRUNCATED AT 250 WORDS)

[Favorable outcome of neoplastic disseminated intravascular coagulation treated with ketoconazole and estrogen derivative].

A prostate biopsy was carried out in a 53-year-old male outpatient with disseminated prostatic carcinoma. Two days later, he was admitted with severe acute anaemia (haemoglobin: 48 g.l-1) and macroscopic haematuria. Biological investigations revealed a disseminated intravascular coagulation (DIC). Symptomatic treatment was undertaken (transfusion of packed red blood cells, platelets, fresh frozen plasma and fibrinogen). However, the patient's condition worsened, and he was admitted to the intensive care unit 48 h later. Despite appropriate symptomatic treatment, the patient's condition continued to worsen. The prostatic origin of this condition was therefore suspected, and anti-androgenic treatment was started on day 9 (1,200 mg.day-1 ketoconazole and 2,000 mg.day-1 sodium fosfestrol). Within 48 h, the patient had began to recover in quite a spectacular manner. Ketoconazole starts blocking steroid synthesis within 4 h of giving it. This treatment can be used until oestrogen therapy starts having an effect (about one week). The low levels of testosterone in this case, before starting treatment, suggest that ketoconazole acted on the DIC by a possible cytotoxic effect on the carcinomatous cells.

[Hepatic veno-occlusive disease caused by Deticene: a cause of acute hypovolemic shock].

A case is reported of severe hypovolaemic shock occurring in a 53-year-old female patient undergoing a second course of chemotherapy with intravenous DTIC for a malignant melanoma. A few hours after the injection of DTIC, she became shocked, with loss of peripheral pulses, polypnoea and cutaneous vasoconstriction. She also had pain and guarding of the right hypocondrium. She was given 4 litres of colloids within 2 hours, together with 10 micrograms.kg-1.min-1 dopamine. Abdominal echography showed hepatomegaly, with a permeable portal vein. However biological investigations revealed lactic acidosis with hepatic cytolysis and hepatic failure. Nuclear magnetic resonance imaging displayed a reduced portal venous flow, with abnormally small hepatic veins. Fluid replacement was continued, together with administration of small doses of heparin (1 mg.kg-1.day-1) and hydrocortisone hemisuccinate 5 mg.kg-1.day-1. The status of patient worsened over the next few hours, because of the development of a very large volume of exudative ascites and bilateral pleural effusions. Despite continuing fluid replacement (91), she became anuric at the 24th hour, requiring haemodialysis. However, her condition became stable, and then slowly improved. Fluid replacement was stopped after 72 h, steroids after a fortnight. Liver function tests returned to normal after the third week, together with diuresis. The patient was able to leave the ICU after 24 days. Physicians should be aware of this rare, often fatal side-effect, probably of immuno-allergic origin.

Allergic contact dermatitis to colophony included in the formulation of flexible collodion BP, the vehicle of a salicylic and lactic acid wart paint.

The authors describe two cases of allergic contact dermatitis to colophony included in the formulation of flexible collodion BP, the vehicle of a wart paint. Patch and repeated open application tests confirmed absence of contact allergy to other constituents. The patients were known to be allergic to various adhesive plasters. The use of flexible collodion USP, which does not contain colophony, is highly advisable whenever possible.

[Access to case records in department for child and adolescent psychiatry].

The object of this investigation was registration of all requests for access to case records in 1987 in all departments for child and adolescent psychiatry in Denmark. A total of 34 requests were made concerning child psychiatric departments and 55 concerning departments for adolescent psychiatry. This corresponds to requests for access to case records in 0.8% of all patients admitted to departments of child psychiatry, and also 0.8% of the outpatients. In the departments for adolescent psychiatry, access to the case records was desired concerning 16.5% of the hospitalized adolescents. No complaints have been made concerning access to the case records and it is concluded that the new Danish law functions satisfactorily also where parents were concerned, where the possibility of providing limited insight for adolescents or parents may be of value. The significance of introduction of a period of limitation for access to case records in departments for child and adolescent psychiatry is emphasized.

Psychiatric care of refugees exposed to organized violence. A comparative study of refugees and immigrants in Frederiksborg County, Denmark.

Psychiatry in Scandinavia has a tradition for research and clinical work with people from other countries, especially related to refugees and survivors after the Second World War. In recent years an increasing number of refugees have arrived, especially from the countries of the Middle East. The aim of this study was to evaluate the refugee situation in a Danish county, on the basis of data on 49 refugees treated in the county's psychiatric departments from January 1, 1986 to December 31, 1987 and 44 immigrants seen from January 1, 1983 to December 31, 1987. The clinical data illustrated marked differences between immigrants and refugees with respect to social situation and background, symptom pattern and exposure to organized violence. Both groups terminated treatment after very few contacts, and the psychiatric staff seems to need better training. The general need for psychiatric and psychological treatment of immigrants and refugees is still uncertain and further research is required.

Estrogen-like effect of 5-androstene-3 beta,17 beta-diol on the induction of fetal thymidine kinase in the rat uterus.

The action of 5-androstene-3 beta,17 beta-diol (Adiol) on Thymidine Kinase (TK) was studied in the rat uterus. It appeared that Adiol, like 17 beta-estradiol (E2), could induce the synthesis of the fetal isoenzyme of TK. Its effects compared with those of E2, were delayed by 6 h and higher doses were required to reach the same levels of induction. The action of Adiol on TK was exclusively observed in tissues where E2 receptors were present. Moreover, Adiol could stimulate the activity of uterine DNA-polymerase alpha. These observations are additional evidence of the estrogen-like behaviour of Adiol. They suggest that it could participate in proliferative processes of tissues which are dependent on estrogens.
