The levels of carcinoembryonic antigen (CEA), embryonic prealbumin 1 and 2 (EPA-1 and EPA-2), thermostable alkaline phosphatase of placental type (TSAP), alpha-fetoproteins (AFP), trophoblastic beta-globulin (TBG), chorionic globulins 1 and 2 (CAG-1 and CAG-2) were determined in 36 fetal, 78 definitive, 31 hyperplastic, 38 adenomatous and 49 malignant tissues of the thyroid (control: 100 donor blood sera) using the immunodiffusion method after O. Ouchterlony modified by N. I. Khramkova and G. I. Abelev and the method of counter immunoelectrophoresis after K. Wilms. Oncofetal antigens TSAP, EPA-1, EPA-2 and CEA characteristic for antigenic reversion of thyroid tumors were identified in thyroid tissues. Antigenic reversion was most noticeable in tissues of papillary thyroid cancer. CEA and EPA-1 identification in the patients' blood sera could be applied to serological diagnosis of thyroid cancer.
The levels of TSH, T4, T3, calcitonin (CT), PTH, TGA and TG were determined in 96 persons (87 patients and 9 healthy persons) before and after operation; 46 patients had papillary carcinoma, 15 follicular cancer, 24 medullary carcinoma, 2 undifferentiated cancer. There was no common clinicomorphological approach to the studied problem therefore it was impossible to compare the data of different researchers. Radionuclide tests were employed to specify the importance of different methods in clinical practice. Elevated levels of TG, TSH, PTH, T4 and a lowered level of CT were revealed in differentiated thyroid carcinomas; elevated levels of CT, TSH, PTH, T4 and a lowered level of TG were revealed in medullary carcinoma. A decrease in T4 and T3 levels was noted after operations in patients with differentiated thyroid cancers necessitating the use of substitution therapy and thyroid hormones.
The author observed 293 patients suffering from autoimmune thyroiditis (Hasimoto's thyroiditis--in 236). Four symptom complexes of the disease were distinguished: typical chronic autoimmune thyroidits in euthyroidal condition, chronic autoimmune thyroiditis with thyrotoxicosis, chronic autoimmune thyroiditis with hypothyroidism, relapsing autoimmune thyroiditis. Radioindication and scanning of the thyroid gland with radioactive iodine-131 was conducted preoperatively in 74 patients and at various postoperative periods in 26 patients. Radioisotopic methods were of importance in the comprehensive clinical examination of patients suffering from autoimmune thyroiditis. By means of these methods diagnosis of autoimmune thyroiditis was confirmed in 6.7% of patients. All the patients with autoimmune thyroiditis were operated on. Resection of the peritracheal segments of the thyroid gland with the isthmus extirpation was performed. During the postoperative period the patients required prolonged dispensary observation and treatment with immunodepressors and thyroid hormones.
A total of 293 patients with chronic autoimmune thyroiditis were observed. The circulating antibody titre to thyroglobulin was determined and 131I-radioindication of the thyroid gland was made in 65 patients. The mentioned studies were diagnostically informative in the complex examination of patients with chronic autoimmune thyroiditis.
