The accuracy of measurements of femoral neck fractures. Conventional radiography versus roentgen stereophotogrammetric analysis.

Postoperative displacement of the femoral head center was measured in 60 intracapsular hip fractures at repeated conventional radiographic (CR) and roentgen stereophotogrammetric (RSA) examinations. The measurements of the conventional radiographs (405 pairs) were done on a digitizing table. Totally, 105 pairs of these radiographs (AP and lateral, CR) were measured by 2 observers. Manual measurements were also made on 176 pairs of the conventional radiographs. Conventional radiographs tended to overestimate or underestimate the average displacements up to about 2 mm. Two standard deviations of the differences (RSA-CR) varied from 5.8 to 9.6 mm depending on the direction of the movements. The accuracy of the measurements on conventional radiographs did not differ between the 2 examiners. The digitizing table was found to be more accurate than the manual measurements. The accuracy of conventional radiography can most likely be improved by using a strictly standardized examination technique.

Femoral Neck Fractures

Trichorhinophalangeal syndrome type I and systemic lupus erythematosus with complement C4A homozygous null alleles in the same family.

A three generation family from northern Sweden with both trichorhinophalangeal syndrome type I (TRP I) and systemic lupus erythematosus (SLE)-like syndrome with complement C4 homozygous null alleles is described. Five family members in three generations were affected by the TRP I syndrome, indicating autosomal dominant inheritance. Two members had clinical and laboratory signs of SLE and two other members SLE-like syndrome. All living family members in the first and second generation had homozygous C4A null alleles. In three of the adults the two syndromes occurred simultaneously, probably in this family by coincidence.

Abnormalities, Multiple

Reduction in mortality from breast cancer after mass screening with mammography. Randomised trial from the Breast Cancer Screening Working Group of the Swedish National Board of Health and Welfare.

A randomised controlled trial to investigate the efficacy of mass screening with single-view mammography in reducing mortality from breast cancer was started in Sweden in 1977. 162 981 women aged 40 years or more and living in the counties of Kopparberg and Ostergötland were enrolled in the study and divided at random into 2 groups. Each woman in the study group was offered screening every 2 or 3 years depending on age. Women in the control group were not offered screening. This report is confined to the 134 867 women aged 40-74 years at date of entry. The results to the end of 1984 show a 31% reduction in mortality from breast cancer and a 25% reduction in the rate of stage II or more advanced breast cancers in the group invited to screening. 7 years after the start of the study the excess of stage I cancers in the study group largely outweighs the deficit of advanced cancers.

Experimental and clinical evaluation of cholangiography with xeroradiography.

Conventional screen film radiography is compared with xeroradiography in an experimental model for cholangiography. The wider exposure latitude of the xerographic technique confers certain diagnostic advantages. However, the actual clinical application presents practical difficulties, which, combined with low sensitivity, are of such a nature that the method can hardly be considered suitable at the present time. If certain improvements can be made, xeroradiography could become a superior method for radiography of the bile ducts.

Animals

Bile duct diameter and diagnostic reliability at cholangiography.

Cases of exploratory choledochotomy with widened bile duct as the indication have been assessed with respect to missed bile duct concretion in a retrospective study covering a period of five years. During this period 2,000 cholecystectomies were performed. The 110 cases, in which (a) pre-exploratory cholangiography was completely normal apart from a diameter of more than 10 mm, (b) exploratory choledochotomy was performed, and (c) secondary cholangiography had been carried out, were studied in detail. No calculi were demonstrable either at exploratory surgery or secondary cholangiography. With a good radiographic technique it does not seem likely that bile duct concretions would be missed in widened ducts at otherwise normal operative cholangiography--at any rate not with bile duct diameters up to 20 mm.

Calcium infusion in suspected hyperparathyroidism: changes in parathyroid hormone concentration related to histopathological findings.

Changes in parathormone, total and free calcium concentrations in serum were studied in 27 patients with suspected hyperparathyroidism (25 with normocalcemia judged from CaT). Six patients were classified as normals. In 21 patients neck explorations was performed. Patients with definite pathology in the parathyroid glands had higher parathormone, total and free calcium levels than patients with borderline pathology or normal glands. The mean parathormone concentration fell significantly during calcium infusion in patients with histologically normal parathyroid glands. In patients with definitely pathological glands the parathormone concentration remained unchanged during the calcium infusion.

Adenoma

Peroperative estimation of the size of the thyroid remnant.

