Effect of propranolol on post-traumatic hypophosphataemia and catecholamine secretion.

Trauma may induce disturbance in the homeostasis and metabolism of phosphate. Serum phosphate decreases rapidly after trauma, and this rapid fall is followed by more slowly progressing hypophosphataemia, reaching minimum values on post-trauma days 2-5. The effect of propranolol on post-traumatic hypophosphataemia was studied in an experimental model, using anaesthetized pigs. Propranolol prevented the initial rapid decrease in serum phosphate. Twelve hours after the trauma, however, both the propranolol-treated and the untreated pigs showed significant serum phosphate depletion, with progression during the 48-hour observation period. The urinary phosphate excretion did not differ between the two groups, and after 48 hours the cumulative loss was the same in both groups. The adrenergic response to trauma was significantly greater in the propranolol-treated than in the untreated group. The results suggested that the rapid serum phosphate fall immediately after trauma was caused by adrenaline release. The second, slower phase was not related to adrenaline or noradrenaline release, but showed correlation with the urinary loss of phosphate.

Specific plasma proteins as indices of inflammation during a modified fast in patients with rheumatoid arthritis.

Fourteen patients with classical rheumatoid arthritis and 7 healthy controls underwent a modified fast, containing diluted fruit and vegetable juices, for a 10-day period. To evaluate the effect of fasting on the inflammatory activity, plasma protein determinations were performed before and on the 10th day of fasting. Both patients and healthy controls developed reduced plasma levels of complement factor 3 (C3), orosomucoid and haptoglobin, although only the decrease in C3 and orosomucoid was greater in the patient group. An unchanged level in plasma of albumin, IgG, IgA, IgM and complement factor 4 was found. We conclude that determination of C3 and orosomucoid in plasma may be useful as indices of inflammation during fasting conditions.

Traumatically induced hypophosphataemia in anaesthetized pigs.

Clinical studies in severely injured patients have regularly shown hypophosphataemia probably directly related to the humoral response to trauma. In this study 72 anaesthetized pigs were exposed to a standardized missile trauma to find out whether the same type of change could be induced in an experimental animal as far as possible in the absence of other factors known to cause hypophosphataemia. The serum phosphate, calcium, and albumin concentrations and the fractional excretion of phosphate were recorded for 72 hours after inflicting the injury. The fall in serum phosphate after trauma follows a biphasic pattern, with a rapid drop immediately after the injury, a brief increase, and then a gradual slow decrease with a minimum at the end of the experimental period. This decrease was not caused by glucose infusion or phosphaturia due to haemodilution and increase in extracellular volume. The results support the hypothesis that hypophosphataemia in the posttraumatic period is caused by the humoral response to trauma. A new experimental model has been developed for the study of posttraumatic metabolic events.

Cardiac output distribution after left colon anastomosis in the rat.

Resection and anastomosis of the left colon result in an aysmmetric breakdown of preexisting collagen in the colonic wall during the first days of healing. Suture technique influences this reaction. To study the regional blood flow in the colon after resection and anastomosis a tracer microsphere technique was used for determination of the cardiac output distribution in four groups of rats: unoperated and sham-operated controls and rats with left colon anastomosis sutured either by continuous or interrupted sutures. There were no differences in the organ distribution of caridac output between unoperated and sham-operated controls. Left colon resection and anastomosis led to an increased regional blood flow in the colon, which was most marked proximal to the anastomosis and furthermore more pronounced in animals with anastomosis made of continuous suture than in those made of interrupted sutures. Further, resection and anastomosis resulted in an increased cardiac output distribution to the visceral organs. The visceral increase was most pronounced in animals with continuous suture. This increase was predominantly observed in the kidneys and in the colon. The latter is interpreted as being due to an increased metabolic demand induced by resection and anastomosis.

Hypophosphatemia in severe burns. A prospective study.

In 33 severely burned patients, who were followed for two weeks after injury, a transient hypophosphatemia was registered with the decrease in serum phosphate occurring on the second to tenth day after injury. When a previous retrospective study was combined with the present one, there were 50 patients in all. Seven patients died and, of these, five showed very low values of serum phosphate (S-P) at the time of their death. Simultaneous reduction of urinary phosphate excretion indicates that the depletion of phosphate is mainly prerenal. However, studies of fractional excretion of phosphate demonstrates that renal losses of phosphate might contribute to the hypophosphatemia. A remarkable finding was a general, marked rise in serum calcitonin (S-h-CT). The relationship between the rise in S-h-CT, the increased catecholamine secretion and the hypophosphatemia remains to be further clarified.

Studies in pharmacokinetics and tolerance of substances temporarily retained in the liver by microsphere embolization.

Earlier investigation has shown that a temporary arrest of arterial blood flow can be achieved by embolization with degradable microspheres. This study was made to investigate the change in pharmacokinetics, and drug tolerance which takes place when a substance is retained in the liver by a microsphere embolization. 14C-labelled inulin and 5-fluorouracil were studied. The administration of these substances with microspheres led to a delay in their systemic distribution. Furthermore there was an increased tolerance to 5-fluoro-uracil, probably due to a prolonged first pass effect when the substance was temporarily retained in the liver by a microsphere embolization.

Dose assessment analogies.

Assessments of radiation detriment are based on the absorbed dose in the organs and tissues of interest, assuming a linear, nonthreshold dose--response relationship for the induction of cancer or genetic effects at low doses. Similar assessments may be made for nonradioactive, mutagenic, or carcinogenic substances, in which case the time integral of the local tissue concentration of the substance may be the quantity which would correspond to the radiation dose. It can be shown that the assessment then involves calculations which are very similar to those made in the assessment of radiation detriment. It is suggested that an attempt should be made to make such assessments for nonradioactive pollutants, in order to provide a more appropriate basis for comparisons with radiation detriments. In the assessment of the detriment from inhaled substances, the overall collective intake commitment is simply the total amount of each substance ever to be inhaled, irrespective of when or where. This quantity might be assumed to be proportional to the total detriment in form of lung cancer.

Function of the parathyroid glands after total thyroidectomy.

Parathyroid function, defined as the ability to maintain normocalcemia without substitution, was evaluated in 36 patients following total thyroidectomy for carcinoma. In 16 patients, all visualized parathyroid tissue was sliced and autotransplanted into muscle pockets; one year after the operation, 11 patients had substitution for lack of parathyroid function. In 20 patients, the parathyroid glands were preserved in situ with intact vascular supply; one year postoperatively, three patients had evidence of persistent hypoparathyroidism. The difference in the parathyroid insufficiency rate is significant, p less than 0.005. We conclude that parathyroid autotransplantation has been a failure io preserve the parathyroids in situ whenever this is feasible, autotransplantation being reserved for parathyroids unfortunately removed or devascularized.

The circulation in liver tissue and experimental liver metastases before and after embolization of the liver artery.

Simultaneous measurement of cardiac output distribution with 86Rubidium and 57Cobalt-tagged microspheres in rats implanted with liver tumors by intraportal injection of sarcoma cells enables quantitation of arterial and portal tumor circulation. The portal circulation was found to be increased in small tumors as compared to the liver, but as the tumor grew there was a decrease in the portal tumor circulation. When the tumor growth became massive even the total liver circulation was reduced, as measured with 133Xenon wash-out. All the tumors had increased arterial circulation. This arterial hyperperfusion was changed into ischemia when the liver artery was occluded through embolization with degradable microspheres.