A new method for estimation of the size of the remaining tissue after thyroid resection is described. The method has been tested on cadavers and during lobectomy in a series of patients. It is shown that intraoperative quantification of the thyroid remnant can be performed mostly with +/- 0.5 g for an individual thyroid lobe with a coefficient of variation of +/- 7.6%. In patients with toxic nodular goiter and calcifications, however, the error might be larger due to the varying homogeneity of the thyroid tissue. In 73 patients with negative antibody titers operated on for hyperthyroidism it was shown that the size of the thyroid remnant was negatively correlated to the delta TSH, determined 6 weeks after the operation.

Serum activity of ornithine carbamoyl transferase in patients operated with jejunoileal bypass for extreme obesity after pretreatment with aminoacids.

Increased blood level of the liver specific enzyme ornithine carbamoyl transferase (OCT) is known to be a sensitive indicator of liver cell damage. The effect of preoperative aminoacid infusion to obese patients before jejunoileal bypass due to obesity was studied in 7 patients. They had a significantly lower rise on OCT than eight patients receiving only 5.5% glucose. The result seems to indicate that the liver in an obese patient undergoing abdominal surgery is protected against damage by an infusion of aminoacids.

Amino Acids

Clinical experience of parathyroid autotransplantation.

Parathyroid tissue was transplanted from the neck to the muscles of the non-dominant forearm in 18 patients. The difference in concentration of parathormone between the transplant arm and the contralateral arm was used as a test of parathyroid function. All patients showed a difference in concentration between the arms. Three patients needed calcium supplement for 2 months after surgery, and one patient with no parathyroid tissue left in situ also needed vitamin D for 8 months. The results indicate that parathyroid tissue transplanted to the arm continues to function, and is able to keep the patient normocalcaemic even when no parathyroid tissue is left in situ.

Adenoma

Angiography in giant-cell tumours of bone.

The angiographic appearance of 9 cases of genuine giant-cell tumour of bone, 6 cases of aneurysmal bone cyst and one case of non-ossifying fibroma is described. Differential diagnosis would appear to be possible to a certain extent. The non-ossifying fibroma was poorly vascularized. The genuine giant-cell tumours were hypervascularized as well as some of the aneurysmal bone cysts making a differential diagnosis in these cases impossible. On the other hand, a poorly vascularized tumour devoid of soft tissue component and arteriovenous fistulas is probably an aneurysmal bone cyst. A hypervascular soft tissue component is of great prognostic value in cases of geniune giant-cell tumour.

Angiography

Extrahepatic obstruction of the portal vein with bleeding from the gallbladder. Report of a case.

A case of extrahepatic obstruction of the common bile duct and the portal vein by a retroperitoneal neurolemmoma is described. In the initial stages portal hypertension with hepatofugal collaterals and bleeding from oesophageal varices existed. Then gradually, hepatopetal anastomoses developed, partly via veins in the wall of the gallbladder. Such venous collaterals tend to reduce the portal hypertension, but may cause bleeding from the gallbladder.

Adolescent

Function of the thyroid gland after subtotal resection for hyperthyroidism in relation to remnant size.

The size of the remnant gland after subtotal thyroidectomy varied between 5 and 12 g in 85 hyperthyroid patients. The patients were then examined 6 weeks and 6 months after surgery with estimation of TSH and TRH stimulation test as well as thyroxine and triiodothyronine. A significant correlation between TSH response to TRH and remnant weight was found ( r= 0.94, p less than 0.01). The frequency of raised S-TSH was higher in the group with small remnants compared with those with larger ones. There was only minor correlation between raised S-TSH or high TSH response to TRH and remnant size expressed in per cent of the total thyroid weight (relative remnant size). No significant difference in thyroxine and triiodothyronine was found between the weight groups. Hypothyroidism developed in two patients with small remnants (6 g) and in one patient with a remnant weight of 8 g. Recurrent hyperthyroidism occurred in one patient in the 8-gram weight group. These results indicate that the remnant size may be one factor influencing the outcome of thyroid function after surgery.

Adult

Radiographic measurements of the radio-carpal joint in normal adults.

Radiography of 60 normal wrists was performed in 50 adults to determine the inclination of the distal joint surface of the radius. The central ray was directed 10 degrees proximally in postero-anterior projection and 15 degree proximally in lateral projection. The values obtained were compared with those from perpendicular projections. No dfferences were found between the two p.a. projections. In the lateral projection with the central ray directed proximally the measuring points were better defined, resulting in significantly increased exactness of the measurement. The mean value for the volar inclination was 9 degrees in the latter projection.
